Project description:Clinical outcomes for patients with COVID-19 are heterogeneous and there is interest in defining subgroups for prognostic modeling and development of treatment algorithms. We obtained 28 demographic and laboratory variables in patients admitted to hospital with COVID-19. These comprised a training cohort (n = 6099) and two validation cohorts during the first and second waves of the pandemic (n = 996; n = 1011). Uniform manifold approximation and projection (UMAP) dimension reduction and Gaussian mixture model (GMM) analysis was used to define patient clusters. 29 clusters were defined in the training cohort and associated with markedly different mortality rates, which were predictive within confirmation datasets. Deconvolution of clinical features within clusters identified unexpected relationships between variables. Integration of large datasets using UMAP-assisted clustering can therefore identify patient subgroups with prognostic information and uncovers unexpected interactions between clinical variables. This application of machine learning represents a powerful approach for delineating disease pathogenesis and potential therapeutic interventions.

Project description:The coronavirus disease 2019 (COVID-19) pandemic has posed unprecedented challenges in critical care medicine, including extreme demand for intensive care unit (ICU) resources and rapidly evolving understanding of a novel disease. Up to one-third of hospitalized patients with COVID-19 experience critical illness. The most common form of organ failure in COVID-19 critical illness is acute hypoxemic respiratory failure, which clinically presents as acute respiratory distress syndrome (ARDS) in three-quarters of ICU patients. Noninvasive respiratory support modalities are being used with increasing frequency given their potential to reduce the need for intubation. Determining optimal patient selection for and timing of intubation remains a challenge. Management of mechanically ventilated patients with COVID-19 largely mirrors that of non-COVID-19 ARDS. Organ failure is common and portends a poor prognosis. Mortality rates have improved over the course of the pandemic, likely owing to increasing disease familiarity, data-driven pharmacologics, and improved adherence to evidence-based critical care.

Project description:The coronavirus (COVID-19) started in China in 2019, has spread rapidly in every single country and has spread in millions of cases worldwide. This paper presents a proposed approach that involves identifying the relative impact of COVID-19 on a specific gender, the mortality rate in specific age, investigating different safety measures adopted by each country and their impact on the virus growth rate. Our study proposes data-driven analysis and prediction modeling by investigating three aspects of the pandemic (gender of patients, global growth rate, and social distancing). Several machine learning and ensemble models have been used and compared to obtain the best accuracy. Experiments have been demonstrated on three large public datasets. The motivation of this study is to propose an analytical machine learning based model to explore three significant aspects of COVID-19 pandemic as gender, global growth rate, and social distancing. The proposed analytical model includes classic classifiers, distinctive ensemble methods such as bagging, feature based ensemble, voting and stacking. The results show a superior prediction performance comparing with the related approaches.

Project description:Optimizing the impact on the economy of control strategies aiming at containing the spread of COVID-19 is a critical challenge. We use daily new case counts of COVID-19 patients reported by local health administrations from different Metropolitan Statistical Areas (MSAs) within the US to parametrize a model that well describes the propagation of the disease in each area. We then introduce a time-varying control input that represents the level of social distancing imposed on the population of a given area and solve an optimal control problem with the goal of minimizing the impact of social distancing on the economy in the presence of relevant constraints, such as a desired level of suppression for the epidemics at a terminal time. We find that with the exception of the initial time and of the final time, the optimal control input is well approximated by a constant, specific to each area, which contrasts with the implemented system of reopening 'in phases'. For all the areas considered, this optimal level corresponds to stricter social distancing than the level estimated from data. Proper selection of the time period for application of the control action optimally is important: depending on the particular MSA this period should be either short or long or intermediate. We also consider the case that the transmissibility increases in time (due e.g. to increasingly colder weather), for which we find that the optimal control solution yields progressively stricter measures of social distancing. We finally compute the optimal control solution for a model modified to incorporate the effects of vaccinations on the population and we see that depending on a number of factors, social distancing measures could be optimally reduced during the period over which vaccines are administered to the population.

Project description:Healthcare management and COVID-19 has been broadly studied during the recent few days, especially after declaration of the COVID-19 outbreak in almost all countries in the world. Therefore, the present research article aims to provide an extensive overview of the scientific literature about the study of healthcare management and COVID-19 for choosing the new topic of related research. It conducts four types of analyses where the first analysis is a trend analysis and other three analyses are related to network and density maps. The second analysis is analyzed decisively in order to produce all keywords, author keywords and index keywords co-occurrence network map and country co-authorship network map and tables summarizing the significant scientific trends under the present topics. The third analysis is analyzed purposefully in order to produce all documents, journals, authors and countries bibliographic coupling network maps and tables summarizing the significant scientific trends. The last analysis provides valuable approaching of the most significant used keywords on the research topic and the links among them using keyword co-occurrence network and density maps respectively. Project description:The contribution of this paper is twofold. First, a new data driven approach for predicting the Covid-19 pandemic dynamics is introduced. The second contribution consists in reporting and discussing the results that were obtained with this approach for the Brazilian states, with predictions starting as of 4 May 2020. As a preliminary study, we first used an Long Short Term Memory for Data Training-SAE (LSTM-SAE) network model. Although this first approach led to somewhat disappointing results, it served as a good baseline for testing other ANN types. Subsequently, in order to identify relevant countries and regions to be used for training ANN models, we conduct a clustering of the world's regions where the pandemic is at an advanced stage. This clustering is based on manually engineered features representing a country's response to the early spread of the pandemic, and the different clusters obtained are used to select the relevant countries for training the models. The final models retained are Modified Auto-Encoder networks, that are trained on these clusters and learn to predict future data for Brazilian states. These predictions are used to estimate important statistics about the disease, such as peaks and number of confirmed cases. Finally, curve fitting is carried out to find the distribution that best fits the outputs of the MAE, and to refine the estimates of the peaks of the pandemic. Predicted numbers reach a total of more than one million infected Brazilians, distributed among the different states, with São Paulo leading with about 150 thousand confirmed cases predicted. The results indicate that the pandemic is still growing in Brazil, with most states peaks of infection estimated in the second half of May 2020. The estimated end of the pandemics (97% of cases reaching an outcome) spread between June and the end of August 2020, depending on the states.

Project description:The COVID-19 pandemic has created unprecedented challenges worldwide. Strained healthcare providers make difficult decisions on patient triage, treatment and care management on a daily basis. Policy makers have imposed social distancing measures to slow the disease, at a steep economic price. We design analytical tools to support these decisions and combat the pandemic. Specifically, we propose a comprehensive data-driven approach to understand the clinical characteristics of COVID-19, predict its mortality, forecast its evolution, and ultimately alleviate its impact. By leveraging cohort-level clinical data, patient-level hospital data, and census-level epidemiological data, we develop an integrated four-step approach, combining descriptive, predictive and prescriptive analytics. First, we aggregate hundreds of clinical studies into the most comprehensive database on COVID-19 to paint a new macroscopic picture of the disease. Second, we build personalized calculators to predict the risk of infection and mortality as a function of demographics, symptoms, comorbidities, and lab values. Third, we develop a novel epidemiological model to project the pandemic's spread and inform social distancing policies. Fourth, we propose an optimization model to re-allocate ventilators and alleviate shortages. Our results have been used at the clinical level by several hospitals to triage patients, guide care management, plan ICU capacity, and re-distribute ventilators. At the policy level, they are currently supporting safe back-to-work policies at a major institution and vaccine trial location planning at Janssen Pharmaceuticals, and have been integrated into the US Center for Disease Control's pandemic forecast.

Project description:Wastewater-based epidemiology (WBE) has been regarded as a potential tool for the prevalence estimation of coronavirus disease 2019 (COVID-19) in the community. However, the application of the conventional back-estimation approach is currently limited due to the methodological challenges and various uncertainties. This study systematically performed meta-analysis for WBE datasets and investigated the use of data-driven models for the COVID-19 community prevalence in lieu of the conventional WBE back-estimation approach. Three different data-driven models, i.e. multiple linear regression (MLR), artificial neural network (ANN), and adaptive neuro fuzzy inference system (ANFIS) were applied to the multi-national WBE dataset. To evaluate the robustness of these models, predictions for sixteen scenarios with partial inputs were compared against the actual prevalence reports from clinical testing. The performance of models was further validated using unseen data (data sets not included for establishing the model) from different stages of the COVID-19 outbreak. Generally, ANN and ANFIS models showed better accuracy and robustness over MLR models. Air and wastewater temperature played a critical role in the prevalence estimation by data-driven models, especially MLR models. With unseen datasets, ANN model reasonably estimated the prevalence of COVID-19 (cumulative cases) at the initial phase and forecasted the upcoming new cases in 2-4 days at the post-peak phase of the COVID-19 outbreak. This study provided essential information about the feasibility and accuracy of data-driven estimation of COVID-19 prevalence through the WBE approach.
