Identification of oral symptoms associated with atopic dermatitis in adolescents: Results from the Korea national representative survey 2009-2017.

ABSTRACT: The relationship between oral health and atopic dermatitis (AD) remains unclear. Here we investigated the association between oral health status and AD using data from 634,299 subjects in the Korean Youth Risk Behavior Survey (KYRBS). Participants with oral symptoms were defined as those with any of following: sensitive teeth, toothache, bleeding gums or gum pain, and dry mouth. Current AD was determined by the question if participant had been diagnosed with AD from doctor within the past 12 months. We estimated the odds ratio (OR) for AD diagnosis according to the presence of oral symptoms. Project description:BackgroundAtopic dermatitis (AD) is an important global health problem affecting children and adolescents and detailed national information of disease burden in China is lacking. We aimed to evaluate the national disease burden of AD in Chinese children and adolescent, to provide the temporal trends over the past 30 years and to predict the burden for the next 10 years.MethodsThe data of AD in China, including incidence, prevalence, and DALY, and population data were obtained from the Global Burden of Disease Study 2019 (GBD study 2019), which were estimated using the DisMod-MR 2.1. We analyzed the three measures by age and sex; the age groups were <5 years, 5-9 years, 10-14 years, and 15-19 years. The joinpoint regression analyses was conducted to assess the temporal trends from 1990 to 2019. The Bayesian age-period cohort (BAPC) model was used to predict measures from 2020 to 2030.ResultsIn 2019, the highest incidence case and rate were observed in <5 years group; for prevalence and disability adjusted life year (DALY), the groups of <5 years and 5-9 years showed similar higher levels and the groups of 10-14 years and 15-19 years had similar relatively lower levels. Overall, the male-to-female ratios were >1 in <5 years group and <1 in 10-14 and 15-19 age groups. The trend analyses found an overall trend of decrease in cases of the three measures; in recent about 3 years, slight increase trends were shown in cases and rates of the three measures in <5 years group. The prediction analyses found a slight decreasing trend for cases of these measures and a slight increasing trend for rates of these measures in the <5 years group in the next 10 years; the 5-9 years group was predicted to increase slightly in rates of the three measures.ConclusionIn conclusion, the groups of <5 years and 5-9 years are two important populations that need targeted measures to reduce disease burden of AD in China. Regarding sex disparity, we should pay more attention to males in <5 years group and to females in 10-19 years group.

Project description:BackgroundA significant improvement in clinical signs was demonstrated with abrocitinib relative to placebo in adolescents with moderate-to-severe atopic dermatitis (AD) in three phase 3, randomized, double-blinded, placebo-controlled studies (JADE TEEN [ClinicalTrials.gov, NCT03796676], JADE MONO-1 [NCT03349060] and JADE MONO-2 [NCT03575871]).ObjectivesTo evaluate the impact of abrocitinib on patient-reported signs/symptoms, including sleep loss and quality of life among adolescents with moderate-to-severe AD.MethodsJADE TEEN, JADE MONO-1 and JADE MONO-2 were conducted in the Asia-Pacific region, Europe and North America and included patients aged 12-17 years with moderate-to-severe AD and inadequate response to ≥ 4 consecutive weeks of topical medication or treatment with systemic therapy for AD. Patients were randomly assigned (1 : 1 : 1, JADE TEEN; 2 : 2 : 1, JADE MONO-1/-2) to receive once-daily oral abrocitinib (200 or 100 mg) or placebo for 12 weeks in combination with topical therapy (JADE TEEN) or as monotherapy (JADE MONO-1/-2). Data from adolescent patients in JADE MONO-1/-2 were pooled for these analyses.ResultsAt week 12, more adolescents treated with abrocitinib (200 or 100 mg) vs. placebo achieved a ≥ 4-point improvement from baseline in the Patient-Oriented Eczema Measure in JADE TEEN (83.9% and 77.0% vs. 60.2%) and JADE MONO-1/-2 (83.0% and 69.4% vs. 43.5%) and a ≥ 6-point improvement from baseline in the Children's Dermatology Life Quality Index in JADE TEEN (73.8% and 67.5% vs. 56.5%) and JADE MONO-1/-2 (70.0% and 57.1% vs. 19.0%). Significant improvements in SCORing Atopic Dermatitis Visual Analog Scale for sleep loss scores were demonstrated with abrocitinib vs. placebo at weeks 2-12 in JADE TEEN and JADE MONO-1/-2.ConclusionsPatient-reported signs/symptoms, including reduction of sleep loss and quality of life, were substantially improved with abrocitinib monotherapy or combination therapy relative to placebo in adolescents with moderate-to-severe AD.

Project description:The purpose of this study was to evaluate the association between oral health and asthma/allergic rhinitis/atopic dermatitis in a large representative Korean adolescent population.A total of 136,027 participants (aged 12-18 years old) were selected from the Korea Youth Risk Behavior Web-based Survey 2014 to 2015. The subjects' history of asthma, allergic rhinitis, and atopic dermatitis was determined by inquiring whether they had been diagnosed by a medical doctor within the previous 12 months or throughout life. Participants were asked if they had experienced the following 6 symptoms regarding oral health in the past 12 months: "chipped or broken tooth," "toothache when eating or drinking," "throbbing and sore tooth," "sore and bleeding gums," "pain in the tongue or inside the cheeks," and "unpleasant breath." The participants were divided into 3 groups according to the number of oral health-related symptoms as follows: good oral health (symptoms = 0), moderate oral health (symptoms = 1), and poor oral health (symptoms = 2-6). Multiple logistic regression analyses calculated the adjusted odds ratios (aORs) with 95% confidence intervals (CIs) for asthma/allergic rhinitis/atopic dermatitis within the past 12 months and throughout life among groups with oral health-related symptom groups.Older age, female sex, fewer days of physical activity, lower parental economic level, lower parental education level, smoking, and frequent alcohol consumption were associated with poor oral health (each P < .001). The aORs for asthma/allergic rhinitis/atopic dermatitis were 1.48 (95% CI = 1.34-1.63, P < .001), 1.42 (95% CI = 1.36-1.47, P < .001), and 1.35 (95% CI = 1.28-1.43, P < .001), respectively, in the poor oral health group compared to the good oral health group.Poor oral health was significantly correlated with the prevalence of asthma/allergic rhinitis/atopic dermatitis within the past 12 months and throughout life in Korean adolescents. Based on our comparative data, oral health-related education may be considered to adolescents with allergic disease for good oral health.
