Project description:BackgroundCancer screening tests are recommended to prevent cancer-associated mortality by detecting precancerous and cancerous lesions in early stages. The COVID-19 pandemic disrupted the use of preventive health care services. Although there was an increase in the number of cancer screening tests beginning in late 2020, screenings remained 29% to 36% lower than in the prepandemic era.ObjectiveThe aim of this review is to assist health care providers in identifying approaches for prioritizing patients and increasing breast, cervical, and colorectal cancer screening during the uncertainty of the COVID-19 pandemic.MethodsWe used the scoping review framework to identify articles on PubMed and EBSCO databases. A total of 403 articles were identified, and 23 articles were selected for this review. The literature review ranged from January 1, 2020, to September 30, 2021.ResultsThe articles included two primary categories of recommendations: (1) risk stratification and triage to prioritize screenings and (2) alternative methods to conduct cancer screenings. Risk stratification and triage recommendations focused on prioritizing high-risk patients with an abnormal or suspicious result on the previous screening test, patients in certain age groups and sex, patients with a personal medical or family cancer history, patients that are currently symptomatic, and patients that are predisposed to hereditary cancers and cancer-causing mutations. Other recommended strategies included identifying areas facing the most disparities, creating algorithms and using artificial intelligence to create cancer risk scores, leveraging in-person visits to assess cancer risk, and providing the option of open access screenings where patients can schedule screenings and can be assigned a priority category by health care staff. Some recommended using telemedicine to categorize patients and determine screening eligibility for patients with new complaints. Several articles noted the importance of implementing preventive measures such as COVID-19 screening prior to the procedures, maintaining hygiene measures, and social distancing in waiting rooms. Alternative screening methods that do not require an in-person clinic visit and can effectively screen patients for cancers included mailing self-collection sampling kits for cervical and colorectal cancers, and implementing or expanding mobile screening units.ConclusionsAlthough the COVID-19 pandemic had devastating effects on population health globally, it could be an opportunity to adapt and evolve cancer screening methods. Disruption often creates innovation, and focus on alternative methods for cancer screenings may help reach rural and underresourced areas after the pandemic has ended.

Project description:Dental clinics were suspected to be a hotspot for nosocomial transmission of coronavirus disease 19 (COVID-19), yet there has been no clear recommendation about emergency dental care and appropriate personal protective equipment during pandemics. In this paper, we aim to summarize recommendations for (i) patient risk assessment, (ii) patient triage, and (iii) measures to prevent infection of health professionals and nosocomial transmission in dental clinics. The available evidence was collected by performing searches on PubMed, Embase, and Cochrane databases. We reviewed papers on COVID-19, severe acute respiratory syndrome (SARS), Middle East respiratory syndrome (MERS), influenza, and related respiratory viral diseases. Legal and ethical frameworks, as well as international (e.g., World Health Organization (WHO)) and national (e.g., public health institutes, dental associations) guidelines were screened to summarize recommendations related to dental emergency care. To assess the patient risk, a questionnaire was developed to classify patients at unknown, high, and very high risk. Patient triage recommendations were summarized in a flow chart that graded the emergency level of treatments (i.e., urgent, as soon as possible, and postpone). Measures to prevent disease transmission based on current evidence were grouped for dental health professionals, dental clinics, and patients. The present recommendations may support health professionals implement preventative measures during the pandemic.

Project description:Purpose: The coronavirus disease-19 (COVID-19) was declared a pandemic by the World Health Organization in March 2020. COVID-19, caused by SARS-CoV-2 has imposed a significant burden on health care systems, economies, and social systems in many countries around the world. The provision of rehabilitation services for persons with active COVID-19 infection poses challenges to maintaining a safe environment for patients and treating providers. Materials and Methods: Established frameworks were used to guide the scoping review methodology. Medline, Embase, Pubmed, CINAHL databases from inception to August 1, 2020, and prominent rehabilitation organizations' websites were searched. Study Selection: We included articles and reports if they were focused on rehabilitation related recommendations for COVID-19 patients, treating providers, or the general population. Data Extraction: Pairs of team members used a pre-tested data abstraction form to extract data from included full-text articles. The strength and the quality of the extracted recommendations were evaluated by two reviewers using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach. Results: We retrieved 6,468 citations, of which 2,086 were eligible for review, after duplicates were removed. We excluded 1,980 citations based on title and abstract screening. Of the screened full-text articles, we included all 106 studies. A summary of recommendations is presented. We assessed the overall evidence to be strong and of fair quality. Conclusion: The rehabilitation setting, and processes, logistics, and patient and healthcare provider precaution recommendations identified aim to reduce the spread of SARS-CoV-2 infection and ensure adequate and safe rehabilitation services, whether face-to-face or through teleservices. The COVID-19 pandemic is rapidly changing. Further updates will be needed over time in order to incorporate emerging best evidence into rehabilitation guidelines.

Project description:To understand and analyse the global impact of COVID-19 on outpatient services, inpatient care, elective surgery, and perioperative colorectal cancer care, a DElayed COloRectal cancer surgery (DECOR-19) survey was conducted in collaboration with numerous international colorectal societies with the objective of obtaining several learning points from the impact of the COVID-19 outbreak on our colorectal cancer patients which will assist us in the ongoing management of our colorectal cancer patients and to provide us safe oncological pathways for future outbreaks.

Project description:PurposeNumerous guideline recommendations for airway and perioperative management during the COVID-19 pandemic have been published. We identified, synthesized, and compared guidelines intended for anesthesiologists.SourceMember society websites of the World Federation of Societies of Anesthesiologists and the European Society of Anesthesiologists were searched. Recommendations that focused on perioperative airway management of patients with proven or potential COVID-19 were included. Accelerated screening was used; data were extracted by one reviewer and verified by a second. Data were organized into themes based on perioperative phase of care.Principal findingsThirty unique sets of recommendations were identified. None reported methods for systematically searching or selecting evidence to be included. Four were updated following initial publication. For induction and airway management, most recommended minimizing personnel and having the most experienced anesthesiologist perform tracheal intubation. Significant congruence was observed among recommendations that discussed personal protective equipment. Of those that discussed tracheal intubation methods, most (96%) recommended videolaryngoscopy, while discordance existed regarding use of flexible bronchoscopy. Intraoperatively, 23% suggested specific anesthesia techniques and most (63%) recommended a specific operating room for patients with COVID-19. Postoperatively, a minority discussed extubation procedures (33%), or care in the recovery room (40%). Non-technical considerations were discussed in 27% and psychological support for healthcare providers in 10%.ConclusionRecommendations for perioperative airway management of patients with COVID-19 overlap to a large extent but also show significant differences. Given the paucity of data early in the pandemic, it is not surprising that identified publications largely reflected expert opinion rather than empirical evidence. We suggest future efforts should promote coordinated responses and provide suggestions for studying and establishing best practices in perioperative patients.Study registrationOpen Science Framework ( https://osf.io/a2k4u/ ); date created, 26 March 2020.

Project description:AIM:To compile recommendations and evidence on the practical management of pregnant women with COVIC-19 in order to clarify standards of obstetric care in the face of this new disease. METHODS:Scoping review based on literaature searches in national and international health science databases (Pubmed/Medline, Biblioteca virtual en salud (BVS), Scielo, COCHRANE and CUIDEN) and websites, and additionally by a "snowball" system. MeSH terms were used: "COVID-19", "Pregnancy", "Delivery, Obstetric" "Pregnant Women" and "Maternal". As limits in the search Spanish and English languages were selected. No limits were established in relation to the year of publication or type of article. RESULTS:A total of 49 documents and articles were detected, of which 27 were analyzed, 18 were used, and 9 were discarded because they did not contain practical recommendations. The recommendations were grouped into 9 subjects: Prevention of infection in pregnant women, Prevention of infection in health care personnel attending pregnant women, Form of presentation and severity in pregnant women, Maternal-fetal transmission (vertical and perinatal), Maternal-fetal control of the pregnant woman infected with COVID-19, Control of the severely pregnant woman with COVID-19, Treatment of the pregnant woman with COVID-19, Management and route of termination of labor, Neonatal outcomes in women with COVID-19 and Breastfeeding. CONCLUSIONS:Lack of strong evidence to support many of the recommendations for pregnant women with COVID-19, as they are based on previous experience with SARS-CoV and MERS-CoV infections. Further studies are needed to confirm the appropriateness of many of the recommendations and guidelines for action in the specific case of pregnant women and COVIC-19.

Project description:Objectives This scoping review aims to assess the situation of violence against women during the COVID-19 pandemic in Latin America, mainly in Brazil. Methods This study consists of a Scoping Review based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. To conduct this review, we mapped scientific evidence on domestic violence during the COVID-19 pandemic. The inclusion criteria were: journal articles on domestic violence set in Latin American countries, particularly Brazil, in the context of social isolation during the COVID-19 pandemic, published between March 2020 and June of 2021. Articles that were not published in journals, studies focusing on child violence or the elderly population, suicide-related approaches, editorials and letters to readers were excluded. The databases used were: National Library of Medicine (PubMed), Embase, COVID-19 Global literature on coronavirus disease, Scopus Preview, Web of Science, LILAC's, Database of the best Evidence-Based Health (Epistemonikos) with the following strategic keywords: "domestic violence" "female'/exp OR female" "domestic violence AND women" "SARS-CoV-19") AND ("domestic violence") SARS-CoV-2" OR "2020-nCoV". The databases above were accessed in July 2021. The articles selected from the databases were synthetized following the double-check procedure and the topics that were most relevant to the subject discussed were separated. The articles used were in English, Portuguese, and Spanish. Results We discussed domestic violence against women in the context of social isolation, showing a significant increase and highlighting aggravating factors - financial instability, exacerbation of the female workload, increase in drug and alcohol consumption, lack of access to care services - and statistics that demonstrate the significant increase in countries such as Argentina, Ecuador, Paraguay and Brazil, especially in São Paulo, Rio de Janeiro, Paraná, among other states. Despite the meaningful increase in the number of cases and the aggravating factors for domestic violence against women in Latin America and Brazil during the COVID-19 pandemic, the reality of underreporting, resulting, among others, from limited legal, social and economic support and the lack of well-prepared intersectoral services may be a limitation in this scenario, since the situation can be even more acute. We presented measures adopted in Brazil and alternatives to confront the critical situation of domestic violence against women, aggravated by social isolation during the COVID-19 pandemic. The 18 most relevant articles were read twice by each author, following the “Double-Check” protocol. The articles used addressed important questions around the established goals: (a) characteristics of the current scenarios in Latin America and, specifically, in Brazil, with regard to violence against women; (b) factors that intensify this type of violence; (c) numbers that demonstrate an increase in cases; and finally (e) strategies to combat domestic violence against women. A total of 595 articles were found, then, based on the abstracts presented, 18 articles were used to write this scoping review.

Project description:AimThis study aimed to clarify how the COVID-19 pandemic impacts psychological stress levels in postpartum women in Yokohama, Japan.MethodsWe retrospectively compared the rates of positive screening tests for postpartum depression (Edinburgh Postnatal Depression Scale score of 9 or more or self-injury factors) and liaison/psychiatric intervention rates between pre-pandemic and pandemic groups of postpartum women who delivered a live birth at our tertiary perinatal center in Yokohama, Japan between January 2019 and May 2020. Those whose postpartum health screening was performed in 2019 (March-June 2019) were considered the pre-pandemic group, and those whose screening was performed between March and June 2020 were considered the pandemic group. Adjustments were made for maternal background, pregnancy, and delivery outcomes, including a history of psychiatric disorders, complications, preterm delivery, NICU admission, and maternal postpartum complications, and the data were analyzed using a logistic regression analysis.ResultsPostpartum health screenings were performed on 339 women before the pandemic and 279 women during the pandemic. Positive screening rates for postpartum depression did not change before and during the pandemic (adjusted odds ratio 1.48, 95% confidence interval 0.885-2.46).ConclusionsThere was no apparent increase in maternal psychological stress due to the COVID-19 pandemic in Yokohama, Japan. Postpartum women responded differently in settings with varied levels of disease severity and social restrictions.

Project description:BackgroundThe COVID-19 pandemic has been uniquely challenging for pregnant and postpartum women. Uncontrollable stress amplifies risk for maternal depression and anxiety, which are linked to adverse mother and child outcomes. This study examined change in internalizing symptoms from before to during the COVID-19 pandemic among pregnant and postpartum women longitudinally, and investigated moderation by loneliness and other contextual risk factors.Methods135 women (M age = 31.81; 26% Latina, 55% non-Hispanic White, 11% Black; 39% low-income) enrolled in an existing prospective study completed self-report questionnaires including the EPDS and STAI-short form during early pregnancy, prior to COVID-19, and during COVID-19.ResultsDepressive symptoms were higher during COVID-19 compared to pre-COVID-19 and just as high as during early pregnancy. Anxiety symptoms were higher during COVID-19 compared to both pre-COVID-19 and early pregnancy. Higher loneliness was associated with increased depressive symptoms during COVID-19. Greater COVID-19 specific adversity was linked to greater increases in internalizing symptoms during COVID-19. Lower income-to-needs-ratio most strongly predicted symptoms during early pregnancy.LimitationsThe present study is longitudinal, but the correlational nature of the data precludes causal conclusions regarding the effect of the pandemic on mental health.ConclusionLongitudinal analyses suggest the COVID-19 pandemic resulted in increased internalizing symptoms among diverse pregnant and postpartum women. Interpersonal and contextual factors exacerbate risk and the impact of the pandemic on women's mental health. Identifying mothers at-risk for depression and anxiety may increase treatment utilization, mitigate fetal and infant exposure to maternal depression and anxiety, and help prevent adverse child outcomes.

Project description:BackgroundPalliative and hospice care facilities face different challenges emerging from the COVID-19 pandemic. In particular, this results from the high age and pre-existing diseases of patients as well as the physically close contact between staff and patients.ObjectiveThe aim of this study was to identify existing strategies, guidelines and recommendations in Germany and other countries with respect to addressing the challenges of the COVID-19 pandemic in palliative and hospice care.Material and methodsThe databases PubMed, CINAHL, Web of Science and PsycInfo were searched as part of a scoping review. Additionally, Google Search and Google Scholar were used to identify "grey literature". All German and English language articles related to inpatient palliative and hospice care were included which were published between January 2020 and August 2021.ResultsA total of 51 publications were included in the analysis. The identified measures can be clustered into 10 different categories: infection control measures, structural measures, visiting regulations, communication structures, education and training, psychosocial support measures, specific considerations for people with dementia, advance care planning, farewell and measures after death.ConclusionWith respect to addressing the COVID-19 pandemic, the primary focus is on measures to enable communication between patients, staff and relatives, psychosocial support measures, structural measures and recommendations for training and education of all stakeholders. Measures for the palliative care of people with dementia in times of a pandemic as well as the farewell and grief management of bereaved families received less attention.
