Presence of problematic and disordered gambling in older age and validation of the South Oaks Gambling Scale.

ABSTRACT: The use of instruments originally developed for measuring gambling activity in younger populations may not be appropriate in older age individuals. The aim of this study was to examine the presence of problematic and disordered gambling in seniors aged 50 or over, and study the reliability and validity properties of the SOGS (a screening measure to identify gambling related problems). Two independent samples were recruited: a clinical group of n = 47 patients seeking treatment at a Pathological Gambling Outpatient Unit, and a population-based group of n = 361 participants recruited from the same geographical area. Confirmatory factor analysis verified the bifactor structure for the SOGS with two correlated underlying dimensions [measuring the impact of gambling on the self primarily (Cronbach's alpha ? = 0.87) or on both the self and others also (? = 0.82)], and a global dimension of gambling severity (also with excellent internal consistency, ? = 0.90). The SOG obtained excellent accuracy/validity for identifying gambling severity based on the DSM-5 criteria (area under the ROC curve AUC = 0.97 for discriminating disordered gambling and AUC = 0.91 for discriminating problem gambling), and good convergent validity with external measures of gambling (Pearson's correlation R = 0.91 with the total number of DSM-5 criteria for gambling disorder, and R = 0.55 with the debts accumulated due to gambling) and psychopathology (R = 0.50, 0.43 and 0.44 with the SCL-90R depression, anxiety and GSI scales). The optimal cutoff point for identifying gambling disorder was 4 (sensitivity Se = 92.3% and specificity Sp = 98.6%) and 2 for identifying problem gambling (Se = 78.8% and Sp = 96.7%). This study provides empirical support for the reliability and validity of the SOGS for assessing problem gambling in elders, and identifies two specific factors that could help both research and clinical decision-making, based on the severity and consequences of the gambling activity.

Project description:Over the past decade, the world population has experienced rapid and widespread adoption of smartphones due to their usefulness and convenience. However, researchers have identified a range of adverse behaviours associated with the adoption of smartphones and their higher use. These behaviours are collectively described as Problematic Smartphone Use and Dependence (PSUD). Despite growing research, the underlying processes and drivers leading to these behaviours are inadequately understood. This can partly be attributed to the absence of developed statistical tools and measures that allow researchers to build a comprehensive conceptual understanding of PSUD. To address this issue, this study proposes and evaluates a validated extension to the Problematic Use of Mobile Phones (PUMP) scale. The extension of this tool incorporates factors associated with substance dependence outlined in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), including additional items to measure PSUD accurately, referred to as the modified problematic use of mobile phones (MPUMP) scale. The newly developed tool was used in a cross-sectional online survey during September and October 2022, drawing on 1018 adult Australian participants. Principal Component Analysis (PCA) and Exploratory Factor Analysis (EFA) were conducted to derive the underlying factors. The EFA revealed two distinct factors: Distraction and Dysregulation. Both factors exhibited high internal consistency, with Cronbach's Alpha coefficients of 0.92 and 0.86, respectively. A one-way analysis of variance (ANOVA) revealed significant variations inthe identified factors' mean scores across different socio-demographic characteristics. The study provides evidence that the MPUMP scale is a validated and reliable measure for accurately assessing PSUD. The study findings offer novel insights into the psychosocial and physical aspects of PSUD, providing a foundation for exploring the causes and potential interventions for PSUD.

Project description:AimsTo measure the association between problem gambling severity and 19 different gambling activities among emerging adults (aged 16-26).DesignAn online non-probability longitudinal survey collecting data in two waves: wave 1, July/August 2019; wave 2, July/October 2020.SettingGreat Britain PARTICIPANTS: A total of 2080 young adults participating in both waves.MeasurementsProblem gambling scores were collected using the Problem Gambling Severity Index (PGSI). Binary variables recorded past year participation in 19 different gambling forms, ranging from lotteries to online casino and gambling-like practices within digital games (e.g. loot box purchase, skin betting). Controls included socio-demographic/economic characteristics, the Eysenck Impulsivity Scale and the number of gambling activities undertaken.FindingsZero inflated negative binomial model lacked evidence of an effect between past year participation in any individual activities and subsequent PGSI scores. However, negative binomial random effects models for current gamblers (n = 497) showed that skin betting (incidence-rate ratio [IRR] = 2.32; 95% CI = 1.69-3.19), fixed odd betting terminals (IRR = 2.21, 95% CI = 1.61-3.05), slot/fruit machines (IRR = 1.43, 95% CI = 1.07-1.91), online betting on horse/dog races (IRR = 1.53, 95% CI = 1.17-2.00) and online betting on non-sports events (IRR = 1.44, 95% CI = 1.11-1.89) were associated with increased PGSI scores. Online casino gambling had a significant interaction by wave; the impact of online casino betting in wave 2 on PGSI scores increased by a factor of 1.61.ConclusionsPast year participation of young adults (aged 16-26) in certain forms of gambling does not appear to be associated with future Problem Gambling Severity Index scores. Among young adults who are current gamblers, past year participation in certain land-based (e.g. electronic gaming machines) and online forms (e.g. skin betting) of gambling appears to be strongly associated with elevated Problem Gambling Severity Index scores.
