One clot after another in COVID-19 patient: diagnostic utility of handheld echocardiogram.

ABSTRACT: A 63-year-old woman was admitted with severe respiratory distress requiring mechanical ventilation and shock requiring vasopressor support. She was found to have COVID-19 pneumonia. Focused cardiac ultrasound performed for evaluation of shock was significant for right ventricular dilation and dysfunction with signs of right ventricular pressure overload. Given worsening shock and hypoxemia systemic thrombolysis was administered for presumed massive pulmonary embolism with remarkable improvement of hemodynamics and respiratory failure. In next 24 h patient's neurologic status deteriorated to the point of unresponsiveness. Emergent computed tomography showed multiple ischemic infarcts concerning for embolic etiology. Focused cardiac ultrasound with agitated saline showed large right to left shunt due to a patent foramen ovale. This was confirmed by transesophageal echocardiogram, 5 months later. This case highlights strengths of focused cardiac ultrasound in critical care setting and in patients with COVID-19 when access to other imaging modalities can be limited.

COVID-19 and mental health: How one pandemic can reveal another.

Project description:The COVID-19 pandemic disproportionately affected individuals with mental disorders, and revealed fundamental flaws in how vulnerable persons are treated in the context of such crises. Much of this difficulty may be attributed to ignorance of the prevalence, severity and economic burden associated with these conditions, as well as to enduring inequalities in how physical illness is treated in comparison to mental illness. As mental disorders are now the single greatest cause of disability, we have reached the point where the tremendous personal and societal costs associated with these conditions can no longer be ignored. Dramatic changes are needed to replace the slow, incremental efforts that most often characterize public health policy. Such changes can no longer wait for the national or international-level solutions that were once hoped, but they may be just as effective through the use of new technologies, grass-roots organization, and initiatives on a local scale.

Diagnostic utility of the Covichem score in predicting COVID-19 disease.

Project description:BackgroundIdentifying which patients with COVİD-19 have a high risk of severe illness is essential to optimizing management and resource utilization strategies.ObjectivesThe aim of this study was to externally validate the diagnostic utility of the Covichem score for predicting COVID-19 disease severity, and secondarily to evaluate its utility in predicting intensive care unit (ICU) admission, and in-hospital mortality.MethodsAll consecutive COVID-19 patients who presented to the emergency department (ED) were included, and patients' demographic data, comorbidities, vital signs, oxygen requirement, and laboratory results were recorded. We calculated patients' Covichem scores and estimates (using a threshold of 0.5) and evaluated the utility of the Covichem score for predicting disease severity, ICU admission, and mortality.ResultsThe median Covichem score was significantly higher for patients with severe illness (Covichem score: 0.170, IQR: 0.298, n = 300 vs. Covichem score: 0.026, IQR: 0.065, n: 191; p < 0.001). Based on their Covichem scores, 12.4% (61/491) of the patients were predicted to experience severe illness (threshold: 0.5), the accuracy of the Covichem score was poor, as the area under curve (AUC) was 48.5% (18.1% sensitivity and 93.8% specificity). When we calculated a new ideal threshold, the AUC reached 82%, but the sensitivity was 79.9% and the specificity was 71.2%.ConclusionIn this external validation of the Covichem score, we found that it performed worse than in the original derivation and validation study, even with the assistance of a new cutoff.

COVID-19: Living through Another Pandemic.

Project description:Novel beta-coronavirus SARS-CoV-2 is the pathogenic agent responsible for coronavirus disease-2019 (COVID-19), a globally pandemic infectious disease. Due to its high virulence and the absence of immunity among the general population, SARS-CoV-2 has quickly spread to all countries. This pandemic highlights the urgent unmet need to expand and focus our research tools on what are considered "neglected infectious diseases" and to prepare for future inevitable pandemics. This global emergency has generated unprecedented momentum and scientific efforts around the globe unifying scientists from academia, government and the pharmaceutical industry to accelerate the discovery of vaccines and treatments. Herein, we shed light on the virus structure and life cycle and the potential therapeutic targets in SARS-CoV-2 and briefly refer to both active and passive immunization modalities, drug repurposing focused on speed to market, and novel agents against specific viral targets as therapeutic interventions for COVID-19.

A novel One-pot rapid diagnostic technology for COVID-19

Project description:Novel coronavirus disease (COVID-19) caused by SARS-CoV-2 is an ongoing global pandemic associated with high rates of morbidity and mortality. RT-qPCR has become the diagnostic standard for the testing of SARS-CoV-2 in most countries. COVID-19 diagnosis generally relies upon RT-qPCR-mediated identification of SARS-CoV-2 viral RNA, which is costly, labor-extensive, and requires specialized training and equipment. Herein, we established a novel one-tube rapid diagnostic approach based upon formamide and colorimetric RT-LAMP (One-Pot RT-LAMP) that can be used to diagnose COVID-19 without the extraction of specific viral RNA. The technique could visually detect SARS-CoV-2 within 45 min with a limit of detection of 5 copies per reaction in extracted RNA, and about 7.66 virus copies per ?L in viral transport medium. The One-Pot RT-LAMP test showed a high specificity without cross-reactivity with 12 viruses including SARS-CoV, MERS-CoV, and human infectious influenza virus (H1N1/H3N2 of influenza A and B virus, ect. We validated this One-Pot RT-LAMP approach by its successful use for the analysis of 45 clinical nasopharyngeal swab samples, yielding results identical to those of traditional RT-qPCR analyses, while achieving good selectivity and sensitivity relative to a commercial RT-qPCR approach. As such, this One-Pot RT-LAMP technology may be a valid means of conducting high-sensitivity, low-cost and rapid SARS-CoV-2 identification without the extraction of viral RNA.

Recurrent Chest Pain After COVID-19: Diagnostic Utility of Cardiac Magnetic Resonance Imaging.

Project description:We report a case of myocarditis in an adult patient with recent coronavirus disease 2019 (COVID-19) infection presenting as recurrent ST-segment elevation, mimicking coronary vasospasm. This case highlights the wide range of presentations of COVID-19-related myocarditis. The novel teaching point is that COVID-19 myocarditis can present with acute manifestations such as chest pain and transient ST-segment elevation even several weeks after complete recovery from the initial infection. Cardiac magnetic resonance imaging should be considered in patients with chest pain syndromes and angiographically normal coronary arteries, as the presence of late gadolinium enhancement and a high T2 signal can be diagnostic. Follow-up cardiac magnetic resonance imaging may be used to assess resolution.

One train may hide another: Acute cardiovascular diseases could be neglected because of the COVID-19 pandemic.

Project description:BackgroundCoronavirus disease 2019 (COVID-19) is likely to have significant implications for the cardiovascular care of patients. In most countries, containment has already started (on 17 March 2020 in France), and self-quarantine and social distancing are reducing viral contamination and saving lives. However, these considerations may only be the tip of the iceberg; most resources are dedicated to the struggle against COVID-19, and this unprecedented situation may compromise the management of patients admitted with cardiovascular conditions.AimWe aimed to assess the effect of COVID-19 containment measures on cardiovascular admissions in France.MethodsWe asked nine major cardiology centres to give us an overview of admissions to their nine intensive cardiac care units for acute myocardial infarction or acute heart failure, before and after containment measures.ResultsBefore containment (02-16 March 2020), the nine participating intensive cardiac care units admitted 4.8±1.6 patients per day, versus 2.6±1.5 after containment (17-22 March 2020) (rank-sum test P=0.0006).ConclusionsWe confirm here, for the first time, a dramatic drop in the number of cardiovascular admissions after the establishment of containment. Many hypotheses might explain this phenomenon, but we feel it is time raise the alarm about the risk for patients presenting with acute cardiovascular disease, who may suffer from lack of attention, leading to severe consequences (an increase in the number of ambulatory myocardial infarctions, mechanical complications of myocardial infarction leading to an increase in the number of cardiac arrests, unexplained deaths, heart failure, etc.). Similar consequences can be feared for all acute situations, beyond the cardiovascular disease setting.
