In this article, we conduct the first-ever systematic study of Australian aid project appraisals. Using a previously unstudied data set of appraisals, we study project and recipient country factors influencing Australian aid effectiveness. We find effectiveness varies more within recipient countries than between countries. We find larger projects are more likely to be successful. Project description:Since 2013, Australian aid has been reduced and increasingly focused on delivering benefits to Australia. Motivated by these changes, this paper fills three gaps in the existing literature on public opinion about aid. It provides the only recent detailed study of Australians' opinions about aid. It studies specific policy questions in addition to the broader questions typical of international research. And it studies views on the purpose of aid, an area not previously researched. Although Australians are generally supportive of aid, most backed major aid cuts in 2015. However, most Australians think the purpose of Australian aid should be helping people in poor countries, not bringing benefits to Australia. There is a clear left-right divide in responses to all questions; however, some variables correlated with support for aid fail to explain variation in views about aid's purpose. The paper concludes by discussing ramifications for those who seek to change aid policy.

Project description:ObjectivesHealthcare budgets are limited, and therefore, research funds should be wisely allocated to ensure high-quality, useful and cost-effective research. We aimed to critically review the criteria considered by major Australian organisations in prioritising and selecting health research projects for funding.MethodsWe reviewed all grant schemes listed on the Australian Competitive Grants Register that were health-related, active in 2017 and with publicly available selection criteria on the funders' websites. Data extracted included scheme name, funding organisation, selection criteria and the relative weight assigned to each criterion. Selection criteria were grouped into five representative domains: relevance, appropriateness, significance, feasibility (including team quality) and cost-effectiveness (ie, value for money).ResultsThirty-six schemes were included from 158 identified. One-half of the schemes were under the National Health and Medical Research Council. The most commonly used criteria were research team quality and capability (94%), research plan clarity (94%), scientific quality (92%) and research impact (92%). Criteria considered less commonly were existing knowledge (22%), fostering collaboration (22%), research environment (19%), value for money (14%), disease burden (8%) and ethical/moral considerations (3%). In terms of representative domains, relevance was considered in 72% of the schemes, appropriateness in 92%, significance in 94%, feasibility in 100% and cost-effectiveness in 17%. The relative weights for the selection criteria varied across schemes with 5%-30% for relevance, 20%-60% for each appropriateness and significance, 20%-75% for feasibility and 15%-33% for cost-effectiveness.ConclusionsIn selecting research projects for funding, Australian research organisations focus largely on research appropriateness, significance and feasibility; however, value for money is most often overlooked. Research funding decisions should include an assessment of value for money in order to maximise return on research investment.

Project description:This article addresses the timely subject of the reactions toward a Sunday trade ban in Poland. The law introduced in March 2018 created a division among service employees: (1) those who used to work on Sundays before the law and now enjoy work-free Sundays, and (2) those who used to work and still have to work on Sundays. Although the objective circumstances did not change for this latter group, their current status quo (0) now had a new better state (+1) as a contrast reference point. Hence, this group experiences a non-gain rather than a loss. Using the framework of regulatory focus (Higgins, 1997) and regulatory fit (Higgins, 2000) theories, we predicted that the individuals in the non-gain condition would process a promotion (vs. prevention)-framed message more fluently. We also predicted that processing fluency would enhance fairness perceptions among these employees. To test these predictions, we conducted a field experiment, manipulating message framing among two groups of service employees: those who gained and those who did not gain as a result the Sunday trade ban. Analysis of variance revealed that employees in a non-gain promotion framing condition processed the message more fluently than those in a prevention framing condition. A moderated mediation analysis also showed that the processing fluency resulting from fit created higher fairness perceptions. Repercussions for communicating about organizational non-gain changes are discussed.

Project description:Australian bat lyssavirus (ABLV) is a known causative agent of neurological disease in bats, humans and horses. It has been isolated from four species of pteropid bats and a single microbat species (Saccolaimus flaviventris). To date, ABLV surveillance has primarily been passive, with active surveillance concentrating on eastern and northern Australian bat populations. As a result, there is scant regional ABLV information for large areas of the country. To better inform the local public health risks associated with human-bat interactions, this study describes the lyssavirus prevalence in microbat communities in the South West Botanical Province of Western Australia. We used targeted real-time PCR assays to detect viral RNA shedding in 839 oral swabs representing 12 species of microbats, which were sampled over two consecutive summers spanning 2016?2018. Additionally, we tested 649 serum samples via Luminex® assay for reactivity to lyssavirus antigens. Active lyssavirus infection was not detected in any of the samples. Lyssavirus antibodies were detected in 19 individuals across six species, with a crude prevalence of 2.9% (95% CI: 1.8?4.5%) over the two years. In addition, we present the first records of lyssavirus exposure in two Nyctophilus species, and Falsistrellus mackenziei.

Project description:BackgroundAustralian national mental health policy outlines the need for a nationally coordinated strategy to address stigma and discrimination, particularly towards people with complex mental illness that is poorly understood in the community. To inform implementation of this policy, this review aimed to identify and examine the effectiveness of existing Australian programs or initiatives that aim to reduce stigma and discrimination.MethodPrograms were identified via a search of academic databases and grey literature, and an online survey of key stakeholder organisations. Eligible programs aimed to reduce stigma towards people with complex mental illness, defined as schizophrenia, psychosis, personality disorder, or bipolar disorder; or they focused on nonspecific 'mental illness' but were conducted in settings relevant to individuals with the above diagnoses, or they included the above diagnoses in program content. Key relevant data from programs identified from the literature search and survey were extracted and synthesized descriptively.ResultsWe identified 61 programs or initiatives currently available in Australia. These included face-to-face programs (n = 29), online resources (n = 19), awareness campaigns (n = 8), and advocacy work (n = 5). The primary target audiences for these initiatives were professionals (health or emergency), people with mental illness, family or carers of people with mental illness, and members of the general population. Most commonly, programs tended to focus on stigma towards people with non-specific mental illness rather than on particular diagnostic labels. Evidence for effectiveness was generally lacking. Face-to-face programs were the most well-evaluated, but only two used a randomised controlled trial design.ConclusionsThis study identified areas of strength and weakness in current Australian practice for the reduction of stigma towards people with complex mental illness. Most programs have significant input from people with lived experience, and programs involving education and contact with a person with mental illness are a particular strength. Nevertheless, best-practice programs are not widely implemented, and we identified few programs targeting stigma for people with mental illness and their families, or for culturally and linguistically diverse communities, Aboriginal and Torres Strait Islander communities and LGBTIQ people. These can inform stakeholder consultations on effective options for a national stigma and discrimination reduction strategy.

Project description:Background:Working dog handlers and breeders have very different behavioural requirements in the animals that they employ for managing livestock. The Australian Working Kelpie breed may be used in several working contexts, notably yards, paddocks and a combination of both. The working context influences the skillsets required and gives rise to three corresponding work-types: Yard, Paddock and Utility Kelpies. In particular, dogs used for working stock in the confines of yards and trucks interact with stock more forcefully than those mustering in larger areas (paddocks) where they can herd stock effectively from a greater distance. This article explores owner assessments of dog working quality and assessment of genomic similarity by multidimensional scaling, to ask whether it is sufficient for breeders to aim for a multipurpose breeding objective, or whether breeding only specialist lines maximises user satisfaction for yard and paddock work. Results:Reported owner perceptions of 298 dogs assessed with the Livestock Herding Dog assessment tool showed that dog handlers across all working types were very happy with their dogs' level of general skills.Compared with both Yard and Utility Kelpies, Paddock Kelpies had significantly lower trait scores for force (pressure applied by the dog to move livestock), willingness to back the stock (run along a sheep's dorsum) and bite (frequency of using the mouth to grab or bite the livestock). Meanwhile, compared with both Paddock and Utility Kelpies, the Yard Kelpies had significantly higher scores for hyperactivity and excitability (both with and without stock) and impulsiveness without stock. As one would predict for all-rounders, Utility Kelpies had intermediate scores for all behaviours and working traits. Conclusions:Specialist characteristics were displayed by dogs in the Yard Kelpie and Paddock Kelpie groups. In particular, Yard Kelpies demonstrate higher excitability, willingness to back the stock, and a higher tendency to bark and bite the stock. Conversely, Paddock Kelpies rarely display these characteristics. Utility Kelpies, as the name suggests, are intermediate between the other two groups and display the characteristics of both. Genetic analysis suggests that the Yard, Utility and Paddock Kelpies are not distinguishable at a DNA level. In conclusion, at this time there is no suggestion of a breed split in the Australian Working Kelpie generated by selection for work type. A common breeding objective should enable dogs to be produced that fulfil all potential working requirements. This reinforces the importance of breeder skill in recognising the phenotypic potential of pups in order to place them in appropriate working contexts.

Project description:BACKGROUND:Australian bat lyssavirus (ABLV) belongs to the genus Lyssavirus which also includes classic rabies virus and the European lyssaviruses. To date, the only three known human ABLV cases, all fatal, have been reported from Queensland, Australia. ABLV is widely distributed in Australian bats, and any bite or scratch from an Australian bat is considered a potential exposure to ABLV. METHODOLOGY/PRINCIPAL FINDINGS:Potential exposure to ABLV has been a notifiable condition in Queensland since 2005. We analysed notification data for potential exposures occurring between 2009 and 2014. There were 1,515 potential exposures to ABLV notified in Queensland, with an average annual notification rate of 5.6 per 100,000 population per year. The majority of notified individuals (96%) were potentially exposed to ABLV via bats, with a small number of cases potentially exposed via two ABLV infected horses and an ABLV infected human. The most common routes of potential exposure were through bat scratches (47%) or bites (37%), with less common routes being mucous membrane/broken skin exposure to bat saliva/brain tissue (2.2%). Intentional handling of bats by the general public was the major cause of potential exposures (56% of notifications). Examples of these potential exposures included people attempting to rescue bats caught in barbed wire fences/fruit tree netting, or attempting to remove bats from a home. Following potential exposures, 1,399 cases (92%) were recorded as having appropriate post-exposure prophylaxis (PEP) as defined in national guidelines, with the remainder having documentation of refusal or incomplete PEP. Up to a quarter of notifications occurred after two days from the potential exposure, but with some delays being more than three weeks. Of 393 bats available for testing during the reporting period, 20 (5.1%) had ABLV detected, including four species of megabats (all flying foxes) and one species of microbats (yellow-bellied sheathtail bat). CONCLUSIONS/SIGNIFICANCE:Public health strategies should address the strong motivation of some members of the public to help injured bats or bats in distress, by emphasising that their action may harm the bat and put themselves at risk of the fatal ABLV infection. Alternative messaging should include seeking advice from professional animal rescue groups, or in the event of human contact, public health units. Further efforts are required to ensure that when potential exposure occurs, timely reporting and appropriate post-exposure prophylaxis occur.

Project description:BackgroundCaesarean sections (CS) rates continue to increase worldwide without a clear understanding of the main drivers and consequences. The lack of a standardized internationally-accepted classification system to monitor and compare CS rates is one of the barriers to a better understanding of this trend. The Robson's 10-group classification is based on simple obstetrical parameters (parity, previous CS, gestational age, onset of labour, fetal presentation and number of fetuses) and does not involve the indication for CS. This classification has become very popular over the last years in many countries. We conducted a systematic review to synthesize the experience of users on the implementation of this classification and proposed adaptations.MethodsFour electronic databases were searched. A three-step thematic synthesis approach and a qualitative metasummary method were used.Results232 unique reports were identified, 97 were selected for full-text evaluation and 73 were included. These publications reported on the use of Robson's classification in over 33 million women from 31 countries. According to users, the main strengths of the classification are its simplicity, robustness, reliability and flexibility. However, missing data, misclassification of women and lack of definition or consensus on core variables of the classification are challenges. To improve the classification for local use and to decrease heterogeneity within groups, several subdivisions in each of the 10 groups have been proposed. Group 5 (women with previous CS) received the largest number of suggestions.ConclusionsThe use of the Robson classification is increasing rapidly and spontaneously worldwide. Despite some limitations, this classification is easy to implement and interpret. Several suggested modifications could be useful to help facilities and countries as they work towards its implementation.

Project description:On-call work arrangements are commonly utilised in the emergency services sector and are consistency associated with inadequate sleep. Despite sleep being a common shared behaviour, studies are yet to assess the impact of on-call work on the sleep of co-sleeping partners. This study aimed to investigate whether frequent 24/7 on-call work impacted the sleep and relationship happiness of firefighters' partners. Two key research questions were investigated: (1) Does the frequency of calls impact sleep and relationship happiness? and, (2) Does the (a) sleep quantity and (b) sleep quality of partners impact perceived relationship happiness? A cross-sectional study was conducted using an online questionnaire completed by partners of on-call workers (n = 66; 93% female). The questionnaire included items on (i) sleep quantity and quality, (ii) on-call sleep disturbances and, (iii) relationship happiness. Responses were analysed using logistic regression models. Higher overnight call frequency was associated with greater self-reported levels of inadequate sleep (<7 h per night; p = 0.024). Support for continuance of a firefighter's role was less likely if the partner reported they regularly had trouble falling asleep within 30 min (p < 0.001). There were no other significant relationships between the frequency of calls or other sleep quantity or quality variables and relationship happiness. This study provides important first insights into how firefighters' on-call work arrangements impact partners' sleep. Future research is needed across periods of high and low call demand, using objective measures of sleep to further define the impacts of on-call work on partners' sleep.

Project description:Background:Working in good jobs is associated with good health. High unemployment rates are reported in those disabled with musculoskeletal pain. Supported employment interventions work well for helping people with mental health difficulties to gain and retain employment. With adaptation, these may be useful for people with chronic pain. We aimed to develop and explore the feasibility of delivering such an adapted intervention. Methods:We developed an intervention and recruited unemployed people with chronic pain from NHS pain clinics and employment services. We trained case managers to assess participants and match them to six-week work placements in the Midlands and provide ongoing support to them and their managers. Participants attended a two-day work preparation session prior to placement. Outcome measures included quality of life at baseline, six-?weeks, 14-weeks, and six-months, and return to work at 14-weeks and six-months. We held focus groups or interviews with stakeholders to examine acceptability and experiences of the intervention. Results:We developed an intervention consisting of work preparation sessions, work experience placements, and individualised employment support. We enrolled 31 people; 27 attended work preparation sessions, and 15 attended placements. Four of our participants started jobs during the study period. We are aware of two others starting jobs shortly after cessation of follow-up. We experienced challenges to recruitment in one area where we had many and diverse placement opportunities and good recruitment in another area where we had a smaller range of placement opportunities. All stakeholders found the intervention acceptable, and it was valued by those given a placement. While there was some disappointment among those not placed, this group still valued the work preparation sessions. Conclusions:The developed intervention was acceptable to participants and partners. Trialling the developed intervention could be feasible with attention to three main processes. To ensure advanced availability of a sufficiently wide range of work placements in each area, multiple partners would be needed. Multiple recruitment sites and focus on employment services will yield better recruitment rates than reliance on NHS pain clinics. Maintaining an adequate follow-up response rate will likely require additional approaches with more than the usual effort.
