This paper aims to examine the correlates of foreign policy attitudes in Moldova by a multilevel analysis, and to also reveal some characteristics of the Moldova's difficult geopolitical and economic context, such as the ethnical conflicts and poverty. Project description:Background: Nigeria's former Prime Minister, Sir Abubakar Tafawa Balewa, in his addresses of August and October 1, 1960, declared Africa as the centrepiece of Nigeria's foreign policy. This policy thrust has remained a constant variable in the country's diplomatic engagements over the years. The doctrine of Afrocentrism is predicated on the supposed manifest leadership role placed on Nigeria by nature. This made her leaders define Africa's interest as Nigeria's national interest, a development that has been contended to have no empirical bearing on the welfare of Nigerians thereby generating intense scrutiny. Consequently, this study evaluates the impact of Nigeria's Afrocentric foreign policy thrust on the welfare of the ordinary Nigerians. The study further analyses the country's gravitation towards citizen-centred diplomacy in 2007. These will help in comprehending the interaction between national interest and foreign policy in Nigeria, and to identify whose interests have been protected the most in Nigeria's foreign policy pursuit - that of the ordinary citizens or the elites? Methods: Through the qualitative research method, in-depth interviews (IDIs) were conducted with Key Informants (KIs) for data collection. Responses from field study are merged with other primary and secondary sources of data to provide an incisive and balanced analysis that is premised on political realism. Results: Findings indicate that Nigeria's international generosity and leadership role has never been predicated on a clear vision of national interest. Notwithstanding the flaws in Nigeria's foreign policy over the years, this study also discovered that the outcome has not been a total failure as some respondents maintain. Conclusions: With the nation's gravitation towards citizen-centred diplomacy, it is hoped that the country will put the interest of its citizens first in her policy pursuits.

Project description:The large inflow of asylum-seekers in recent years has heralded a diversification in adopted asylum policies across European societies. Although a growing body of research has addressed these versatile approaches and their implications for the European integration project, insight into the social basis of these restrictive or open asylum policies remains underdeveloped. Hence, the current study provides detailed insight into public preferences for asylum policies and offers a new understanding of how these attitudes are affected by diverging socio-economic realities across Europe. In addition, this paper considers the role of individual factors that coincide with publicly adopted frames in contemporary asylum debates. In particular, to explain how contextual differences reflect on opinion climates, the impacts of the policy, economic, and migratory context are studied. On the individual-level, we focus on threat perception and human values, which represent humanitarian, economic, and cultural frames. To explore these relations, data on 20 countries from the European Social Survey Round 8 (2016) are analyzed through a multilevel structural equation modeling approach. Results indicate that, on the contextual-level, only unemployment rates have a significant impact and, rather surprisingly, lower unemployment rates provoke a more negative opinion climate. Yet, this relationship seems to be largely driven by some specific countries that are characterized by large unemployment rates and relatively positive opinion climates simultaneously. The migratory and policy context, on the other hand, do not influence attitudes toward asylum policy. This indicates that it is not necessarily the countries facing the largest inflow of asylum-seekers or issuing the most positive decisions on asylum applications that have the most restrictive opinion climates. As shown by the important roles of human values and threat perceptions, which represent widely adopted frames, public discourses seem much more important in explaining attitudes toward asylum policy across Europe.

Project description:BACKGROUND:The shaping of gender beliefs and attitudes in early adolescence affects the way young people internalize and self-enforce prevalent notions of masculinity and femininity, with lifelong consequences for sexual and reproductive health. This cross-sectional study examines determinants of gender attitudes among some of the poorest and most vulnerable adolescents in Tanzania using an ecological model. METHODS:Data come from baseline interviews with 2458 males and females aged 14-19?years conducted as part of a larger impact evaluation. Structural equation models are used to examine how factors at the community-, household-, and individual-levels influence gender attitudes in the four domains measured by the Gender Equitable Men (GEM) Scale (i.e. violence, sexual relationships, reproductive health and disease prevention, and domestic chores and daily life). RESULTS:A structural equation model of the four latent domains of the GEM scale regressed on individual, social-interactional and structural level characteristics indicated that secondary school attendance was associated with more equitable gender attitudes, while females held less equitable attitudes than males in the sample. Having had sexual intercourse was associated with more gender equitable attitudes among females, but the reverse was true among males. CONCLUSIONS:Addressing gender inequity requires understanding gender socialisation at the socio-interactional level. As females had more inequitable gender attitudes than males in the study, a special emphasis on highlighting the rights of women to girls should be considered. This study will inform future analysis of programme impacts on gender attitudes and sexual and reproductive health.

Project description:INTRODUCTION:Human papillomavirus vaccination is less prevalent among foreign-born than U.S.-born women and may lead to disparities in human papillomavirus-related cancers in the future. There is limited research on factors associated with vaccination uptake between these two groups. This study examined the association between place of birth and human papillomavirus vaccine uptake, and what determinants of vaccination attenuate this relationship. METHODS:The 2013-2015 National Health Interview Survey data on women was analyzed in 2016, to determine differences in prevalence of human papillomavirus vaccination between foreign- and U.S.-born women. Multivariate binary logistic regression analysis was used to examine the association between foreign-born status and human papillomavirus vaccine initiation, after controlling for health insurance status, having a usual source of care, obstetrician/gynecologist visits, Pap tests, length of U.S. residency, and citizenship. RESULTS:Human papillomavirus vaccination prevalence varied significantly among women born in different regions of the world. European and South-American women had the highest vaccination rates among all foreign-born women. Compared with U.S.-born women, foreign-born women were significantly less likely to report human papillomavirus vaccine initiation. This relationship was partially attenuated after adjusting for the covariates. Among foreign-born women, Asians were significantly less likely to report human papillomavirus vaccination uptake than white women. Additionally, living in the U.S. for >5 years was significantly associated with vaccine initiation, but attenuated by U.S. citizenship status. CONCLUSIONS:Public health interventions to improve human papillomavirus vaccination need to be developed to address multicultural audiences with limited access to health insurance and health care.

Project description:The current paper presents three studies, which suggest that perceiving one's nation as transgenerational (TG) is related to a differentiation in the evaluation of ethnically German diaspora migrants and ethnically non-German ('foreign') migrants. First, we find that unlike 'classical' concepts such as right-wing authoritarianism (RWA), social dominance orientation (SDO), and hierarchic self-interest (HSI), TG explains differences in derogatory sentiments expressed towards diaspora and 'foreign' migrants. Second, TG is differentially related to positive emotions and behavioral intentions expressed towards these two groups of migrants. Lastly, results indicate that people who perceive the ingroup as TG require 'foreign' migrants to fulfill more criteria that make them eligible for citizenship and are thereby more exclusionist than people who include only the current generation into their concept of national identity. The social implications of these findings in face of the so-called refugee crisis in Germany and the wider European Union are discussed.

Project description:South Africa, as an emerging middle-income country, is becoming increasingly influential in global health diplomacy (GHD). However, little empirical research has been conducted to inform arguments for the integration of domestic health into foreign policy by state and non-state actors. This study seeks to address this knowledge gap. It takes the form of an empirical case study which analyses how South Africa integrates domestic health into its foreign policy, using the lens of access to antiretroviral (ARV) medicines.To explore state and non-state actors' perceptions regarding how domestic health policy is integrated into foreign policy. The ultimate goal of this study was to achieve better insights into the health and foreign policy processes at the national level.Employing qualitative approaches, we examined changes in the South African and global AIDS policy environment. Purposive sampling was used to select key informants, a sample of state and non-state actors who participated in in-depth interviews. Secondary data were collected through a systematic literature review of documents retrieved from five electronic databases, including review of key policy documents. Qualitative data were analysed for content. This content was coded, and the codes were collated into tentative categories and sub-categories using Atlas.ti v.7 software.The findings of this work illustrate the interplay among social, political, economic and institutional conditions in determining the success of this integration process. Our study shows that a series of national and external developments, stakeholders, and advocacy efforts and collaboration created these integrative processes. South Africa's domestic HIV/AIDS constituencies, in partnership with the global advocacy movement, catalysed the mobilization of support for universal access to ARV treatment nationally and globally, and the promotion of access to healthcare as a human right.Transnational networks may influence government's decision making by providing information and moving issues up the agenda.

Project description:BackgroundForeign aid has been shown to be favourably biased towards small countries. This study investigated whether country size bias also occurs in national malaria policy and development assistance from international agencies.MethodsData from publicly available sources were collected with countries as observational units. The exploratory data analysis was based on the conceptual framework with socio-economic, environmental and institutional parameters. The strength of relationships was estimated by the Pearson and polychoric correlation coefficients. The correlation matrix was explored by factor analysis.ResultsMalaria burden is strongly correlated with GDP per capita, total health expenditure per capita, HDI; moderately with latitude, weakly with elevation, urban population share, per capita funding from the Global Fund, PMI USAID, UK government and UNICEF. Small country status is strongly correlated with population size, land area, island status; moderately with development assistance received per capita, weakly with funding per capita from Global Fund, government NMP and PMI USAID. Policy score 1, a variable derived from our factor analysis and related to malaria endemicity, is significantly strongly correlated with the malaria burden, moderately with HDI, GDP per capita, total health expenditure per capita, PMI USAID funding; weakly with island status, urban population share, latitude, coastal population share, total government expenditure and trade openness, Global Fund funding, World Bank funding, UK government funding, and UNICEF funding per capita. Policy score 2, which captures variation not related to malaria endemicity, is significantly weakly related to the ICRG index, PMI USAID funding per capita and small country status.ConclusionsThe results suggest that malaria burden and economic development are bidirectionally related. Economic development can contribute to a reduction in the malaria burden. Country size does not negatively impact malaria burden, but it does account for greater development assistance per capita from selected international agencies. National malaria policy is associated with parameters related to public governance and is modified in small countries. Small country bias is present in the distribution of socio-economic resources and the allocation of foreign aid. Small countries are characterized by distinct environmental and socio-political properties.

Project description:BACKGROUND:Relatively little is known about American medical student's attitudes toward caring for the uninsured, limiting physician reimbursement and the role of cost-effectiveness data in medical decision-making. We assessed American medical student's attitudes regarding these topics as well as demographic predictors of those attitudes, and compared them to practicing physicians. METHODS AND FINDINGS:A survey instrument was explicitly designed to compare medical student attitudes with those previously reported by physicians. Between December 1st 2010 and March 27th 2011 survey responses were collected from more than 2% of the total estimated 2010-2011 US medical student population enrolled at 111 of 159 accredited US medical schools within the 50 United States (n = 2414 of possible 98197). Medical students were more likely to object to reimbursement cuts, and more likely to object to the use of cost effectiveness data in medical decision making than current physicians according to the literature. Specialty preference, political persuasion, and medical student debt were significant predictors of health policy attitudes. Medical students with anticipated debt in excess of $200,000 were significantly less willing to favor limiting reimbursement to improve patient access (OR: 0.73 [95% confidence interval (CI): 0.59-0.89]), and significantly more likely to object to using cost effectiveness data to limit treatments (OR 1.30, 95% CI 1.05-1.60) when compared to respondents with anticipated debt less than $200,000. CONCLUSIONS:When compared to physicians in the literature, future physicians may be less willing to favor cuts to physician reimbursements and may be more likely to object to the use of cost effectiveness data. Political orientation, specialty preference and anticipated debt may be important predictors of health policy attitudes among medical students. Early career medical providers with primary care ambitions and those who anticipate less debt may be more likely to support healthcare cost containment.

Project description:Background and objectivesThe increasing number of senior drivers may introduce new road risks due to age-related declines in physical and cognitive abilities. Advanced driver assistance systems (ADAS) have been proposed as solutions to minimize age-related declines, thereby increasing both senior safety and mobility. This study examined factors that influence seniors' attitudes toward adopting ADAS after significant exposure to the technology in naturalistic settings.Research design and methodsThis study recruited 18 senior drivers aged 70-79 to drive vehicles equipped with ADAS for 6 weeks in their own environments. Afterward, each participant was enrolled in 1 of the 3 focus group sessions to discuss their changes in attitude toward ADAS based on their driving experiences. We applied structural topic modeling (STM) on the focus group transcripts to reveal key topics deemed important to seniors.ResultsSTM revealed 5 topics of importance for seniors. In order of prevalence, these were (i) safety, (ii) confidence concerning ADAS, (iii) ADAS functionality, (iv) user interface/usability, and (v) non-ADAS-related features. Based on topics and associated keywords, seniors perceived safety improvement with ADAS but expressed concerns about its limitations in coping with adverse driving conditions. Experience and training were suggested for improving seniors' confidence in ADAS. Blind spot alert and adaptive cruise control received the most discussion regarding perceived safety and comfort.Discussion and implicationsThis study indicated that promoting road safety for senior drivers through ADAS is feasible. Acceptance and appropriate use of ADAS may be supported through intuitive and senior-friendly user interfaces, in-depth training programs, and owner's manuals specifically designed and tested for senior drivers.
