Optimizing Kidney Replacement Therapy During the COVID-19 Pandemic Across a Complex Healthcare System.

ABSTRACT: The unprecedented surge of nephrology inpatients needing kidney replacement therapy placed hospital systems under extreme stress during the COVID-19 pandemic. In this article, we describe the formation of a cross campus "New-York Presbyterian COVID-19 Kidney Replacement Therapy Task Force" with intercampus physician, nursing, and supply chain representation. We describe several strategies including the development of novel dashboards to track supply/demand of resources, urgent start peritoneal dialysis, in-house preparation of kidney replacement fluid, the use of unconventional personnel resources to ensure the safe and continued provision of kidney replacement therapy in the face of the unanticipated surge. These approaches facilitated equitable sharing of resources across a complex healthcare-system and allowed for the rapid implementation of standardized protocols at each hospital.

Project description:Rationale & objectiveDuring the coronavirus disease 2019 (COVID-19) pandemic, New York encountered shortages in continuous kidney replacement therapy (CKRT) capacity for critically ill patients with acute kidney injury stage 3 requiring dialysis. To inform planning for current and future crises, we estimated CKRT demand and capacity during the initial wave of the US COVID-19 pandemic.Study designWe developed mathematical models to project nationwide and statewide CKRT demand and capacity. Data sources included the Institute for Health Metrics and Evaluation model, the Harvard Global Health Institute model, and published literature.Setting & populationUS patients hospitalized during the initial wave of the COVID-19 pandemic (February 6, 2020, to August 4, 2020).InterventionCKRT.OutcomesCKRT demand and capacity at peak resource use; number of states projected to encounter CKRT shortages.Model, perspective, & timeframeHealth sector perspective with a 6-month time horizon.ResultsUnder base-case model assumptions, there was a nationwide CKRT capacity of 7,032 machines, an estimated shortage of 1,088 (95% uncertainty interval, 910-1,568) machines, and shortages in 6 states at peak resource use. In sensitivity analyses, varying assumptions around: (1) the number of pre-COVID-19 surplus CKRT machines available and (2) the incidence of acute kidney injury stage 3 requiring dialysis requiring CKRT among hospitalized patients with COVID-19 resulted in projected shortages in 3 to 8 states (933-1,282 machines) and 4 to 8 states (945-1,723 machines), respectively. In the best- and worst-case scenarios, there were shortages in 3 and 26 states (614 and 4,540 machines).LimitationsParameter estimates are influenced by assumptions made in the absence of published data for CKRT capacity and by the Institute for Health Metrics and Evaluation model's limitations.ConclusionsSeveral US states are projected to encounter CKRT shortages during the COVID-19 pandemic. These findings, although based on limited data for CKRT demand and capacity, suggest there being value during health care crises such as the COVID-19 pandemic in establishing an inpatient kidney replacement therapy national registry and maintaining a national stockpile of CKRT equipment.

Project description:BackgroundWhile the etiology for the upsurge in testosterone testing and prescriptions is likely multifactorial, increased direct-to-consumer marketing and the expansion of clinical care centers devoted to testosterone treatment likely play a role. Many of these centers require patients to report, in-person, on a regular basis for their injectable therapy and/or lab studies. The purpose of our study was to investigate barriers of care that patients receiving treatment for testosterone deficiency may be experiencing in the setting of COVID-19.MethodsOur survey was posted on a closed Facebook support page for males currently receiving testosterone treatment and members of the group were invited to participate. The survey asked participants several questions related to how they received their injections, if they've experienced difficulties obtaining their injections due to COVID-19 restrictions, and about their interest in telemedicine services for their care.ResultsThe majority of patients were able to receive their treatment despite barriers enforced by the pandemic. Of the 104 participants, almost half received their testosterone prescriptions from an outpatient clinic dedicated to testosterone replacement, while the other half received their therapy from a PCP, endocrinologist, or urologist. Only 5 patients (4.8%) noted difficulties obtaining their injections during this pandemic, 4 of which received their prescriptions from dedicated testosterone clinics, and the other from a PCP. Nearly 90% of respondents self-administered their testosterone therapy. With regards to telemedicine, 57.8% of patients have utilized the technology in some capacity, however 74.4% said that they would prefer to use telemedicine video services with a urologist or APP with expertise in andrology over in-person services.ConclusionsIn our survey, the majority of the respondents have been able to receive their injectable testosterone therapy despite the ongoing pandemic. The majority of respondents self-administer their treatments, which may explain the lack of barriers. This study is the first of its kind to investigate the effect of a pandemic on the receipt of care for those being treated for testosterone deficiency with injectable testosterone.

Project description:BackgroundAcute kidney injury (AKI) can affect hospitalized patients with coronavirus disease 2019 (COVID-19), with estimates ranging between 0.5% and 40%. We performed a systematic review and meta-analysis of studies reporting incidence, mortality and risk factors for AKI in hospitalized COVID-19 patients.MethodsWe systematically searched 11 electronic databases until 29 May 2020 for studies in English reporting original data on AKI and kidney replacement therapy (KRT) in hospitalized COVID-19 patients. Incidences of AKI and KRT and risk ratios for mortality associated with AKI were pooled using generalized linear mixed and random-effects models. Potential risk factors for AKI were assessed using meta-regression. Incidences were stratified by geographic location and disease severity.ResultsA total of 3042 articles were identified, of which 142 studies were included, with 49 048 hospitalized COVID-19 patients including 5152 AKI events. The risk of bias of included studies was generally low. The pooled incidence of AKI was 28.6% [95% confidence interval (CI) 19.8-39.5] among hospitalized COVID-19 patients from the USA and Europe (20 studies) and 5.5% (95% CI 4.1-7.4) among patients from China (62 studies), whereas the pooled incidence of KRT was 7.7% (95% CI 5.1-11.4; 18 studies) and 2.2% (95% CI 1.5-3.3; 52 studies), respectively. Among patients admitted to the intensive care unit, the incidence of KRT was 20.6% (95% CI 15.7-26.7; 38 studies). Meta-regression analyses showed that age, male sex, cardiovascular disease, diabetes mellitus, hypertension and chronic kidney disease were associated with the occurrence of AKI; in itself, AKI was associated with an increased risk of mortality, with a pooled risk ratio of 4.6 (95% CI 3.3-6.5).ConclusionsAKI and KRT are common events in hospitalized COVID-19 patients, with estimates varying across geographic locations. Additional studies are needed to better understand the underlying mechanisms and optimal treatment of AKI in these patients.
