Transmission of Legionnaires' Disease through Toilet Flushing.

ABSTRACT: We describe 2 cases of healthcare-associated Legionnaires' disease in patients in France hospitalized 5 months apart in the same room. Whole-genome sequencing analyses showed that clinical isolates from the patients and isolates from the room's toilet clustered together. Toilet contamination by Legionella pneumophila could lead to a risk for exposure through flushing.

Emission strength of airborne pathogens during toilet flushing.

Project description:The flushing of toilets generates contaminated aerosols, the transmission of which may cause the spread of disease, particularly in the immunocompromised or the elderly. This study investigated the emission strength of three types of airborne bacteria, namely Staphylococcus epidermidis, Escherichia coli, and Pseudomonas alcaligenes, during toilet flushing in a custom-built toilet under a controlled environment. Flushing was activated by a flushometer operated at two pressure levels, 400 kPa (high pressure [HP]) and 200 kPa (low pressure [LP]), and by a water cistern tank placed 95 cm (high tank [HT]) and 46 cm (low tank [LT]) above the toilet seat. The pathogens emitted by the first flush were calculated, with the correlations between airborne pathogen emissions and droplet concentration (HP, r=0.944, P<.001; LP, r=0.803, P<.001, HT, r=0.885, P<.05) and bacterial size (HP, r=-0.919, P<.001; LP, r=-0.936, P<.001; HT, r=-0.967, P<.05) in the different conditions then tested. The emission strength in the HP condition was statistically greater than that in the LP condition, whereas the cistern tank system produced less emissions than the flushometer system, and tank height was not found to be a sensitive parameter.

Particle exposure risk to a lavatory user after flushing a squat toilet.

Project description:Squat toilets are widely used in developing countries due to local customs and low costs. The flushing of a squat toilet can entrain strong airflow and produce aerosols. This investigation constructed a lavatory mock-up with a squat toilet. The flushing-induced airflow was both visualized and quantitatively measured by particle image velocimetry. The maximum height of the impacted airflow was identified by an ultrasonic anemometer. For inference of the particle emission rate, the toilet bowl was covered by an enclosed box for particle concentration measurement. The risks from skin contact of the deposited particles on the flushing button and the door handle and the possible inhalation of the released aerosols were evaluated. The results revealed that flushing a squat toilet can drive toilet plume to rise up to 0.9 m above the toilet bowl. A single flushing process can produce 0.29 million particles with diameters greater than 0.3 μm, among which 90% of the particles are submicron-sized. The flushing may cause particles to deposit on the flushing button and lavatory door handle as well as inhalation exposure even remaining in the lavatory for half a minute after flushing, especially for those lavatory users whose respiratory zones are below 1.0 m.

Virus Emissions from Toilet Flushing: Comparing Urine-Diverting to Mix Flush Toilets.

Project description:High levels of viruses can be found in human excrement from infected individuals, a fraction of which can be emitted from toilet flushing. Unlike the common mix flush toilet (MFT), the urine-diverting toilet (UDT) separates urine from the toilet water. Specific focus on urine-associated viruses is needed because the UDT can emit different levels of urine-associated and fecal-borne viruses and urine has different properties compared to feces that can affect emission levels (e.g., protein content). In this work, we quantified emission levels of surrogate bacteriophages for urine-associated and fecal-borne viruses, MS2 and T3, from flushing a UDT and an MFT, with and without protein in the water. Emission levels of viruses in the water of the UDT were lower than that of the MFT by up to 1.2-log10 and 1.3-log10 for T3 and MS2, respectively. If urine is completely diverted in the UDT, virus emissions can be reduced by up to 4-log10. Based on these results and typical levels in urine and feces, we estimate that up to 107 and 108 gene copies of human viruses per flush can be released from the UDT and MFT, respectively. Lower emissions observed with the UDT suggest reduced exposure to viruses from flushing the UDT.

Practical implementation of true on-site water recycling systems for hand washing and toilet flushing.

Project description:On-site wastewater reuse can improve global access to clean water, sanitation and hygiene. We developed a treatment system (aerated bioreactor, ultrafiltration membrane, granular activated carbon and electrolysis for chlorine disinfection) that recycles hand washing and toilet flush water. Three prototypes were field-tested in non-sewered areas, one in Switzerland (hand washing) and two in South Africa (hand washing, toilet flushing), over periods of 63, 74 and 94 days, respectively. We demonstrated that the system is able to recycle sufficient quantities of safe and appealing hand washing and toilet flush water for domestic or public use in real-life applications. Chemical contaminants were effectively removed from the used water in all prototypes. Removal efficiencies were 99.7% for the chemical oxygen demand (COD), 98.5% for total nitrogen (TN) and 99.9% for phosphate in a prototype treating hand washing water, and 99.8% for COD, 95.7% for TN and 89.6% for phosphate in a prototype treating toilet flush water. While this system allowed for true recycling for the same application, most on-site wastewater reuse systems downcycle the treated water, i.e., reuse it for an application requiring lower water quality. An analysis of 18 selected wastewater reuse specifications revealed that at best these guidelines are only partially applicable to innovative recycling systems as they are focused on the downcycling of water to the environment (e.g., use for irrigation). We believe that a paradigm shift is necessary and advocate for the implementation of risk-based (and thus end-use dependent) system performance targets to evaluate water treatment systems, which recycle and not only downcycle water.

Resolving the relative contributions of cistern and pour flushing to toilet water usage: Measurements from urban test sites in India.

Project description:A challenge in water reuse for toilet flushing in India and other Asian countries derives from pour flushing practices. It is a common assumption that the amount of pour flushed water used for personal cleansing is small in comparison to the cistern flush volume, however there is a knowledge gap regarding the actual contribution of each water source to the blackwater amount. In this study, digital water meters were used to measure the fraction of water from personal wash tap relative to cistern water that is used for toilet flushing. High temporal resolution measurements were carried in three different urban sites in the city of Coimbatore in the southern Indian state of Tamil Nadu where onsite sanitation treatment prototypes that may provide reclaimed water for cistern flushing are being tested. Data collected over periods of up to 2 months show that the contribution of the cistern flush to the total blackwater volume is low (14-40%). These data highlight an important factor to inform interventions designed around water reuse for flushing in world geographies where personal toilet cleansing by water is the common practice.

Project description:Legionella londiniensis has been isolated from aqueous environments. However, to our knowledge, this organism has never been isolated from clinical specimens. A case of Legionnaires' disease in a hematopoietic stem cell transplant recipient caused by this organism is described, which confirms that L. londiniensis can be an opportunistic pathogen.

Project description:During June 2012-September 2014, we tested patients with severe respiratory illness for Legionella spp. infection and conducted a retrospective epidemiologic investigation. Of 1,805 patients tested, Legionella was detected in samples of 21 (1.2%); most were adults who had HIV or tuberculosis infections and were inappropriately treated for Legionella.

Healthcare-Associated Legionnaires' Disease, Europe, 2008-2017.

Project description:Healthcare-associated Legionnaires' disease (HCA LD) can cause nosocomial outbreaks with high death rates. We compared community-acquired LD cases with HCA LD cases in Europe during 2008-2017 using data from The European Surveillance System. A total of 29 countries reported 40,411 community-acquired and 4,315 HCA LD cases. Of the HCA LD cases, 2,937 (68.1%) were hospital-acquired and 1,378 (31.9%) were linked to other healthcare facilities. The odds of having HCA LD were higher for women, children and persons <20 years of age, and persons >60 years of age. Out of the cases caused by Legionella pneumophila with a known serotype, community-acquired LD was more likely to be caused by L. pneumophila serogroup 1 (92.3%) than was HCA LD (85.1%). HCA LD patients were more likely to die. HCA LD is associated with specific patient demographics, causative strains, and outcomes. Healthcare facilities should consider these characteristics when designing HCA LD prevention strategies.

National Surveillance of Legionnaires' Disease, China, 2014-2016.

Project description:We report national surveillance of Legionnaires' disease in China. Urine samples from 11 (3.85%) of 286 patients with severe pneumonia of unknown cause were positive for the Legionella pneumophila serogroup 1 antigen. We isolated Legionella strains from 7 patients. Improved diagnostic testing is needed for this underestimated disease in China.
