Risk Factors for Brain Metastases in Patients with Renal Cell Carcinoma.

ABSTRACT: Background:Patients with brain metastases (BM) from renal cell carcinoma (RCC) were considered to experience a poor prognosis. However, there is little knowledge on the risk factors for BM from RCC at diagnosis. This study was aimed at exploring the risk factors for patients with BM from RCC and the interaction among these risk factors. Methods:A total of 38759 cases of RCC were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Risk factors for BM from RCC were evaluated by univariate and multivariate logistic regression analyses. Interaction effect between age and tumor size was tested. Results:There was a significant difference in univariate analysis, including T stage, tumor size, grades III and IV, lymph node metastasis, bone metastasis, liver metastasis, lung metastasis, and surgery. There was a significant difference in multivariate analysis, including age, T stage, tumor size < 10?cm, grade IV, lymph node metastasis, bone metastasis, lung metastasis, and surgery. Patients older than 70 had 0.653-fold lower risk of developing BM compared with those younger than 70. Patients with tumor size ? 4?cm and <10?cm had higher risk of developing BM compared with those < 4?cm. The larger the tumor size, the higher the incidence of BM from RCC in those whose tumor size was less than 10?cm. An interaction test between the tumor size and age on brain metastasis was statistically significant in the crude analysis (P = 0.0114) and model II analysis (P = 0.0114) and model II analysis (P = 0.0114) and model II analysis (. Conclusion:Both tumor size and age were independent risk factors for brain metastases in patients with RCC. The impact of age on the risk of developing BM from RCC was limited to patients with tumor size ? 7?cm. Patients with a larger tumor size and younger age might have the higher risk of developing BM at diagnosis of RCC.

Project description:BackgroundNivolumab plus ipilimumab (NIVO + IPI) has demonstrated long-term efficacy and safety in patients with previously untreated, advanced renal cell carcinoma (aRCC). Although most phase 3 clinical trials exclude patients with brain metastases, the ongoing, multicohort phase 3b/4 CheckMate 920 trial (ClincalTrials.gov identifier NCT02982954) evaluated the safety and efficacy of NIVO + IPI in a cohort that included patients with aRCC and brain metastases, as reported here.MethodsPatients with previously untreated aRCC and asymptomatic brain metastases received NIVO 3 mg/kg plus IPI 1 mg/kg every 3 weeks × 4 followed by NIVO 480 mg every 4 weeks. The primary end point was the incidence of grade ≥3 immune-mediated adverse events (imAEs) within 100 days of the last dose of study drug. Key secondary end points were progression-free survival and the objective response rate according to Response Evaluation Criteria in Solid Tumors, version 1.1 (both determined by the investigator). Exploratory end points included overall survival, among others.ResultsAfter a minimum follow-up of 24.5 months (N = 28), no grade 5 imAEs occurred. The most common grade 3 and 4 imAEs were diarrhea/colitis (n = 2; 7%) and hypophysitis, rash, hepatitis, and diabetes mellitus (n = 1 each; 4%). The objective response rate was 32% (95% CI, 14.9%-53.5%) with a median duration of response of 24.0 months; 4 of 8 responders remained without reported progression. Seven patients (25%) had intracranial progression. The median progression-free survival was 9.0 months (95% CI, 2.9-12.0 months), and the median overall survival was not reached (95% CI, 14.1 months to not estimable).ConclusionsIn patients who had previously untreated aRCC and brain metastases-a population with a high unmet medical need that often is underrepresented in clinical trials-the approved regimen of NIVO + IPI followed by NIVO showed encouraging antitumor activity and no new safety signals.

Project description:BackgroundThe optimal tool for predicting the survival of renal cell carcinoma (RCC) patients with lung metastases remains controversial.MethodsWe selected patients diagnosed with RCC and lung metastases, from 2010 to 2015, from the Surveillance, Epidemiology, and End Results (SEER) database. After the selection of inclusion criteria and exclusion criterion, the rest of the patients were incorporated into model analysis. Least absolute shrinkage and selection operator (LASSO) regression was used to select the most important features for construction of a nomogram predicting cancer-specific survival. A calibration plot and the concordance index (C-index) were used to estimate nomogram efficacy in a validation cohort. The association between important factors selected by LASSO regression, and prognosis was assessed by the Kaplan-Meier (KM) survival curve. The receiver operating characteristic (ROC) curves were drawn to compare sensitivity and specificity between the nomogram we built and the TNM stage-based model.ResultsA total of 1,369 patients met the inclusion criteria, but not the exclusion criteria. The LASSO regression model reduced 15 features to seven potential predictors of survival, including tumor grade, the extent of surgery, N and T status, histological profile, and brain and bone metastasis status. Such features had good discrimination in the KM survival curves. The nomogram showed excellent discriminatory power (C-index, 0.71; 95% confidence interval: 0.70 to 0.72) and good calibration in terms of both 1- and 2-year cancer-specific survival. The nomogram showed great discriminatory power (C-index 0.68) and adequate calibration when applied to the validation cohort. The areas under the curve (AUCs) of nomogram were 0.767 and 0.780, respectively, and the AUCs of TNM stage were 0.617 and 0.618 at 1 and 2 years, respectively.ConclusionsOur nomogram might play a major role in predicting the cancer-specific survival of RCC patients with lung metastases.
