A method for material decomposition and quantification with grating based phase CT.

ABSTRACT: Material decomposition (MD) is an important application of computer tomography (CT). For phase contrast imaging, conventional MD methods are categorized into two types with respect to different operation sequences, i.e., "before" or "after" image reconstruction. Both categories come down to two-step methods, which have the problem of noise amplification. In this study, we incorporate both phase and absorption (PA) information into MD process, and correspondingly develop a simultaneous algebraic reconstruction technique (SART). The proposed method is referred to as phase & absorption material decomposition-SART (PAMD-SART). By iteratively solving an optimization problem, material composition and substance quantification are reconstructed directly from absorption and differential phase projections. Comparing with two-step MD, the proposed one-step method is superior in noise suppression and accurate decomposition. Numerical simulations and synchrotron radiation based experiments show that PAMD-SART outperforms the classical MD method (image-based and dual-energy CT iterative method), especially for the quantitative accuracy of material equivalent atomic number.

Project description:BackgroundQuantitative CT imaging, particularly iodine and calcium quantification, is an important CT-based biomarker.PurposeThis study quantifies sources of errors in quantitative CT imaging in both single-energy and spectral CT.Materials and methodsThis work examines the theoretical relationship between CT numbers, linear attenuation coefficient, and material quantification. We derive four understandings: (1) CT numbers are not proportional with element mass in vivo, (2) CT numbers are proportional with element mass only when contained in a voxel of pure water, (3) iodine-water material decomposition is never accurate in vivo, and (4) for error-free material decomposition a voxel must only consist of the basis decomposition vectors. Misinterpretation-based errors are calculated using the National Institute of Standards and Technology (NIST) XCOM database for: tissue chemical compositions, clinical concentrations of hydroxyapatite (HAP), and iodine. Quantification errors are also demonstrated experimentally using phantoms.ResultsIn single-energy CT, misinterpretation-induced errors for HAP density in adipose, muscle, lung, soft tissue, and blood ranged from 0-132%, i.e., a mass error of 0-749 mg/cm3. In spectral CT, errors with iodine in the same tissues resulted in a range of < 0.1-33% error, resulting in a mass error of < 0.1-1.2 mg/mL.ConclusionOur work demonstrates material quantification is fundamentally limited when measured in vivo due to measurement conditions differing from assumed and the errors are at or above detection limits for bone mineral density (BMD) and spectral iodine quantification. To define CT-derived biomarkers, the errors we demonstrate should either be avoided or built into uncertainty bounds.Clinical relevance statementImproving error bounds in quantitative CT biomarkers, specifically in iodine and BMD quantification, could lead to improvements in clinical care aspects based on quantitative CT.Key pointsCT numbers are only proportional with element mass only when contained in a voxel of pure water, therefore iodine-water material decomposition is never accurate in vivo. Project description:PurposeThe dual-energy CT (DECT) LiverVNC application class in the Siemens Syngo.via software has been used to perform non-iodine material decompositions. However, the LiverVNC application is designed with an optional size-specific calibration based on iodine measurements. This work investigates the effects of this iodine-based size-specific calibration on non-iodine material decomposition and benchmarks alternative methods for size-specific calibrations.MethodsCalcium quantification was performed with split-filter and sequential-scanning DECT techniques on the Siemens SOMATOM Definition Edge CT scanner. Images were acquired of the Gammex MECT abdomen and head phantom containing calcium inserts with concentrations ranging from 50-300 mgCa/ml. Several workflows were explored investigating the effects of size-specific dual-energy ratios (DERs) and the beam hardening correction (BHC) function in the LiverVNC application. Effects of image noise were also investigated by varying CTDIvol and using iterative reconstruction (ADMIRE).ResultsWith the default BHC activated, Syngo.via underestimated the calcium concentrations in the abdomen for sequential-scanning acquisitions, leaving residual calcium in the virtual non-contrast images and underestimating calcium in the enhancement images for all DERs. Activation of the BHC with split-filter images resulted in a calcium over- or underestimation depending on the DER. With the BHC inactivated, the use of a single DER led to an under- or overestimate of calcium concentration depending on phantom size and DECT modality. Optimal results were found with BHC inactivated using size-specific DERs. CTDIvol levels and ADMIRE had no significant effect on results.ConclusionWhen performing non-iodine material decomposition in the LiverVNC application class, it is important to understand the implications of the BHC function and to account for patient size appropriately. The BHC in the LiverVNC application is specific to iodine and leads to inaccurate quantification of other materials. The inaccuracies can be overcome by deactivating the BHC function and using size-specific DERs, which provided the most accurate calcium quantification.

Project description:BackgroundDual-energy computed tomography (DECT) is a noninvasive diagnostic tool for gouty arthritis. This study aimed to compare two postprocessing techniques for monosodium urate (MSU) detection: conventional two-material decomposition and material map-based decomposition.MethodsA raster phantom and an ex vivo biophantom, embedded with four different MSU concentrations, were scanned in two high-end CT scanners. Scanner 1 used the conventional postprocessing method while scanner 2 employed the material map approach. Volumetric analysis was performed to determine MSU detection, and image quality parameters, such as signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), were computed.ResultsThe material map-based method demonstrated superior MSU detection. Specifically, scanner 2 yielded total MSU volumes of 5.29 ± 0.28 mL and 4.52 ± 0.29 mL (mean ± standard deviation) in the raster and biophantom, respectively, versus 2.35 ± 0.23 mL and 1.15 ± 0.17 mL for scanner 1. Radiation dose correlated positively with detection for the conventional scanner, while there was no such correlation for the material map-based decomposition method in the biophantom. Despite its higher detection rate, material map-based decomposition was inferior in terms of SNR, CNR, and artifacts.ConclusionWhile material map-based decomposition resulted in superior MSU detection, it is limited by challenges such as increased artifacts. Our findings highlight the potential of this method for gout diagnosis while underscoring the need for further research to enhance its clinical reliability.Relevance statementAdvanced postprocessing such as material-map-based two-material decomposition might improve the sensitivity for gouty arthritis in clinical practice, thus, allowing for lower radiation doses or better sensitivity for gouty tophi.Key pointsDual-energy CT showed limited sensitivity for tophi with low MSU concentrations. Materiel-map-based decomposition increased sensitivity compared to conventional two-material decomposition. The advantages of material-map-based decomposition outweigh lower image quality and increased artifact load.

Project description:The authors present initial progress toward a clinically compatible x-ray phase contrast CT system, using glancing-angle x-ray grating interferometry to provide high contrast soft tissue images at estimated by computer simulation dose levels comparable to conventional absorption based CT.DPC-CT scans of a joint phantom and of soft tissues were performed in order to answer several important questions from a clinical setup point of view. A comparison between high and low fringe visibility systems is presented. The standard phase stepping method was compared with sliding window interlaced scanning. Using estimated dose values obtained with a Monte-Carlo code the authors studied the dependence of the phase image contrast on exposure time and dose.Using a glancing angle interferometer at high x-ray energy (∼ 45 keV mean value) in combination with a conventional x-ray tube the authors achieved fringe visibility values of nearly 50%, never reported before. High fringe visibility is shown to be an indispensable parameter for a potential clinical scanner. Sliding window interlaced scanning proved to have higher SNRs and CNRs in a region of interest and to also be a crucial part of a low dose CT system. DPC-CT images of a soft tissue phantom at exposures in the range typical for absorption based CT of musculoskeletal extremities were obtained. Assuming a human knee as the CT target, good soft tissue phase contrast could be obtained at an estimated absorbed dose level around 8 mGy, similar to conventional CT.DPC-CT with glancing-angle interferometers provides improved soft tissue contrast over absorption CT even at clinically compatible dose levels (estimated by a Monte-Carlo computer simulation). Further steps in image processing, data reconstruction, and spectral matching could make the technique fully clinically compatible. Nevertheless, due to its increased scan time and complexity the technique should be thought of not as replacing, but as complimentary to conventional CT, to be used in specific applications.
