School-type differences in e-cigarette use and its correlates among Chinese adolescents.

ABSTRACT: INTRODUCTION:Studies examining e-cigarette use among adolescents in Shanghai, China, have focused largely on middle school students. Given the vast differences between vocational and traditional schools, we aimed to explore school-type differences in e-cigarette use and correlates among adolescents in Shanghai. METHODS:The study was conducted in September 2017 through multistage and stratified cluster random sampling, which consisted of 10699 adolescents aged 13-17 years attending traditional and vocational schools in Shanghai. Descriptive statistics and multivariate logistic regressions were conducted to assess the weighted prevalence and correlates of ever e-cigarette use stratified by school type. RESULTS:The weighted prevalence of e-cigarette use was 5.21% among all respondents. Although e-cigarette use was more prevalent among students attending vocational schools (p<0.001), its correlates were similar across both school types. Among vocational school students, ever tobacco use (OR=3.10; 95% CI: 2.36-4.08) was the most significant correlate, followed by having most friends as smokers (OR=2.97; 95% CI: 1.84-4.81) and having morning cravings (OR=1.90; 95% CI: 1.64-2.20). Among traditional school students, having most friends as smokers (OR=4.87; 95% CI: 2.78-8.54) and ever tobacco use (OR=3.78; 95% CI: 2.68-5.34) were the most significant correlates, followed by knowledge of pro-tobacco advertisements (OR=2.12; 95% CI: 1.54-2.91). CONCLUSIONS:Joint efforts from the national, school and family levels are needed to control e-cigarette use among adolescents in China, and such efforts should be tailored to address differences in school characteristics.

Project description:PURPOSE:Little is known about whether adolescent cigarette and e-cigarette use patterns over time differ by ethnicity. METHODS:Data were pooled from three prospective cohort studies of adolescents in California and Connecticut (baseline: 2013-2014; 12-month follow-up: 2014-2015; N = 6,258). Adjusted polytomous regression models evaluated the association of baseline exclusive ever e-cigarette use, exclusive ever cigarette use, ever use of both e-cigarettes and cigarettes (dual use) with past 30-day use at follow-up (exclusively e-cigarettes, exclusively cigarettes, dual use; no use at baseline/follow-up were the referent groups). Interaction analyses evaluated differences by race/ethnicity (Hispanic white [HW], non-Hispanic white [NHW], Other). RESULTS:A significant global interaction was observed for the association of baseline with follow-up tobacco use by ethnicity (p = .009). Among NHW participants, ever e-cigarette or cigarette users at baseline (vs. never users) had significantly higher odds of every past 30-day use tobacco use pattern at follow-up. Among HW participants, compared with never users, exclusive e-cigarette users at baseline had increased odds of continued e-cigarette use (ORexclusive e-cigarettes = 5.22; 95% confidence interval [CI]: 3.50, 7.79; ORdual use = 3.64; 95% CI: 1.62, 8.18) but not of transition to exclusive cigarette use at follow-up (ORexclusive cigarettes = 1.27; 95% CI: .47, 3.46), and HW exclusive cigarette users at baseline had greater odds of continued cigarette use (ORexclusive e-cigarettes = 12.3; 95% CI: 5.87, 25.8; ORdual use = 3.82; 95% CI: 1.06, 13.7) but not of transition to exclusive e-cigarette use at follow-up (ORexclusive cigarettes = 1.61; 95% CI: .62, 4.18). CONCLUSIONS:Findings that NHW youth report more transitional use patterns and HW youth report more stable use patterns suggest a potential for differential impacts of e-cigarettes, by ethnicity, in increasing subsequent transition to or cessation from cigarette smoking.

Project description:Introduction: Few studies have examined the association between the individual-, family-, and school-level ecological correlates and physical fitness among Chinese children and adolescents, which is the purpose of the present study. Methods: A total of 157,168 children and adolescents, 10-18 years of age, with physical fitness data in 2014, participated in the study. Physical fitness was assessed, using six tests: forced vital capacity, standing long jump, sit and reach, body muscle strength, 50-m dash, and endurance running. Scores were aggregated to form a summary physical fitness indicator (PFI), which was then classified into five levels: low, low-middle, middle, middle-high, and high. Each option of individual-, family-, and school-level measures were constructed into a positive or negative correlate of physical fitness and then accumulated as a composite ecological score. Results: Among the 20 individual-, family-, and school-level correlates, 18 were found to be significantly associated with PFI, with high PFI levels being correlated with the positive group of correlates and low PFI levels correlated with the negative group of correlates (p < 0.05). High scores of ecological correlates were associated with a high level of PFI [adjusted odds ratio (OR) = 1.06, 95% CI: 1.06, 1.07)] whereas low scores were associated with a low level of PFI (OR = 0.95, 95% CI:0.95, 0.95). The ecological correlates at the individual, school- and family-levels were shown to be significantly related to high PFI among Chinese children and adolescents aged 10-18 years with different ORs of 5.97 (95% CI: 5.51, 6.47), 3.94 (3.66, 4.24), and 1.25 (1.19, 1.31). The PAR% of 20 cumulative factors effects due to the negative and positive responses for low PFI levels were 35.9 and 16.1%, and, for high PFI levels, were 15.3 and 24.1%, among Chinese children and adolescents, respectively. Compared with the correlates at family and school levels, the correlates at individual levels had the largest PAR%. Conclusion: Individual-, family-, and school-multilevel factors had a significant cumulative association with either improving or worsening aspects of physical fitness. Individual level factors remain at the core of physical fitness improvement. Comprehensive policies and measures are urgently needed to enhance the physical fitness of Chinese children and adolescents through involving further individual and environmental factors.
