Technique for Fixation of Semitendinosus to the Patella in Patellar Tendon Reconstruction.

ABSTRACT: Chronic patellar tendon injuries are rare yet challenging to treat. Timing of surgery and graft choices are debatable. Many techniques and methods of fixation have been described with pros and cons. Functional impairment of the extensor mechanism and postoperative complications triggers the quest for finding the ideal technique. In this Technical Note, we use distally based semitendinosus (ST) looped over 2 ETHIBOND sutures. The ETHIBOND is passed through 2 vertical tunnels in the patella and retrieved proximally, docking the ST in a blind tunnel created in the lower pole of the patella. The whip-stitched free ST end is passed through a tunnel behind the tibial tubercle and sutured back to its base. A polyester tape is used to augment the repair in a circumferential manner. Postoperatively full weight bearing is allowed as tolerated in a hinged knee brace locked in extension with only passive range of motion of 0-90° allowed for 6 weeks.

Project description:Medial patellofemoral ligament (MPFL) reconstruction is a reliable surgical method for stabilizing a dislocating patella, with multiple techniques previously described. Although outcomes are generally favorable, the procedure is technically demanding and relies on precise identification of native MPFL insertion sites, secure fixation of the graft to these sites, and appropriate graft tension. We describe a technique for MPFL reconstruction with a looped semitendinosus tendon. The 2 free limbs of the graft are secured into blind-end patellar sockets with knotless anchors, and the looped end is initially secured into a medial femoral socket with a button on the opposite (lateral) cortex. Use of an adjustable-loop button allows for gradual adjustment of graft tension, as well as re-tensioning after cycling of the knee, before final aperture fixation on the femur with an interference screw.

Isometric Anterolateral Ligament Reconstruction Using the Semitendinosus Tendon With Suspensory Tibial Fixation.

Project description:The anterolateral ligament (ALL) plays an important role in isolated anterior cruciate ligament reconstruction by providing rotational stability in a combined anterior cruciate ligament and anterolaterally injured knee. Although the ALL has been difficult to characterize anatomically, it originates near the lateral epicondyle of the distal femur and inserts into the proximal tibia between Gerdy's tubercle and the fibula head. Here, we describe an isometric ALL reconstruction using the semitendinosus tendon and an adjustable length loop suspensory tibial fixation. With this technique, it is possible to provide good stability and satisfactory results for patients with rotational instability of the knee.

Augmentation of Patellar Tendon Repair With Autologous Semitendinosus Graft-Porto Technique.

Project description:Patellar tendon ruptures can lead to significant functional deficiency of the extensor mechanism of the knee. These injuries, because of their inherent nature and associated complications, may require a complex treatment and remains a challenge for orthopaedic surgeons. Current surgical techniques present significant complications, including patellar fracture, damage to patellar articular cartilage, and abnormal patella height. This note describes a surgical technique to provide an additional reinforcement to the patellar tendon repair with a semitendinous autograft, without the necessity to perform any transosseous tunnels at the patella bone. First, the patellar tendon is repaired with an end-to-end technique and the semitendinous tendon is harvested. A transosseous tunnel at the tibial tubercle is drilled and 2 rents are made, both medial and lateral to the retinaculum at the level of the intermedial segment of the patella close to the patellar margin. The graft is passed through the tunnel and rents in a U-shaped form. The graft is sutured along the length of the patellar tendon on both margins in tension at 30° of knee flexion. Fluoroscopy imaging is performed to assess the patella height. This technique provides a significant augmentation of patellar tendon, avoiding the potential patella bone tunnel complications.

Patellar Tendon Repair With Ipsilateral Semitendinosus Autograft Augmentation.

Project description:Patellar tendon ruptures are rare but potentially devastating injuries. Acute repair after patellar tendon rupture affords the best opportunity for tension-free restoration of the extensor mechanism. Biological augmentation of primary repair is believed to decrease strain across the repair site and reduce the risk of rerupture. We present a technique for primary patellar tendon repair with bidirectional fixation using transosseous tunnels, suture anchor fixation, and ipsilateral hamstring autograft augmentation in a distal patellar pole socket.

A new anterior cruciate ligament reconstruction fixation technique (quadrupled semitendinosus anterior cruciate ligament reconstruction with polyetheretherketone cage fixation).

Project description:Fixation of the graft during anterior cruciate ligament reconstruction surgery has been the subject of numerous technical innovations but still remains a challenge. This article describes a novel technique of graft fixation for hamstring tendon reconstruction: the Cage For One system (Sacimex, Aix-en-Provence, France). The technique uses only the semitendinosus tendon, which is looped to create a 4-strand graft. Leaving the gracilis tendon intact probably reduces the loss of knee flexion strength. The graft is indirectly anchored into both tunnels with polyetheretherketone cages by use of polyethylene terephthalate tape strips. Both cages and strips are magnetic resonance imaging compatible and do not create artifacts. The tunnels are drilled by an outside-in method with minimal incisions. This type of fixation creates a 360° bone contact at 1.5 cm in each tunnel and is compatible with double-bundle reconstruction. This easy-to-use novel technique of fixation for anterior cruciate ligament reconstruction produces a strong 4-strand graft while harvesting only the semitendinosus tendon and leaving the gracilis tendon intact to reduce flexion strength loss and preserve rotatory stability of the knee. It creates an immediate solid fixation that is independent of graft integration in the early postoperative period, allowing the patient to start immediate rehabilitation without the use of a brace.

Arthroscopic ligamentum teres reconstruction using semitendinosus tendon: surgical technique and an unusual outcome.

Project description:Although the exact biomechanical function of the ligamentum teres (LT) remains unclear, an important role in hip stability has been suggested. In some patients, perhaps because of their specific anatomy or physical activity, it seems to have a major function as a hip stabilizer. Therefore, after a complete LT tear, this group of patients may have persistent hip pain and mechanical symptoms probably related to microinstability and subtle subluxation. We present the case of a female patient with a complete LT tear. This had been treated by debridement and anterior capsule tightening, followed by prolonged rehabilitation. However, she had persistent symptoms. An LT reconstruction with a novel surgical technique was performed. The LT was reconstructed with double-stranded semitendinosus graft. The acetabular end was fixed with 2 anchors, and the femoral end was passed into a bone tunnel and fixed with an interference screw. After surgery, the patient's modified Harris Hip Score and Non-Arthritic Hip Score improved from 53 and 73 to 100 and 95, respectively, at 12 months of follow-up. At repeat arthroscopy 15 months after surgery, it was observed that the graft had resorbed, although the 2 Ticron stitches (Covidien, Mansfield, MA) remained firmly attached and were still acting as a small synthetic ligament.

Patellar Tendon Lengthening: Rescue Procedure for Patella Baja.

Project description:Patella baja is a challenging pathologic condition that causes pain and functional restrictions and can even lead to premature osteoarthritis-even more so in cases of patella infera or cases associated with degeneration of the patellar tendon in which simple conservative treatment frequently is not resolutive. Several surgical options have been described for symptomatic patella baja: excision of the lower third of the patella, lengthening of the patellar tendon, reconstruction of the patellar tendon with allograft, and proximalization of the tibial tubercle. A combination of 2 or more of these treatments may be recommended in cases of significant patella baja. We present a simple and reproducible technique to address patella baja that combines a partial transposition of the tibial tubercle and patellar tendon lengthening using a subperiosteal patellar flap in continuity with the patellar tendon.

Anatomical Double-Bundle Medial Patellofemoral Ligament Reconstruction With Autologous Semitendinosus, a Single Patellar Tunnel Technique.

Project description:Patellofemoral instability is one of the knee problems that can be found in up to 3% of knee injuries, especially in younger aged females. Recent biomechanical studies showed that the main soft tissue stabilizer for patellofemoral stability is the medial patellofemoral ligament (MPFL). More than 200 articles on MPFL reconstruction have been published. Some surgical techniques create multiple holes in the patellar area that may increase the risk of patellar fractures. This technique that we present here attempts to reduce the chance of patella-related complications, while maintaining stability of the reconstructed construct, reducing the use of a lengthy tendon graft within the patella and covering the footprint at the border of the patella closed to the native anatomy of the MPFL.

Bone-on-Bone Anatomic Patellar Tendon Graft Anterior Cruciate Ligament Reconstruction: A Reproducible Technique Combining Press-Fit and Extracortical Fixation.

Project description:Scientific interest in optimizing outcomes after anterior cruciate ligament (ACL) reconstruction is ongoing, and some recent developments have focused on graft shape as one of the most important factors of anatomic graft placement. The double-bundle or fanlike structure of the native ACL seems to more closely restore normal function and control of rotational stability of the knee with implant-free or press-fit techniques, creating a favorable situation in terms of direct graft-tunnel healing. The ACL reconstruction technique presented in this article enables safe, reproducible, and anatomic fixation of the patellar tendon autograft, providing the biological and biomechanical benefits of direct bony integration of the ribbonlike bone-patellar tendon-bone graft. Because press-fit fixation represents a technically challenging surgical procedure, the goal of the described technique is to enable direct bone-to-bone healing by using secondary extracortical femoral and tibial fixation without the need for a true press-fit situation. Safe and anatomic femoral tunnel drilling is achieved with an outside-in technique (retrograde drilling), hence providing advantages in the routine clinical setting in terms of applicability and time effort. To reduce donor-site morbidity caused by bone block harvesting, refilling of the harvest sites with autologous material is performed.
