This paper is a collection of telemedicine techniques used by wireless body area networks (WBANs) for emergency conditions. Furthermore, Bayes' theorem is proposed for predicting emergency conditions. With prior knowledge, the posterior probability can be found along with the observed evidence. Project description:This paper proposes an emergency Traffic Adaptive MAC (eTA-MAC) protocol for WBANs based on Prioritization. The main advantage of the protocol is to provide traffic ranking through a Traffic Class Prioritization-based slotted-Carrier Sense Multiple Access/Collision Avoidance (TCP-CSMA/CA) scheme. The emergency traffic is handled through Emergency Traffic Class Provisioning-based slotted-CSMA/CA (ETCP-CSMA/CA) scheme. The emergency-based traffic adaptivity is provided through Emergency-based Traffic Adaptive slotted-CSMA/CA (ETA-CSMA/CA) scheme. The TCP-CSMA/CA scheme assigns a distinct, minimized and prioritized backoff period range to each traffic class in every backoff during channel access in Contention Access Period (CAP). The ETCP-CSMA/CA scheme delivers the sporadic emergency traffic that occurs at a single or multiple BMSN(s) instantaneously, with minimum delay and packet loss. It does this while being aware of normal traffic in the CAP. Then, the ETA-CSMA/CA scheme creates a balance between throughput and energy in the sporadic emergency situation with energy preservation of normal traffic BMSNs. The proposed protocol is evaluated using NS-2 simulator. The results indicate that the proposed protocol is better than the existing Medium Access Control (MAC) protocols by 86% decrease in packet delivery delay, 61% increase in throughput, and a 76% decrease in energy consumption.

Project description:Wireless Body Area Network (WBAN) is effectively used in healthcare to increase the value of the patient's life and also the value of healthcare services. The biosensor based approach in medical care system makes it difficult to respond to the patients with minimal response time. The medical care unit does not deploy the accessing of ubiquitous broadband connections full time and hence the level of security will not be high always. The security issue also arises in monitoring the user body function records. Most of the systems on the Wireless Body Area Network are not effective in facing the security deployment issues. To access the patient's information with higher security on WBAN, Game Theory with Stackelberg Security Equilibrium (GTSSE) is proposed in this paper. GTSSE mechanism takes all the players into account. The patients are monitored by placing the power position authority initially. The position authority in GTSSE is the organizer and all the other players react to the organizer decision. Based on our proposed approach, experiment has been conducted on factors such as security ratio based on patient's health information, system flexibility level, energy consumption rate, and information loss rate. Stackelberg Security considerably improves the strength of solution with higher security.

Project description:The importance of body area sensor networks (BASNs) is increasing day by day because of their increasing use in Internet of things (IoT)-enabled healthcare application services. They help humans in improving their quality of life by continuously monitoring various vital signs through biosensors strategically placed on the human body. However, BASNs face serious challenges, in terms of the short life span of their batteries and unreliable data transmission, because of the highly unstable and unpredictable channel conditions of tiny biosensors located on the human body. These factors may result in poor data gathering quality in BASNs. Therefore, a more reliable data transmission mechanism is greatly needed in order to gather quality data in BASN-based healthcare applications. Therefore, this study proposes a novel, multiobjective, lion mating optimization inspired routing protocol, called self-organizing multiobjective routing protocol (SARP), for BASN-based IoT healthcare applications. The proposed routing scheme significantly reduces local search problems and finds the best dynamic cluster-based routing solutions between the source and destination in BASNs. Thus, it significantly improves the overall packet delivery rate, residual energy, and throughput with reduced latency and packet error rates in BASNs. Extensive simulation results validate the performance of our proposed SARP scheme against the existing routing protocols in terms of the packet delivery ratio, latency, packet error rate, throughput, and energy efficiency for BASN-based health monitoring applications.

Project description:Wireless Body Area Network (WBAN) has become the emerging technology due to its ability to provide intelligent and cost-effective healthcare monitoring solution. The biological sensors used in WBAN are energy-constrained and required to be functional for a longer duration. Also, the sensed data should be communicated in reasonable time. Therefore, network lifetime and delay have become the primary concerns in the design of WBAN. In this paper, Receive Diversity based Transmission Data Rate Optimization (RDTDRO) scheme is proposed to improve the network lifetime and delay efficiency of Multi level-Quadrature Amplitude Modulation (M-QAM) based WBAN. In the proposed RDTDRO scheme, minimum energy consumption is ensured by optimizing the transmission data rate with respect to a given transmission distance and number of receive antennas while satisfying the Bit Error Rate (BER) requirements. The performance of proposed RDTDRO is analyzed in terms of network lifetime and delay difference and is compared with conventional Baseline and Rate optimized schemes. The results show that at a transmission distance of 0.3 m, the proposed RDTDRO scheme with a receive diversity order of 4 achieves 1.30 times and 1.27 times improvement in network lifetime over conventional Baseline and Rate optimized schemes respectively. From the results, it is also evident that at a transmission distance of 0.3 m, the proposed RDTDRO scheme with a receive diversity order of 4 is delay efficient as it achieves delay difference of 0.75 μs and 0.29 μs over conventional Baseline and Rate optimized schemes respectively.

Project description:This research work proposes a novel priority aware schedule based charging algorithm that uses wireless power transfer (WPT) technique in order to charge embedded sensor nodes (SNs) in a wireless body area network (WBAN). Implanted sensor nodes in WBANs require energy for both information extraction and data transmission to the remote controller unit. Thus, energy shortage of these SNs deteriorates due to the data transmission process of the patient health monitoring system. However, continuous operation by means of electromagnetic induction for energy harvesting, obtained from ambient sources, reduces the overall efficiency of the primary unit. With this paradigm in sight, an algorithm demonstrating the modeling of a priority-based mechanism is proposed in order to ensure proper sensor voltage level and to reduce the transmission losses. Medium access control (MAC) protocols are used for inductive powering from the primary unit to the secondary unit in a collision-free centralized scheduling scheme. Therefore, the proposed wireless charging algorithm for implanted SNs in WBAN is designed as per carrier sense multiple access with collision avoidance (CSMA/CA) technique. Because of this, the overall power consumption of SNs for certain operation periods, successful charging probabilities for multiple SNs, and instantaneous power requirements are considered as key performance measures of analysis. It is assumed that proper energy storage in both transmitters and receivers can handle channel interference and traffic contention. Simulation results verify that a significant reduction in power consumption for the proposed priority aware algorithm will maintain almost similar output. For this reason, saturating class-C as well as class-E driver circuits have been used to justify the performance in two different circuit topologies. Effects of priority with respect to the full charge period have also been observed for the multi-node system. Furthermore, from performance analysis, it has been demonstrated that the scheduling scheme causes both single MOSFET composed saturating class-C and Lchoke modeled class-E associated driver circuits to be considerably more loss efficient than corresponding existing ones.

Project description:Targeting the medical monitoring applications of wireless body area networks (WBANs), a hybrid medium access control protocol using an interrupt mechanism (I-MAC) is proposed to improve the energy and time slot utilization efficiency and to meet the data delivery delay requirement at the same time. Unlike existing hybrid MAC protocols, a superframe structure with a longer length is adopted to avoid unnecessary beacons. The time slots are mostly allocated to nodes with periodic data sources. Short interruption slots are inserted into the superframe to convey the urgent data and to guarantee the real-time requirements of these data. During these interruption slots, the coordinator can break the running superframe and start a new superframe. A contention access period (CAP) is only activated when there are more data that need to be delivered. Experimental results show the effectiveness of the proposed MAC protocol in WBANs with low urgent traffic.

Project description:BackgroundThe Health Resources and Services Administration (HRSA), Federal Office of Rural Health Policy (FORHP) funded the Evidence-Based Tele-Emergency Network Grant Program (EB TNGP) to serve the dual purpose of providing telehealth services in rural emergency departments (teleED) and systematically collecting data to inform the telehealth evidence base. This provided a unique opportunity to examine trends across multiple teleED networks and examine heterogeneity in processes and outcomes.Method and findingsSix health systems received funding from HRSA under the EB TNGP to implement teleED services and they did so to 65 hospitals (91% rural) in 11 states. Three of the grantees provided teleED services to a general patient population while the remaining three grantees provided teleED services to specialized patient populations (i.e., stroke, behavioral health, critically ill children). Over a 26-month period (November 1, 2015 -December 31, 2017), each grantee submitted patient-level data for all their teleED encounters on a uniform set of measures to the data coordinating center. The six grantees reported a total of 4,324 teleED visits and 99.86% were technically successful. The teleED patients were predominantly adult, White, not Latinx, and covered by Medicare or private insurance. Across grantees, 7% of teleED patients needed resuscitation services, 58% were rated as emergent, and 30% were rated as urgent. Across grantees, 44.2% of teleED patients were transferred to another inpatient facility, 26.0% had a routine discharge, and 24.5% were admitted to the local inpatient facility. For the three grantees who served a general patient population, the most frequent presenting complaints for which teleED was activated were chest pain (25.7%), injury or trauma (17.1%), stroke symptoms (9.9%), mental/behavioral health (9.8%), and cardiac arrest (9.5%). The teleED consultation began before the local clinician exam in 37.8% of patients for the grantees who served a general patient population, but in only 1.9% of patients for the grantees who provided specialized services.ConclusionsGrantees used teleED services for a representative rural population with urgent or emergent symptoms largely resulting in transfer to a distant hospital or inpatient admission locally. TeleED was often available as the first point of contact before a local provider examination. This finding points to the important role of teleED in improving access for rural ED patients.

Project description:Recent technological advancement in wireless communication has led to the invention of wireless body area networks (WBANs), a cutting-edge technology in healthcare applications. WBANs interconnect with intelligent and miniaturized biomedical sensor nodes placed on human body to an unattended monitoring of physiological parameters of the patient. These sensors are equipped with limited resources in terms of computation, storage, and battery power. The data communication in WBANs is a resource hungry process, especially in terms of energy. One of the most significant challenges in this network is to design energy efficient next-hop node selection framework. Therefore, this paper presents a green communication framework focusing on an energy aware link efficient routing approach for WBANs (ELR-W). Firstly, a link efficiency-oriented network model is presented considering beaconing information and network initialization process. Secondly, a path cost calculation model is derived focusing on energy aware link efficiency. A complete operational framework ELR-W is developed considering energy aware next-hop link selection by utilizing the network and path cost model. The comparative performance evaluation attests the energy-oriented benefit of the proposed framework as compared to the state-of-the-art techniques. It reveals a significant enhancement in body area networking in terms of various energy-oriented metrics under medical environments.

Project description:Wireless Body Area Networks (WBANs) are in the spotlight of researchers and engineering industries due to many applications. Remote health monitoring for general as well as military purposes where tiny sensors are attached or implanted inside the skin of the body to sense the required attribute is particularly prominent. To seamlessly accomplish this procedure, there are various challenges, out of which temperature control to reduce thermal effects and optimum power consumption to reduce energy wastage are placed at the highest priority. Regular and consistent operation of a sensor node for a long-time result in a rising of the temperature of respective tissues, where it is attached or implanted. This temperature rise has harmful effects on human tissues, which may lead to the tissue damage. In this paper, a Temperate Aware and Energy Optimized (TAEO) routing protocol is proposed that not only deals with the thermal aspects and hot spot problem, but also extends the stability and lifetime of a network. Analytical simulations are conducted, and the results depict better performance in terms of the network lifetime, throughput, energy preservation, and temperature control with respect to state of the art WBAN protocols.

Project description:BackgroundTelemedicine offers a unique opportunity to improve coordination and administration for urgent patient care remotely. In an emergency setting, it has been used to support first responders by providing telephone or video consultation with specialists at hospitals and through the exchange of prehospital patient information. This technological solution is evolving rapidly, yet there is a concern that it is being implemented without a demonstrated clinical need and effectiveness as well as without a thorough economic evaluation.ObjectiveOur objective is to systematically review whether the clinical outcomes achieved, as reported in the literature, favor telemedicine decision support for medical interventions during prehospital care.MethodsThis systematic review included peer-reviewed journal articles. Searches of 7 databases and relevant reviews were conducted. Eligibility criteria consisted of studies that covered telemedicine as data- and information-sharing and two-way teleconsultation platforms, with the objective of supporting medical decisions (eg, diagnosis, treatment, and receiving hospital decision) in a prehospital emergency setting. Simulation studies and studies that included pediatric populations were excluded. The procedures in this review followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) statement. The Risk Of Bias In Non-randomised Studies-of Interventions (ROBINS-I) tool was used for the assessment of risk of bias. The results were synthesized based on predefined aspects of medical decisions that are made in a prehospital setting, which include diagnostic decision support, receiving facility decisions, and medical directions for treatment. All data extractions were done by at least two reviewers independently.ResultsOut of 42 full-text reviews, 7 were found eligible. Diagnostic support and medical direction and decision for treatments were often reported. A key finding of this review was the high agreement between prehospital diagnoses via telemedicine and final in-hospital diagnoses, as supported by quantitative evidence. However, a majority of the articles described the clinical value of having access to remote experts without robust quantitative data. Most telemedicine solutions were evaluated within a feasibility or short-term preliminary study. In general, the results were positive for telemedicine use; however, biases, due to preintervention confounding factors and a lack of documentation on quality assurance and protocol for telemedicine activation, make it difficult to determine the direct effect on patient outcomes.ConclusionsThe information-sharing capacity of telemedicine enables access to remote experts to support medical decision making on scene or in prolonged field care. The influence of human and technology factors on patient care is poorly understood and documented.
