A Multidimensional, Multisensory and Comprehensive Rehabilitation Intervention to Improve Spatial Functioning in the Visually Impaired Child: A Community Case Study.

ABSTRACT: Congenital visual impairment may have a negative impact on spatial abilities and result in severe delays in perceptual, social, motor, and cognitive skills across life span. Despite several evidences have highlighted the need for an early introduction of re-habilitation interventions, such interventions are rarely adapted to children's visual capabilities and very few studies have been conducted to assess their long-term efficacy. In this work, we present a case study of a visually impaired child enrolled in a newly developed re-habilitation intervention aimed at improving the overall development through the diversification of re-habilitation activities based on visual potential and developmental profile, with a focus on spatial functioning. We argue that intervention for visually impaired children should be (a) adapted to their visual capabilities, in order to increase re-habilitation outcomes, (b) multi-interdisciplinary and multidimensional, to improve adaptive abilities across development, (c) multisensory, to promote the integration of different perceptual information coming from the environment.

Project description:BackgroundVisually impaired and blind adolescents fare poorly in educational attainment compared to adolescents without vision impairment. Rehabilitation holds the potential to compensate for the hindrances that the impairment causes. Many rehabilitation initiatives exist. However, the efficacy of these initiatives remains uncertain. This systematic review assessed which rehabilitation initiatives improve participation in an educational setting for visually impaired and blind adolescents.MethodsPubMed, Embase, Scopus, Cinahl, and Cochrane library databases were searched. Only primary studies as randomized controlled trial (parallel group or crossover), cohort studies, case-control studies, qualitative studies, and case-studies were included. Data on the study characteristics, visual impairment, type of intervention, research question, main findings, and implications for practice were extracted from the papers. Critical appraisal was performed using the Critical Appraisal Checklist for Qualitative Research and the Checklist for Quasi-Experimental Studies both from the Joanna Briggs Institute. The data extraction and the critical appraisal were performed independently by two reviewers.ResultsA total of 10 studies with visually impaired and blind adolescents were considered eligible, from an original search result of 3210 studies. In the thematic analysis we identified a heightened focus on different means for studying by making the curriculum content more accessible by applying different audio, tactile, or electronic devices (n = 8). A minor focus in the identified studies (n = 2) was placed on the impact of support from the environment on the development of literacy, for example the support from teachers or parents. Outcome parameters representing more diverse rehabilitation initiatives have not been adequately investigated in the literature. The scientific evidence that we identified was based on few publications with contradictory results and some studies were of questionable quality, limiting the applicability of their findings.ConclusionsOverall, the review identified a gap in the evidence regarding rehabilitation initiatives for visually impaired and blind adolescents that enables participation in an educational setting. The overall quality assessment of the 10 studies identified several risks of bias, for which reason the current scientific evidence does not qualify as a basis for decision making, leaving the adolescents in a heightened risk to fall even further behind in the educational system. Further high quality randomized controlled trials are required to establish high-quality evidence.

Project description:SignificanceVisually impaired participants were surprisingly fast in learning a new sensory substitution device, which allows them to detect obstacles within a 3.5-m radius and to find the optimal path in between. Within a few hours of training, participants successfully performed complex navigation as well as with the white cane.PurposeGlobally, millions of people live with vision impairment, yet effective assistive devices to increase their independence remain scarce. A promising method is the use of sensory substitution devices, which are human-machine interfaces transforming visual into auditory or tactile information. The Sound of Vision (SoV) system continuously encodes visual elements of the environment into audio-haptic signals. Here, we evaluated the SoV system in complex navigation tasks, to compare performance with the SoV system with the white cane, quantify training effects, and collect user feedback.MethodsSix visually impaired participants received eight hours of training with the SoV system, completed a usability questionnaire, and repeatedly performed assessments, for which they navigated through standardized scenes. In each assessment, participants had to avoid collisions with obstacles, using the SoV system, the white cane, or both assistive devices.ResultsThe results show rapid and substantial learning with the SoV system, with less collisions and higher obstacle awareness. After four hours of training, visually impaired people were able to successfully avoid collisions in a difficult navigation task as well as when using the cane, although they still needed more time. Overall, participants rated the SoV system's usability favorably.ConclusionsContrary to the cane, the SoV system enables users to detect the best free space between objects within a 3.5-m (up to 10-m) radius and, importantly, elevated and dynamic obstacles. All in all, we consider that visually impaired people can learn to adapt to the haptic-auditory representation and achieve expertise in usage through well-defined training within acceptable time.
