Graft Retensioning by Dialing of Suture Disk on Tibial-Side Fixation in Arthroscopic Anterior Cruciate Ligament Reconstruction: Pathak Pune Dialing Technique.

ABSTRACT: Arthroscopic anterior cruciate ligament (ACL) reconstruction is a common procedure performed for symptomatic ACL tears, especially in athletes. The desired surgical end product with any surgical fixation device remains a taut ACL graft, which is crucial during postoperative rehabilitation to reduce the risk of knee instability and rerupture of the ACL graft. The purpose of this Technical Note and accompanying video is to describe a simple and cost-effective technique to easily retension the ACL graft after tibial fixation in ACL reconstruction using a suture disk device. The technique uses a simple suture disk device to provide strong tibial fixation, along with the unique ability to retension the ACL graft by dialing it in a clockwise direction.

Arthroscopic 4-point suture fixation of anterior cruciate ligament tibial avulsion fractures.

Project description:Tibial eminence avulsion fractures are rare injuries occurring mainly in adolescents and young adults. When necessary, regardless of patient age, anatomic reduction and stable internal fixation are mandatory for fracture healing and accurate restoration of normal knee biomechanics. Various arthroscopically assisted fixation methods with sutures, anchors, wires, or screws have been described but can be technically demanding, thus elongating operative times. The purpose of this article is to present a technical variation of arthroscopic suture fixation of anterior cruciate ligament avulsion fractures. Using thoracic drain needles over 2.4-mm anterior cruciate ligament tibial guidewires, we recommend the safe and easy creation of four 2.9-mm tibial tunnels at different angles and at specific points. This technique uses thoracic drain needles as suture passage cannulas and offers 4-point fixation stability, avoiding potential complications of bony bridge fracture and tunnel connection.

Project description:Few cases of tibial spine avulsion injuries occur in adolescents. An open or arthroscopic surgical approach is indicated for displaced and nonreducible fractures, but evidence for a gold standard is insufficient. Various arthroscopic techniques are available. Suture fixation is popular and shows good results. The proposed technique is a modified suture-bridge fixation using 2 high-strength sutures tied through 2 transosseous tunnels. This simple and low-cost technique avoids the potential complications of hardware fixation within a joint. It represents an arthroscopic treatment option for anterior cruciate ligament tibial avulsion injuries.

Arthroscopic Suture-to-Loop Fixation of Posterior Cruciate Ligament Tibial Avulsion Fracture.

Project description:Avulsion fracture of the tibial insertion of the posterior cruciate ligament (PCL) receives constant concern. Arthroscopic procedures have long been attempted because of their minimally invasive nature, and various related techniques have been reported. However, the best arthroscopic method is still being pursued. In this article, we introduce an arthroscopic suture ligation and backup adjustable-loop fixation technique for PCL tibial avulsion fracture. The critical points of this technique are proper ligation of the PCL, proper location of the 2 tibial tunnels to create pulleys for posterior-inferior reduction of the bone fragment, and additional backup suture loop fixation. Our experience indicates that this technique is efficient and relatively simple. We consider that the introduction of this technique will provide a reasonable choice in the treatment of PCL tibial avulsion fracture.

Project description:Adult tibial avulsion fracture of the anterior cruciate ligament (ACL) occurs not as frequently as ACL tear but still is concerning. There are many methods of arthroscopic fixation of this special fracture. However, a simple and effective method is still being pursued. We would like to introduce an arthroscopic suture-to-adjustable loop fixation technique, which features are a reduction of lateral displacement of the fragment by special suture configuration and tensioning, and a final reduction of residual displacement by tensioning the adjustable loop. We consider the introduction of this technique will provide additional choice in the treatment of adult ACL tibial avulsion fracture.

Graft Retensioning Technique Using an Adjustable-Loop Fixation Device in Arthroscopic Anterior Cruciate Ligament Reconstruction.

Project description:Anterior cruciate ligament (ACL) tears are an unfortunate and devastating injury in the athletic patient. Surgical reconstruction of the ACL with a taut graft construct is necessary to restore knee stability and help expedite the patient's return to an active lifestyle. Arthroscopy-assisted ACL reconstruction can be a technically involved procedure, where strong fixation limits graft motion to allow sufficient healing within the tibial and femoral tunnels at the bone-tendon interfaces. Loss of tension in a graft can cause symptomatic anterior or rotatory instability and potential retear of the ACL graft. We describe in this Technical Note and accompanying video a simple and effective technique to implant and retension a tendon graft during ACL reconstruction using a suspensory fixation device. The technical description uses a simple adjustable-loop device to provide strong cortical fixation, along with the unique ability to retension the graft in ACL reconstruction, not otherwise possible with previously described graft fixation techniques.

Arthroscopic Fixation of Anterior Cruciate Ligament Avulsion Fracture Using FiberWire Suture With Suture Disc.

Project description:Controversy still exists regarding how to reduce and fix a displaced tibial avulsion fracture. Open reduction and internal fixation may lead to morbidity due to soft-tissue injury and arthrotomy. As a result, arthroscopic techniques are increasing in popularity. In the literature, sutures, K wires, and screws are suggested to be used as the fixation devices. Screws cannot be used in small or comminuted fractures, whereas K wires and sutures may not provide strong stability. Recently, with the advent of ultrahigh molecular weight polyethylene sutures like FiberWire, it has become possible to use it as the fixation device in even comminuted avulsion fractures with acceptable reduction stability. In this paper, we describe a simple arthroscopic technique using a FiberWire to manage the displaced tibial eminence avulsion fracture.

Suture Bridge Fixation for Posterior Cruciate Ligament Tibial Avulsion Fracture in Children.

Project description:Posterior cruciate ligament (PCL) tibial avulsion fractures in children are extremely rare. Due to the rarity of these injuries, careful attention to the specific physical examination and imaging findings is necessary for a proper diagnosis. PCL avulsion fractures can be missed on plain radiography in skeletally immature patients. Magnetic resonance imaging should be considered if sagging or posterior drawer sign is positive after a strong hit to the anterior aspect of the lower leg. With this knowledge, clinicians can formulate treatment plans that can return patients to their original functionality while avoiding potential morbidity from misdiagnoses. We treated these patients using the suture bridge method. In children, ossification is incomplete, and they possess a lot of cartilage, so screw fixation easily destroys avulsed fragment. The suture bridge method can firmly fix the avulsed fragments, reducing the risk of damage to the bone fragment; therefore, a secondary surgery for implant removal is not needed. Arthroscopic surgery also was expected to be technically difficult in children due to the limited scope of the operation. We used open fixation because the outcome was unaffected by open surgery and arthroscopic surgery, and all patients returned to full sporting activity postoperatively.

Arthroscopic Suture Fixation in Femoral-Sided Avulsion Fracture of Anterior Cruciate Ligament.

Project description:A femoral-sided avulsion fracture of the anterior cruciate ligament (ACL) is a rare and challenging condition. Most reported cases have occurred in childhood or adolescence. Many techniques of ACL repair have been reported, and in recent years, techniques in arthroscopic surgery have been developed and have become ever more popular with orthopaedic surgeons. We created a technique of arthroscopic ACL repair with suture anchor fixation for a femoral-sided ACL avulsion fracture. This technique saves the natural ACL stump. It is available for cases in which creation of a tibial tunnel is not allowed. Moreover, it does not require a skin incision for fixation on the far femoral cortex and, therefore, does not require a second operation to remove the fixation device. The arthroscopic technique also has a good cosmetic outcome.

Arthroscopic Assisted Anterior Cruciate Ligament Tibial Spine Avulsion Reduction and Cortical Button Fixation.

Project description:Tibial spine avulsion fractures, or tibial eminence fractures, are intra-articular knee injuries that affect the bony attachment of the anterior cruciate ligament (ACL). It is commonly seen in children and adolescents aged 8 to 15 years old and can be caused by noncontact pivot shift injuries or by traumatic hyperextension knee injuries, as seen in adult ACL patients. A thorough history and physical exam is important in these patients alongside proper imaging that will confirm the diagnosis of a tibial spine avulsion. Proper imaging may also demonstrate other associated conditions or injuries to the cartilage, meniscus, or ligamentous structures. Following diagnosis, treatment can be both nonoperative versus operative, depending upon the degree of displacement and reducibility of the fragment, as well as other concomitant injuries. For nondisplaced or minimally displaced, and reducible injuries, the patient can be immobilized in full extension for several weeks. For displaced fragments that are unable to be reduced by closed methods, open reduction internal fixation or arthroscopic fixation is recommended. In this Technical Note, we describe an arthroscopy-assisted reduction and internal fixation with suture tape through 2 transtibial tunnels with a cortical suture button fixation technique.
