Pediatric surgical wait priority score (pSWAPS): Modifying a health system's adult-based elective surgery prioritization system for children's surgery during the COVID-19 pandemic.

ABSTRACT:

Background

With the rise of COVID-19 cases, societies recommended canceling all elective surgical procedures because of perioperative concerns, transmission risk, and the need to divert resources. Once the number of cases stabilized, there was recognition that a system was needed to triage and prioritize scheduling operations.

Methods

A universal scoring system to triage surgical elective cases was developed for the Advocate Aurora Health system (Surgical Wait Priority Score, SWAPS) and was modified for use in pediatrics (pSWAPS). Resource-related, patient-related, and case urgency factors were used to create the overall score. Interrater reliability of ten cases was determined by four surgeons' scores and calculating Fleiss' Kappa coefficient. The system has been used for two months at two operating rooms with different resource restrictions with the goal of prioritizing elective cases.

Results

18 factors were identified as significant contributors to the pWAPS creating a cumulative score ranging from 0 to 120. In the first month, 61 and 99 procedures were screened at the Oak Lawn (OL) and Park Ridge (PR) campuses respectively, and in the second month, 94 (OL) and 135 (PR) procedures were evaluated. The average pSWAPS scores were 37.9 at OL and 54.3 at PR. All cases that had scores within the immediate group were scheduled and completed.

Conclusion

The pSWAPS system is a simple, flexible scoring system that takes into consideration resource constraints. pSWAPS has been used for two months. It has served as an effective tool for safe and methodical reintroduction of elective procedures during the COVID-19 pandemic and could be used again for another surge.

Project description:Assessment of need for intravascular volume resuscitation remains challenging for anesthesiologists. Dynamic waveform indices, including systolic and pulse pressure variation, are demonstrated as reliable measures of fluid responsiveness for mechanically ventilated patients. Despite widespread use, real-world reference distributions for systolic and pulse pressure variation values have not been established for euvolemic intraoperative patients. The authors sought to establish systolic and pulse pressure variation reference distributions and assess the impact of modifying factors.The authors evaluated adult patients undergoing general anesthetics for elective noncardiac surgery. Median systolic and pulse pressure variations during a 50-min postinduction period were noted for each case. Modifying factors including body mass index, age, ventilator settings, positioning, and hemodynamic management were studied via univariate and multivariable analyses. For systolic pressure variation values, effects of data entry method (manually entered vs. automated recorded) were similarly studied.Among 1,791 cases, per-case median systolic and pulse pressure variation values formed nonparametric distributions. For each distribution, median values, interquartile ranges, and reference intervals (2.5th to 97.5th percentile) were, respectively, noted: these included manually entered systolic pressure variation (6.0, 5.0 to 7.0, and 3.0 to 11.0 mmHg), automated systolic pressure variation (4.7, 3.9 to 6.0, and 2.2 to 10.4 mmHg), and automated pulse pressure variation (7.0, 5.0 to 9.0, and 2.0 to 16.0%). Nonsupine positioning and preoperative ? blocker were independently associated with altered systolic and pulse pressure variations, whereas ventilator tidal volume more than 8?ml/kg ideal body weight and peak inspiratory pressure more than 16?cm H2O demonstrated independent associations for systolic pressure variation only.This study establishes real-world systolic and pulse pressure variation reference distributions absent in the current literature. Through a consideration of reference distributions and modifying factors, the authors' study provides further evidence for assessing intraoperative volume status and fluid management therapies.

Project description:BackgroundCurrent principles of postoperative pain management are primarily based on the types and extent of surgical intervention. This clinical study measured patient's self-anticipated pain score before surgery, and compared the anticipated scores with the actual pain levels and analgesic requirements after surgery.MethodsThis prospective observational study recruited consecutive patients who received elective surgery in the E-Da Hospital, Taiwan from June to August 2018. Patients were asked to subjectively rate their highest anticipated pain level (numeric rating scale, NRS 0-10) for the scheduled surgical interventions during their preoperative anesthesia assessment. After the operation, the actual pain intensity (NRS 0-10) experienced by the patient in the post-anesthesia care unit and the total dose of opioids administered during the perioperative period were recorded. Pain scores ≥4 on NRS were regarded as being unacceptable levels for anticipated or postoperative pain that required more aggressive intervention.ResultsA total of 996 patients were included in the study. Most of the patients (86%) received general anesthesia and 73.9% of them had a history of previous operation. Female anticipated significantly higher overall pain intensities than the male patients (adjusted odd ratio 1.523, 95% confidence interval 1.126-2.061; P = 0.006). Patients who took regular benzodiazepine at bedtime (P = 0.037) and those scheduled to receive more invasive surgical procedures were most likely to anticipate for higher pain intensity at the preoperative period (P < 0.05). Higher anticipated pain scores (preoperative NRS ≥ 4) were associated with higher actual postoperative pain levels (P = 0.007) in the PACU and higher total equivalent opioid use (P < 0.001) for acute pain management during the perioperative period.ConclusionThis observational study found that patients who are female, use regular benzodiazepines at bedtime and scheduled for more invasive surgeries anticipate significantly higher surgery-related pain. Therefore, appropriate preoperative counseling for analgesic control and the management of exaggerated pain expectation in these patients is necessary to improve the quality of anesthesia delivered and patient's satisfaction.
