The COVID-ASSESS dataset - COVID19 related anxiety and stress in prEgnancy, poSt-partum and breaStfeeding during lockdown in Italy.

ABSTRACT: The COVID-ASSESS questionnaire (COVID-19 related Anxiety and StreSs in prEgnancy, poSt-partum and breaStfeeding) was developed and distributed by CiaoLapo Foundation, an Italian charity for healthy pregnancy and perinatal loss support. Data were collected during phase 1 and phase 2 of COVID-19 lockdown in Italy (March, April and May 2020). The final dataset consists of 2448 women, of whom 1307 during pregnancy and 1141 women during post-partum or breastfeeding period. Variables collected for each subject are: sociodemographic and clinical information (previous losses, history of psychological disorders), birth expectations before and after COVID-19, concerns regarding pandemic consequences, perception of media and health professionals' information and communication on COVID-19, psychopathological assessment (anxiety, post-traumatic stress and general psychopathology).

Project description:Covid19-induced lockdown measures caused modifications in atmospheric pollutant and greenhouse gas emissions. Urban road traffic was the most impacted, with 48-60% average reduction in Italy. This offered an unprecedented opportunity to assess how a prolonged (∼2 months) and remarkable abatement of traffic emissions impacted on urban air quality. Six out of the eight most populated cities in Italy with different climatic conditions were analysed: Milan, Bologna, Florence, Rome, Naples, and Palermo. The selected scenario (24/02/2020-30/04/2020) was compared to a meteorologically comparable scenario in 2019 (25/02/2019-02/05/2019). NO2, O3, PM2.5 and PM10 observations from 58 air quality and meteorological stations were used, while traffic mobility was derived from municipality-scale big data. NO2 levels remarkably dropped over all urban areas (from -24.9% in Milan to -59.1% in Naples), to an extent roughly proportional but lower than traffic reduction. Conversely, O3 concentrations remained unchanged or even increased (up to 13.7% in Palermo and 14.7% in Rome), likely because of the reduced O3 titration triggered by lower NO emissions from vehicles, and lower NOx emissions over typical VOCs-limited environments such as urban areas, not compensated by comparable VOCs emissions reductions. PM10 exhibited reductions up to 31.5% (Palermo) and increases up to 7.3% (Naples), while PM2.5 showed reductions of ∼13-17% counterbalanced by increases up to ∼9%. Higher household heating usage (+16-19% in March), also driven by colder weather conditions than 2019 (-0.2 to -0.8 °C) may partly explain primary PM emissions increase, while an increase in agriculture activities may account for the NH3 emissions increase leading to secondary aerosol formation. This study confirmed the complex nature of atmospheric pollution even when a major emission source is clearly isolated and controlled, and the need for consistent decarbonisation efforts across all emission sectors to really improve air quality and public health.

Project description:BackgroundMaternal mental health problems often develop prenatally and predict post-partum mental health. However, the circumstances before and following childbirth differ considerably. We currently lack an understanding of dynamic variation in the profiles of depressive and anxiety symptoms over the perinatal period.MethodsDepressive and anxiety symptoms were self-reported by 980 women at 26-week pregnancy and 3 months post-partum. We used network analysis of depressive and anxiety symptoms to investigate if the symptoms network changed during and after pregnancy. The pre- and post-partum depressive-anxiety symptom networks were assessed for changes in structure, unique symptom-symptom interactions, central and bridging symptoms. We also assessed if central symptoms had stronger predictive effect on offspring's developmental outcomes outcomes at birth and 24, 54, and 72 months old than non-central symptoms. Bridging symptoms between negative and positive mental health were also assessed.ResultsThough the depressive-anxiety network structures were stable during and after pregnancy, the post-partum network was more strongly connected. The central depressive-anxiety symptoms were also different between prenatal and post-partum networks. During pregnancy, central symptoms were mostly related to feeling worthless or useless; after pregnancy, central symptoms were mostly related to feeling overwhelmed or being punished. Central symptoms during pregnancy were associated with poorer developmental outcomes for the child. Anxiety symptoms were strongest bridging symptoms during and after pregnancy. The interactions between negative and positive mental health symptoms were also different during and after pregnancy.ConclusionsThe differences between pre- and post-partum networks suggest that the presentation of maternal mental health problems varies over the peripartum period. This variation is not captured by traditional symptom scale scores. The bridging symptoms also suggest that anxiety symptoms may precede the development of maternal depression. Interventions and public health policies should thus be tailored to specific pre- and post-partum symptom profiles.
