Case report of anterior ST-elevation myocardial infarction in a patient with coronavirus disease-2019.

ABSTRACT:

Background

Coronavirus disease-2019 (COVID-19) is an infectious disease appeared in China in December 2019 and, since then, has spread worldwide at a rapid pace.

Case summary

A patient with COVID-19 was hospitalized in our institution for a diabetic foot ulcer and presented afterwards a pulmonary oedema and concomitant anterior ST-segment elevation myocardial infarction. We report here on the initial presentation, coronary care and intervention, and clinical course of this patient.

Discussion

Emergent percutaneous coronary intervention is feasible and safe in COVID-19 patients but requires a multidisciplinary effort involving caregivers from infectious disease, intensive care, and cardiology teams.

Project description:BackgroundDuchenne's muscular dystrophy (DMD) is an X-linked muscular disease which is caused by the absence of dystrophin. This results in the death of muscle cells and cardiomyocytes and consequent substitution by fat and fibrous tissue. The clinical translation of this is muscle weakness and cardiomyopathy. We report on the case of a young patient with dilated cardiomyopathy on a background of DMD who developed ST-elevation myocardial infarction (STEMI).Case summaryA 19-year-old male patient with DMD, known dilated cardiomyopathy, and no risk factors for ischaemic heart disease presented with central crushing chest pain. His electrocardiogram revealed anterior ST elevation. His angiogram revealed distally occluded left anterior descending and second diagonal branch with no evidence of underlying coronary artery disease. He was treated with balloon angioplasty. An echocardiogram raised the suspicion of a left ventricular thrombus, and the mechanism of STEMI was felt to be embolism from the left ventricular thrombus on a background of dilated cardiomyopathy in the context of DMD. The patient was treated with anticoagulants (warfarin). On a repeat echocardiogram a few months later, the thrombus had resolved. At 3 years of follow-up, the patient did not present any more embolic events.DiscussionTo our knowledge, this is the first case of STEMI secondary to thrombotic coronary occlusion that has been described in a patient with DMD. This case highlights an unusual complication of DMD. Based on this case, we discuss the dilemmas in the management and follow-up of this complex patient population.

Foreign body associated non-ST elevation myocardial infarction: a case report.

Project description:The incidence of guiding catheter tip fracture, is quite rare during percutaneous coronary interventions (PCI). Retained broken catheter tip in coronary artery and circulation can result in serious complications such as thrombosis, embolism, acute occlusion and myocardial infarction, arrhythmias and sepsis. It is mandatory to remove the fractured catheter as soon as possible to avoid these complications. Transcatheter removal is the treatment of choice. If percutaneous removal is failed, surgical intervention is advised.

Intravascular ultrasound-guided dissection recanalization in a patient with ST-segment elevation myocardial infarction: a case report.

Project description:BackgroundIn primary percutaneous coronary intervention (PCI) for acute myocardial infarction, we occasionally experience challenging cases where conventional guidewires cannot pass through the lesion. In such cases, if the use of a tapered guidewire or polymer jacket guidewire is also unsuccessful, coronary artery bypass surgery becomes inevitable. Therefore, other methods to enable revascularization in a reliable and timely manner are desirable.Case summaryWe present the first case of intravenous ultrasound (IVUS)-guided tip detection (TD)-antegrade dissection re-entry (ADR) in a 73-year-old man who suffered ST-segment elevation myocardial infarction (STEMI). The patient had a total thrombotic occlusion of the right coronary artery and stenotic lesion of the left anterior descending artery. Primary PCI was unsuccessful and IVUS-guided rewiring using a chronic total occlusion (CTO) wire failed due to thrombus attenuation. However, IVUS imaging revealed the presence of intimal and subintimal space, which led us to perform IVUS-guided TD-ADR using Conquest Pro 12 ST (Asahi Intecc). Using the TD method, we were successful in swiftly puncturing the true lumen wall, and a stent was implanted following successful re-entry. Final angiography showed the establishment of Thrombolysis in Myocardial Infraction-3 flow and resolution of ST-segment elevation.DiscussionIVUS-guided TD enables accurate puncture in an ADR procedure, enabling successful recanalization in a relatively short time. Thus, IVUS-guided TD-ADR is a reliable option for revascularization in STEMI cases wherein the guidewire fails to pass the occlusion using conventional techniques.

ST elevation myocardial infarction.

Project description:ST segment elevation myocardial infarction remains a significant contributor to morbidity and mortality worldwide, despite a declining incidence and better survival rates. It usually results from thrombotic occlusion of a coronary artery at the site of a ruptured or eroded plaque. Diagnosis is based on characteristic symptoms and electrocardiogram changes, and confirmed subsequently by raised cardiac enzymes. Prognosis is dependent on the size of the infarct, presence of collaterals and speed with which the occluded artery is reopened. Mechanical reperfusion by primary percutaneous coronary intervention is superior to fibrinolytic therapy if delivered by an experienced team in a timely fashion. Post-reperfusion care includes monitoring for complications, evaluation of left ventricular function, secondary preventive therapy and cardiac rehabilitation.

ST-segment elevation myocardial infarction due to septic coronary embolism: a case report.

Project description:BackgroundCoronary artery embolism is an infrequent cause of type 2 myocardial infarction which can be due to arterial thromboembolism or septic embolism. While systemic embolization is one of the most acknowledged and threatened complications of infective endocarditis, coronary localization of the emboli causing acute myocardial infarction is exceedingly rare occurring in <1% of cases.Case summaryA 52-year-old man with a history of Bentall procedure and redo aortic valve replacement due to prosthetic degeneration (11 years prior to the current presentation) presented to the emergency department with high-grade fever and myalgias. Shortly after his arrival, he experienced typical chest pain and an electrocardiogram demonstrated signs of inferior ST-elevation myocardial infarction: coronary angiography showed a lesion of presumed embolic origin at the level of the mid-distal circumflex coronary artery which was treated with embolectomy. Transthoracic and transoesophageal echocardiography highlighted the presence of a periaortic abscess. The final diagnosis of infective endocarditis as the cause of septic coronary artery embolization was confirmed with a Positron Emission Tomography-Computed Tomography (PET-CT) exam and by the growth of Staphylococcus lugdunensis on repeated blood cultures. The patient underwent successful redo Bentall surgery the good outcome was confirmed at 1-month follow-up.DiscussionType 2 myocardial infarction caused by coronary embolism is a rare presentation of infective endocarditis and requires a high level of suspicion for its diagnosis. Prosthetic heart valves are a predisposing factor for infective endocarditis: aortic root abscess requires surgery as it rarely regresses with antibiotic therapy.

Project description:BackgroundMyocardial infarction (MI) with non-obstructive coronary arteries presenting with ST-segment elevation can be challenging. Understanding the cardiac and non-cardiac causes aid in identifying the underlying diagnosis and deciding on the management. Neurological insult resulting in a mismatch of oxygen supply or demand to cardiomyocytes can lead to type 2 MI. Acute brain injury, such as intracranial haemorrhage, can induce cardiac dysfunction secondary to brain-heart interaction via hypothalamic-pituitary-adrenal axis and catecholamine surge.Case summaryA 50-year-old Caucasian male who vaped cannabis presented with epileptic seizures. A Glasgow coma scale of 7/15 necessitated urgent intubation. Electrocardiogram showed ST-segment elevation in inferior leads. Computed tomography of the head suggested intracerebral haemorrhage. He was stabilized in the intensive care unit (ICU). Subsequent imaging confirmed anterior communicating cerebral artery aneurysm and haematoma. Echocardiogram showed severe left ventricular dysfunction and hypokinesia in the left circumflex (LCx) territory. After step down from ICU, cardiac magnetic resonance imaging revealed transmural MI and myocardial oedema at LCx territory. Coronary angiogram was normal. Patient was treated with Levetiracetam and heart failure regimen. A cardiac defibrillator was implanted for secondary prevention and he was scheduled for elective neurosurgical intervention. A follow-up outpatient echocardiogram was normal.DiscussionMyocardial infarction with non-obstructive coronary arteries is uncommon. Though the majority is due to either plaque disruption or myocarditis, non-cardiac causes, such as acute neurological insults and substance use, should be considered. Scrutinizing the clinical presentation and using a meticulous approach with appropriate investigations are required to reach the correct diagnosis and appropriate management.

A case report of a giant hiatal hernia mimicking an ST-elevation myocardial infarction.

Project description:BackgroundAcute coronary syndrome (ACS) can be a life-threatening condition. However, identification of patients with ACS can be challenging, especially among women, and clinical presentation can often overlap with other medical entities.Case summaryA 61-year-old woman with a history of stable bronchial asthma presented with worsening dyspnoea for spiroergometry. During bicycle exercise testing, she developed acute chest pain and her electrocardiogram showed significant ST-segment elevations. High-sensitivity cardiac troponin was elevated and a coronary angiography was performed showing normal coronary arteries. Cardiac magnetic resonance imaging showed no signs of myocardial infarction, myocarditis or Takotsubo cardiomyopathy but the incidental finding of a giant hiatal hernia impeding the filling of the left atrium. The giant hernia was surgically corrected, and the patient's exertional dyspnoea fully relieved during follow-up.DiscussionHiatal hernia might compress cardiac structures, cause exertional dyspnoea and mimic ST-elevation myocardial infarction. 10.1093/ehjcr/ytz138_audio1 ytz138_audio1 6074443146001.

Simultaneous ST-elevation myocardial infarction in monozygotic twins: a case report of entangled twins.

Project description:BackgroundThe development of coronary artery disease (CAD) is strongly influenced by genetic and environmental factors. Monozygotic twins represent a unique population that provides insights into the interaction of genetic, environmental, and social factors and their effects on the development of CAD.Case summaryTwo 54-year-old identical twins presented to an outside hospital with acute chest pain. Twin B developed chest pain after witnessing Twin A in distress from an acute chest pain episode. Electrocardiogram performed on each of them was diagnostic of ST-elevation myocardial infarction. Upon arrival at the angioplasty centre, Twin A was taken for emergency coronary angiography yet his pain subsided on the way to the catheterization lab hence, Twin B was taken for angiography instead. Twin B angiography demonstrated acute occlusion of the proximal left anterior descending coronary artery and was treated with percutaneous coronary intervention. Twin A coronary angiogram demonstrated 60% ostial first diagonal branch stenosis with a normal distal flow. He was diagnosed with possible coronary vasospasm.ConclusionThis is the first report of a simultaneous presentation of monozygotic twins with ST-elevation acute coronary syndrome. While genetic and environmental contributions to the development of CAD have been described, this case highlights the strong social bond that exists between monozygotic twins. Once CAD is diagnosed in one twin, aggressive risk factor modification and screening should be implemented in the other.

Watch out for ST-elevation myocardial infarction: a case report of ST-elevation in single-lead electrocardiogram tracing of a smartwatch.

Project description:BackgroundIt was recently demonstrated that the detection of atrial fibrillation based on heart rate tracking by optical sensors is feasible and reliable using the Apple Watch and the corresponding application. There are already a number of smartwatches and other wearable devices alongside the Apple Watch that can additionally record a single-lead electrocardiogram (ECG) and it is reasonable to expect this technology to become a standard feature, as is already the case with automated heart rate tracking. This could potentially have enormous impact regarding the early diagnosis of several cardiac diseases.Case summaryA 61-year-old male patient without previously known coronary artery disease was admitted with subacute ST-elevation myocardial infarction (STEMI) caused by occlusion of left anterior descending artery. Due to mildness of symptoms, the patient did only seek medical attention due to morphological changes in the single-lead ECG tracing acquired on his Apple Watch 5. The ECG recording of his smartwatch clearly showed ST-elevation, QRS widening, R-wave loss, and T-wave inversion. Coronary angiography revealed occlusion of the left anterior descending and recanalization was performed. The patient recovered without any complications and was discharged from the hospital 4 days after admission.DiscussionWhile the potential of ECG recordings by smartwatches to detect atrial fibrillation is currently under scientific investigation, this case highlights the possible potential of these devices to detect STEMI.
