Transitive Sequencing Medical Records for Mining Predictive and Interpretable Temporal Representations.

ABSTRACT: Electronic health records (EHRs) contain important temporal information about the progression of disease and treatment outcomes. This paper proposes a transitive sequencing approach for constructing temporal representations from EHR observations for downstream machine learning. Using clinical data from a cohort of patients with congestive heart failure, we mined temporal representations by transitive sequencing of EHR medication and diagnosis records for classification and prediction tasks. We compared the classification and prediction performances of the transitive sequential representations (bag-of-sequences approach) with the conventional approach of using aggregated vectors of EHR data (aggregated vector representation) across different classifiers. We found that the transitive sequential representations are better phenotype "differentiators" and predictors than the "atemporal" EHR records. Our results also demonstrated that data representations obtained from transitive sequencing of EHR observations can present novel insights about the progression of the disease that are difficult to discern when clinical data are treated independently of the patient's history.

Project description:Humans constantly assess the appearance of materials to plan actions, such as stepping on icy roads without slipping. Visual inference of materials is important but challenging because a given material can appear dramatically different in various scenes. This problem especially stands out for translucent materials, whose appearance strongly depends on lighting, geometry, and viewpoint. Despite this, humans can still distinguish between different materials, and it remains unsolved how to systematically discover visual features pertinent to material inference from natural images. Here, we develop an unsupervised style-based image generation model to identify perceptually relevant dimensions for translucent material appearances from photographs. We find our model, with its layer-wise latent representation, can synthesize images of diverse and realistic materials. Importantly, without supervision, human-understandable scene attributes, including the object's shape, material, and body color, spontaneously emerge in the model's layer-wise latent space in a scale-specific manner. By embedding an image into the learned latent space, we can manipulate specific layers' latent code to modify the appearance of the object in the image. Specifically, we find that manipulation on the early-layers (coarse spatial scale) transforms the object's shape, while manipulation on the later-layers (fine spatial scale) modifies its body color. The middle-layers of the latent space selectively encode translucency features and manipulation of such layers coherently modifies the translucency appearance, without changing the object's shape or body color. Moreover, we find the middle-layers of the latent space can successfully predict human translucency ratings, suggesting that translucent impressions are established in mid-to-low spatial scale features. This layer-wise latent representation allows us to systematically discover perceptually relevant image features for human translucency perception. Together, our findings reveal that learning the scale-specific statistical structure of natural images might be crucial for humans to efficiently represent material properties across contexts.

Project description:Determining the discrepancy between chronological and physiological age of patients is central to preventative and personalized care. Electronic medical records (EMR) provide rich information about the patient physiological state, but it is unclear whether such information can be predictive of chronological age. Here we present a deep learning model that uses vital signs and lab tests contained within the EMR of Mount Sinai Health System (MSHS) to predict chronological age. The model is trained on 377,686 EMR from patients of ages 18-85 years old. The discrepancy between the predicted and real chronological age is then used as a proxy to estimate physiological age. Overall, the model can predict the chronological age of patients with a standard deviation error of ∼7 years. The ages of the youngest and oldest patients were more accurately predicted, while patients of ages ranging between 40 and 60 years were the least accurately predicted. Patients with the largest discrepancy between their physiological and chronological age were further inspected. The patients predicted to be significantly older than their chronological age have higher systolic blood pressure, higher cholesterol, damaged liver, and anemia. In contrast, patients predicted to be younger than their chronological age have lower blood pressure and shorter stature among other indicators; both groups display lower weight than the population average. Using information from ∼10,000 patients from the entire cohort who have been also profiled with SNP arrays, genome-wide association study (GWAS) uncovers several novel genetic variants associated with aging. In particular, significant variants were mapped to genes known to be associated with inflammation, hypertension, lipid metabolism, height, and increased lifespan in mice. Several genes with missense mutations were identified as novel candidate aging genes. In conclusion, we demonstrate how EMR data can be used to assess overall health via a scale that is based on deviation from the patient's predicted chronological age.
