Determination of Albumin, Glucose, and Creatinine Employing a Single Sequential Injection Lab-at-Valve with Mono-Segmented Flow System Enabling In-Line Dilution, In-Line Single-Standard Calibration, and In-Line Standard Addition.

ABSTRACT: A mono-segmented sequential injection lab-at-valve (SI-LAV) system for the determination of albumin, glucose, and creatinine, three key biomarkers in diabetes screening and diagnosis, was developed as a single system for multi-analyte analysis. The mono-segmentation technique was employed for in-line dilution, in-line single-standard calibration, and in-line standard addition. This made adjustments to the sample preparation step easy unlike the batch-wise method. The results showed that the system could be used for both fast reaction (albumin) and slow reaction (glucose with enzymatic reaction and creatinine). In the case of slow reaction, the analysis time could be shortened by using the reaction rate obtained with the SI-LAV system. This proposed system is for cost-effective and downscaling analysis, which would be applicable for small hospitals and clinics in remote places with a small number of samples but relatively fast screening would be needed.

Project description:BackgroundThe standard configuration's set of twelve electrocardiogram (ECG) leads is optimal for the medical diagnosis of diverse cardiac conditions. However, it requires ten electrodes on the patient's limbs and chest, which is uncomfortable and cumbersome. Interlead conversion methods can reconstruct missing leads and enable more comfortable acquisitions, including in wearable devices, while still allowing for adequate diagnoses. Currently, methodologies for interlead ECG conversion either require multiple reference (input) leads and/or require input signals to be temporally aligned considering the ECG landmarks.MethodsUnlike the methods in the literature, this paper studies the possibility of converting ECG signals into all twelve standard configuration leads using signal segments from only one reference lead, without temporal alignment (blindly-segmented). The proposed methodology is based on a deep learning encoder-decoder U-Net architecture, which is compared with adaptations based on convolutional autoencoders and label refinement networks. Moreover, the method is explored for conversion with one single shared encoder or multiple individual encoders for each lead.ResultsDespite the more challenging settings, the proposed methodology was able to attain state-of-the-art level performance in multiple target leads, and both lead I and lead II seem especially suitable to convert certain sets of leads. In cross-database tests, the methodology offered promising results despite acquisition setup differences. Furthermore, results show that the presence of medical conditions does not have a considerable effect on the method's performance.ConclusionsThis study shows the feasibility of converting ECG signals using single-lead blindly-segmented inputs. Although the results are promising, further efforts should be devoted towards the improvement of the methodologies, especially the robustness to diverse acquisition setups, in order to be applicable to cardiac health monitoring in wearable devices and less obtrusive clinical scenarios.

Project description:BackgroundAll behaviour requires energy, and measuring energy expenditure in standard units (joules) is key to linking behaviour to ecological processes. Animal-borne accelerometers are commonly used to infer proxies of energy expenditure, termed 'dynamic body acceleration' (DBA). However, converting acceleration proxies (m/s2) to standard units (watts) involves costly in-lab respirometry measurements, and there is a lack of viable substitutes for empirical calibration relationships when these are unavailable.MethodsWe used past allometric work quantifying energy expenditure during resting and locomotion as a function of body mass to calibrate DBA. We used the resulting 'power calibration equation' to estimate daily energy expenditure (DEE) using two models: (1) locomotion data-based linear calibration applied to the waking period, and Kleiber's law applied to the sleeping period (ACTIWAKE), and (2) locomotion and resting data-based linear calibration applied to the 24-h period (ACTIREST24). Since both models require locomotion speed information, we developed an algorithm to estimate speed from accelerometer, gyroscope, and behavioural annotation data. We applied these methods to estimate DEE in free-ranging meerkats (Suricata suricatta), and compared model estimates with published DEE measurements made using doubly labelled water (DLW) on the same meerkat population.ResultsACTIWAKE's DEE estimates did not differ significantly from DLW (t(19) = - 1.25; P = 0.22), while ACTIREST24's estimates did (t(19) = - 2.38; P = 0.028). Both models underestimated DEE compared to DLW: ACTIWAKE by 14% and ACTIREST by 26%. The inter-individual spread in model estimates of DEE (s.d. 1-2% of mean) was lower than that in DLW (s.d. 33% of mean).ConclusionsWe found that linear locomotion-based calibration applied to the waking period, and a 'flat' resting metabolic rate applied to the sleeping period can provide realistic joule estimates of DEE in terrestrial mammals. The underestimation and lower spread in model estimates compared to DLW likely arise because the accelerometer only captures movement-related energy expenditure, whereas DLW is an integrated measure. Our study offers new tools to incorporate body mass (through allometry), and changes in behavioural time budgets and intra-behaviour changes in intensity (through DBA) in acceleration-based field assessments of daily energy expenditure.
