ABSTRACT:

Background

Proposition analysis of the discourse of persons with aphasia has a long history, yielding important advancements in our understanding of communication impairments in this population. Recently, discourse measures have been considered primary outcome measures, and multiple calls have been made for improved psychometric properties of discourse measures.

Aims

To advance the use of discourse analysis in persons with aphasia by providing Main Concept Analysis checklists and descriptive statistics for healthy control performance on the analysis for the Cat in the Tree and Refused Umbrella narrative tasks utilized in the AphasiaBank database protocol.

Methods & procedures

Ninety-two control transcripts, stratified into four age groups (20-39 years; 40-59; 60-79; 80+), were downloaded from the AphasiaBank database. Relevant concepts were identified, and those spoken by at least one-third of the control sample were considered to be a main concept. A multi-level coding system was used to determine the accuracy and completeness of the main concepts produced by control speakers.

Outcomes & results

Main concept checklists for two discourse tasks are provided. Descriptive statistics are reported and examined to assist readers with evaluation of the normative data.

Conclusions

These checklists provide clinicians and researchers with a tool to reliably assess the discourse of persons with aphasia. They also help address the gap in available psychometric data with which to compare persons with aphasia to healthy controls.

Project description:BackgroundFunctional MRI (fMRI) tasks are increasingly being used to advance knowledge of the etiology and maintenance of obesity and eating disorders. Thus, understanding the test-retest reliability of BOLD signal contrasts from these tasks is important.ObjectivesTo evaluate test-retest reliability of responses in reward-related brain regions to food receipt paradigms (palatable tastes, anticipated palatable tastes), food picture paradigms (high-calorie food pictures), a monetary reward paradigm (winning money and anticipating winning money), and a thin female model picture paradigm (thin female model pictures).MethodWe conducted secondary univariate contrast-based analyses in data drawn from 4 repeated-measures fMRI studies. Participants (Study 1: N = 60, mean [M] age = 15.2 ± 1.1 y; Study 2: N = 109, M age = 15.1 ± 0.9 y; Study 3: N = 39, M age = 21.2 ± 3.7 y; Study 4: N = 62, M age = 29.7 ± 6.2 y) completed the same tasks over 3-wk to 3-y test-retest intervals. Studies 3 and 4 included participants with eating disorders and obesity, respectively.ResultsTest-retest reliability of the food receipt and food picture paradigms was poor, with average ICC values ranging from 0.07 to 0.20. The monetary reward paradigm and the thin female model picture paradigm also showed poor test-retest reliability: average ICC values 0.21 and 0.12, respectively. Although several regions demonstrated moderate to good test-retest reliability, these results did not replicate across studies using similar paradigms. In Studies 3 and 4, but not Study 1, test-retest reliability in visual processing regions was moderate to good when contrasting single conditions with a low-level baseline.ConclusionsResults underscore the importance of examining the temporal reliability of fMRI tasks and call for the development and use of well-validated standardized fMRI tasks in eating- and obesity-related studies that can provide more reliable measures of neural activation. Project description:Retrieval of semantic representations is a central process during overt speech production. There is an increasing consensus that an amodal semantic 'hub' must exist that draws together modality-specific representations of concepts. Based on the distribution of atrophy and the behavioral deficit of patients with the semantic variant of fronto-temporal lobar degeneration, it has been proposed that this hub is localized within both anterior temporal lobes (ATL), and is functionally connected with verbal 'output' systems via the left ATL. An alternative view, dating from Geschwind's proposal in 1965, is that the angular gyrus (AG) is central to object-based semantic representations. In this fMRI study we examined the connectivity of the left ATL and parietal lobe (PL) with whole brain networks known to be activated during overt picture description. We decomposed each of these two brain volumes into 15 regions of interest (ROIs), using independent component analysis. A dual regression analysis was used to establish the connectivity of each ROI with whole brain-networks. An ROI within the left anterior superior temporal sulcus (antSTS) was functionally connected to other parts of the left ATL, including anterior ventromedial left temporal cortex (partially attenuated by signal loss due to susceptibility artifact), a large left dorsolateral prefrontal region (including 'classic' Broca's area), extensive bilateral sensory-motor cortices, and the length of both superior temporal gyri. The time-course of this functionally connected network was associated with picture description but not with non-semantic baseline tasks. This system has the distribution expected for the production of overt speech with appropriate semantic content, and the auditory monitoring of the overt speech output. In contrast, the only left PL ROI that showed connectivity with brain systems most strongly activated by the picture-description task, was in the superior parietal lobe (supPL). This region showed connectivity with predominantly posterior cortical regions required for the visual processing of the pictorial stimuli, with additional connectivity to the dorsal left AG and a small component of the left inferior frontal gyrus. None of the other PL ROIs that included part of the left AG were activated by Speech alone. The best interpretation of these results is that the left antSTS connects the proposed semantic hub (specifically localized to ventral anterior temporal cortex based on clinical neuropsychological studies) to posterior frontal regions and sensory-motor cortices responsible for the overt production of speech.
