Exposure to Leptospira spp. and Associated Risk Factors in the Human, Cattle and Dog Populations in Bhutan.

ABSTRACT: Leptospirosis is a neglected worldwide zoonotic bacterial disease with a high prevalence in subtropical and tropical countries. The prevalence of Leptospira spp. in humans, cattle and dogs is unknown in Bhutan. Therefore, we sought to find out whether humans, cattle or dogs had been infected in the past with leptospires by measuring antibodies in the serum. We therefore collected blood from 864 humans ≥13 years of age, 130 bovines and 84 dogs from different rural and urban areas in Bhutan and tested the serum for antibodies specific for leptospires with a screening of enzyme-linked immunosorbent assays (ELISA) and a confirmatory microscopic agglutination test (MAT). In humans, 17.6% were seropositive by ELISA and 1.6% by MAT. The seropositivity was stronger in bovines (36.9%) and dogs (47.6%). "Having had a fever recently" (OR 5.2, p = 0.004), "working for the military" (OR 26.6, p = 0.028) and "being unemployed" (OR 12.9, p = 0.041) (reference category = housemaker) were statistically significantly associated with seropositivity when controlled for the effects of other risk factors. However, due to the small number of positive test results, the findings on risk factors should be interpreted with caution. Based on the serogroups found in the three species, dogs could be a source of infection for humans, or dogs and humans are exposed to the same environmental risk factors Clinical leptospirosis in humans and domestic animals should be investigated by testing blood and urine for the presence of leptospires by molecular methods (qPCR).

Project description:BackgroundSmallholder dairy farming is crucial for the Tanzanian dairy sector which generates income and employment for thousands of families. This is more evident in the northern and southern highland zones where dairy cattle and milk production are core economic activities. Here we estimated the seroprevalence of Leptospira serovar Hardjo and quantified potential risk factors associated with its exposure in smallholder dairy cattle in Tanzania.MethodsFrom July 2019 to October 2020, a cross-sectional survey was carried out in a subset of 2071 smallholder dairy cattle. Information about animal husbandry and health management was collected from farmers, and blood was taken from this subset of cattle. Seroprevalence was estimated and mapped to visualize potential spatial hotspots. The association between a set of animal husbandry, health management and climate variables and ELISA binary results was explored using a mixed effects logistic regression model.ResultsAn overall seroprevalence of 13.0% (95% CI 11.6-14.5%) for Leptospira serovar Hardjo was found in the study animals. There was marked regional variations with the highest seroprevalence in Iringa 30.2% (95% CI 25.1-35.7%) and Tanga 18.9% (95% CI 15.7-22.6) with odds ratios of OR = 8.13 (95% CI 4.23-15.63) and OR = 4.39 (95% CI 2.31-8.37), respectively. Multivariate analysis revealed the individual animal factors that were a significant risk for Leptospira seropositivity in smallholder dairy cattle were: animals over 5 years of age (OR = 1.41, 95% CI 1.05-1.9); and indigenous breed (OR = 2.78, 95% CI 1.47-5.26) compared to crossbred animals SHZ-X-Friesian (OR = 1.48, 95% CI 0.99-2.21) and SHZ-X-Jersey (OR = 0.85, 95% CI 0.43-1.63). Farm management factors significantly associated with Leptospira seropositivity included: hiring or keeping a bull for raising purposes (OR = 1.91, 95% CI 1.34-2.71); distance between farms of more than 100 meters (OR = 1.75, 95% CI 1.16-2.64); cattle kept extensively (OR = 2.31, 95% CI 1.36-3.91); farms without cat for rodent control (OR = 1.87, 95% CI 1.16-3.02); farmers with livestock training (OR = 1.62, 95% CI 1.15-2.27). Temperature (OR = 1.63, 95% CI 1.18-2.26), and the interaction of higher temperature and precipitation (OR = 1.5, 95%CI 1.12-2.01) were also significant risk factors.ConclusionThis study indicated seroprevalence of Leptospira serovar Hardjo, as well as the risk factors driving dairy cattle leptospirosis exposure in Tanzania. The study showed an overall high leptospirosis seroprevalence with regional variations, where Iringa and Tanga represented the highest seroprevalence and risk. The study highlighted the urgent need to understand the human exposures and risks from this important zoonosis to develop control measures and awareness of the problem and quantify the economic and production impacts through abortion and milk loss. In addition, given that the available data was limited to Leptospira serovar Hardjo, the study recommends more studies to identify serologically the most common serovars in cattle for targeted vaccination and risk reduction.

Project description:Tick-borne pathogens (TBPs) seriously affect cattle production and can be economically damaging. The epidemiology of these organisms in the Chongqing municipality of China is not well described. This study aimed to investigate the prevalence and risk factors of TBPs including Anaplasma spp., Babesia spp. and Theileria spp. in cattle in Chongqing municipality. The results showed that 43.48% (150/345) of cattle were infected with at least one TBP, of which single infections were detected in 104 (30.14%), double infections in 34 cattle (9.86%) and triple infections in 12 (3.48%) of the cattle. The overall prevalence of Anaplasma spp., Theileria spp. and B. bigemina were 22.32%, 23.19% and 7.24%, respectively. Among these, the prevalence of A. bovis, A. central, A. phagocytophilum, A. platys, A. marginale, T. sinensisi and T. orientalis were 8.41%, 7.83%, 4.93%, 4.35%, 2.61%, 22.32% and 2.60%, respectively. We could not detect B. bovis, T. annulata, T. luwenshuni or T. uilenbergi in cattle. Cattle ≥1-year-old were more likely to be infected with Theileria spp. [adjusted odd ratio (AOR) = 2.70, 95% CI = 1.12-6.56)] compared with younger cattle, while cattle ≥1-year-old had reduced susceptibility to B. bigemina (AOR = 0.14, 95% CI = 0.03-0.60). Cattle living at higher altitude (≥500 m) were more susceptible to B. bigemina (AOR = 6.97, 95% CI = 2.08-23.35) and Theileria spp. infection (AOR = 1.87, 95% CI = 1.06-3.32). The prevalence of Theileria spp. on farms with cats was significantly higher than that without cats (AOR = 2.56, 95% CI = 1.12-5.88). Infection with A. bovis and A. central were significantly associated with A. phagocytophilum infection. Furthermore, there were significant associations between A. bovis and A. central infection, T. sinensisi and A. marginale infection, and B. bigemina and T. orientalis infection. This study provides new data on the prevalence of Anaplasma spp., Babesia spp. and Theileria spp. in cattle in Chongqing, and for the first time we reveal a possible relationship between the afore-mentioned pathogens, which will help in formulating appropriate control strategies for these pathogens in this area.

Project description:Leptospirosis is a significant zoonotic disease that causes high economic losses in cattle production due to its association with abortions, stillbirths, infertility, and reduced milk yields. However, the epidemiology of bovine leptospirosis in Ethiopia is poorly understood. From October 2020 to October 2021, a cross-sectional study was conducted to investigate the seroprevalence of serovar Hardjo in cattle in southwest Ethiopia, as well as the associated risk factors. To test for the existence of L. Hardjo antibodies, blood samples were taken from 461 cattle. Indirect ELISA was used to identify the presence of antibodies against L. Hardjo in sera samples. We conducted a multivariable random-effect logistic regression analysis to identify potential risk factors associated with L. Hardjo seropositivity. An overall L. Hardjo seroprevalence of 24.7% (95% CI: 20.2-48.8) and 53.5% (95% CI: 45.7-90.5) was recorded at the animal level and the herd level, respectively, in the study areas. This study revealed six factors influencing L. Hardjo seropositivity in cattle herds. Large herds had twice the odds of seropositivity (OR = 2.0, 95% CI: 1.1-3.8) compared to small herds. Co-grazing cattle exhibited higher odds (OR = 2.2, 95% CI: 1.2-4.1) of seropositivity. Extensive management systems significantly increased the odds (OR = 10.3, 95% CI: 1.7-61.8) compared to semi-intensive systems. Highland cattle had higher odds (OR = 3.7, 95% CI: 1.4-10.3) than lowland cattle. Older cattle (OR = 4.6, 95% CI: 2.4-8.9) were more likely to be seropositive. At the herd level, extensive management (OR = 2.8, 95% CI: 1.3-5.8) and large herds (OR = 2.5, 95% CI: 1.3-4.7) increased the risk of seropositivity. Herds with sheep/goats (OR = 2.3, 95% CI: 1.3-4.1) were also at higher risk, highlighting significant L. Hardjo seropositivity risk factors in cattle herds. The study findings showed that leptospirosis was highly prevalent across the study areas. As a result, use proper management, raise zoonotic awareness for leptospirosis, and conduct molecular bovine leptospirosis research in study areas were recommended.

Project description:Shedding of DNA of pathogenic Leptospira spp. has been documented in naturally infected cats in several countries, but urinary shedding of infectious Leptospira spp. has only recently been proven. The climate in Southern Chile is temperate rainy with high annual precipitations which represents ideal preconditions for survival of Leptospira spp., especially during spring and summer. The aims of this study were to investigate shedding of pathogenic Leptospira spp. in outdoor cats in Southern Chile, to perform molecular characterization of isolates growing in culture, and to assess potential risk factors associated with shedding. Urine samples of 231 outdoor cats from rural and urban areas in southern Chile were collected. Urine samples were investigated for pathogenic Leptospira spp. by 4 techniques: qPCR targeting the lipL32 gene, immunomagnetic separation (IMS)-coupled qPCR (IMS-qPCR), direct culture and IMS-coupled culture. Positive urine cultures were additionally confirmed by PCR. Multilocus sequence typing (MLST) was used to molecularly characterize isolates obtained from positive cultures. Overall, 36 urine samples (15.6%, 95% confidence interval (CI) 11.4-20.9) showed positive results. Eighteen (7.8%, 95% CI 4.9-12.1), 30 (13%, 95% CI 9.2-18), 3 (1.3%, 0.3-3.9) and 4 cats (1.7%; 95% CI 0.5-4.5) were positive in qPCR, IMS-qPCR, conventional culture, and IMS-coupled culture, respectively. MLST results of 7 culture-positive cats revealed sequences that could be assigned to sequence type 17 (6 cats) and sequence type 27 (1 cat) corresponding to L. interrogans (Pathogenic Leptospira Subgroup 1). Shedding of pathogenic Leptospira spp. by cats might be an underestimated source of infection for other species including humans. The present study is the first one reporting growth of leptospires from feline urine in culture in naturally infected cats in South-America and characterisation of culture-derived isolates. So far, very few cases of successful attempts to culture leptospires from naturally infected cats are described worldwide.
