Project description:BackgroundThe COVID-19 pandemic has increased mental health problems among healthcare workers globally. However, studies from low- and middle-income countries on this matter were minimal. This study assessed the change in depression prevalence during the first year of the COVID-19 pandemic and associated factors among healthcare providers in Addis Ababa, Ethiopia.MethodsWe conducted surveys among healthcare workers in Addis Ababa at two-time points, September 2020 and October 2021. A total of 577 study participants were randomly selected based on registers obtained from professional associations. Computer-assisted telephone interviewing technique was used for data collection. The Patient Health Questionnaire (PHQ-9) was used to screen for depression. Multivariable logistic regression analysis was performed to identify potential factors associated with depression.ResultThe prevalence of depression among healthcare workers was 2.3% (95% CI [1.1-4.8]) in Time 1 and 6.5% (95% CI [4.1-10.1]) in Time 2; nearly a three-fold increase in Time 2 compared to Time 1. The most frequently reported symptoms at both times based on the PHQ-9 item were having poor energy, sleep problem, and anhedonia, while reported suicidal ideation was less than 5%. Depression showed a positive and significant association with a positive COVID-19 test result (AOR 7.25 95% CI [1.32-39.4]) in Time 1, and with being a female healthcare provider (AOR 3.96 95% CI [1.08-14.51]) and lack of COVID-19 related policy or guidelines at the workplace (AOR 3.22 95% CI [1.11-9.35]) in Time 2.ConclusionThe prevalence of depression among healthcare workers tripled during the first year of the COVID-19 pandemic. Panic reaction to positive COVID-19 test result seems to have a negative effect at the beginning while lack of disease-specific prevention guidelines and comprehensive psychological interventions for healthcare providers had an adverse effect on the mental health of health workers.

Project description:The COVID-19 pandemic has considerably increased food insecurity. To identify where intervention and policy solutions are most needed, we explored barriers to obtaining food and predictors of experiencing food insecurity due to the COVID-19 pandemic. Between May and July 2020, we conducted cross-sectional online surveys with two convenience samples of U.S. adults (Study 1: n = 2,219, Study 2: n = 810). Roughly one-third of participants reported experiencing food insecurity due to the COVID-19 pandemic (Study 1: 32%, Study 2: 35%). Between one-third and half reported using the charitable food system (Study 1: 36%, Study 2: 46%). The majority of participants experienced barriers to getting food (Study 1: 84%, Study 2: 88%), of which the most commonly reported were not having enough money to buy food (Study 1: 48%; Study 2: 53%) and worrying about getting COVID-19 at the store (Study 1: 50%; Study 2: 43%). Higher education was associated with greater risk of food insecurity in both studies (all p < 0.05). Receipt of aid from SNAP buffered against the association between financial struggles and food insecurity in Study 1 (p = 0.03); there was also some evidence of this effect in Study 2 (p = 0.05). Our findings suggest that food insecurity might be reduced by mitigating financial struggles (e.g., by increasing access to SNAP) and by addressing barriers to obtaining food (e.g., by expanding accessibility of food delivery programs).

Project description:The present study aimed to analyze changes in food consumption among teachers of state schools in Minas Gerais in the context of the pandemic of coronavirus disease 2019 (COVID-19). This is a cross-sectional study of teachers at elementary, middle, and high schools in Minas Gerais. An online questionnaire was made available to all participants through the Google Forms platform. For the analysis, sociodemographic, economic, occupational profile, health conditions, and behavioral/habitual variables were used. Food consumption was assessed through the weekly frequency of eating foods considered healthy and unhealthy before and during the pandemic. The changes observed in the frequency were classified as: decreased intake, no change in intake, and increased intake during the pandemic. Statistical analysis was performed using bivariate and multiple analysis using the Multinomial Logistics Regression model. The data were tabulated with the aid of the Statistical Package for Social Sciences (SPSS), version 18.0. A total of 15,641 teachers participated in this study, with an average age of 42.96 (±9.27) years, and most of them were female (81.9%). During the pandemic, approximately 40% reported altered sleep quality and alcohol consumption, and lack of physical activity with 60.4% of the professionals categorized as overweight. During this period, there was an increase in the consumption of sweets (19.5%), soft drinks (13.3%), sausages (12.0%), frozen foods (8.9%), salty foods (6.3%), vegetables (13.1%), fruits (12.6%), and wholegrain products (8.3%). In the regression analysis, several factors were found to be associated with changes in teachers' food consumption in a bidirectional way when associated with variables related to sociodemographic characteristics, occupational profile, general health, and lifestyle.

Project description:Little is known about the differences in dietary practices among food secure and food insecure populations during the early COVID-19 pandemic restrictions. The purpose of this study was to examine differences in dietary practices the early COVID-19 pandemic restrictions between adults reporting food security versus food insecurity. An online cross-sectional survey using validated measures was administered between April and September 2020 to explore both dietary patterns and practices and food security status among persons residing in five U.S. states from different regions of the country during the COVID-19 pandemic. Between-group differences (food secure versus food insecure) were examined for dietary practice outcomes using Pearson's Chi-Square test statistic, with Fisher's Exact test for cell counts less than five. There were 3,213 adult respondents. Food insecurity increased among the survey sample from 15.9% before the COVID-19 pandemic to 23.1% during the onset of the COVID-19 pandemic (p < 0.01). Compared to food secure respondents, those experiencing food insecurity reported more group gatherings for meals during the pandemic, decreased fruit and vegetable intake, and a need for more nutrition support resources than food secure respondents (p < 0.05). Food secure individuals reported increasing alcohol consumption, more frequent take-out or delivery ordering from fast food or restaurants, and more interest in supporting the local food system (p < 0.05). Results indicate a clear risk of disparities in dietary practices based on food security status during the early COVID-19 pandemic restrictions. Public health research, practice, and policy efforts should tailor specific efforts towards both food secure and food insecure groups.

Project description:AbstractThere is limited examination about coronavirus disease 19 (COVID-19)-related food handling concerns and practices that cause chemical or microbial contamination and illness, particularly among those with food insecurity. We investigated consumer food handling concerns and practices during the COVID-19 pandemic and whether they differed by food insecurity status. An online survey was distributed among Chicago, IL, residents between 15 July and 21 August 2020 (n = 437). Independent t tests and Fisher's exact tests were used to identify differences in food handling concerns and practices between those with and without food insecurity (alpha = 0.05). Survey items included questions about food handling practices that were considered safe or neutral (i.e., washing hands and produce with water, sanitizing food packaging) and unsafe (i.e., using cleaning agents to wash foods, leaving perishable foods outside) by using 5-point Likert-style scales or categorical responses (i.e., yes, no). Participant responses fell between "slightly" and "somewhat" concerned about contracting COVID-19 from food and food packaging (mean ± standard error [SE]: 2.7 ± 0.1). Although participants reported washing their hands before eating and before preparing foods at least "most of the time" (mean ± SE: 4.4 ± 0.0 and 4.5 ± 0.0, respectively), only one-third engaged in unsafe practices. The majority of participants (68%) indicated that they altered food handling practices due to the COVID-19 pandemic and received information about food safety from social media (61%). When investigating differences in concerns and practices by food insecurity status, food insecure participants were more concerned about COVID-19 foodborne transmission for all food items (all P < 0.001) and more frequently performed unsafe food handling practices than those with food security (all P < 0.001). Results from this study suggest more investigation is needed to understand barriers to safe food handling knowledge and practices, particularly among those with food insecurity.Highlights

Project description:BackgroundCOVID-19 caused a significant impact on food security; particularly among low- and middle-income countries. The objective of the current study was to assess both the direct and indirect impact of COVID-19 on individuals' food security in the metropolitan and regional context of Ethiopia.MethodsThis project utilized a retrospective study design with a mixed methods approach. It assessed the food insecurity experience of individuals both before and after the emergence of COVID-19 through the Food Insecurity Experience Scale (FIES). Food sellers were interviewed to identify the factors that increased post-COVID-19 emergence food insecurity. Inferential analysis using ordinal logistic regression was undertaken based on the Polytomous Universal Model (PLUM) procedure.ResultsThe size of the family and the type of work for income generation were statistically associated (p < 0.05) with the food security of individuals, both before and after COVID-19 emergence. Location (p = 0.002, odds = 0.37), age (p = 0.002, odds = 2.57) and educational status (p = 0.001, odds = 0.24/) of individuals had a statistically significant effect on the food security of individuals before COVID-19 emergence only. The ordinal value of all FIES indicators increased after COVID-19 emergence compared with pre-pandemic food insecurity. Overall food security of individuals was reduced by 21.5%, with the moderate and severe food insecurity of individuals increasing by 13.1% and 15.9%, respectively. The COVID-19 preventive measures that affected the individuals' food security, in the order of their priority, were: transport bans; food price increment; lockdown measures; job loss; market bans; social distance restrictions; fear of the pandemic; movement restrictions; over-buying; food inaccessibility; and, lack of cash due to bank closure. In addition, pre-existing non-pandemic related natural and man-made disasters played a role in the food security crisis, including drought, war, and desert locust emergence.ConclusionThe COVID-19 pandemic has directly and indirectly affected individuals' food security. Learning from the experiences of COVID-19 may assist governments in preparing for future pandemics. Suggested improvements include forming impact reduction task forces and establishing disease prevention strategies that will not compromise food security.

Project description:BackgroundWorldwide, health systems have been challenged by the overwhelming demands of the COVID-19 pandemic. In Ethiopia, maintaining essential health services during the COVID-19 pandemic is critical to preventing severe outcomes and protecting the gains made over the past years in the health sector. This project aims to explore the health system's response to maintaining essential healthcare services in Addis Ababa, Ethiopia.MethodsA total of 60 key informant interviews were conducted by purposively selecting key stakeholders from Federal Ministry of Health, Addis Ababa Regional Health Bureau, Sub-city Health Offices, and frontline healthcare providers. Interviews were transcribed verbatim and coded using Open Code. Thematic analysis was employed to analyze the data.ResultCOVID-19 affected the delivery of essential health services in several ways, namely: decline in health service utilization, fear of infection among healthcare providers, stigma towards healthcare providers, and perceived decrease in quality-of-service provision. However, the health system actors made efforts to sustain services while responding to the pandemic by enacting changes in the service delivery modality. The most significant service delivery changes included repurposing health centers and prolonged prescriptions (multi-month medication dispensing). The primary challenges encountered were burnout of the health workforce and a shortage of personal protective equipment.ConclusionCOVID-19 has affected the delivery of essential health services in multifaceted ways. System actors have accordingly made efforts to sustain services while responding to the pandemic.

Project description:This paper aims to study the perceptions of the impacts of the COVID-19 pandemic on behaviors related to diet and food shopping on a sample of 356 adults in Oman. The study is based on the results of an Arabic-language online survey conducted between September 15 and October 10, 2020, using the Survey Monkey platform. The questionnaire had 25 questions (multiple options and one option), subdivided into three parts. Respondents were asked to disseminate the survey to their networks as part of the study's snowball sampling method. Descriptive statistics and various statistical tests (e.g., U-Mann Whitney, Kruskal-Wallis, chi-square) have been used to evaluate the study results. The study showed a significant shift in the attitude and behavior of respondents regarding food and health. Indeed, the paper findings indicated (i) a shift to healthier diets, as shown by the fact that 45.5% of the participants increased their intake of fruits and vegetables, 42.4% ate more healthy foods, and 53.1% reduced their intake of unhealthy foods; (ii) an increase in the consumption of local products, owing to food safety concerns, with 25.8% of the cohort stating that they purchase more local food items; (iii) a shift in grocery shopping behaviors, especially with 28.1% of the participants buying more groceries online; (iv) the absence of panic buying in Oman, since 62.36% of the participants said they did not stockpile food items; and (v) a reduction of food waste. Indeed, 78.9% of the participants specified they were not wasting more food than average since the beginning of the pandemic, and 74.72% indicated they were more aware of how much food they were wasting. Surprisingly, COVID-19 appears to bring many beneficial adjustments in Oman to make food consumption more sustainable and healthier.

Project description:BackgroundHealthcare professionals (HCPs) are at the frontline in the fight against COVID-19 and are at an increased risk of becoming infected with coronavirus. Risk of infection can be minimized by use of proper personal protective equipment (PPE). The aim of this study was to assess the availability and use of PPE, and satisfaction of HCPs with PPE in six public hospitals in Addis Ababa, Ethiopia.MethodsA cross-sectional study was conducted among 1134 HCPs in June 2020. A systematic random sampling and consecutive sampling techniques were used to select the study participants. Data were collected using a self-administered questionnaire. Descriptive statistics were used to describe the data and Chi-square test was used to assess the association between the groups. Bivariate and multivariable logistic regression models were used to assess factors associated with satisfaction of healthcare workers.ResultsThe mean (±SD) age of the participants was 30.26 ± 6.43 year and 52.6% were females. Nurses constituted about 40% of the overall sample, followed by physicians (22.2%), interns (10.8%), midwives (10.3%) and others (16.7%). The majority (77%) of the HCPs reported that their hospital did not have adequate PPE. A critical shortage of N95 respirators was particularly reported, it only increased from 13 to 24% before and during COVID-19, respectively. The use of N95 increased from 9 to 21% before and during COVID-19, respectively. Almost 72% of the respondents were dissatisfied with the availability and use of PPE in their hospital. The independent predictors of the respondents' satisfaction level about PPE were healthcare workers who reported that PPE was adequately available in the hospital (adjusted OR = 7.65, 95% CI:5.09-11.51), and preparedness to provide care to COVID-19 cases (adjusted OR = 2.07, 95% CI:1.42-3.03).ConclusionsA critical shortage of appropriate PPE and high level of dissatisfaction with the availability and use of PPE were identified. Therefore, urgent efforts are needed to adequately supply the healthcare facilities with appropriate PPE to alleviate the challenges.

Project description:The effects of COVID-19 are continuing to increase around the world as the pandemic claims thousands of lives. Bangladesh is no exception and has been greatly affected by SARS-CoV-2. Apart from the number of people who are or have been directly getting infected with this disease, millions of people are directly or indirectly facing many challenges to their livelihoods and the security of their food and nutritional supply, along with other societal issues created by the pandemic. In this study, a hybrid approach of online and telephone questionnaire surveys was used to investigate the food security of Dhaka city's inhabitants at household level. Approximately 80% of the respondents reported reduced income, and a quarter of respondents lost their jobs between March and June 2020. The frequency of fish consumption, an essential component of Bangladeshi diets, significantly reduced during the pandemic. This was especially apparent in affluent segments of the community. Out of the respondents, 75% reported an increase in the price of fish in Dhaka city. A range of coping strategies were observed: including decreasing the frequency of grocery shopping, shifting to online shopping, reducing consumption of high price commodities, reducing junk food consumption, cleaning fish and meat with hot water and vinegar, and increasing the consumption of protein and vitamin C rich food items. Prior to COVID-19, 80% of the households surveyed bought fish from wet markets. This number dropped to 45% during the pandemic. Many households substituted fish and meat with poultry, eggs and dried fish. About half of the households stockpiled rice, lentils and potatoes during the peak of the pandemic. However, if the pandemic lasts for a prolonged period, those living on low incomes in urban areas will experience some level of food insecurity from a reduced income or loss of work. Because of this, a large-scale sustainability policy should be undertaken to secure the food and nutritional security of low-income and middle-class household.
