Saccades are a key component for the assessment and diagnosis of Neuro-ophthalmological disorders. Traditionally, clinicians have been taught to use large amplitude saccades (LAS) to assess saccadic velocity (SV), when small amplitude saccades (SAS) may be more effective. Project description:Saccadic peak velocity increases in a stereotyped manner with the amplitude of eye movements. This relationship, known as the main sequence, has classically been considered to be fixed, although several recent studies have demonstrated that velocity can be modulated to some extent by external incentives. However, the ability to voluntarily control saccadic velocity and its association with motivation has yet to be investigated. Here, in three separate experimental paradigms, we measured the effects of incentivisation on saccadic velocity, reaction time and preparatory pupillary changes in 53 young healthy participants. In addition, the ability to voluntarily modulate saccadic velocity with and without incentivisation was assessed. Participants varied in their ability to increase and decrease the velocity of their saccades when instructed to do so. This effect correlated with motivation level across participants, and was further modulated by addition of monetary reward and avoidance of loss. The findings show that a degree of voluntary control of saccadic velocity is possible in some individuals, and that the ability to modulate peak velocity is associated with intrinsic levels of motivation.

Project description:ObjectiveTo determine whether saccadic velocity in the suppression head impulse paradigm (SHIMP) test is a reliable indicator of vestibular loss at the acute and at the chronic stage in patients suffering from different vestibular pathologies.MethodsThirty-five normal subjects and 57 patients suffering from different vestibular pathologies associated with unilateral vestibular loss (UVL) or bilateral vestibular loss (BVL) were tested in the SHIMPs paradigm. SHIMPs were performed by turning the head 10 times at high velocities to the left or right side, respectively. The patients were instructed to fixate on a red spot generated by a head-fixed laser projected on the wall. In this SHIMPs paradigm, healthy subjects made a large anti-compensatory saccade at the end of the head turn (a SHIMP saccade). The peak saccadic velocity, the percentage of the trials completed with saccades in 10 trials, and the latency of the saccades were quantified in each group. A video-head impulse test (v-HIT) was systematically performed in all of our subjects as well as a caloric test. The dizziness handicap inventory questionnaire was also given to chronic UVL and BVL patients.ResultsAt the acute stage after a complete UVL, patients had zero or a few anti-compensatory saccades for low velocity head turns toward the lesioned side. These saccades had lower velocity than the anti-compensatory saccades recorded during head rotation toward the intact side and/or compared with the saccades measured in control subjects. At the chronic stage, some of the patients recovered the ability to perform SHIMP saccades at each head turn toward the lesioned side, but very often these saccades were of significantly lower velocity. In BVL patients, no anti-compensatory saccades, or only significantly smaller ones, could be detected for head turns to both sides.ConclusionSHIMP is a specific and sensitive test to detect a complete horizontal canal loss at the acute stage. In addition, it reflects the ability of patients with moderate horizontal vestibulo-ocular reflex gain decrease to generate anti-compensatory saccades in the chronic stage. In association with v-HIT, it allows determination of the residual vestibular function and to detect anti-compensatory saccades.

Project description:Trans-saccadic memory consists of keeping track of objects' locations and features across saccades; pre-saccadic information is remembered and compared with post-saccadic information. It has been shown to have limited resources and involve attention with respect to the selection of objects and features. In support, a previous study showed that recognition of distinct post-saccadic objects in the visual scene is impaired when pre-saccadic objects are relevant and thus already encoded in memory (Poth, Herwig, Schneider, 2015). Here, we investigated the inverse (i.e. how the memory of pre-saccadic objects is affected by abrupt but irrelevant changes in the post-saccadic visual scene). We also modulated the amount of attention to the relevant pre-saccadic object by having participants either make a saccade to it or elsewhere and observed that pre-saccadic attentional facilitation affected how much post-saccadic changes disrupted trans-saccadic memory of pre-saccadic objects. Participants identified a flashed symbol (d, b, p, or q, among distracters), at one of six placeholders (figures "8") arranged in circle around fixation while planning a saccade to one of them. They reported the identity of the symbol after the saccade. We changed the post-saccadic scene in Experiment one by removing the entire scene, only the placeholder where the pre-saccadic symbol was presented, or all other placeholders except this one. We observed reduced identification performance when only the saccade-target placeholder disappeared after the saccade. In Experiment two, we changed one placeholder location (inward/outward shift or rotation re. saccade vector) after the saccade and observed that identification performance decreased with increased shift/rotation of the saccade-target placeholder. We conclude that pre-saccadic memory is disrupted by abrupt attention-capturing post-saccadic changes of visual scene, particularly when these changes involve the object prioritized by being the goal of a saccade. These findings support the notion that limited trans-saccadic memory resources are disrupted when object correspondence at saccadic goal is broken through removal or location change.

Project description:Understanding streamflow in montane watersheds on regional scales is often incomplete due to a lack of data for small-order streams that link precipitation and snowmelt processes to main stem discharge. This data deficiency is attributed to the prohibitive cost of conventional streamflow measurement methods and the remote location of many small streams. Expedient and low-cost streamflow measurement methods used by resource professionals or citizen scientists can provide scientifically useful solutions to this data deficiency. To this end, four current velocity measurement methods were evaluated in a laboratory flume: the surface float, rising body, velocity head rod, and rising air bubble methods. The methods were tested under a range of stream velocities, cross-sectional depths, and streambed substrates. The resulting measurements provide estimates of precision and bias of each method, as well as method-specific insight and calibration formulas. The mean values of the coefficient of variation, a measure of precision, were 10% for the surface float, 10% for the velocity head rod, 14% for the rising body, and 9% for the air bubble method. The values of scaled mean error, a measure of bias, were -8% for the surface float, -4% for the velocity head rod, -1% for the rising body, and 4% for the air bubble. The velocity head rod and surface float methods were the easiest methods to use, providing greater precision at large (> = 0.6 m/s) and small (<0.6 m/s) velocities, respectively. However, the reliance on a velocity ratio for each of these methods can generate inaccuracy in their results. The rising body method is more challenging to execute and of lower precision than the former two methods but provides low bias measurements. The rising air bubble method has a complex instrument assembly that is considered impractical for potential field user groups.

Project description:Stimuli that are briefly presented around the time of saccades are often perceived with spatiotemporal distortions. These distortions do not always have deleterious effects on the visibility and identification of a stimulus. Recent studies reported that when a stimulus is the target of an intended saccade, it is released from both masking and crowding. Here, we investigated pre-saccadic changes in single and crowded letter recognition performance in the absence (Experiment 1) and the presence (Experiment 2) of backward masks to determine the extent to which saccadic "uncrowding" and "unmasking" mechanisms are similar. Our results show that pre-saccadic improvements in letter recognition performance are mostly due to the presence of masks and/or stimulus transients which occur after the target is presented. More importantly, we did not find any decrease in crowding strength before impending saccades. A simplified version of a dual-channel neural model, originally proposed to explain masking phenomena, with several saccadic add-on mechanisms, could account for our results in Experiment 1. However, this model falls short in explaining how saccades drastically reduced the effect of backward masking (Experiment 2). The addition of a remapping mechanism that alters the relative spatial positions of stimuli was needed to fully account for the improvements observed when backward masks followed the letter stimuli. Taken together, our results (i) are inconsistent with saccadic uncrowding, (ii) strongly support saccadic unmasking, and (iii) suggest that pre-saccadic letter recognition is modulated by multiple perisaccadic mechanisms with different time courses.

Project description:Accurate and efficient self-assessment is a critical skill for medical students to develop as part of their professional development. Along with clinical training reform at Fukushima Medical University, rubric-based student self-assessment and teacher assessment of students' performance using our proposed assessment tool, which includes several aspects of clinical skills and abilities, was initiated to improve the clinical clerkship process. To investigate how students identified their weaknesses and strengths, we analyzed the results of 119 fourth-year medical students' self-assessment and corresponding teacher assessment. Our study revealed substantial consistency between student self-assessment and teacher assessment, despite some overestimation and underestimation in student self-assessments. Students who incorrectly assess themselves require a variety of feedback to increase their self-efficacy and self-confidence, as well as to identify their weaknesses.

Project description:BackgroundGoal-directed ultrasound protocols have been developed to facilitate efficiency, throughput, and patient care. Hands-on instruction and training workshops have been shown to positively impact ultrasound training.ObjectivesWe describe a novel undifferentiated chest pain goal-directed ultrasound algorithm-focused education workshop for the purpose of enhancing emergency medicine resident training in ultrasound milestones competencies.MethodsThis was a cross-sectional study performed at an academic medical center. A novel goal-directed ultrasound algorithm was developed and implemented as a model for teaching and learning the sonographic approach to a patient with undifferentiated chest pain. This algorithm was incorporated into all components of the 1-day workshop: asynchronous learning, didactic lecture, case-based learning, and hands-on stations. Performance comparisons were made between postgraduate year (PGY) levels.ResultsA total of 38 of the 40 (95%) residents who attended the event participated in the chest pain objective standardized clinical exam, and 26 of the 40 (65%) completed the entire questionnaire. The average number of ultrasounds performed by resident class year at the time of our study was as follows: 19 (standard deviation [SD]=19) PGY-1, 238 (SD=37) PGY-2, and 289 (SD=73) PGY-3. Performance on the knowledge-based questions improved between PGY-1 and PGY-3. The application of the novel algorithm was noted to be more prevalent among the PGY-1 class.ConclusionThe 1-day algorithm-based ultrasound educational workshop was an engaging learning technique at our institution.

Project description:We measured saccadic latencies in a large sample (total n = 459) of individuals with amblyopia or risk factors for amblyopia, e.g., strabismus or anisometropia, and normal control subjects. We presented an easily visible target randomly to the left or right, 3.5° from fixation. The interocular difference in saccadic latency is highly correlated with the interocular difference in LogMAR (Snellen) acuity-as the acuity difference increases, so does the latency difference. Strabismic and strabismic-anisometropic amblyopes have, on average, a larger difference between their eyes in LogMAR acuity than anisometropic amblyopes and thus their interocular latency difference is, on average, significantly larger than anisometropic amblyopes. Despite its relation to LogMAR acuity, the longer latency in strabismic amblyopes cannot be attributed either to poor resolution or to reduced contrast sensitivity, because their interocular differences in grating acuity and in contrast sensitivity are roughly the same as for anisometropic amblyopes. The correlation between LogMAR acuity and saccadic latency arises because of the confluence of two separable effects in the strabismic amblyopic eye-poor letter recognition impairs LogMAR acuity while an intrinsic sluggishness delays reaction time. We speculate that the frequent microsaccades and the accompanying attentional shifts, made while strabismic amblyopes struggle to maintain fixation with their amblyopic eyes, result in all types of reactions being irreducibly delayed.

Project description:The fundamental role of the visual system is to guide behavior in natural environments. To optimize information transmission, many animals have evolved a non-homogeneous retina and serially sample visual scenes by saccadic eye movements. Such eye movements, however, introduce high-speed retinal motion and decouple external and internal reference frames. Until now, these processes have only been studied with unnatural stimuli, eye movement behavior, and tasks. These experiments confound retinotopic and geotopic coordinate systems and may probe a non-representative functional range. Here we develop a real-time, gaze-contingent display with precise spatiotemporal control over high-definition natural movies. In an active condition, human observers freely watched nature documentaries and indicated the location of periodic narrow-band contrast increments relative to their gaze position. In a passive condition under central fixation, the same retinal input was replayed to each observer by updating the video's screen position. Comparison of visual sensitivity between conditions revealed three mechanisms that the visual system has adapted to compensate for peri-saccadic vision changes. Under natural conditions we show that reduced visual sensitivity during eye movements can be explained simply by the high retinal speed during a saccade without recourse to an extra-retinal mechanism of active suppression; we give evidence for enhanced sensitivity immediately after an eye movement indicative of visual receptive fields remapping in anticipation of forthcoming spatial structure; and we demonstrate that perceptual decisions can be made in world rather than retinal coordinates.

Project description:IntroductionBedside cardiac assessment (BCA) is deficient across a spectrum of noncardiology trainees. Learners not taught BCA well may become instructors who do not teach well, creating a self-perpetuating problem. To improve BCA teaching and learning, we developed a high-quality, patient-centered curriculum for medicine clerkship students that could be flexibly implemented and accessible to other health professions learners.MethodsWith a constructivist perspective, we aligned learning goals, activities, and assessments. The curriculum used a "listen before you auscultate" framework, capturing patient history as context for a six-step, systematic approach. In the flipped classroom, short videos and practice questions preceded two 1-hour class activities that integrated diagnostic reasoning, pathophysiology, physical diagnosis, and reflection. Activities included case discussions, jugular venous pressure evaluation, heart sound competitions, and simulated conversations with patients. Two hundred sixty-eight students at four US and international medical schools participated. We incorporated feedback, performed thematic analysis, and assessed learners' confidence and knowledge.ResultsLow posttest data capture limited quantitative results. Students reported increased confidence in BCA ability. Knowledge increased in both BCA and control groups. Thematic analysis suggested instructional design strategies were effective and peer encounters, skills practice, and encounters with educators were meaningful.DiscussionThe curriculum supported active learning of day-to-day clinical competencies and promoted professional identity formation alongside BCA ability. Feedback and increased confidence on the late-clerkship posttest suggested durable learning. We recommend approaches to confirm this and other elements of knowledge, skill acquisition, or behaviors and are surveying impacts on professional identity formation-related constructs.
