Mycobacterium uberis Infection in the Subcutaneous Tissue of the Radius/Ulna Area of a Cow.

ABSTRACT: Mycobacterium uberis (M. uberis) is a recently described non-tuberculous mycobacterium phylogenetically close to Mycobacterium leprae (M. leprae) and Mycobacterium lepromatosis (M. lepromatosis). This pathogen classically causes nodular thelitis in cattle and goats. Here, we discuss what seems to be the first described case of M. uberis infection in a novel anatomical site, in the proximal or distal position (information not available) of the radius/ulna area of a cow. As this case was discovered in the framework of bovine tuberculosis (bTB) surveillance program in France, this type of infection could interfere with the screening and diagnostic tools employed for bTB.

Project description:Confinement and restriction of movement are a reality for most dairy cows. Providing outdoor access is one method to increase movement opportunities. However, leading cows to an outdoor exercise area increases their exposure to manipulations different from those of an indoor housing system. These situations have the potential to induce fear reactions, which can lead to injuries for the cow and danger or economic losses for the farmer. Our aim was to evaluate the development of the human-cow relationship and general reactivity of cows after a 12-week period of outdoor access provision in winter, summer and fall. A total of 16 cows in the winter, 16 in the summer, and 15 in the fall were enrolled in the study and either allocated to the treatment (Out) or stayed in the tiestall (NonOut). A human reactivity test and suddenness test were performed before and after the 12-week treatment period. In winter and to a lesser extent in fall, Out cows had a better human reaction score compared to NonOut cows, suggesting that cows with outdoor access during the winter associated human approaches with positive events. Conversely, no difference in the human reaction score was found between treatments during the summer. For summer and fall, Out cows did, however, show a decrease in their reaction score to the suddenness test compared to NonOut cows. The results of the human reactivity test in the summer suggested that cows with outdoor access did not associate the manipulation with a positive event. Interestingly, this result is not due to the cows being more frightened, since the suddenness test suggested that the Out cows were less fearful than NonOut cows. The way in which cows were led to the outdoor area could explain the differences in cow responses. Here, summer cows faced greater movement restrictions during trips to the outdoor area than in the winter, which may have been negatively perceived by the cows. We conclude that, besides the provision of outdoor access, the manner in which cows are handled during these events may have significant impacts on their reactions and could facilitate future handling.

Project description:BACKGROUND:The objective of the present study was to evaluate effectiveness, complications and cost-effectiveness of any surgical or non-surgical treatment for radius or ulna fractures in elderly patients. Secondary objectives were to analyze present treatment traditions of distal radius fractures (DRF) in Sweden and to calculate resource usage for its treatment. METHODS AND FINDINGS:The assessment contains a systematic review of clinical and health economic studies comparing treatment options for radius or ulna fractures. The results regarding the effectiveness of the treatments are summarized in meta-analyses. In addition, the assessment contains a cost analysis for different treatment options commonly used for DRF care, and an analysis of registry data on the incidence and treatment of DRF. In total 31 randomized controlled trials were included in meta-analyses. When comparing functional outcome for plate fixation versus non-surgical treatment for DRF, there were no clinically important differences at one-year follow-up (mean difference [MD], -3.29, 95% CI, -7.03; 0.44). Similar results were found when comparing plating and percutaneous methods with respect to functional outcome (standardized mean difference [SMD], -0.07, 95% CI, -0.21; 0.07) and grip strength (MD, -3.47, 95% CI, -11.21; 4.28). There were no differences for minor complications, (risk difference [RD], -0.01, 95% CI, -0.07; 0.05) whereas major complications were less common for the percutaneous group, (RD, 0.02, 95% CI, 0.02; 0.03). Given the low number of studies, the evidence above was rated as moderate certainty. The cost for plate fixation versus plaster cast was estimated to 1698 compared to 137 US dollars. For DRF, plate fixation increased in Sweden between 2005 and 2013, and was the most common surgical method in 2013. CONCLUSIONS:Surgical treatment of moderately displaced distal radius fractures in elderly patients offers no clear benefit compared to non-surgical treatment. Plating procedures have become more common during the second millennium and involve higher costs and higher risk of major complications than percutaneous options.
