Facial aesthetic injections in clinical practice: Pretreatment and posttreatment consensus recommendations to minimise adverse outcomes.

ABSTRACT: Facial aesthetic treatment with injectable neuromodulators and hyaluronic acid fillers is well established, with favourable safety profiles and consistent outcomes. As with any medical treatment, adverse events and complications may occur. Adverse events associated with these products are typically transient and mild to moderate in severity. Serious adverse events, such as infection and intravascular occlusion, are rare. Proper patient selection, consent and counselling, preparation and impeccable injection technique are important risk reduction strategies. Both clinicians and patients must be alert to the signs and symptoms of complications so that appropriate treatment can be started promptly. In this article, the authors review the current literature and provide their consensus recommendations for minimising adverse outcomes when treating patients with botulinum toxin or hyaluronic acid fillers.

Project description:Although preoperative chemoradiotherapy (CRT) and surgical mesorectal resection is the standard of care for locally advanced rectal carcinomas, it is still difficult to predict which patients will respond to treatment. We explored how differential stromal transcriptomic profiles from microdissected pretreatment rectal biopsies can be used to define an immunohistochemical score based on two CAF-specific proteins for predicting neoadjuvant treatment response. The analysis of differentially expressed genes (DEGs) of stroma and tumour glands from responder and non-responder patients shows that most changes were associated with the stromal compartment. Gene ontology analysis revealed that the DEGs codify mainly for extracellular matrix and ribosomal components. We built a CAF-specific classifier with genes showing monotonic changes in expression according to the tumour regression grade (coefficient >1; FN1, COL3A1, COL1A1, MMP2 and IGFBP5). These are the genes that display the biggest a priori differences in expression between non-responder and responder stroma. We translated these five genes at the protein level by means of immunohistochemical staining in a cohort of 38 patients. For predictive purposes we used a leave-one-out cross-validated (LOOCV) model with a positive predictive value (PPV) of 80%. Classifier optimisation with Random Forest identified FN1 and COL3A1 as the best predictors. Rebuilding the LOOCV regression model improved the classification performance with a PPV of 89.5% and a negative predictive value (NPV) of 73.7%. An independent cohort of 36 patients was used to validate the classifier performance, which had a PPV of 84.2% and an NPV of 70.6%. In a multivariate analysis the two-protein classifier proved to be the only independent predictor of response (HR=2.58; P=0.003). We also explored a pharmacogenomic approach to gather information about possible therapeutic strategies for non-responder patients. In conclusion, we developed a two-protein immunohistochemical classifier that performs well at predicting the absence of response to neoadjuvant treatment in rectal cancer.

Project description:ObjectiveScleromyxedema (SMX) is a rare systemic sclerosis mimic that often responds to intravenous immunoglobulin (IVIG) therapy, yet the resulting clinical and biochemical changes have not been well characterized. To better understand the pathogenesis of the disease and the efficacy of IVIG, we sought to explore whether IVIG would introduce a measurable biologic effect corresponding with clinical improvement.MethodsFifteen patients with SMX were recruited for the study. Clinical information and peripheral blood mononuclear cells for flow cytometry were obtained immediately before and again 1-2 weeks after patients received IVIG therapy. Ten patients also underwent skin biopsies for gene expression analysis both before and after IVIG therapy. Clinical data included measures of skin involvement (modification of the modified Rodnan skin thickness score [MMRSS] and percentage of body surface area) and several patient-reported outcome measures assessing patients' skin.ResultsPosttreatment, the average MMRSS score decreased from mean ± SD 13.6 ± 2.6 to 10.3 ± 1.9; P = 0.003. There were also significant improvements in skin flexibility (mean ± SD 5.4 ± 0.8 to 3.2 ± 0.7; P = 0.003) and softening (mean ± SD 4.9 ± 0.9 to 2.6 ± 0.6; P = 0.022). Baseline levels of Tc17 cells (CD8+CCR6+CXCR3+CCR4-) correlated with the extent of skin involvement as measured by MMRSS pretreatment (r = 0.69, P = 0.012) and decreased after IVIG therapy (mean ± SD 3.4% ± 3.2% to 1.3% ± 1.7%; P = 0.008). Posttreatment analysis of RNA in skin tissue revealed a decrease in gene expression of transforming growth factor β (TGFβ) cytokines as well as several interferon-inducible proteins.ConclusionThis open-label study further supports the evidence that patients with SMX respond both objectively and subjectively to IVIG therapy. Biologic studies suggest a role for T lymphocytes in the pathogenesis of the disease and reveal the potential significance of TGFβ and interferon pathways.
