Sequencing of Historical Isolates, K-mer Mining and High Serological Cross-Reactivity with Ross River Virus Argue against the Presence of Getah Virus in Australia.

ABSTRACT: Getah virus (GETV) is a mosquito-transmitted alphavirus primarily associated with disease in horses and pigs in Asia. GETV was also reported to have been isolated from mosquitoes in Australia in 1961; however, retrieval and sequencing of the original isolates (N544 and N554), illustrated that these viruses were virtually identical to the 1955 GETVMM2021 isolate from Malaysia. K-mer mining of the >40,000 terabases of sequence data in the Sequence Read Archive followed by BLASTn confirmation identified multiple GETV sequences in biosamples from Asia (often as contaminants), but not in biosamples from Australia. In contrast, sequence reads aligning to the Australian Ross River virus (RRV) were readily identified in Australian biosamples. To explore the serological relationship between GETV and other alphaviruses, an adult wild-type mouse model of GETV was established. High levels of cross-reactivity and cross-protection were evident for convalescent sera from mice infected with GETV or RRV, highlighting the difficulties associated with the interpretation of early serosurveys reporting GETV antibodies in Australian cattle and pigs. The evidence that GETV circulates in Australia is thus not compelling.

Project description:To evaluate the frequency of errors in the diagnosis of medical laboratory-diagnosed Chikungunya virus (CHIKV) infections in Australia, we studied 42 laboratory-diagnosed CHIKV serum samples from one Queensland medical laboratory by ELISA IgG/IgM and measured the specific neutralization antibodies (Nab) against Barmah Forest virus (BFV), CHIKV and Ross River virus (RRV). The sero-positivity rates for the sera were as follows: anti-BFV IgG+ 19% (8/42), IgM+ 2.4% (1/42) and Nab+ 16.7% (7/42); anti-CHIKV IgG+ 90.5% (38/42), IgM+ 21.4% (9/42) and Nab+ 90.5% (38/42); anti-RRV IgG+ 88.1% (37/42), IgM+ 28.6% (12/42) and Nab+ 83.2% (35/42), respectively. Among the samples with multiple antibody positivity, 2.4% (1/42) showed triple ELISA IgM+, and 14.3% (6/42) exhibited double IgM RRV+CHIKV+; 9.5% (4/42) showed triple IgG+, 76.2% (32/42) displayed double IgG RRV+CHIKV+, 4.8% (2/42) showed IgG BFV+RRV+ and 4.8% (2/42) showed IgG BFV++CHIKV+; and 9.5% (4/42) showed triple Nab+ and 69% (29/42) exhibited double Nab RRV+CHIKV+, respectively. Our analysis of the single-virus infection control Nab results suggested no cross-neutralization between RRV and BFV, and only mild cross-neutralization between CHIKV and RRV, BFV and CHIKV, all with a ≥4-fold Nab titre ratio difference between the true virus infection and cross-reactivity counterpart virus. Subsequently, we re-diagnosed these 42 patients as 1 BFV+, 8 CHIKV+ and 23 RRV+ single-virus infections, along with five RRV+/BFV+ and four RRV+/CHIKV+ double infections, and one possible RRV+/BFV+ or RRV+CHIKV+, respectively. These findings suggests that a substantial proportion of medically attended RRV and BFV infections were misdiagnosed as CHIKV infections, highlighting the imperative need for diagnostic laboratory tests capable of distinguishing between CHIKV infections and actively co-circulating RRV and BFV. For a correct diagnosis, it is crucial to consider reliable diagnostic methods such as the neutralization assay to exclude RRV and BFV.

Project description:BackgroundThe automated monitoring of routinely collected disease surveillance data has the potential to ensure that important changes in disease incidence are promptly recognised. However, few studies have established whether the signals produced by automated monitoring methods correspond with events considered by epidemiologists to be of public health importance. This study investigates the correspondence between retrospective epidemiological evaluation of notifications of Ross River virus (RRv) disease in Western Australia, and the signals produced by two cumulative sum (cusum)-based automated monitoring methods.MethodsRRv disease case notification data between 1991 and 2004 were assessed retrospectively by two experienced epidemiologists, and the timing of identified outbreaks was compared with signals generated from two different types of cusum-based automated monitoring algorithms; the three Early Aberration Reporting System (EARS) cusum algorithms (C1, C2 and C3), and a negative binomial cusum.ResultsWe found the negative binomial cusum to have a significantly greater area under the receiver operator characteristic curve when compared with the EARS algorithms, suggesting that the negative binomial cusum has a greater level of agreement with epidemiological opinion than the EARS algorithms with respect to the existence of outbreaks of RRv disease, particularly at low false alarm rates. However, the performance of individual EARS and negative binomial cusum algorithms were not significantly different when timeliness was also incorporated into the area under the curve analyses.ConclusionOur retrospective analysis of historical data suggests that, compared with the EARS algorithms, the negative binomial cusum provides greater sensitivity for the detection of outbreaks of RRv disease at low false alarm levels, and decreased timeliness early in the outbreak period. Prospective studies are required to investigate the potential usefulness of these algorithms in practice.
