Prevalence, risk factors and global impact of musculoskeletal injuries among endoscopists: a nationwide European study.

ABSTRACT: Background and study aims? Previous studies have suggested a high prevalence of musculoskeletal injuries (MI) in endoscopists. Little evidence has come from European countries. Our main aim was to evaluate the prevalence, type, and impact of MI among Portuguese endoscopists. We also sought to identify risk factors for the development, severity and number of endoscopy-related MI. Material and methods? A 48-question electronic survey was developed by a multidisciplinary group.?The electronic survey was sent to all members of Portuguese Society of Gastroenterology (n?=?705) during May 2019.?Study data were collected and managed using REDCap electronic data capture tools hosted at SPG?-?CEREGA. Results? The survey was completed by 171 endoscopists (response rate of 24.3?%), 55.0?% female with a median age of 36 years (range 26-78). The prevalence of at least one MI related to endoscopy was 69.6?% (n?=?119), the most frequent being neck pain (30.4?%) and thumb pain (29.2?%). The median time for MI development was 6 years (range 2 months-30 years). Severe pain was reported by 19.3?%. Change in endoscopic technique was undertaken by 61.3?% and reduction in endoscopic caseload was undertaken by 22.7?%. Missing work was reported by 10.1?%, with the median time off from work being 30 days (range 1-90). Female gender and???15 years in practice were independently associated with MI and severe pain. Years in practice, weekly-time performing endoscopy, and gender were significant predictors of the number of MI. Conclusions? Prevalence of MI was significant among Portuguese endoscopists and had a relevant impact on regular and professional activities.

Project description:Background Musculoskeletal disorders (MSKDs) present a significant burden to health care systems worldwide. Evidence suggests that university students may have unique risk factors for developing MSKDs; however, research on the corresponding prevalence and risk factors of MSKDs in Canadian students is limited. Methods Using a multi-year cross-sectional survey, we aimed to understand the prevalence and risk factors of MSKDs in students at McMaster University. A survey on the prevalence of MSKD as well as potential risk factors was conducted online in the years 2018–2022. Our outcomes were the prevalence of MSKDs over the last 7 days and the last 12 months, as well as presence of lower body, upper body, and spine MSKDs. We investigated risk factors using negative binomial regression analysis, including a sex-stratified analysis. Results There were a total of 289 respondents in 2018 with a decrease in the number of participants in the subsequent years (n2019 = 173, n2020 = 131, n2021 = 76). Participants reported a median of 2–3 pain sites in the last year and 1–2 pain sites in the last week in all four years. The most prevalent sources of self-reported pain were the lower back and neck. Depending on the year and outcome studied, 59–67% of participants reported neck/lower back pain in the last year, and 43–49% reported it in the last week. Although risk factors were different depending on the year and sex, overall, poorer mental health, being in health care studies, regular sports participation (males only), older age, and less hours of sleep were significantly associated with higher prevalence of MSKDs. Conclusions This study identified that MSKDs are a prevalent source of pain in university students. While some risk factors, such as mental health, are known to play a role in developing MSKDs, sports activity and academic pressure are risk factors that are unique to students. Our study also suggests that there may be differences in risk factors between sexes. Project description:Injurious pecking can cause a wide range of damage and is an important welfare and economic issue in turkey production. Aggressive pecking typically targets the head/neck (HN) area, and feather pecking typically targets the back/tail (BT) area; injuries in these separate areas could be used as a proxy for the level of aggressive and feather pecking in a flock. The objective of this study was to identify risk factors for integument injuries in Canadian turkey flocks. A survey containing a questionnaire about housing and management practices and a scoring guide was distributed to 500 turkey farmers across Canada. The farmer scored pecking injuries in two different body areas (HN and BT) on a 0-2 scale on a subset of birds within each flock. Multivariable logistic regression modeling was used to identify factors associated with the presence of HN and BT injuries. The prevalence of birds with integument injuries ranged widely between the flock subsets (HN = 0-40%, BT = 0-97%), however the mean prevalence was low (HN = 6%, BT = 10%). The presence of injuries for logistic regression was defined as flocks with an injury prevalence greater than the median level of injury prevalence in the dataset (3.3% HN and 6.6% BT). The final logistic regression model for HN injuries contained five variables: flock sex, flock age, number of daily inspections, number of different people during inspections, and picking up birds during inspections (N = 62, pR2 = 0.23, α = 0.05). The final logistic regression model for BT injuries contained six variables: flock sex, flock age, litter depth, litter condition, inspection duration, and use of hospital pens for sick/injured birds (N = 59, pR2 = 0.29, α = 0.05). Flock age, and to a lesser extent, sex was associated with both types of injuries. From a management perspective, aggressive pecking injuries appear to be influenced by variables related to human interaction, namely during inspections. On the other hand, the presence of feather pecking injuries, was associated with litter condition and other management factors like separating sick birds. Future research on injurious pecking in turkeys should focus on these aspects of housing and management to better describe the relationship between the identified variables and the prevalence and severity of these conditions.

Project description:ImportanceUnderstanding causes and correlates of health loss among children and adolescents can identify areas of success, stagnation, and emerging threats and thereby facilitate effective improvement strategies.ObjectiveTo estimate mortality and morbidity in children and adolescents from 1990 to 2017 by age and sex in 195 countries and territories.Design, setting, and participantsThis study examined levels, trends, and spatiotemporal patterns of cause-specific mortality and nonfatal health outcomes using standardized approaches to data processing and statistical analysis. It also describes epidemiologic transitions by evaluating historical associations between disease indicators and the Socio-Demographic Index (SDI), a composite indicator of income, educational attainment, and fertility. Data collected from 1990 to 2017 on children and adolescents from birth through 19 years of age in 195 countries and territories were assessed. Data analysis occurred from January 2018 to August 2018.ExposuresBeing under the age of 20 years between 1990 and 2017.Main outcomes and measuresDeath and disability. All-cause and cause-specific deaths, disability-adjusted life years, years of life lost, and years of life lived with disability.ResultsChild and adolescent deaths decreased 51.7% from 13.77 million (95% uncertainty interval [UI], 13.60-13.93 million) in 1990 to 6.64 million (95% UI, 6.44-6.87 million) in 2017, but in 2017, aggregate disability increased 4.7% to a total of 145 million (95% UI, 107-190 million) years lived with disability globally. Progress was uneven, and inequity increased, with low-SDI and low-middle-SDI locations experiencing 82.2% (95% UI, 81.6%-82.9%) of deaths, up from 70.9% (95% UI, 70.4%-71.4%) in 1990. The leading disaggregated causes of disability-adjusted life years in 2017 in the low-SDI quintile were neonatal disorders, lower respiratory infections, diarrhea, malaria, and congenital birth defects, whereas neonatal disorders, congenital birth defects, headache, dermatitis, and anxiety were highest-ranked in the high-SDI quintile.Conclusions and relevanceMortality reductions over this 27-year period mean that children are more likely than ever to reach their 20th birthdays. The concomitant expansion of nonfatal health loss and epidemiological transition in children and adolescents, especially in low-SDI and middle-SDI countries, has the potential to increase already overburdened health systems, will affect the human capital potential of societies, and may influence the trajectory of socioeconomic development. Continued monitoring of child and adolescent health loss is crucial to sustain the progress of the past 27 years.
