Hemophilia management: Huge impact of a tiny difference.

ABSTRACT: Hemophilia A and B are inherited X-linked disorders of hemostasis, associated with an increased bleeding tendency. Patients with severe hemophilia have undetectable clotting factor levels and experience spontaneous bleeds. In patients with nonsevere hemophilia, the clotting factor levels are 2% to 40% of normal and bleeds predominantly occur after provocative events such as trauma and surgery. Despite this milder phenotype, patients with nonsevere hemophilia may suffer from considerable morbidity and have an increased mortality risk. However, many aspects of the course of disease and treatment remain unclear. Information on the factors influencing interindividual differences in bleeding phenotype is lacking, and misdiagnosis may occur due to assay discrepancies in the diagnostic workup. Desmopressin is the preferred treatment modality, but some patients and indications require treatment with clotting factor concentrates. This may elicit inhibitor formation, which is associated with an increased burden of disease and a higher mortality rate. It has been found that patients with nonsevere hemophilia A carry a lifelong risk for this serious complication. In this review, we provide an overview of the current knowledge of the diagnosis and management of nonsevere hemophilia. A report of science presented at the International Society on Thrombosis and Haemostasis 2019 Annual Congress is also provided.

Project description:Known with certainty solely from a unique male specimen collected in central Brazil in the first quarter of the 19th century, the Critically Endangered (Possibly Extinct) Hooded Seedeater Sporophila melanops has been one of the great enigmas of Neotropical ornithology, arguably the only one of a host of long-lost species from Brazil to remain obstinately undiscovered. We reanalysed the morphology of the type specimen, as well as a female specimen postulated to represent the same taxon, and sequenced mitochondrial DNA (COI and Cyt-b) from both individuals. Furthermore, we visited the type locality, at the border between Goiás and Mato Grosso, and its environs on multiple occasions at different seasons, searching for birds with similar morphology to the type, without success. Novel genetic and morphological evidence clearly demonstrates that the type of S. melanops is not closely related to Yellow-bellied Seedeater S. nigricollis, as has been frequently postulated in the literature, but is in fact a representative of one of the so-called capuchinos, a clade of attractively plumaged seedeaters that breed mostly in the Southern Cone of South America. Our morphological analysis indicates that S. melanops has a hitherto unreported dark-coffee throat and that it is probably a Dark-throated Seedeater S. ruficollis collected within its wintering range, acquiring breeding plumage and showing melanism on the cap feathers. Alternatively, it may be a melanistic-capped individual of a local population of seedeaters known to breed in the Esteros del Iberá, Corrientes, Argentina, to which the name S. ruficollis might be applicable, whilst the name S. plumbeiceps might be available for what is currently known as S. ruficollis. A hybrid origin for S. melanops cannot be ruled out from the available data, but seems unlikely. The purported female specimen of S. melanops pertains either to S. nigricollis or to Double-collared Seedeater S. caerulescens based on genetic and morphological data, and thus cannot be a female of S. melanops. We conclude that Sporophila melanops is not typical of any natural population of seedeaters, appears to have been collected far from its breeding grounds while overwintering in central Brazil, and should not be afforded any conservation status.

Project description:Importance:Unruptured intracranial aneurysms (UIAs) are relatively common in the general population and are being increasingly diagnosed; a significant proportion are tiny (≤3 mm) aneurysms. There is significant heterogeneity in practice and lack of clear guidelines on the management of incidental, tiny UIAs. It is important to quantify the implications of different management strategies in terms of health benefits to patients. Objective:To evaluate the effectiveness of routine treatment (aneurysm coiling) vs 3 strategies for imaging surveillance compared with no preventive treatment or routine follow-up of tiny UIAs. Design, Setting, and Participants:A decision-analytic model-based comparative effectiveness analysis was conducted from May 1 to June 30, 2017, using inputs from the medical literature. PubMed searches were performed to identify relevant literature for all key model inputs, each of which was derived from the clinical study with the most robust data and greatest applicability. Analysis included 10 000 iterations simulating adult patients with incidental detections of UIAs 3 mm or smaller and no history of subarachnoid hemorrhage. Interventions:The following 5 management strategies for tiny UIAs were evaluated: annual magnetic resonance angiography (MRA) screening, biennial MRA screening, MRA screening every 5 years, aneurysm coiling and follow-up, and no treatment or preventive follow-up. Main Outcomes and Measures:A Markov decision model for lifetime rupture was constructed from a societal perspective per 10 000 patients with incidental, tiny UIAs. Outcomes were assessed in terms of quality-adjusted life-years. Probabilistic, 1-way, and 2-way sensitivity analyses were performed. Results:In this analysis of 10 000 iterations simulating adult patients with a mean age of 50 years, the base-case calculation shows that the management strategy of no treatment or preventive follow-up has the highest health benefit (mean [SD] quality-adjusted life-years, 19.40 [0.31]). Among the management strategies that incorporate follow-up imaging, MRA every 5 years is the best strategy with the next highest effectiveness (mean [SD] quality-adjusted life-years, 18.05 [0.62]). The conclusion remains robust in probabilistic and 1-way sensitivity analyses. No routine follow-up remains the optimal strategy when the annual growth rate and risk of rupture of growing aneurysms are varied. When the annual risk of rupture of nongrowing UIAs is less than 1.7% (0.23% in base case scenario), no follow-up is the optimal strategy. If annual risk of rupture is more than 1.7%, coiling should be performed directly. Conclusions and Relevance:Given the current literature, no preventive treatment or imaging follow-up is the most effective strategy in patients with aneurysms that are 3 mm or smaller, resulting in better health outcomes. More aggressive imaging surveillance for aneurysm growth or preventive treatment should be reserved for patients with a high risk of rupture. Given these findings, it is important to critically evaluate the appropriateness of current clinical practices, and potentially determine specific guidelines to reflect the most effective management strategy for patients with incidental, tiny UIAs.

Project description:The world’s 2.5 billion poorest people - small farmers living at the far fringe of the developing world – and their billion or so slightly better off neighbors burn 10.5 billion metric tonnes (tonnes) of crop waste annually. Smoke from their fires reddens the sun, closes airports, shuts schools and governments – and kills millions of people (World Health Organization (WHO). who.int/health-topics/air-pollution#tab=tab_1). Their fires release 16.6 billion tonnes of CO2, and emit 9.8 billion tonnes CO2e, 1.1 billion tonnes of smog precursors and 66 million tonnes of PM2.5. Seminars, health warnings, bans, threats, jailings, shootings – nothing has worked, because not one has offered farmers a better alternative. This is the story of how Warm Heart, a small, community development NGO, learned enough about small farmers’ plight to collaborate with them to develop the technology, training and social organization to mobilize villages to form biochar social enterprises. These make it profitable for farmers to convert crop waste into biochar, reducing CO2e, smog precursor and PM2.5 emissions, improving health and generating new local income – in short, to address the big three SDGs (1, 2 and 3) from the bottom. Warm Heart, however, wanted more; it wanted a system so appealing that it would spread by imitation and not require outside intervention. Based on what it has learned, Warm Heart wants to teach others that the knowledge to stop the smoke and improve the quality of one’s life does not require outside experts and lots of money. It wants to teach that anyone can learn to create a more sustainable world by themselves. This article traces the experiential learning process by which Warm Heart and its partners achieved their goals and shares Warm Heart’s open source solution. It serves four purposes. The article closely explores an experiential learning process. It details the underlying logic, workings and consequences of crop waste burning in the developing world. It demonstrates the application of this knowledge to the development of a sustainable – even profitable – solution to this global problem that does not require costly outside intervention but can be undertaken by local communities and small NGOs anywhere. Finally, it models how local communities, small NGOs and social investors can turn this global problem into a profitable business opportunity.
