Cynical Hostility and Loneliness in Older Adult Married Couples: An Indirect Effect Through Friendships.

ABSTRACT: Objectives
Cynical hostility is a cognitive schema according to which people cannot be trusted, and it has associations with individuals' loneliness. The present study takes a dyadic approach to examine whether cynical hostility is related to one's own and their spouse's loneliness. We further explore whether friendship factors serve as a mediator between individuals' and spouses' cynical hostility and loneliness.Method
We used 2 waves of the Health and Retirement Study (N = 1,065 couples) and Actor-Partner Interdependence Models (APIMs) with mediation to examine the proposed model. Mediation was tested with the construction of path models and significance levels were reached using bootstrapping.Results
For both husbands and wives, cynical hostility was significantly associated with loneliness. Husband's loneliness was also significantly associated with his wife's cynical hostility, but wife's loneliness was not associated with her husband's cynical hostility. We further found that the association between wife's own cynical hostility and loneliness was mediated by lower levels of contact with, and support from friends. Friendship factors did not serve as mediators for husbands.Discussion
Husbands and wives who have higher levels of cynical hostility may be more vulnerable to loneliness. High levels of cynical hostility in women may be related to deficits in their quantity and quality of friendship, and thus be associated with loneliness. Men who are married to women with a higher level of cynical hostility may experience increased loneliness, but this relationship is not explained by men's friendships.

Project description:The focus in this paper is on the social domain of quality of life, and more particularly loneliness. The empirical literature on older adult loneliness is reviewed, thereby challenging three often-held assumptions that figure prominently in public debates on loneliness. The first assumption that loneliness is a problem specifically for older people finds only partial support. Loneliness is common only among the very old. The second assumption is that people in individualistic societies are most lonely. Contrary to this belief, findings show that older adults in northern European countries tend to be less lonely than those in the more familialistic southern European countries. The scarce data on Central and Eastern Europe suggest a high prevalence of older adult loneliness in those countries. The third assumption that loneliness has increased over the past decades finds no support. Loneliness levels have decreased, albeit slightly. The review notes the persistence of ageist attitudes, and underscores the importance of considering people's frame of reference and normative orientation in analyses of loneliness.

Project description:BackgroundIt is hypothesised that hostility accentuates the association between stressful conditions and health. This study aims to test this hypothesis by analysing the joint effect of unemployment and hostility on all-cause mortality among men and women.MethodsThe population was 3677 men and 4138 women from the Danish workforce who participated in a survey in 2000. The joint exposure variable was defined as 1) employed, not hostile, 2) unemployed, not hostile, 3) hostile and employed, 4) unemployed and hostile. Outcome was defined as all-cause mortality between 2000 and 2014. Data was analysed with Cox proportional hazards models with age as the underlying time scale. The interaction between unemployment and hostility was studied using the synergy index.ResultsCompared to employed non-hostile men, men who were both hostile and unemployed were at markedly higher risk of premature death with a hazard ratio (HR) of 3.19 (95% CI 2.22-4.69). A similar picture was found for hostile and unemployed women, with a HR of 1.97 (95% CI 1.24-3.12). However, the mortality in men and women exposed to both did not exceed what was expected from the combination of their individual effects. Hence, we did not find that hostility enhances the association between unemployment and all-cause mortality.ConclusionMen and women exposed to both unemployment and hostility were at markedly high risk of premature mortality. However, this study did not support the hypothesis that the deleterious health effect of the combination of unemployment and hostility exceeds their individual effects.

Project description:ObjectivesA growing body of research has identified factors related to loneliness among older adults. Fewer have investigated predictors of loneliness within married couples. This analysis investigates how spousal support and strain relate with loneliness within older couples (age 50+), and whether these associations are modified by functional limitation. The study focuses on Mexico, a country experiencing rapid aging occurring alongside historically limited institutional support for older adults, and where traditional gender roles extend to marriage.MethodsThe analytic sample consisted of 3,584 husband-wife dyads from the 2012 and 2015 Mexican Health and Aging Study. Loneliness was measured using the Three-Item Loneliness Scale. Associations between spousal support, strain, and loneliness were estimated within husband-wife dyads using the Actor-Partner Interdependence Model.ResultsExperiencing more spousal support was associated with less loneliness, whereas experiencing spousal strain was associated with more loneliness 3 years later among married adults. The associations between spousal support/strain and loneliness were stronger among husbands with limitations in activities of daily living (ADL) when compared to their counterparts without ADL limitations.DiscussionAmong married adults, spousal support and strain may be important factors to understand loneliness within marriage. Effects should be interpreted within the context of functional limitation.

Project description:ObjectivesThe topic of older adult loneliness commands increasing media and policy attention around the world. Are surveys of aging equipped to measure it? We assess the measurement of loneliness in large-scale aging studies in 31 countries by describing the available measures, testing correlations between them, and documenting their construct validity.MethodsWe use data from several "sister studies" of aging adults around the world. In each country, we document available loneliness measures, test for measurement reliability by examining correlations between different measures of loneliness, and assess how these correlations differ by gender and age group. We then evaluate construct validity by estimating correlations between loneliness measures and theoretically hypothesized constructs related to loneliness: living alone and not having a spouse.ResultsThere is substantial heterogeneity in available measures of loneliness across countries. Within countries with multiple measures, the correlations between measures are high (range 0.384-0.777, median 0.636). Although we find several statistically significant differences in these correlations by gender and age, the differences are small (gender: range -0.098 to 0.081, median -0.026; age group: range -0.194 to 0.092, median -0.003). Correlations between loneliness measures and living alone and being without a spouse are all positive, almost universally statistically significant, and similar in magnitude across countries, supporting construct validity.DiscussionThis article establishes that even single-item measures of loneliness contribute meaningful information in diverse settings. Similar to the measurement of self-rated health, there are nuances to the measurement of older adult loneliness in different contexts, but it has reliable and consistent measurement properties within many countries.

Project description:Traits of optimism and cynical hostility are features of personality that could influence the risk of falls and fractures by influencing risk-taking behaviors, health behaviors, or inflammation. To test the hypothesis that personality influences falls and fracture risk, we studied 87,342 women enrolled in WHI-OS. Optimism was assessed by the Life Orientation Test-Revised and cynical hostility, the cynicism subscale of the Cook-Medley questionnaire. Higher scores indicate greater optimism and hostility. Optimism and hostility were correlated at r?=?-0. 31, p?<?0.001. Annual self-report of falling ?2 times in the past year was modeled using repeated measures logistic regression. Cox proportional hazards models were used for the fracture outcomes. We examined the risk of falls and fractures across the quartiles (Q) of optimism and hostility with tests for trends; Q1 formed the referent group. The average follow-up for fractures was 11.4 years and for falls was 7.6 years. In multivariable (MV)-adjusted models, women with the highest optimism scores (Q4) were 11% less likely to report ?2 falls in the past year (odds ratio [OR]?=?0.89; 95% confidence intervals [CI] 0.85-0.90). Women in Q4 for hostility had a 12% higher risk of ?2 falls (OR?=?1.12; 95% CI 1.07-1.17). Higher optimism scores were also associated with a 10% lower risk of fractures, but this association was attenuated in MV models. Women with the greatest hostility (Q4) had a modest increased risk of any fracture (MV-adjusted hazard ratio?=?1. 05; 95% CI 1.01-1.09), but there was no association with specific fracture sites. In conclusion, optimism was independently associated with a decreased risk of ?2 falls, and hostility with an increased risk of ?2 falls, independent of traditional risk factors. The magnitude of the association was similar to aging 5 years. Whether interventions aimed at attitudes could reduce fall risks remains to be determined. © 2016 American Society for Bone and Mineral Research.

Project description:BackgroundTrait optimism (positive future expectations) and cynical, hostile attitudes toward others have not been studied together in relation to incident coronary heart disease (CHD) and mortality in postmenopausal women.Methods and resultsParticipants were 97 253 women (89 259 white, 7994 black) from the Women's Health Initiative who were free of cancer and cardiovascular disease at study entry. Optimism was assessed by the Life Orientation Test-Revised and cynical hostility by the cynicism subscale of the Cook Medley Questionnaire. Cox proportional hazard models produced adjusted hazard ratios (AHRs) for incident CHD (myocardial infarction, angina, percutaneous coronary angioplasty, or coronary artery bypass surgery) and total mortality (CHD, cardiovascular disease, or cancer related) over approximately 8 years. Optimists (top versus bottom quartile ["pessimists"]) had lower age-adjusted rates (per 10 000) of CHD (43 versus 60) and total mortality (46 versus 63). The most cynical, hostile women (top versus bottom quartile) had higher rates of CHD (56 versus 44) and total mortality (63 versus 46). Optimists (versus pessimists) had a lower hazard of CHD (AHR 0.91, 95% CI 0.83 to 0.99), CHD-related mortality (AHR 0.70, 95% CI 0.55 to 0.90), cancer-related mortality (blacks only; AHR 0.56, 95% CI 0.35 to 0.88), and total mortality (AHR 0.86, 95% CI 0.79 to 0.93). Most (versus least) cynical, hostile women had a higher hazard of cancer-related mortality (AHR 1.23, 95% CI 1.09 to 1.40) and total mortality (AHR 1.16, 95% CI 1.07 to 1.27; this effect was pronounced in blacks). Effects of optimism and cynical hostility were independent.ConclusionsOptimism and cynical hostility are independently associated with important health outcomes in black and white women. Future research should examine whether interventions designed to change attitudes would lead to altered risk.

Project description:Intimate partner aggression (IPA) is a significant public health burden, and thus efforts to identify individual characteristics that predict perpetration of IPA is an important research question. Personality disorders are a significant correlate of IPA, both cross-sectionally and longitudinally. Much of that work, however, had been conducted in younger samples. IPA in older adults is a relatively understudied area of research generally. Thus, examining the association between personality pathology and IPA in older adults is novel and important. In previous work using the St. Louis Personality and Aging Network sample, we found that borderline personality disorder was a robust correlate of perpetrating partner aggression in older, romantically involved participants. In the current analyses, we attempted to replicate our original findings in the same sample assessed 2.5 years later. We also extended the original work by using both self- and partner-reported IPA and by using latent factors of personality pathology. We found that prevalence of IPA in this sample was similar to the baseline assessment; 69.1% reported IPA in the past year, with the vast majority being psychological/verbal rather than physical aggression. Agreement between partners on perpetration was modest. Borderline personality pathology was again the strongest and most consistent predictor of IPA. Our findings speak to the continuing public health burden of borderline personality pathology into older age as evidenced by associations with perpetration of IPA. Project description:Loneliness has been linked to poor mental and physical health outcomes. Past research suggests that inflammation is a potential pathway linking loneliness and health, but little is known about how loneliness assessed in daily life links with inflammation, or about linkages between loneliness and inflammation among older adults specifically. As part of a larger investigation, we examined the cross-sectional associations between loneliness and a panel of both basal and LPS-stimulated inflammatory markers. Participants were 222 socioeconomically and racially diverse older adults (aged 70-90 years; 38% Black; 13% Hispanic) systematically recruited from the Bronx, NY. Loneliness was measured in two ways, with a retrospective trait measure (the UCLA Three Item Loneliness Scale) and an aggregated momentary measure assessed via ecological momentary assessment (EMA) across 14 days. Inflammatory markers included both basal levels of C-reactive protein (CRP) and cytokines (IL-1β, IL-4, IL-6, IL-8, IL-10, TNF-α) and LPS-stimulated levels of the same cytokines. Multiple regression analyses controlled for age, body-mass index, race, and depressive symptoms. Moderation by gender and race were also explored. Both higher trait loneliness and aggregated momentary measures of loneliness were associated with higher levels of CRP (β = 0.16, p = 0.02; β = 0.15, p = 0.03, respectively). There were no significant associations between loneliness and basal or stimulated cytokines and neither gender nor race were significant moderators. Results extend prior research linking loneliness with systemic inflammation in several ways, including by examining this connection among a sample of older adults and using a measure of aggregated momentary loneliness.

Project description:More than two decades of research has consistently indicated that feelings of loneliness among older people are more common in southern Europe than in its northern parts, with the lowest rates in Denmark and Sweden. Our analyses based on analysis of 2004-2006 data from 8,787 individuals aged 65 years or older in the SHARE project replicate, update, and extend these findings. We found, similar to previous studies, that the prevalence of feelings of loneliness was more common in the Mediterranean countries than in Northern Europe. Living together with a spouse/partner was consistently associated with the lower prevalence of loneliness across countries. The combination of living alone and having bad health was associated with 10 times higher odds of feeling lonely as compared with living together with someone and having good health. With regard to gender and health, we found signs of differences between countries in how these factors were related to loneliness. Our results indicate the importance of both contextual features and cultural expectations in interpreting reported loneliness, that is, loneliness across Europe has both nomothetic and idiographic features.

Project description:Background and objectivesMany older adults remain inactive despite the known positive health implications of physical activity (improved mood, reduced mortality risk). Physical inactivity is an interdependent phenomenon in couples, but most research examines physical inactivity at the individual level. We estimated the average amount of prolonged physical inactivity for older adult couples and, using dyadic analysis, identified physical and mental health determinants thereof.Research design and methodsForty-six heterosexual older adult couples (age = 70.61 ± 6.56) from the Veterans Integrated Service Network 20 cohort of the Collaborative Aging Research using Technology (CART) initiative were included. The average number per day of prolonged inactive periods (no step counts or sleep activity for ≥30 min) was estimated using actigraphy data collected over a month.ResultsMultilevel modeling revealed that, within couples, there was no significant difference between partners in the average amount of inactive periods (p = .28). On average across couples, males and females had an average of 6.90 ± 2.02 and 6.56 ± 1.93 inactive periods per day, respectively. For males, older age was the only variable associated with more inactive periods (β = 0.15, p = .002). For females, having more depressive symptoms in both dyad members was associated with fewer inactive periods (female: β = -0.30, p = .03; male: β = -0.41, p < .001), and more dependence in completing their own instrumental activities of daily living predicted more inactive periods (β = 2.58, p < .001).Discussion and implicationsViewing couples' activity as an interdependent phenomenon, rather than individual, provides a novel approach to identifying pathways to reduce inactivity in older adults, especially when focusing on the mental health and level of independence within the couple.
