COVID-19 impact assessment on the French radiological centers: a nationwide survey.

ABSTRACT:

Purpose

To determine the impact of the COVID-19 on the CT activities in French radiological centers during the epidemic peak.

Materials and methods

A cross-sectional prospective CT scan survey was conducted between March 16 and April 12, 2020, in accordance with the local IRB. Seven hundred nine radiology centers were invited to participate in a weekly online survey. Numbers of CT examinations related to COVID-19 including at least chest (CT_covid) and whole chest CT scan activities (CT_chest) were recorded each week. A sub-analysis on French departments was performed during the 4 weeks of the study. The impact of the number of RT-PCRs (reverse transcriptase polymerase chain reactions) on the CT workflow was tested using two-sample t test and Pearson's test.

Results

Five hundred seventy-seven structures finally registered (78%) with mean response numbers of 336 ± 18.9 (323; 351). Mean CT_chest activity per radiologic structure ranged from 75.8 ± 133 (0-1444) on week 12 to 99.3 ± 138.6 (0-1147) on week 13. Mean ratio of CT_covid on CT_chest varied from 0.36 to 0.59 on week 12 and week 14 respectively. There was a significant relationship between the number of RT-PCR performed and the number of CT_covid (r = 0.73, p = 3.10^-16) but no link with the number of positive RT-PCR results.

Conclusion

In case of local high density COVID-19, CT workflow is strongly modified and redirected to the management of these specific patients.

Project description:During the COVID-19 pandemic, lockdown posed a number of unprecedented problems. Accidents to children are a major public health concern. This study sought to determine the impact of France's 2020 COVID-19 lockdown on hospital admissions for paediatric accidents. This study was a retrospective cohort analysis of data from the French national inpatient database. Children admitted to a French hospital for an accident from January to July 2020 and 2019 were included. The number of admissions for accidents between January and July in 2020 (the study period) vs. the same period (the control period) in 2019 was recorded. Hospital admission rate ratios (HRRs) comparing 2020 with 2019 were calculated for three periods in 2020 and 2019: before lockdown (P1, January 1 to March 15), during lockdown (P2, March 16 to May 10), and after lockdown (P3, May 11 to July 31). A total of 72,089 children were admitted to hospital for accidents between January and July 2020 (mean (SD) age, 8.7 (5.8)), and 84,961 between January and July 2019 (mean (SD) age, 9.2 (5.7)). A total of 12,864 children (18%) were admitted during the lockdown period (P2) in 2020, and 23,098 (27%) during the equivalent period in 2019 (HRR, 0.56; 95% confidence interval (CI), 0.55-0.57; p < .001). There were significantly fewer admissions for accidents in 2020 than in 2019; the HRR was 0.97 (95% CI, 0.96-0.99; p = .003) for P1 and 0.95 (95% CI, 0.93-0.96; p < .001) for P3. During the 2020 lockdown period (P2), the risk of accident-related paediatric admissions with critical care lasting for at least 1 day was lower than in 2019 (HRR 0.67; 95% CI, 0.62-0.73; p < .001).ConclusionsFurther research could usefully examine how parents create a safe home environment for their children.What is known• During the COVID-19 pandemic, lockdown posed a number of unprecedented problems. Project description:BackgroundGeographical variation in health care services challenges the basic principle of fair allocation of health care resources. This study aimed to investigate geographical variation in the use of X-ray, CT, MRI and Ultrasound examinations in Norway, the contribution from public and private institutions, and the impact of accessibility and socioeconomic factors on variation in examination rates.MethodsA nationwide survey of activity in all radiological institutions for the year 2002 was used to compare the rates per thousand of examinations in the counties. The data format was files/printouts where the examinations were recorded according to a code system.ResultsOverall rates per thousand of radiological examinations varied by a factor of 2.4. The use of MRI varied from 170 to 2, and CT from 216 to 56 examinations per 1000 inhabitants. Single MRI examinations (knee, cervical spine and head/brain) ranged high in variation, as did certain other spine examinations. For examination of specific organs, the counties' use of one modality was positively correlated with the use of other modalities. Private institutions accounted for 28% of all examinations, and tended towards performing a higher proportion of single examinations with high variability. Indicators of accessibility correlated positively to variation in examination rates, partly due to the figures from the county of Oslo. Correlations between examination rates and socioeconomic factors were also highly influenced by the figures from this county.ConclusionThe counties use of radiological services varied substantially, especially CT and MRI examinations. A likely cause of the variation is differences in accessibility. The coexistence of public and private institutions may be a source of variability, along with socioeconomic factors. The findings represent a challenge to the objective of equality in access to health care services, and indicate a potential for better allocation of overall health care resources.

Project description:PurposeTo evaluate the impact of the COVID-19 pandemic on Canadian intensive care unit (ICU) workers.MethodsBetween June and August 2020, we distributed a cross-sectional online survey of ICU workers evaluating the impact of the pandemic, coping strategies, symptoms of post-traumatic stress disorder (PTSD; Impact of Events Scale-Revised), and psychological distress, anxiety, and depression (Kessler Psychological Distress Scale). We performed regression analyses to determine the predictors of psychological symptoms.ResultsWe analyzed responses from 455 ICU workers (80% women; 67% from Ontario; 279 nurses, 69 physicians, and 107 other healthcare professionals). Respondents felt that their job put them at great risk of exposure (60%), were concerned about transmitting COVID-19 to family members (76%), felt more stressed at work (67%), and considered leaving their job (37%). Overall, 25% had probable PTSD and 18% had minimal or greater psychological distress. Nurses were more likely to report PTSD symptoms (33%) and psychological distress (23%) than physicians (5% for both) and other health disciplines professionals (19% and 14%). Variables associated with PTSD and psychological distress included female sex (beta-coefficient [B], 1.59; 95% confidence interval [CI], 1.20 to 2.10 and B, 3.79; 95% CI, 1.79 to 5.78, respectively; P < 0.001 for differences in scores across groups) and perceived increased risk due to PPE shortage or inadequate PPE training (B, 1.87; 95% CI, 1.51 to 2.31 and B, 4.88; 95% CI, 3.34 to 6.43, respectively). Coping strategies included talking to friends/family/colleagues (80%), learning about COVID-19 (78%), and physical exercise (68%). Over half endorsed the following workplace strategies as valuable: hospital-provided scrubs, clear communication and protocols by hospitals, knowing their voice is heard, subsidized parking, and gestures of appreciation from leadership.ConclusionsThis survey study shows that ICU workers have been impacted by the COVID-19 pandemic with high levels of stress and psychological burden. Respondents endorsed communication, protocols, and appreciation from leadership as helpful mitigating strategies.
