Simulating the progression of the COVID-19 disease in Cameroon using SIR models.

ABSTRACT: This paper analyses the evolution of COVID-19 in Cameroon over the period March 6-April 2020 using SIR models. Specifically, we 1) evaluate the basic reproduction number of the virus, 2) determine the peak of the infection and the spread-out period of the disease, and 3) simulate the interventions of public health authorities. Data used in this study is obtained from the Cameroonian Public Health Ministry. The results suggest that over the identified period, the reproduction number of COVID-19 in Cameroon is about 1.5, and the peak of the infection should have occurred at the end of May 2020 with about 7.7% of the population infected. Furthermore, the implementation of efficient public health policies could help flatten the epidemic curve.

Simulating SIR processes on networks using weighted shortest paths.

Project description:We present a framework to simulate SIR processes on networks using weighted shortest paths. Our framework maps the SIR dynamics to weights assigned to the edges of the network, which can be done for Markovian and non-Markovian processes alike. The weights represent the propagation time between the adjacent nodes for a particular realization. We simulate the dynamics by constructing an ensemble of such realizations, which can be done by using a Markov Chain Monte Carlo method or by direct sampling. The former provides a runtime advantage when realizations from all possible sources are computed as the weighted shortest paths can be re-calculated more efficiently. We apply our framework to three empirical networks and analyze the expected propagation time between all pairs of nodes. Furthermore, we have employed our framework to perform efficient source detection and to improve strategies for time-critical vaccination.

Incorporating global dynamics to improve the accuracy of disease models: Example of a COVID-19 SIR model.

Project description:Mathematical models of infectious diseases exhibit robust dynamics, such as stable endemic, disease-free equilibriums or convergence of the solutions to periodic epidemic waves. The present work shows that the accuracy of such dynamics can be significantly improved by including global effects of host movements in disease models. To demonstrate improved accuracy, we extended a standard Susceptible-Infected-Recovered (SIR) model by incorporating the global dynamics of the COVID-19 pandemic. The extended SIR model assumes three possibilities for susceptible individuals traveling outside of their community: • They can return to the community without any exposure to the infection. • They can be exposed and develop symptoms after returning to the community. • They can be tested positively during the trip and remain quarantined until fully recovered. To examine the predictive accuracy of the extended SIR model, we studied the prevalence of the COVID-19 infection in six randomly selected cities and states in the United States: Kansas City, Saint Louis, San Francisco, Missouri, Illinois, and Arizona. The extended SIR model was parameterized using a two-step model-fitting algorithm. The extended SIR model significantly outperformed the standard SIR model and revealed oscillatory behaviors with an increasing trend of infected individuals. In conclusion, the analytics and predictive accuracy of disease models can be significantly improved by incorporating the global dynamics of the infection.

COVID-19 Pandemic Data Modeling in Pakistan Using Time-Series SIR.

Project description:Pakistan is currently facing the fourth wave of the deadly coronavirus, which was first reported in Wuhan, China, in December 2019. This work utilizes the epidemiological models to analyze Pakistan's COVID-19 data. The basic susceptible, infected, and recovered (SIR) model is studied assuming Bayesian and time-series SIR (tSIR) approaches. Many studies have been conducted from different perspectives, but to the best of our knowledge, no study is available using the SIR models for Pakistan. The coronavirus incubation period has been set to 14 days across the globe; however, this study noticed that the assumption of 14 days is not suitable for Pakistan's data. Furthermore, on the basis of R 0, we infer that COVID-19 is not a pandemic in Pakistan, as it was in other nations, such as the United States, India, Brazil, and Italy, among others. We attribute this to the best strategy adopted by the Government of Pakistan to minimize the burden of COVID-19 cases in Pakistani hospitals. It is also noticed that the posterior-based SIR (pSIR) model with uniform prior toR 0 and Poisson distribution (of log-likelihood) provides better results as compared to other distributions. From time-series SIR (tSIR), we observed that the value of the reporting rate (ρ) is less than 1 which means that cases are underreported.

Mechanistic Models of COVID-19: Insights into Disease Progression, Vaccines, and Therapeutics.

Project description:The COVID-19 pandemic has severely impacted health systems and economies worldwide. Significant global efforts are therefore ongoing to improve vaccine efficacies, optimize vaccine deployment, and develop new antiviral therapies to combat the pandemic. Mechanistic viral dynamics and quantitative systems pharmacology models of SARS-CoV-2 infection, vaccines, immunomodulatory agents, and antiviral therapeutics have played a key role in advancing our understanding of SARS-CoV-2 pathogenesis and transmission, the interplay between innate and adaptive immunity to influence the outcomes of infection, effectiveness of treatments, mechanisms and performance of COVID-19 vaccines, and the impact of emerging SARS-CoV-2 variants. Here, we review some of the critical insights provided by these models and discuss the challenges ahead.

COVID-19 vaccination simulating lymph node progression in a patient with prostate cancer.

Project description:Several cases of cancer patients with 18-fluorodeoxyglucose (18FDG) Positron Emission Tomography/Computed Tomography (PET/CT) evidence of metabolically active axillary lymph nodes after COVID-19 vaccination have been described, creating a diagnostic dilemma and sometimes leading to further unnecessary examinations. A 62-year-old male, diagnosed with prostate cancer, treated with hormone-therapy and radiotherapy of the prostate 2 years before, underwent fluorine-18 choline (F-FCH) PET/CT for restaging purpose, less than 3 weeks after he had received the second dose of the Pfizer BioNTech-BNT162b2 mRNA COVID-19 vaccine. This exam showed an increased F-FCH uptake and an enlargement of the left axillary, paratracheal, para-aortic, subcarinal, and hilar bilateral lymph nodes. Fourteen weeks later, the patient underwent a new F-FCH PET-CT scan, displaying an almost complete regularization of the FCH uptake in all the previously involved regions. The patient was not treated after the first PET-CT scan, thus, the aforementioned PET/CT findings represented inflammatory vaccine-related lymph nodes. This case highlights the significance of knowing vaccination history to correctly interpret imaging findings and to avoid false-positive reports.

Simulating heterogeneous populations using Boolean models.

Project description:BackgroundCertain biological processes, such as the development of cancer and immune activation, can be controlled by rare cellular events that are difficult to capture computationally through simulations of individual cells. Information about such rare events can be gleaned from an attractor analysis, for which a variety of methods exist (in particular for Boolean models). However, explicitly simulating a defined mixed population of cells in a way that tracks even the rarest subpopulations remains an open challenge.ResultsHere we show that when cellular states are described using a Boolean network model, one can exactly simulate the dynamics of non-interacting, highly heterogeneous populations directly, without having to model the various subpopulations. This strategy captures even the rarest outcomes of the model with no sampling error. Our method can incorporate heterogeneity in both cell state and, by augmenting the model, the underlying rules of the network as well (e.g., introducing loss-of-function genetic alterations). We demonstrate our method by using it to simulate a heterogeneous population of Boolean networks modeling the T-cell receptor, spanning ∼ 1020 distinct cellular states and mutational profiles.ConclusionsWe have developed a method for using Boolean models to perform a population-level simulation, in which the population consists of non-interacting individuals existing in different states. This approach can be used even when there are far too many distinct subpopulations to model individually.

Predicting severity in COVID-19 disease using sepsis blood gene expression signatures

Project description:Severely-afflicted COVID-19 patients can exhibit disease manifestations representative of sepsis, including acute respiratory distress syndrome and multiple organ failure. We hypothesized that diagnostic tools used in managing all-cause sepsis, such as clinical criteria, biomarkers, and gene expression signatures, should extend to COVID-19 patients. Here we analyzed the whole blood transcriptome of 124 early (1-5 days post-hospital admission) and late (6-20 days post-admission) sampled patients with confirmed COVID-19 infections from hospitals in Quebec, Canada. Mechanisms associated with COVID-19 severity were identified between severity groups (ranging from mild disease to the requirement for mechanical ventilation and mortality), and established sepsis signatures were assessed for dysregulation. Specifically, gene expression signatures representing pathophysiological events, namely cellular reprogramming, organ dysfunction, and mortality, were significantly enriched and predictive of severity and lethality in COVID-19 patients. Mechanistic endotypes reflective of distinct sepsis aetiologies and therapeutic opportunities were also identified in subsets of patients, enabling prediction of potentially-effective repurposed drugs. The expression of sepsis gene expression signatures in severely-afflicted COVID-19 patients indicates that these patients should be classified as having severe sepsis. Accordingly, in severe COVID-19 patients, these signatures should be strongly considered for the mechanistic characterization, diagnosis, and guidance of treatment using repurposed drugs.
