We present in this work an economic analysis of ransomware, a relatively new form of cyber-enabled extortion. We look at how the illegal gains of the criminals will depend on the strategies they use, examining uniform pricing and price discrimination. We also explore the welfare costs to society of such strategies. Project description:We use an event study approach to examine the economic consequences of hospital admissions for adults in two datasets: survey data from the Health and Retirement Study, and hospitalization data linked to credit reports. For non-elderly adults with health insurance, hospital admissions increase out-of-pocket medical spending, unpaid medical bills and bankruptcy, and reduce earnings, income, access to credit and consumer borrowing. The earnings decline is substantial compared to the out-of-pocket spending increase, and is minimally insured prior to age-eligibility for Social Security Retirement Income. Relative to the insured non-elderly, the uninsured non-elderly experience much larger increases in unpaid medical bills and bankruptcy rates following a hospital admission. Hospital admissions trigger less than 5 percent of all bankruptcies.

Project description:Acute kidney injury (AKI) is an increasingly common condition associated with poor health outcomes. Combined with its rising incidence, AKI has emerged as a major public health concern with high human and financial costs. In England, the estimated inpatient costs related to AKI consume 1% of the National Health Service budget. In the United States, AKI is associated with an increase in hospitalization costs that range from $5.4 to $24.0 billion. The most expensive patients are those with AKI of sufficient severity to require dialysis, where cost increases relative to patients without AKI range from $11,016 to $42,077 per hospitalization. Even with these high costs, significant hospital-level variation still exists in the cost of AKI care. In this article, we review the economic consequences of AKI for both the general and critically ill AKI population. Our primary objective is to shed light on an opportunity for hospitals and policymakers to develop new care processes for patients with AKI that have the potential to yield substantial cost savings. By exposing the high rates of death and disability experienced by affected patients and the immense financial burden attributable to AKI, we also hope to motivate scientists and entrepreneurs to pursue a variety of innovative therapeutic strategies to combat AKI in the near term.

Project description:This study examined economic disconnection, an extreme case of economic exclusion in which families lack both employment and cash assistance, among families in the child welfare system. To build hypotheses about the intersection of the child welfare system and economic disconnection we used a multilevel framework that considers federal policy, local practice, and processes within families. We hypothesized that child welfare intervention has the potential to be a mechanism of economic inclusion or exclusion for vulnerable families, with implications for family reunification. We utilized a novel administrative data set containing data from three state agencies to construct income histories of parents relative to their child's placement in foster care (N = 15,159 parents). We identified eight trajectories using group-based trajectory modeling. About two-thirds of parents experience economic disconnection over a three-year period; these families are least likely to reunify. Although providing economic resources to families is typically beyond the scope of child welfare, efforts to minimize the negative impact of child placement on parents' economic connection is likely to improve both the economic inclusion of poor families and family reunification.

Project description:BackgroundAn understanding of global, regional, and national macroeconomic losses caused by stroke is important for allocation of clinical and research resources. The authors investigated the macroeconomic consequences of stroke disease burden in the year 2019 in 173 countries.MethodsDisability-adjusted life year data for overall stroke and its subtypes (ischemic stroke, intracerebral hemorrhage, and subarachnoid hemorrhage) were collected from the GBD study (Global Burden of Disease) 2019 database. Gross domestic product (GDP, adjusted for purchasing power parity [PPP]) data were collected from the World Bank; GDP and disability-adjusted life year data were combined to estimate macroeconomic losses using a value of lost welfare (VLW) approach. All results are presented in 2017 international US dollars adjusted for PPP.ResultsGlobally, in 2019, VLW due to stroke was $2059.67 billion or 1.66% of the global GDP. Global VLW/GDP for stroke subtypes was 0.78% (VLW=$964.51 billion) for ischemic stroke, 0.71% (VLW=$882.81 billion) for intracerebral hemorrhage, and 0.17% (VLW=$212.36 billion) for subarachnoid hemorrhage. The Central European, Eastern European, and Central Asian GBD super-region reported the highest VLW/GDP for stroke overall (3.01%), ischemic stroke (1.86%), and for subarachnoid hemorrhage (0.26%). The Southeast Asian, East Asian, and Oceanian GBD super-region reported the highest VLW/GDP for intracerebral hemorrhage (1.48%).ConclusionsThe global macroeconomic consequences related to stroke are vast even when considering stroke subtypes. The present quantification may be leveraged to help justify increased spending of finite resources on stroke in an effort to improve outcomes for patients with stroke globally.

Project description:Existing animal welfare standards for legislation and food certification programmes are primarily designed to avoid harms to the livestock, with minimal consideration given to their behavioural freedoms. Recent research has shown, however, that animal welfare should not only be evaluated by the absence of negative states but also by the presence of "good life" or positive experiences enjoyed by animals. The objective of the present study is to investigate the scientific validity and on-farm cost implications of utilising potential input-based measures of positive welfare as part of evaluation criteria for farm assurance schemes. Building upon the Farm Animal Welfare Council's concept of good life opportunities, an assessment was undertaken on 49 noncaged laying hen farms across the UK by measuring on-farm resources to facilitate positive experiences alongside commonly measured metrics for welfare outcomes. The financial cost of providing these resources on each enterprise was also estimated using a farm-scale costing tool. The results suggested that 63% of resource needs that facilitate the behaviour opportunities of laying hens are already being provided by these producers, far above legal and commercial requirements. This practice attracts no reward mechanism or direct financial benefit under the current market structure. Additional provision of opportunities was positively associated with behavioural outcomes, but only limited impact was observed on health and productivity measures. Economic modelling indicated that significant room exists to further improve welfare scores on these farms, on average by 97%, without incurring additional costs. Together we argue that these results can be seen as evidence of market failure since producers are providing positive welfare value to society that is not being currently recognised. It is therefore contended that measuring and rewarding the supply of good life opportunities could be a novel policy instrument to create an effective marketplace that appropriately recognises high welfare production.

Project description:Both proper animal welfare and economic benefit are important to the broiler industry, so it is better to consider these two factors together. The purpose of this study was to investigate the relationship between economic benefit and animal welfare in different production systems of white-feathered broilers in China. Based on the Welfare Quality Assessment (WQA) protocol for poultry, the authors compared and evaluated the results of the Welfare Quality model (WQM) and the deterministic model. The present study conducted welfare evaluations and investigations on 66 broiler chicken flocks on 52 farms in China. These flocks included three types: the net floor system (NFS), the normal cage system (NCS), and the high standard cage system (HCS). In terms of economy, the results were in line with high economic input, high output, and high profit. In terms of animal welfare assessment, the authors calculated the welfare scores per measure and the attributional WQ scores and WQ index scores of each production systems. The results showed that nine welfare measures from four welfare criteria presented different trends in the three production systems. WQ index scores were 778.24 ± 29.45, 691.09 ± 32.97, and 669.82 ± 22.79, respectively. According to Chow test results, significant differences were found between WQ index scores and total cost and profit (all p < 0.01). In conclusion, with the development of white-feathered broiler production in China, from the conventional system to the latest system, both cost and economic profit have been increased, but the welfare score has been decreased.

Project description:BackgroundAlthough alcohol use disorder (AUD) is associated with future risk for psychosocial dysfunction, the degree to which this arises from a direct causal effect of AUD on functioning v. from correlated risk factors (also known as confounders) is less clearly established.MethodAUD was assessed from Swedish medical, criminal and pharmacy registries. In a large general population cohort, using Cox proportional hazard and regression models, we predicted from the onset of AUD four outcomes: early retirement, unemployment, social assistance, and individual income. We then examined the degree to which these associations were attenuated by relevant confounders as well as by the use of discordant cousin, half-sibling, full-sibling, and monozygotic twin pairs.ResultsIn males, AUD most strongly predicted social assistance [hazard ratio (HR) 8.27, 95% confidence interval (CI) 7.96-8.59], followed by early retirement (HR 5.63, 95% CI 5.53-5.72) and unemployment (HR 2.75, 95% CI 2.65-2.85). For income at age 50, AUD was associated with a decrease in income of 0.24 s.d.s (95% CI -0.25 to -0.23). Results were similar in females. Modest to moderate attenuation of these associations was seen in both sexes after the addition of relevant covariates. These associations were reduced but remained robust in discordant co-relative pairs, including monozygotic twins.ConclusionsOur results suggest that AUD has a causal impact on a range of measures reflective of psychosocial dysfunction. These findings provide strong support for the drift hypothesis. However, some of the associations between AUD and dysfunction appear to be non-causal and result from shared risk factors, many of which are likely familial.

Project description:Quantitative estimates of the economic damages of climate change usually are based on aggregate relationships linking average temperature change to loss in gross domestic product (GDP). However, there is a clear need for further detail in the regional and sectoral dimensions of impact assessments to design and prioritize adaptation strategies. New developments in regional climate modeling and physical-impact modeling in Europe allow a better exploration of those dimensions. This article quantifies the potential consequences of climate change in Europe in four market impact categories (agriculture, river floods, coastal areas, and tourism) and one nonmarket impact (human health). The methodology integrates a set of coherent, high-resolution climate change projections and physical models into an economic modeling framework. We find that if the climate of the 2080s were to occur today, the annual loss in household welfare in the European Union (EU) resulting from the four market impacts would range between 0.2-1%. If the welfare loss is assumed to be constant over time, climate change may halve the EU's annual welfare growth. Scenarios with warmer temperatures and a higher rise in sea level result in more severe economic damage. However, the results show that there are large variations across European regions. Southern Europe, the British Isles, and Central Europe North appear most sensitive to climate change. Northern Europe, on the other hand, is the only region with net economic benefits, driven mainly by the positive effects on agriculture. Coastal systems, agriculture, and river flooding are the most important of the four market impacts assessed.

Project description:Background: Rheumatic heart disease (RHD) has declined dramatically in wealthier countries in the past three decades, but it remains endemic in many lower-resourced regions and can have significant costs to households. The objective of this study was to quantify the economic burden of RHD among Ugandans affected by RHD. Methods: This was a cross-sectional cost-of-illness study that randomly sampled 87 participants and their households from the Uganda National RHD registry between December 2018 and February 2020. Using a standardized survey instrument, we asked participants and household members about outpatient and inpatient RHD costs and financial coping mechanisms incurred over the past 12 months. We used descriptive statistics to analyze levels and distributions of costs and the frequency of coping strategies. Multivariate Poisson regression models were used to assess relationships between socioeconomic characteristics and utilization of financial coping mechanisms. Results: Most participants were young or women, demonstrating a wide variation in socioeconomic status. Outpatient and inpatient costs were primarily driven by transportation, medications, and laboratory tests, with overall RHD direct and indirect costs of $78 per person-year. Between 20 and 35 percent of households experienced catastrophic healthcare expenditure, with participants in the Northern and Western Regions 5-10 times more likely to experience such hardship and utilize financial coping mechanisms than counterparts in the Central Region, a wealthier area. Increases in total RHD costs were positively correlated with increasing use of coping behaviors. Conclusion: Ugandan households affected by RHD, particularly in lower-income areas, incur out-of-pocket costs that are very high relative to income, exacerbating the poverty trap. Universal health coverage policy reforms in Uganda should include mechanisms to reduce or eliminate out-of-pocket expenditures for RHD and other chronic diseases.

Project description:Medical interventions that increase life expectancy of patients result in additional consumption of non-medical goods and services in 'added life years'. This paper focuses on the distributional consequences across socio-economic groups of including these costs in cost effectiveness analysis. In that context, it also highlights the role of remaining quality of life and household economies of scale. Data from a Dutch household spending survey was used to estimate non-medical consumption and household size by age and educational attainment. Estimates of non-medical consumption and household size were combined with life tables to estimate what the impact of including non-medical survivor costs would be on the incremental cost effectiveness ratio (ICER) of preventing a death at a certain age. Results show that including non-medical survivor costs increases estimated ICERs most strongly when interventions are targeted at the higher educated. Adjusting for household size (lower educated people less often live additional life years in multi-person households) and quality of life (lower educated people on average spend added life years in poorer health) mitigates this difference. Ignoring costs of non-medical consumption in economic evaluations implicitly favors interventions targeted at the higher educated and thus potentially amplifies socio-economic inequalities in health.
