Project description:In this Update, we document the circulation of Coronavirus Disease 2019 (COVID-19) in Florida, USA, since February 2020. On 8th March 2020, a confirmed case of COVID-19 was notified to the Health Authority of Central Tuscany, Florence, Italy. The patient developed symptoms on 3rd March while staying in Miami where he arrived on 12th February. The case returned to Italy on 6th March and was admitted to a local hospital of Florence on 7th March with fever (38.2°C/100.4°F), cough and breathing difficulties. First COVID-19 cases in Florida where confirmed by the Centers for Disease Control and Prevention on 2nd March as imported cases. Therefore, this event documents COVID-19 circulation in Florida at least since mid-February 2020.

Project description:We quantified the effects of adherence to various non-pharmaceutical interventions (NPIs) on the seasonal influenza epidemic dynamics in Japan during 2020. The total monthly number of seasonal influenza cases per sentinel site (seasonal influenza activity) reported to the National Epidemiological Surveillance of Infectious Diseases and alternative NPI indicators (retail sales of hand hygiene products and number of airline passenger arrivals) from 2014-2020 were collected. The average number of monthly seasonal influenza cases in 2020 had decreased by approximately 66.0% (p < 0.001) compared to those in the preceding six years. An increase in retail sales of hand hygiene products of ¥1 billion over a 3-month period led to a 15.5% (95% confidence interval [CI]: 10.9-20.0%; p < 0.001) reduction in seasonal influenza activity. An increase in the average of one million domestic and international airline passenger arrivals had a significant association with seasonal influenza activity by 11.6% at lag 0-2 months (95% CI: 6.70-16.5%; p < 0.001) and 30.9% at lag 0-2 months (95% CI: 20.9-40.9%; p < 0.001). NPI adherence was associated with decreased seasonal influenza activity during the COVID-19 pandemic in Japan, which has crucial implications for planning public health interventions to minimize the health consequences of adverse seasonal influenza epidemics.

Project description:During the 2019-20 influenza season, influenza-like illness (ILI)* activity first exceeded the national baseline during the week ending November 9, 2019, signaling the earliest start to the influenza season since the 2009 influenza A(H1N1) pandemic. Activity remains elevated as of mid-February 2020. In the United States, annual vaccination against seasonal influenza is recommended for all persons aged ?6 months (1). During each influenza season, CDC estimates seasonal influenza vaccine effectiveness in preventing laboratory-confirmed influenza associated with medically attended acute respiratory illness (ARI). This interim report used data from 4,112 children and adults enrolled in the U.S. Influenza Vaccine Effectiveness Network (U.S. Flu VE Network) during October 23, 2019-January 25, 2020. Overall, vaccine effectiveness (VE) against any influenza virus associated with medically attended ARI was 45% (95% confidence interval [CI] = 36%-53%). VE was estimated to be 50% (95% CI = 39%-59%) against influenza B/Victoria viruses and 37% (95% CI = 19%-52%) against influenza A(H1N1)pdm09, indicating that vaccine has significantly reduced medical visits associated with influenza so far this season. Notably, vaccination provided substantial protection (VE = 55%; 95% CI = 42%-65%) among children and adolescents aged 6 months-17 years. Interim VE estimates are consistent with those from previous seasons, ranging from 40%-60% when influenza vaccines were antigenically matched to circulating viruses. CDC recommends that health care providers continue to administer influenza vaccine to persons aged ?6 months because influenza activity is ongoing, and the vaccine can still prevent illness, hospitalization, and death associated with currently circulating influenza viruses as well as other influenza viruses that might circulate later in the season.

Project description:BackgroundTimely monitoring of COVID-19 impact on mortality is critical for rapid risk assessment and public health action.AimBuilding upon well-established models to estimate influenza-related mortality, we propose a new statistical Attributable Mortality Model (AttMOMO), which estimates mortality attributable to one or more pathogens simultaneously (e.g. SARS-CoV-2 and seasonal influenza viruses), while adjusting for seasonality and excess temperatures.MethodsData from Nationwide Danish registers from 2014-week(W)W27 to 2020-W22 were used to exemplify utilities of the model, and to estimate COVID-19 and influenza attributable mortality from 2019-W40 to 2020-W20.ResultsSARS-CoV-2 was registered in Denmark from 2020-W09. Mortality attributable to COVID-19 in Denmark increased steeply, and peaked in 2020-W14. As preventive measures and national lockdown were implemented from 2020-W12, the attributable mortality started declining within a few weeks. Mortality attributable to COVID-19 from 2020-W09 to 2020-W20 was estimated to 16.2 (95% confidence interval (CI): 12.0 to 20.4) per 100,000 person-years. The 2019/20 influenza season was mild with few deaths attributable to influenza, 3.2 (95% CI: 1.1 to 5.4) per 100,000 person-years.ConclusionAttMOMO estimates mortality attributable to several pathogens simultaneously, providing a fuller picture of mortality by COVID-19 during the pandemic in the context of other seasonal diseases and mortality patterns. Using Danish data, we show that the model accurately estimates mortality attributable to COVID-19 and influenza, respectively. We propose using standardised indicators for pathogen circulation in the population, to make estimates comparable between countries and applicable for timely monitoring.

Project description:Background. In mid-2020, there was significant concern that the overlapping 2020-2021 influenza season and COVID-19 pandemic would overwhelm already stressed health care systems in the Northern Hemisphere, particularly if influenza immunization rates were low. Methods. Using a mathematical susceptible-exposed-infected-recovered (SEIR) compartmental model incorporating the age-specific viral transmission rates and disease severity of Austin, Texas, a large metropolitan region, we projected the incidence and health care burden for both COVID-19 and influenza across observed levels of SARS-CoV-2 transmission and influenza immunization rates for the 2020-2021 season. We then retrospectively compared scenario projections made in August 2020 with observed trends through June 2021. Results. Across all scenarios, we projected that the COVID-19 burden would dwarf that of influenza. In all but our lowest transmission scenarios, intensive care units were overwhelmed by COVID-19 patients, with the levels of influenza immunization having little impact on health care capacity needs. Consistent with our projections, sustained nonpharmaceutical interventions (NPIs) in Austin prevented COVID-19 from overwhelming health care systems and almost completely suppressed influenza during the 2020-2021 respiratory virus season. Limitations. The model assumed no cross-immunity between SARS-CoV-2 and influenza, which might reduce the burden or slow the transmission of 1 or both viruses. Conclusion. Before the widespread rollout of the SARS-CoV-2 vaccine, COVID-19 was projected to cause an order of magnitude more hospitalizations than seasonal influenza because of its higher transmissibility and severity. Consistent with predictions assuming strong NPIs, COVID-19 strained but did not overwhelm local health care systems in Austin, while the influenza burden was negligible. Implications. Nonspecific NPI efforts can dramatically reduce seasonal influenza burden and preserve health care capacity during respiratory virus season.HighlightsAs the COVID-19 pandemic threatened lives worldwide, the Northern Hemisphere braced for a potential "twindemic" of seasonal influenza and COVID-19.Using a validated mathematical model of influenza and SARS-CoV-2 co-circulation in a large US city, we projected the impact of COVID-19-driven nonpharmaceutical interventions combined with influenza vaccination on health care capacity during the 2020-2021 respiratory virus season.We describe analyses conducted during summer 2020 to help US cities prepare for the 2020-2021 influenza season and provide a retrospective evaluation of the initial projections.

Project description:Patients with influenza and SARS-CoV2/Coronavirus disease 2019 (COVID-19) infections have a different clinical course and outcomes. We developed and validated a supervised machine learning pipeline to distinguish the two viral infections using the available vital signs and demographic dataset from the first hospital/emergency room encounters of 3883 patients who had confirmed diagnoses of influenza A/B, COVID-19 or negative laboratory test results. The models were able to achieve an area under the receiver operating characteristic curve (ROC AUC) of at least 97% using our multiclass classifier. The predictive models were externally validated on 15,697 encounters in 3125 patients available on TrinetX database that contains patient-level data from different healthcare organizations. The influenza vs COVID-19-positive model had an AUC of 98.8%, and 92.8% on the internal and external test sets, respectively. Our study illustrates the potentials of machine-learning models for accurately distinguishing the two viral infections. The code is made available at https://github.com/ynaveena/COVID-19-vs-Influenza and may have utility as a frontline diagnostic tool to aid healthcare workers in triaging patients once the two viral infections start cocirculating in the communities.

Project description:ObjectiveThe COVID-19 outbreak in Wuhan, Hubei, China, followed the seasonal influenza epidemic. Since some COVID-19 cases may have been misdiagnosed as seasonal influenza in January 2020, before testing capacity was adequate, it is relevant to study the proportions of influenza and COVID-19 cases among influenza-like-illness (ILI) patients and their temporal pattern.ResultsThis study analyzed the record of the ILI patients with a recent travel history to Wuhan who arrived in Hong Kong between 31 December 2019 and 21 January 2020. We found that the proportion of COVID-19 cases among the total ILI patients is much smaller than a study among ILI in Wuhan. This difference in proportion could be due to the difference in sampling. We argue that it is essential to consider both samples when inferring the number of COVID-19 cases from ILI patients.

Project description:IntroductionDuring the 2019 novel coronavirus infectious disease (COVID-19) pandemic in 2020, limited data from several countries suggested reduced seasonal influenza viruses' circulation. This was due to community mitigation measures implemented to control the pandemic of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We used sentinel surveillance data to identify changes in the 2020 influenza season compared with previous seasons in Bangladesh.MethodsWe used hospital-based influenza surveillance (HBIS) data of Bangladesh that are generated year-round and are population-representative severe acute respiratory infection (SARI) data for all age groups from seven public and two private tertiary care level hospitals data from 2016 to 2019. We applied the moving epidemic method (MEM) by using R language (v4.0.3), and MEM web applications (v2.14) on influenza-positive rates of SARI cases collected weekly to estimate an average seasonal influenza curve and establish epidemic thresholds.ResultsThe 2016-2019 average season started on epi week 18 (95% CI: 15-25) and lasted 12.5 weeks (95% CI: 12-14 weeks) until week 30.5. The 2020 influenza season started on epi week 36 and ended at epi week 41, lasting for only five weeks. Therefore, influenza epidemic started 18 weeks later, was 7.5 weeks shorter, and was less intense than the average epidemic of the four previous years. The 2020 influenza season started on the same week when COVID-19 control measures were halted, and 13 weeks after the measures were relaxed.ConclusionOur findings suggest that seasonal influenza circulation in Bangladesh was delayed and less intense in 2020 than in previous years. Community mitigation measures may have contributed to this reduction of seasonal influenza transmission. These findings contribute to a limited but growing body of evidence that influenza seasons were altered globally in 2020.

Project description:OBJECTIVES:Infection due to the 2019 novel coronavirus disease (COVID-19) is dramatically widespread around the world. The COVID-19 pandemic could increase public concern to prevent infectious disease. The present study aimed to assess the relationship between the COVID-19 epidemic and the potential decrease in seasonal influenza cases. METHODS:This study was performed to show trends in seasonal influenza cases from the 2014-2015 season to the 2019-2020 season in 11 countries and regions, and evaluate whether the trends in the 2019-2020 season were different before and after the COVID-19 pandemic compared to previous seasons using a quasi-experimental difference-in-difference design. RESULTS:In East Asia, the number of seasonal influenza cases in the 2019-20 season was lower after the COVID-19 transmission compared to previous years. However, this was not the case in American countries or in European countries. CONCLUSION:The COVID-19 epidemic might have altered health behaviors, resulting in an unexpected reduction of seasonal influenza cases.
