Comparing the rate of fiberoptic bronchoscopy use with a video double lumen tube versus a conventional double lumen tube-a randomized controlled trial.

ABSTRACT:

Background

Double lumen endotracheal tubes (DLT) are commonly used to provide single lung ventilation during thoracic surgery. A fiberoptic bronchoscope (FOB) is typically used to confirm accurate DLT placement. Accounting for initial purchase, maintenance, repair and cleaning, the use of an FOB can cost as much as $312 per procedure. The VivaSight DLT (VS-DLT) incorporates a built-in camera, which is aimed at reducing FOB use and its associated costs. In this study, we compared the rate of FOB use when intubating using either a VS-DLT or a conventional DLT (c-DLT).

Methods

This is a randomized controlled comparative study performed at a public county teaching hospital. A total of 50 patients were enrolled and randomly assigned to either a c-DLT (n=25) or a VS-DLT (n=25). The primary outcome was the rate of FOB use. Secondary outcomes included time to correct tube placement and incidence of malposition during surgery.

Results

Use of the VS-DLT required significantly less FOB use (28%) compared to use of the c-DLT (100%). While there was no difference in the ease of intubation, the time to correct tube placement was significantly faster using a VS-DLT (54 vs. 156 s, P<0.001). Additionally, the incidence of tube malposition was significantly reduced in the VS-DLT group.

Conclusions

This study demonstrated a significantly lower rate of FOB use when using a VS-DLT compared to a c-DLT. Placement of the VS-DLT was significantly quicker and malposition during surgery occurred significantly less than with the c-DLT. While intubating with a VS-DLT provides clinical benefits, it may not result in significant cost reductions when compared to a c-DLT.

Project description:BackgroundA fiberoptic bronchoscope (FOB) is commonly used to identify the proper placement of a double-lumen endotracheal tube (DLT) for good lung isolation during thoracic surgery. We hypothesized that the FOB-guided method for DLT placement composed of tracheal intubation under initial guidance by a FOB via the bronchial lumen and subsequent selective left-bronchial intubation could be used to reduce the incidence of DLT malposition and reduce the time required for completion of DLT placement and confirmation of proper DLT position during intubation using a left-sided DLT, in comparison to the conventional method under direct laryngoscopy using a Macintosh laryngoscope.Methods/designIn this randomized controlled pilot trial, 50 patients, aged 18-70 years, scheduled for elective thoracic surgery will be recruited and randomly assigned to two groups according to the method of DLT placement: a FOB-guided method (F) group and a conventional method (C) group. Regardless of the group, the DLT placement processes will be followed by subsequent confirmation processes, using a FOB. If the DLT is misplaced, the position would be corrected. The primary outcome is the incidence of DLT malpositioning observed via a FOB during confirmation after DLT placement. Project description:BackgroundThe inflated balloon of a bronchial blocker (BB) obstructs the mainstem bronchus, allowing controlled ventilation of one lung while collapsing the other for lung isolation during thoracic surgery. We compared the effects of lung isolation using electromagnetic navigation bronchoscopy (ENB)-guided versus fiberoptic bronchoscopy (FOB)-guided BB placement to provide a novel BB positioning method for airway management during thoracic anesthesia.MethodsWe randomly divided 106 patients who underwent elective one-lung ventilation (OLV) with intraoperative ENB usage into ENB and FOB groups. We compared subjective lung collapse scores, time required for correct BB placement, incidence of BB malposition, and frequency of intraoperative BB repositioning with FOB between the groups. Heart rate (HR) and mean arterial pressure were recorded before intubation of single-lumen tracheal tube, and before and after BB positioning. Blood gas levels were measured before and at 10 and 30 min after initiating OLV. Postoperative follow-up indices, including blood cell analysis, C-reactive protein levels, and pulmonary complications, were also recorded.ResultsThe subjective lung collapse score was significantly higher in the ENB group than in the FOB group [9.08 (1.36) vs. 8.24 (1.93), P=0.01]. The time required for correct BB placement was significantly shorter in the ENB group than in the FOB group [13.00 (10.00, 20.25) vs. 49.00 (35.00, 75.00), P<0.001]. However, the incidence of BB malposition, frequency of intraoperative BB repositioning with FOB, HR, mean arterial and peak airway pressure before and after BB positioning, arterial blood gas analysis, and postoperative follow-up indices did not differ significantly between the two groups.ConclusionsENB-guided BB positioning is an efficient method of lung isolation, demonstrating superior and more rapid lung collapse effects compared with conventional FOB-guided BB placement.Trial registrationThe trial was registered on China Clinical Trial Registry (registration No. ChiCTR2300076133).
