In this paper, we evaluate 11 measures of inequality, d(p1, p2), between 2 proportions p1 and p2, some of which are new to the health disparities literature. These measures are selected because they are continuous, nonnegative, equal to 0 if and only if |p1 - p2| = 0, and maximal when |p1 - p2| = 1. They are also symmetrical [d(p1, p2) = d(p2, p1)] and complement-invariant [d(p1, p2) = d(1 - p2, 1 - p1)]. Project description:ObjectivesDescribe out-of-pocket payment (OOP) and the proportion of Peruvian households with catastrophic health expenditure (CHE) and evaluate changes in socioeconomic inequalities in CHE between 2008 and 2017.MethodsWe used data from the 2008 and 2017 National Household Surveys on Living and Poverty Conditions (ENAHO in Spanish), which are based on probabilistic stratified, multistage and independent sampling of areas. OOP was converted into constant dollars of 2017. A household with CHE was assumed when the proportion between OOP and payment capacity was ≥0.40. OOP was described by median and interquartile range while CHE was described by weighted proportions and 95% confidence intervals (CIs). To estimate the socioeconomic inequality in CHE we computed the Erreygers concentration index.ResultsThe median OOP reduced from 205.8 US dollars to 158.7 US dollars between 2008 and 2017. The proportion of CHE decreased from 4.9% (95% CI, 4.5 to 5.2) in 2008 to 3.7% (95% CI, 3.4 to 4.0) in 2017. Comparison of socioeconomic inequality of CHE showed no differences between 2008 and 2017, except for rural households in which CHE was less concentrated in richer households (p<0.05) and in households located on the rest of the coast, showing an increase in the concentration of CHE in richer households (p<0.05).ConclusionsAlthough OOP and CHE reduced between 2008 and 2017, there is still socioeconomic inequality in the burden of CHE across different subpopulations. To reverse this situation, access to health resources and health services should be promoted and guaranteed to all populations.

Project description:Scholars and university administrators have a vested interest in building equitable valuation systems of academic work for both practical (e.g., resource distribution) and more lofty purposes (e.g., what constitutes "good" research). Well-established inequalities in science pose a difficult challenge to those interested in constructing a parsimonious and fair method for valuation as stratification occurs within academic disciplines, but also between them. The h-index, a popular research metric, has been formally used as one such method of valuation. In this article, we use the case of the h-index to examine how the distribution of research metrics reveal within and between discipline inequalities. Using bibliometric data from 1960-2019 on over 50,000 high performing scientists-the top 2% most frequently cited authors-across 174 disciplines, we construct random effects within-between models predicting the h-index. Results suggest significant within-discipline variation in several forms, specifically sole-authorship and female penalties. Results also show that a sole authorship penalty plays a significant role in well-known between-discipline variation. Field-specific models emphasize the "apples-to-oranges," or incommensurable, property of cross-discipline comparison with significant heterogeneity in sole-authorship and female penalties within fields. In conclusion, we recommend continued caution when using the h-index or similar metrics for valuation purposes and the prioritization of substantive valuations from disciplinary experts.

Project description:An influential policy idea states that reducing inequality is beneficial for improving health in the low and middle income countries (LMICs). Our study provides an empirical test of this idea: we utilized data collected by the Demographic and Health Surveys between 2000 and 2011 in as much as 52 LMICs, and we examined the relationship between household wealth inequality and two health outcomes: anemia status (of the children and their mothers) and the women' experience of child mortality. Based on multi-level analyses, we found that higher levels of household wealth inequality related to worse health, but this effect was strongly reduced when we took into account the level of individuals' wealth. However, even after accounting for the differences between individuals in terms of household wealth and other characteristics, in those LMICs with higher household wealth inequality more women experienced child mortality and more children were tested with anemia. This effect was partially mediated by the country's level and coverage of the health services and infrastructure. Furthermore, we found higher inequality to be related to a larger health gap between the poor and the rich in only one of the three examined samples. We conclude that an effective way to improve the health in the LMICs is to increase the wealth among the poor, which in turn also would lead to lower overall inequality and potential investments in public health infrastructure and services.

Project description:The Global Gender Gap Index is one of the best-known measures of national gender inequality, used by both academics and policy makers. We argue that that this measure has a number of problems and introduce a simpler measure of national levels of gender inequality. Our proposed measure is based on sex differences in the opportunity to lead a long healthy and satisfied life that is grounded on educational opportunities. The measure better captures variation in gender inequality than other measures, with inclusion of outcomes that can be favorable or unfavorable to either sex, not simply unfavorable to women. We focus on some of the most basic measures available for 134 countries from 2012-2016 (i.e., disadvantages in children's basic education, life satisfaction, and healthy life span) and we relate these to various measures, including the United Nations' Human Development Index. We found that low levels of human development are typically associated with disadvantages for girls and women, while medium and high levels of development are typically associated with disadvantages for boys and men. Countries with the highest levels of human development are closest to gender parity, albeit typically with a slight advantage for women. We argue that the disparities, when they are found, are related to the sexual division of labor (i.e., traditional gender roles) in poorly developed countries as well as the underinvestment in preventative health care in more developed nations.

Project description:The pulsatility of blood flow through cerebral arteries is clinically important, as it is intrinsically associated with cerebrovascular health. In this study we outline a new MRI approach to measuring the real-time pulsatile flow in cerebral arteries, which is based on the inflow phenomenon associated with fast gradient-recalled-echo acquisitions. Unlike traditional phase-contrast techniques, this new method, which we dub dynamic inflow magnitude contrast (DIMAC), does not require velocity-encoding gradients as sensitivity to flow velocity is derived purely from the inflow effect. We achieved this using a highly accelerated single slice EPI acquisition with a very short TR (15 ms) and a 90° flip angle, thus maximizing inflow contrast. We simulate the spoiled GRE signal in the presence of large arteries and perform a sensitivity analysis. The sensitivity analysis demonstrates that in the regime of high inflow contrast, DIMAC shows much greater sensitivity to flow velocity over blood volume changes. We support this theoretical prediction with in-vivo data collected in two separate experiments designed to demonstrate the utility of the DIMAC signal contrast. We perform a hypercapnia challenge experiment in order to experimentally modulate arterial tone within subjects, and thus modulate the arterial pulsatile flow waveform. We also perform a thigh-cuff release challenge, designed to induce a transient drop in blood pressure, and demonstrate that the continuous DIMAC signal captures the complex transient change in the pulsatile and non-pulsatile components of flow. In summary, this study proposes a new role for a well-established source of MR image contrast and demonstrates its potential for measuring both steady-state and dynamic changes in arterial tone.

Project description:BackgroundHealth inequality has been recognized as a problem all over the world. In China, the poor usually have less access to healthcare than the better-off, despite having higher levels of need. Since the proportion of the Chinese population living in urban areas increased tremendously with the urbanization movements, attention has been paid to the association between urban/rural residence and population health. It is important to understand the variation in health across income groups, and in particular to take into account the effects of urban/rural residence on the degree of income-related health inequalities.MethodsThis paper empirically assesses the magnitude of rural/urban disparities in income-related adult health status, i.e., self-assessed health (SAH) and physical activity limitation, using Concentration Indices. It then uses decomposition methods to unravel the causes of inequalities and their variations across urban and rural populations. Data from the China Health and Nutrition Survey (CHNS) 2006 are used.ResultsThe study finds that the poor are less likely to report their health status as "excellent or good" and are more likely to have physical activity limitation. Such inequality is more pronounced for the urban population than for the rural population. Results from the decomposition analysis suggest that, for the urban population, 76.47 per cent to 79.07 per cent of inequalities are driven by non-demographic/socioeconomic-related factors, among which income, job status and educational level are the most important factors. For the rural population, 48.19 per cent to 77.78 per cent of inequalities are driven by non-demographic factors. Income and educational attainment appear to have a prominent influence on inequality.ConclusionThe findings suggest that policy targeting the poor, especially the urban poor, is needed in order to reduce health inequality.

Project description:BackgroundPopulation health indices such as disability adjusted life years (DALY) and quality adjusted life years (QALY) are often used in an effort to measure health of populations and identify areas of concern that require interventions. There has been an increase of number of population health indices since the last review published more than a decade ago. Therefore, this study aims to provide an overview of existing population health indices and examine the methods used to develop them.MethodsThe search was conducted across three databases: PubMed, CINAHL and Emerald using four key concepts: 'health', 'index', 'context', 'develop', which was supplemented with Google searching and reference scanning. A researcher screened the titles, abstracts and subsequently full texts and confirmed the findings with the research team at each stage. Data charting was performed according to the included publications and identified indices. The collation was performed by describing the indices and made observation on its development method using a priori framework consist of four processes: underpinning theory, model or framework; data selection and processing; formation of index; testing of index.ResultsTwenty-six publications describing population health indices were included, and 27 indices were identified. These indices covered the following health topics: overall health outcomes (n = 15), outcomes for specific health topics (n = 4), diseases outcome (n = 6), assist health resource allocation for priority minority subgroup or geographic area (n = 4), quality of health or health care (n = 2). Twenty-one indices measure health for general populations while six measure defined subpopulations. Fourteen of the indices reported at least one of the development processes according to the a priori framework: underpinning theory, model or framework (n = 7); data selection and processing (n = 8); formation of index (n = 12); testing of index (n = 9).ConclusionsFew population health indices measure specific health topics or health of specific sub-population. There is also a lack of usage of theories, models or framework in developing these indices. Efforts to develop a guideline is proposed on how population health indices can be developed systematically and rigorously to ensure validity and comprehensive assessment of the indices.

Project description:IntroductionMany regions in the world are using the population health approach and require a means to measure the health of their population of interest. Population health frameworks provide a theoretical grounding for conceptualization of population health and therefore a logical basis for selection of indicators. The aim of this scoping review was to provide an overview and summary of the characteristics of existing population health frameworks that have been used to conceptualize the measurement of population health.MethodsWe used the Population, Concept and Context (PCC) framework to define eligibility criteria of frameworks. We were interested in frameworks applicable for general populations, that contained components of measurement of health with or without its antecedents and applied at the population level or used a population health approach. Eligible reports of eligible frameworks should include at least domains and subdomains, purpose, or indicators. We searched 5 databases (Pubmed, EMBASE, Web of Science, NYAM Grey Literature Report, and OpenGrey), governmental and organizational sites on Google and websites of selected organizations using keywords from the PCC framework. Characteristics of the frameworks were summarized descriptively and narratively.ResultsFifty-seven frameworks were included. The majority originated from the US (46%), Europe (23%) and Canada (19%). Apart from 1 framework developed for rural populations and 2 for indigenous populations, the rest were for general urban populations. The numbers of domains, subdomains and indicators were highly variable. Health status and social determinants of health were the most common domains across all frameworks. Different frameworks had different priorities and therefore focus on different domains.ConclusionKey domains common across frameworks other than health status were social determinants of health, health behaviours and healthcare system performance. The results in this review serve as a useful resource for governments and healthcare organizations for informing their population health measurement efforts.

Project description:BackgroundLittle is known about the level of burnout among program administrators (PAs) working in graduate medical education.ObjectiveWe created a national database with baseline burnout data for PAs from residency and fellowship programs, including intention to leave their current positions.MethodsA cross-sectional study was conducted in July 2017 to assess levels of burnout in a national cohort of PAs, who were largely members of online specialty forums. The Copenhagen Burnout Inventory (CBI) was used to measure burnout. Univariate analysis produced descriptive statistics for CBI. We performed a 2-sample t test to measure differences in average burnout scores for those who had thoughts of resigning from their positions and those who had not.ResultsOf the approximately 10 205 national PAs, we sampled 1126 (11%). Of the 1126 individuals who received the study information, 931 (83%) completed the baseline survey. Total mean scores for all subscales were elevated (personal: 53.7, SD 21.4; work-related: 52.0, SD 22; and client-related: 30.6, SD 20.8; each scale ranged from 0, low, to 100, high). Burnout scores differed between those contemplating leaving their jobs and those who were not, across all subscales of CBI, including personal (64.2 versus 42.4, -24.18 to -19.44 confidence interval [CI]), work-related (63.5 versus 39.7, -26.12 to -21.35 CI), and client-related (36.6 versus 24.2, -14.95 to -9.84 CI; P < .0001 for all).ConclusionsIn this national survey of PAs, burnout scores measured by the CBI were higher among those who had considered leaving their positions.

Project description:BACKGROUND:Ensuring equal access to care and providing financial risk protection are at the center of the global health agenda. While Rwanda has made impressive progress in improving health outcomes, inequalities in medical care utilization and household catastrophic health spending (HCHS) between the impoverished and non-impoverished populations persist. Decomposing inequalities will help us understand the factors contributing to inequalities and design effective policy instruments in reducing inequalities. This study aims to decompose the inequalities in medical care utilization among those reporting illnesses and HCHS between the poverty and non-poverty groups in Rwanda. METHODS:Using the 2005 and 2010 nationally representative Integrated Living Conditions Surveys, our analysis focuses on measuring contributions to inequalities from poverty status and other sources. We conducted multivariate logistic regression analysis to obtain poverty's contribution to inequalities by controlling for all observed covariates. We used multivariate nonlinear decomposition method with logistic regression models to partition the relative and absolute contributions from other sources to inequalities due to compositional or response effects. RESULTS:Poverty status accounted for the majority of inequalities in medical care utilization (absolute contribution 0.093 in 2005 and 0.093 in 2010) and HCHS (absolute contribution 0.070 in 2005 and 0.032 in 2010). Health insurance status (absolute contribution 0.0076 in 2005 and 0.0246 in 2010) and travel time to health centers (absolute contribution 0.0025 in 2005 and 0.0014 in 2010) were significant contributors to inequality in medical care utilization. Health insurance status (absolute contribution 0.0021 in 2005 and 0.0011 in 2010), having under-five children (absolute contribution 0.0012 in 2005 and 0.0011 in 2010), and having disabled family members (absolute contribution 0.0002 in 2005 and 0.0001 in 2010) were significant contributors to inequality in HCHS. Between 2005 and 2010, the main sources of the inequalities remained unchanged. CONCLUSIONS:Expanding insurance coverage and reducing travel time to health facilities for those living in poverty could be used as policy instruments to mitigate inequalities in medical care utilization and HCHS between the poverty and non-poverty groups.
