Cable Graft: Simple Superior Capsule Reconstruction Technique for Irreparable Rotator Cuff Tear Using a Teflon Patch.

ABSTRACT: We describe a simple superior capsule reconstruction technique for irreparable rotator cuff tear using a Teflon patch. In this technique, a triple-folded Teflon patch, suture tape, and a strong suture penetrating through the graft are fixed to the glenoid and greater tuberosity using a suture anchor. This allows for reconstruction of the superior capsule while simultaneously playing a role as a spacer. This procedure's greatest advantage is its simplicity; it is easy to perform, has a short operative time, and avoids the need to collect autologous tissue. More time is saved, as suturing and tying are not required. We believe our study could aid orthopaedic surgeons in clinical decision-making when encountering irreparable rotator cuff repairs.

Rooting Rotator Cuff Reconstruction for Irreparable Posterior-Superior Rotator Cuff Tear.

Project description:Irreparable posterior-superior rotator cuff tear is encountered quite often in clinical practice. Bridging the tendon defect with various materials is reasonable. However, optimal bridging structures and techniques are still being pursued. We introduce a rotator cuff bridging technique, rooting rotator cuff reconstruction. In this technique, autogenous tendon is used to make grafts. On the medial side, the graft tendons are suspended on the rotator cuff tendon. On the lateral side, the graft tendons are placed into tunnels through the tuberosities. The most critical steps of this technique are properly fabricating the humeral tunnels and suspending the graft tendons onto the rotator cuff tendon. We believe this technique will shed light on rotator cuff reconstruction.

Arthroscopic Superior Capsule Reconstruction for Irreparable Rotator Cuff Tears.

Project description:Massive irreparable rotator cuff tears in young patients are a particular challenge for the orthopaedic surgeon. Surgical treatment options include debridement, partial rotator cuff repair, patch-augmented rotator cuff repair, bridging rotator cuff reconstruction with graft interposition, tendon transfer, and reverse total shoulder arthroplasty. Recently, reconstruction of the superior glenohumeral capsule using a fascia lata autograft has been suggested to reduce superior glenohumeral translation and restore superior stability. Promising clinical results have been reported in 1 case series of 23 patients, indicating that superior capsular reconstruction may be a promising tool to manage massive irreparable rotator cuff tears. This article describes our preferred technique for arthroscopic superior capsule reconstruction.

Arthroscopic Superior Capsule Reconstruction Technique in the Setting of a Massive, Irreparable Rotator Cuff Tear.

Project description:Massive, irreparable rotator cuff tears are challenging to treat and associated with pain and severe limitation in shoulder elevation due to the proximal migration of the humeral head and, consequently, subacromial impingement. Furthermore, retraction of the tendons in combination with fat infiltration and muscular weakness results in unpredictable treatment outcomes. While conservative treatment may be warranted for low-demand patients, surgical treatment is often indicated for a successful return to activities of daily living as well as an improved quality of life. The reported failure rate of rotator cuff repair for the treatment of a massive rotator cuff tear varies. However, this failure is often found at the interface between the tendon and tendon footprint. Several techniques have been reported to address this pathology, including muscular transfer, arthroscopic debridement, augmentation procedures, and superior capsule reconstruction. In particular, superior capsule reconstruction has been reported as a safe and effective method to treat a massive, irreparable rotator cuff tear. The purpose of this Technical Note is to describe our preferred technique of a superior capsule reconstruction for the treatment of a massive, irreparable rotator cuff tear.

Dynamic Biceps Rerouting for Irreparable Posterior-Superior Rotator Cuff Tear.

Project description:Irreparable posterior-superior rotator cuff tear (IPSRCT) is a common clinical condition. Many methods of transfer of the long head of the biceps (LHB) have been developed to augment the shoulder superiorly, with the best method using the LHB still being pursued. In this article, we introduce a special method using the LHB to address IPSRCT: arthroscopic dynamic LHB rerouting. The main steps of this technique include opening the native bicipital groove, creating a new bicipital groove through the greater tuberosity, rerouting the LHB into the new bicipital groove without LHB fixation, and side-to-side rotator cuff repair over the LHB. Our clinical experience indicates this technique can be easily and safely performed according to certain guidelines. We think the introduction of this technique will bring special insight to superior shoulder augmentation in case of IPSRCT.

Project description:The management of irreparable rotator cuff tears remains challenging. In patients in whom a complete repair cannot be obtained, a partial repair remains an option. The goal of a partial rotator cuff repair is to obtain a stable glenohumeral joint fulcrum by restoring the rotator cable complex. Traditionally, partial repair has been performed with independent reattachment(s) of the rotator cable complex with or without margin convergence medially. This Technical Note describes an alternative approach to a partial rotator cuff repair with a suture-based cable reconstruction.

Arthroscopic Superior Capsular Reconstruction Using "Sandwich" Patch Technique for Irreparable Rotator Cuff Tears.

Project description:The technique of superior capsular reconstruction (SCR) using fascia lata autograft, described by Mihata et al. in 2012, has been an acceptable and effective method for treating irreparable massive rotator cuff tears, especially in cases with severe fatty infiltration and tendon retraction. After the SCR procedure of Mihata et al., it was found that some graft failure occurred with thinning and elongation during the follow-up time, which was called graft "creep." To avoid graft creep and reduce graft failure rates after SCR, we created an arthroscopic SCR technique with a "sandwich" patch augmented with polyethylene terephthalate scaffold interspaced between 2 folded layers of fascia lata autograft.

Semitendinosus Allograft Cable Reconstruction Technique for Massive Irreparable Rotator Cuff Tears.

Project description:Massive irreparable rotator cuff tears are difficult to treat. Disruption of the rotator cable and joint capsule leads to altered glenohumeral joint mechanics, superior migration of the humeral head, and limited overhead function. A few graft options exist for reconstruction, with limited outcomes data. A newer technique using a hamstring allograft provides benefits compared with other graft reconstruction options. This Technical Note describes a rotator cable reconstruction using a V-shaped hamstring allograft for treatment of a massive, irreparable rotator cuff tear.

Arthroscopic Humeral Bone Tunnel-Based Tendon Grafting and Trapezius Transfer for Irreparable Posterior Superior Rotator Cuff Tear.

Project description:Irreparable posterior rotator cuff tears pose challenges to orthopaedic surgeons, especially when the medial remaining rotator cuff is not reusable. Trapezius transfer is biomechanically favorable due to the similar vector of the transferred muscle to the native posterior superior rotator cuff. Regarding combined tendon grafting in trapezius transfer, onto-surface tendon attachment to the humerus was reported in most previous reports. For better tendon-humeral head connection, we introduce an humeral bone tunnel-based tendon grafting technique. In this technique, we use the hamstring tendons and the anterior half of the peroneus tendon to make 3 grafts. The most critical steps of this technique are the proper creation of the humeral tunnels and graft implantation. We consider the introduction of this technique will shed light in the field of trapezius transfer for Irreparable posterior rotator cuff tears.

Anatomical Dermal Allograft and Autologous Biceps Long Head Superior Capsule Reconstruction for Irreparable Posterosuperior Rotator Cuff Tears.

Project description:Superior capsule reconstruction (SCR) can be performed using fascia lata, dermal allograft, and long head of the biceps tendon (LHBT). We present a Technical Note combining dermal allograft and autologous LHBT, reconstructing the superior capsule's actual anatomical thickness and augmenting with single-stranded LHBT. The glenoid side consists of intact LHBT insertion and is covered with dermal allograft. The lateral side comprises posteriorly transpositioned LHBT, dermal allograft, and repairable remnant cuff. First, 1 suture-based anchor is used to fix the biceps 5 to 8 mm posterior to the bicipital groove, and tenotomy is done distal to it, while the glenoid side of the biceps is preserved. Second, 2 suture-based anchors are used to fix the dermal allograft at the glenoid side by 1 double-pulley and 2 mattress sutures. Third, 2 SwiveLock anchors are used to fix allograft's lateral side by 2 reverse mattress sutures. The tension and coverage of the graft can be determined by the position of the SwiveLock anchors. In this way, fewer anchors are needed than the conventional dermal allograft SCR and larger footprint coverage can be achieved than LHBT SCR. A better spacer effect may be achieved by combining both biological grafts' thickness, mimicking the intact shoulder's true anatomy.

Rotator Cuff Reconstruction Using Fascia Lata Patch Autograft for the Nonrepairable Rotator Cuff Tear.

Project description:A large to massive rotator cuff tear is a common issue that lacks reliable options to return a patient's range of motion and function when conservative treatment has failed. With up to 96% of massive rotator cuff repairs failing within the first 6 months of repair, surgeons have been searching for a reliable treatment option for this difficult subset of patients. Surgical options for massive, retracted rotator cuff tears include re-establishing the counterforce coupling of the rotator cuff with techniques such as a partial repair or superior capsular reconstruction, preventing superior humeral migration as seen with balloon spacer implantation, and eliminating pain generators with techniques such as biceps tenotomy; however, these do not reconstitute dynamic cuff control. More recently, an acellular dermal allograft, as seen in superior capsular reconstruction, has been used to reconstruct the remaining rotator cuff. We describe a technique using a fascia lata autograft to reconstruct the rotator cuff in the setting of a massive cuff tear. This is of particular importance in centers that lack the funding or institutional approval to use acellular dermal allografts that have been popularized to date.
