Socioeconomic, demographic and healthcare determinants of the COVID-19 pandemic: an ecological study of Spain.

ABSTRACT:

Background

The coronavirus disease 2019 (COVID-19) pandemic has posed a major challenge to health, economic and political systems around the world. Understanding the socioeconomic, demographic and health determinants affecting the pandemic is of interest to stakeholders. The purpose of this ecological study is to analyse the effect of the different socioeconomic, demographic and healthcare determinants on the mortality rate and estimated cumulative incidence of COVID-19 first wave in the Spanish regions.

Methods

From the available data of the 17 Spanish regions (Autonomous Communities), we have carried out an ecological study through multivariate linear regression using ordinary least squares. To do this, we conducted an analysis using two distinct dependent variables: the logarithm of mortality rate per 1,000,000 inhabitants and the estimated cumulative incidence. The study has 12 explanatory variables.

Results

After applying the backward stepwise multivariate analysis, we obtained a model with nine significant variables at different levels for mortality rate and a model with seven significant variables for estimated cumulative incidence. Among them, six variables are statistically significant and of the same sign in both models: "Nursing homes beds", "Proportion of care homes over 100 beds", "Log GDP per capita", "Aeroplane passengers", "Proportion of urban people", and the dummy variable "Island region".

Conclusions

The different socioeconomic, demographic and healthcare determinants of each region have a significant effect on the mortality rate and estimated cumulative incidence of COVID-19 in territories where the measures initially adopted to control the pandemic have been identical.

Project description:BackgroundDuring the COVID-19 pandemic health crisis, in some countries such as Spain, nursing students have offered to provide health assistance, but the role they have played, their degree of preparedness to face the situation, and what must be improved in their training to be ready for these situations is unknown.ObjectiveDescribe the experience and perceptions of students of the Nursing university degree during their participation as health support in the COVID-19 health crisis in Spain.DesignWe conducted a cross-sectional survey study.Method503 students enrolled in the 4th course of the Nursing degree in Spain during the months of March and April 2020. An online questionnaire was developed, based on a pilot study and distributed through the Nursing Association, students' unions and students' associations. Variables were used to describe their participation, degree of preparedness and training needs to determine how to improve training through descriptive statistics, as well as nonparametric tests to analyse the relationship between training and degree of preparedness nursing students. Results are reported according to the STROBE Statement.Results73.2% (368) of students offered to participate in healthcare aid, of which 225 were actively involved. 27.8% carried out nursing tasks without supervision, and 47.7% assisted COVID-19 patients as any other nurse. Only 3.4% felt very prepared to work in the field of intensive care, finding that those students who perceived a higher degree of preparedness had received previous training in personal protective equipment and mechanical ventilation (p < 0.005). The highest scores for training activities that may improve their preparedness were simulations to improve levels of anxiety and stress when managing critical patients, simulation in ventilatory support and mandatory practices in services where ventilators are used.ConclusionsAlthough three out of four students were willing to provide health assistance, they recognise that they were not specially prepared in the field of intensive care and demand training with simulation to improve anxiety and stress levels in the management of critical patients and simulation in ventilatory support.Relevance to clinical practiceStudents have been vital resources for our health system and society when they have been needed. It is now up to us, both teachers and health authorities, to share their efforts by implementing the necessary improvements in training and safety measures not only because these affects the health and safety of the patient, but because they will be essential parts in future pandemics.

Project description:Lombardy, the most populated and industrialized Italian region, was the epicentre of the first wave (March and April, 2020) of COVID-19 in Italy and it is among the most air polluted areas of Europe. We carried out an ecological study to assess the association between long-term exposure to particulate matter (PM) and nitrogen dioxide (NO2) on COVID-19 incidence and all-cause mortality after accounting for demographic, socioeconomic and meteorological variables. The study was based on publicly available data. Multivariable negative binomial mixed regression models were fitted, and results were reported in terms of incidence rate ratios (IRRs) and standardized mortality ratios (SMR). The effect of winter temperature and humidity was modelled through restricted cubic spline. Data from 1,439 municipalities out of 1,507 (95%) were included in the analyses, leading to a total of 61,377 COVID-19 cases and 40,401 deaths from all-causes collected from February 20th to April 16th and from March 1st to April 30th, 2020, respectively. Several demographic and socioeconomic variables resulted significantly associated with COVID-19 incidence and all-cause mortality in a multivariable fashion. An increase in average winter temperature was associated with a nonlinear decrease in COVID-19 incidence and all-cause mortality, while an opposite trend emerged for the absolute humidity. An increase of 10 ?g/m3 in the mean annual concentrations of PM2.5 and PM10 over the previous years was associated with a 58% and 34% increase in COVID-19 incidence rate, respectively. Similarly, a 10 ?g/m3 increase of annual mean PM2.5 concentration was associated with a 23% increase in all-cause mortality. An inverse association was found between NO2 levels and COVID-19 incidence and all-cause mortality. Our ecological study showed that exposure to PM was significantly associated with the COVID-19 incidence and excess mortality during the first wave of the outbreak in Lombardy, Italy.
