Calculating and estimating second cancer risk from breast radiotherapy using Monte Carlo code with internal body scatter for each out-of-field organ.

ABSTRACT: Out-of-field organs are not commonly designated as dose calculation targets during radiation therapy treatment planning, but they might entail risks of second cancer. Risk components include specific internal body scatter, which is a dominant source of out-of-field doses, and head leakage, which can be reduced by external shielding. Our simulation study quantifies out-of-field organ doses and estimates second cancer risks attributable to internal body scatter in whole-breast radiotherapy (WBRT) with or without additional regional nodal radiotherapy (RNRT), respectively, for right and left breast cancer using Monte Carlo code PHITS. Simulations were conducted using a complete whole-body female model. Second cancer risk was estimated using the calculated doses with a concept of excess absolute risk. Simulation results revealed marked differences between WBRT alone and WBRT plus RNRT in out-of-field organ doses. The ratios of mean doses between them were as large as 3.5-8.0 for the head and neck region and about 1.5-6.6 for the lower abdominal region. Potentially, most out-of-field organs had excess absolute risks of less than 1 per 10,000 persons-year. Our study surveyed the respective contributions of internal body scatter to out-of-field organ doses and second cancer risks in breast radiotherapy on this intact female model.

Project description:PURPOSE:In 90 Y microsphere radioembolization (RE), accurate post-therapy imaging-based dosimetry is important for establishing absorbed dose versus outcome relationships for developing future treatment planning strategies. Additionally, accurately assessing microsphere distributions is important because of concerns for unexpected activity deposition outside the liver. Quantitative 90 Y imaging by either SPECT or PET is challenging. In 90 Y SPECT model based methods are necessary for scatter correction because energy window-based methods are not feasible with the continuous bremsstrahlung energy spectrum. The objective of this work was to implement and evaluate a scatter estimation method for accurate 90 Y bremsstrahlung SPECT/CT imaging. METHODS:Since a fully Monte Carlo (MC) approach to 90 Y SPECT reconstruction is computationally very demanding, in the present study the scatter estimate generated by a MC simulator was combined with an analytical projector in the 3D OS-EM reconstruction model. A single window (105 to 195-keV) was used for both the acquisition and the projector modeling. A liver/lung torso phantom with intrahepatic lesions and low-uptake extrahepatic objects was imaged to evaluate SPECT/CT reconstruction without and with scatter correction. Clinical application was demonstrated by applying the reconstruction approach to five patients treated with RE to determine lesion and normal liver activity concentrations using a (liver) relative calibration. RESULTS:There was convergence of the scatter estimate after just two updates, greatly reducing computational requirements. In the phantom study, compared with reconstruction without scatter correction, with MC scatter modeling there was substantial improvement in activity recovery in intrahepatic lesions (from > 55% to > 86%), normal liver (from 113% to 104%), and lungs (from 227% to 104%) with only a small degradation in noise (13% vs. 17%). Similarly, with scatter modeling contrast improved substantially both visually and in terms of a detectability index, which was especially relevant for the low uptake extrahepatic objects. The trends observed for the phantom were also seen in the patient studies where lesion activity concentrations and lesion-to-liver concentration ratios were lower for SPECT without scatter correction compared with reconstruction with just two MC scatter updates: in eleven lesions the mean uptake was 4.9 vs. 7.1 MBq/mL (P = 0.0547), the mean normal liver uptake was 1.6 vs. 1.5 MBq/mL (P = 0.056) and the mean lesion-to-liver uptake ratio was 2.7 vs. 4.3 (P = 0.0402) for reconstruction without and with scatter correction respectively. CONCLUSIONS:Quantitative accuracy of 90 Y bremsstrahlung imaging can be substantially improved with MC scatter modeling without significant degradation in image noise or intensive computational requirements.

Project description:The scope of this study was to estimate total scatter factors (S(c,p)) of the three smallest collimators of the Cyberknife radiosurgery system (5-10 mm in diameter), combining experimental measurements and Monte Carlo simulation. Two microchambers, a diode, and a diamond detector were used to collect experimental data. The treatment head and the detectors were simulated by means of a Monte Carlo code in order to calculate correction factors for the detectors and to estimate total scatter factors by means of a consistency check between measurement and simulation. Results for the three collimators were: S(c,p) (5 mm) = 0.677 +/- 0.004, S(c,p) (7.5 mm) = 0.820 +/- 0.008, S(c,p) (10 mm) = 0.871 +/- 0.008, all relative to the 60 mm collimator at 80 cm source-to-detector distance. The method also allows the full width at half maximum of the electron beam to be estimated; estimations made with different collimators and different detectors were in excellent agreement and gave a value of 2.1 mm. Correction factors to be applied to the detectors for the measurement of S(c,p) were consistent with a prevalence of volume effect for the microchambers and the diamond and a prevalence of scattering from high-Z material for the diode detector. The proposed method is more sensitive to small variations of the electron beam diameter with respect to the conventional method used to commission Monte Carlo codes, i.e., by comparison with measured percentage depth doses (PDD) and beam profiles. This is especially important for small fields (less than 10 mm diameter), for which measurements of PDD and profiles are strongly affected by the type of detector used. Moreover, this method should allow S(c,p) of Cyberknife systems different from the unit under investigation to be estimated without the need for further Monte Carlo calculation, provided that one of the microchambers or the diode detector of the type used in this study are employed. The results for the diamond are applicable only to the specific detector that was investigated due to excessive variability in manufacturing.
