Assessment of the quality of life of prehospital care nursing professionals.

ABSTRACT: Background:Nursing professionals play an active role in Mobile Urgent Care Services (Serviço de Atendimento Móvel de Urgência-SAMU). Together with other staff members they are responsible for providing high-quality care to patients. However, they are exposed to extreme pressure and emotional overload, which might impair their biopsychological balance and quality of life. Objective:To investigate the quality of life of prehospital care nursing professionals. Method:Cross-sectional descriptive study with quantitative approach conducted from April through June 2017 at a SAMU in the northwestern area of the state of Ceara, Brazil. The target population was SAMU nursing professionals, namely, 8 nurses and 11 nursing technicians. Data collection was performed by means of an instrument which contained two quality of life assessment scales, the Flanagan scale and World Health Organization Quality of Life (WHOQOL-bref). Results:The domains with the poorest scores on the Flanagan scale were physical and material well-being and social activities. On WHOQOL-bref, the domains with the lowest mean scores were physical health, environment and self-assessed quality of life. Conclusion:The SAMU nursing professionals were predominantly female and married, their average age was 37 years old, had attended undergraduate education at least, and worked 71.88 (±17.50) hours per week. The results point to the need to implement actions to improve the quality of life of this population of workers.

Project description:BackgroundCardiac diseases are among the leading causes of death worldwide, including sudden cardiac arrest in particular. Nursing professionals are often the first to encounter these scenarios in various settings. Adequate preparation and competent knowledge among nurses significantly impact survival rates positively.AimTo describe the state of knowledge about Basic and Advanced Life Support guidelines among Ecuadorian nursing professionals.MethodologyA nationwide, descriptive, cross-sectional study was conducted from February to April 2023 among Ecuadorian nursing professionals. Participants were invited through official social media groups such as WhatsApp and Facebook. The study utilized a self-administered online questionnaire to evaluate theoretical knowledge of Basic Life Support (BLS) and Advanced Life Support (ALS). Knowledge scores were assigned based on the number of correct answers on the tests. T-tests and one-way ANOVA were used to examine relationships between knowledge scores and demographic and academic training variables.ResultsA total of 217 nursing professionals participated in the study. The majority of the participants were female (77.4%) and held a university degree (79.9%). Among them, only 44.7% claimed to have obtained a BLS training certificate at least once, and 19.4% had ALS certification. The overall BLS knowledge score (4.8/10 ± 1.8 points) was higher than the ALS score (4.3/10 ± 1.8 points). Participants who had obtained BLS certification and those who used evidence-based summaries as a source of extracurricular training achieved higher BLS and ALS knowledge scores.ConclusionEcuadorian nursing professionals in this study exhibited a significant deficiency in theoretical knowledge of BLS and ALS. Formal training and preparation positively impact life support knowledge. Support and inclusion of Ecuadorian nurses in training and academic preparation programs beginning at the undergraduate level are essential for promoting life support knowledge and improving outcomes.

Project description:BackgroundNursing homes are among the most common places of death in many countries.AimTo determine the quality of dying and end-of-life care of nursing home residents in six European countries.DesignEpidemiological survey in a proportionally stratified random sample of nursing homes. We identified all deaths of residents of the preceding 3-month period. Main outcomes: quality of dying in the last week of life (measured using End-of-Life in Dementia Scales - Comfort Assessment while Dying (EOLD-CAD)); quality of end-of-life care in the last month of life (measured using Quality of Dying in Long-Term Care (QoD-LTC) scale). Higher scores indicate better quality.Setting/participantsThree hundred and twenty-two nursing homes in Belgium, Finland, Italy, the Netherlands, Poland and England. Participants were staff (nurses or care assistants) most involved in each resident's care.ResultsStaff returned questionnaires regarding 1384 (81.6%) of 1696 deceased residents. The End-of-Life in Dementia Scales - Comfort Assessment while Dying mean score (95% confidence interval) (theoretical 14-42) ranged from 29.9 (27.6; 32.2) in Italy to 33.9 (31.5; 36.3) in England. The Quality of Dying in Long-Term Care mean score (95% confidence interval) (theoretical 11-55) ranged from 35.0 (31.8; 38.3) in Italy to 44.1 (40.7; 47.4) in England. A higher End-of-Life in Dementia Scales - Comfort Assessment while Dying score was associated with country ( p?=?0.027), older age ( p?=?0.012), length of stay ?1?year ( p?=?0.034), higher functional status ( p?<?0.001). A higher Quality of Dying in Long-Term Care score was associated with country ( p?<?0.001), older age ( p?<?0.001), length of stay ?1?year ( p?<?0.001), higher functional status ( p?=?0.002), absence of dementia ( p?=?0.001), death in nursing home ( p?=?0.033).ConclusionThe quality of dying and quality of end-of-life care in nursing homes in the countries studied are not optimal. Project description:PurposeA newly developed brief measure of nursing facility (NF) resident self-reported quality of life (QOL) has been proposed for inclusion in a modified version of the minimum data set (MDS). There is considerable interest in determining whether it is possible to develop indicators of QOL that are more convenient and less expensive than direct, in-person interviews with residents.Design and methodsQOL interview data from 2,829 residents living in 101 NFs using a 14-item version of a longer instrument were merged with data from the MDS and the Online Survey and Certification Automated Record (OSCAR). Bivariate and multivariate hierarchical linear modeling were used to assess the association of QOL with potential resident and facility level indicators.ResultsResident and facility level indicators were associated with self-reported QOL in the expected direction. At the individual resident level, QOL is negatively associated with physical function, visual acuity, continence, being bedfast, depression, conflict in relationships, and positively associated with social engagement. At the facility level, QOL is negatively associated with citations for failing to accommodate resident needs or providing a clean, safe environment. The ratio of activities staff to residents is positively associated with QOL. This study did not find an association between QOL and either use of restraints or nurse staff levels. Approximately 9 percent of the total variance in self-reported QOL can be attributed to differences among facilities; 91 percent can be attributed to differences among residents. Resident level indicators explained about 4 percent of the variance attributable to differences among residents, and facility factors explained 49 percent of the variance attributable to differences among NFs. However, the different variables explained only 10 percent of the variance in self-reported QOL.ImplicationsA brief self-report measure of NF resident QOL is consistently associated with measures that can be constructed from extant data sources. However, the level of prediction possible from these data sources does not justify reliance on external indicators of resident QOL for policy purposes.
