Project description:Introduction Congenital cholesteatomas of the temporal bone are epidermoid cysts of embryologic origin that result in progressive desquamation and trapping of squamous epithelium behind an intact tympanic membrane. They are benign, slowly progressive lesions that can be found in various areas of the temporal bone. We report a case of a patient with a massive cholesteatoma first detected at the age of 64 years, causing significant destruction of the mastoid and petrous temporal bones, and adjacent occipital bone. Methods We reviewed the literature and a case report of a patient seen in our institution recently. The Medline database was used to search multiple terms including "congenital" and "cholesteatoma." Results The patient's congenital cholesteatoma was detected incidentally on a computed tomography scan when the patient's only symptoms were unilateral conductive hearing loss with a family history of hearing loss. It was subsequently successfully operated on with minimal postoperative complications. Conclusions Congenital cholesteatomas of mastoid origin can often exist for many years in a subclinical state and develop into a massive size before causing symptoms. A high index of suspicion is necessary to detect congenital cholesteatomas in patients with unilateral conductive hearing loss who are otherwise asymptomatic and have a normal tympanic membrane.

A 45-year-old Female with an Atypical Presentation of Pharyngitis.

Project description:IntroductionEmergency physicians are trained to treat a variety of ailments in the emergency department (ED), some of which are emergent, while others are not. A common complaint seen in the ED is a sore throat. While most sore throats are easily diagnosed and treated, less common causes are often not considered in the differential diagnoses. Therefore, the purpose of this case study was to present an atypical case of sore throat and discuss differential diagnoses.Case presentationThe patient was a 45-year-old female who presented to the ED with a three-day history of sore throat that was exacerbated by eating and drinking. The patient was not on any prescription medications, but tried over-the-counter medications for the sore throat without any improvement in symptoms. Review of systems was positive for sore throat, fevers, and chills. Physical examination of her oropharynx revealed mildly dry mucous membranes with confluent plaques and white patchy ulcerative appearance involving the tongue, tonsils, hard palate, and soft palate. Rapid streptococcal antigen, mononucleosis spot test, and KOH test were performed and found to be negative.DiscussionAfter initial testing was negative, a follow-up complete blood count with differential and complete metabolic profile were ordered. The patient was found to have decreased lymphocytes and platelets. Based upon those results, a diagnosis was made in the ED, the patient was started on medication, and further laboratory workup was ordered to confirm the diagnosis. ED providers should consider non-infectious as well as infectious causes for a sore throat, as this might lead to a diagnosis of an underlying condition.

A Case Report on the Atypical Presentation of Hypertrophic Cardiomyopathy (HOCM) in a 19-Year-Old Female.

Project description:Atypical hypertrophic cardiomyopathy (HOCM) is a relatively rare genetic disorder that can affect the left ventricular system. HOCM can lead to various cardiac issues such as sudden cardiac death (SCD). We report a case of a 19-year-old female who was referred to a cardiology clinic after presenting with bi-ventricular hypertrophy on an echocardiogram (ECHO). Results from screening tests for infiltrative diseases and an iron panel came negative. The patient was asymptomatic, with no functional limitations and no family history of any cardiac disease or sudden death. In conclusion, HOCM can present with an atypical pattern, such as biventricular hypertrophy, and has been linked to SCD; therefore, it is important to be aware of this condition and take the necessary precautions to prevent it.

Chronic ischaemic colitis in a 48 year old man: an unusual presentation.

Project description:Ischaemic colitis typically presents with an acute abdominal pain and distension with bloody diarrhoea. However, this can vary and the condition is frequently missed or misdiagnosed, especially if the patient presents with chronic symptoms. Herein, we report a case of chronic presentation of ischaemic colitis in a 48-year-old man with a history of myocardial infarction. This report highlights the importance of having ischaemic colitis as a differential diagnosis whenever a patient with a history of arteriosclerosis presents with atypical chronic abdominal pain.
