Cross-cultural differences in mother-preschooler book sharing practices in the United States and Thailand.

ABSTRACT: Cross-cultural differences in book sharing practices of American and Thai mother-preschooler dyads were examined. Twenty-one Thai monolingual and 21 American-English monolingual mothers and their four-year-olds completed a book sharing task. Results revealed narrative style differences between the American and Thai groups: American mothers adopted a high-elaborative story-builder style and used affirmations, descriptions, extensions, and recasting more than Thai mothers. Thai mothers adopted a low-elaborative story-teller style and used more attention directives and expansions than American mothers. American children produced longer narratives than their Thai peers, whereas Thai children repeated their mothers' utterances more than their American counterparts. Maternal and child narrative styles were associated. These results suggest that maternal scaffolding styles differ across cultures and influence children's developing narrative skills.

Differences in infant feeding practices by mode of conception in a United States cohort.

Project description:OBJECTIVE:To identify associations between fertility treatment use (assisted reproductive technologies, ovulation induction, and artificial insemination) and subsequent infant feeding practices. DESIGN:The Upstate KIDS population-based cohort enrolled mothers who delivered live births in New York (2008-2010), sampling on fertility treatment and plurality. SETTING:Not applicable. PATIENT(S):Data regarding singletons and one randomly selected infant between twins were used. INTERVENTION(S):Not applicable. MAIN OUTCOME MEASURE(S):Mothers reported breast feeding and formula feeding practices at 4, 8, and 12 months postpartum. Modified Poisson regression was used to compare risks for feeding practices by mode of conception. Marginal structural models were used to estimate the controlled direct effects of fertility treatment on feeding, independent of preterm birth. RESULT(S):Among 4,591 mothers, 1,361 (30%) conceived with the use of fertility treatments. Mothers who used fertility treatments were less likely to breast feed to 12 months after birth and were more likely to provide formula, solids, and juice by 4 months than mothers who did not conceive with treatments. Fertility treatment remained associated with breast feeding cessation and formula feeding in mediation analyses, suggesting that preterm birth does not fully explain these associations. CONCLUSION(S):Women who conceived with the use of fertility treatments were less likely to breast feed later in infancy and were more likely to provide formula, solids, and juice earlier in infancy. Our analyses accounted for confounding and preterm birth, but other contributing factors may include difficulties feeding twins or workplace breast feeding accommodations.

Grooming and cultural socialization: a mixed method study of caregiving practices in Burma (Myanmar) and the United States.

Project description:Grooming behaviours are thought to be a crucial aspect of parenting and integral to the sociality of non-human mammals, but there have been few empirical studies on how grooming might be relevant to parenting and socialization processes in humans. Study 1 is a quantitative cross-cultural comparison of grooming practices in two cultural settings: an urban centre in Burma (Myanmar) and an urban centre in the United States. The study uses naturalistic video data of 57 families to analyse grooming behaviours directed at children. A broad range of ages was sampled in each culture to examine the developmental trajectory of grooming behaviours. Results indicate that significant cultural differences exist between Burma and the United States, with Burmese children being groomed by their caregivers more often than U.S. children. Results also indicate that cultural differences in grooming practices begin early and remain constant across age. An unexpected finding was that Burmese families were more variable in their behaviour than U.S. families. Study 2 attempts to explain this variability by using ethnography to describe how sociodemographic changes in Burma are leading to changes in parental values and socialization practices in the schools, but how embodied primary care in the homes appear resistant to change.

The United States Does CAIR About Cultural Safety: Examining Cultural Safety Within Indigenous Health Contexts in Canada and the United States.

Project description:This article examines the concept and use of the term cultural safety in Canada and the United States.To examine the uptake of cultural awareness, cultural sensitivity, cultural competence, and cultural safety between health organizations in Canada and the United States, we reviewed position statements/policies of health care associations.The majority of selected health associations in Canada include cultural safety within position statements or organizational policies; however, comparable U.S. organizations focused on cultural sensitivity and cultural competence.Through the work of the Center for American Indian Resilience, we demonstrate that U.S. researchers engage with the tenets of cultural safety-despite not using the language.We recommend that health care providers and health researchers consider the tenets of cultural safety.To address health disparities between American Indian populations and non-American Indians, we urge the adoption of the term and tenets of cultural safety in the United States.

Neonatal Abstinence Syndrome Practices in the United States.

Project description:ObjectiveTo survey current practices for the treatment of neonatal abstinence syndrome (NAS) among institutions in the United States to identify changes in national practice over time.MethodsPrevious NAS management reports were referenced in the development of our 26-question electronic survey, which was distributed in the fall of 2019 to pediatric practitioners of 2 national clinical pharmacy organizations via email list servers. Not all questions required a response and responses from incomplete surveys were included. Institution demographics and NAS management strategies, including location of care, observation period, and inpatient and outpatient pharmacotherapy, were queried.ResultsSeventy respondents representing institutions from all US geographic regions participated in the survey The most commonly reported inpatient observation durations were 3 (18 of 61, 29%) and 5 (22 of 61, 36%) days. Respondents indicated that neonates were typically transferred to the NICU if pharmacologic management was required (38 of 56, 68%). According to participants, first-line agents used for NAS management were morphine (45 of 56, 80%), methadone (5 of 56, 9%), clonidine (2 of 56, 4%), and buprenorphine (2 of 56, 4%). Among respondents, only 20% (11 of 56) reported that infants may be discharged home on pharmacotherapy, including morphine (n = 6), phenobarbital (n = 3), clonidine (n = 1), and methadone (n = 1).ConclusionsOpioids are the most commonly used first-line agents for NAS management in the United States. The primary site of NAS management is the inpatient setting, as only 20% of institutions report discharging patients on pharmacotherapy.

Self-care perspective taking and empathy in a studentfaculty book club in the United States

Project description:PurposeWe aimed to study the impact of a combined faculty-student book club on education and medical practice as a part of the informal curriculum at the University of Florida College of Medicine in the United States.MethodsSixteen medical students and 7 faculties who participated in the book club were interviewed through phone and recorded. The interview was then transcribed and entered into the qualitative data analysis program QSR NVivo (QSR International, Burlington, MA, USA). The transcripts were reviewed, and thematic codes were developed inductively through collaborative iteration. Based on these preliminary codes, a coding dictionary was developed and applied to all interviews within QSR Nvivo to identify themes.ResultsFour main themes were identified from interviews: The first theme, the importance of literature to the development and maintenance of empathy and perspective-taking, and the second theme, the importance of the book club in promoting mentorship, personal relationships and professional development, were important to both student and faculty participants. The third and fourth themes, the need for the book club as a tool for self-care and the book club serving as a reminder about the world outside of school were discussed by student book club members.ConclusionOur study demonstrated that an informal book club has a significant positive impact on self-care, perspective-taking, empathy, and developing a “world outside of school” for medical school students and faculty in the United States. It also helps to foster meaningful relationships between students and faculty.

Perspectives of United States neonatologists on genetic testing practices.

Project description:PurposeGenetic disorders often present in the neonatal intensive care unit (NICU), and detecting or confirming these diagnoses has been shown to impact care. However, the availability and use of genetic testing, particularly exome or genome sequencing, among NICUs varies widely. We therefore sought to investigate practice patterns related to genetic testing in NICUs around the country to identify and quantify potential discrepancies.MethodsWe designed a survey that was distributed to neonatologists via email. The survey contained questions related to test availability and desirability, the process of test ordering in NICU, and general comfort with ordering and interpreting genetic testing. Demographic data related to the survey participants and characteristics of their NICU were also obtained.ResultsIn total, 162 neonatologists completed the survey, representing 40 states and 112 distinct NICUs. Although nearly all (93.2%) neonatologists attributed a high level of importance to identifying a genetic diagnosis for their patients, genetic consultations were only available at 78% of NICUs and exome or genome sequencing was not available on a regular basis (69% of NICUs).ConclusionAlthough, among US neonatologists surveyed, most feel that genetic tests are indicated for their patients, these are not always clinically available. Further research into implementation barriers is warranted.

Associations between mother-preschooler attachment and maternal depression symptoms: A systematic review and meta-analysis.

Project description:The current study aimed to systematically review and meta-analyze concurrent and longitudinal associations between maternal depression symptoms and mother-child attachment during the preschool period (aged 2 to 7 years) as assessed using the coding systems by Cassidy and Marvin (1992) and Main and Cassidy (1988). The review was pre-registered with PROSPERO (International Prospective Register of Systematic Reviews; Registration number CRD42017073417) and was conducted in accordance with PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. A total of 7,969 records were screened and 18 articles were deemed as eligible for inclusion in the review. Studies were reviewed using qualitative synthesis techniques and, where appropriate, meta-analysis. Qualitative synthesis indicated that mothers of disorganized/controlling children most consistently reported the highest levels of depressive symptoms, both concurrently and longitudinally. The association between disorganized/controlling child attachment and concurrent maternal depressive symptoms was significant (n = 1,787; g = 0.27, 95% CI [0.13,0.40]), and was not moderated by sample type, child gender, or risk of bias. Findings of a relationship between child attachment insecurity and maternal depressive symptoms must be qualified due to significant within-study heterogeneity and publication bias. Results suggest that maternal depressive symptoms may confer risk for disorganized/controlling attachment during the preschool period.

Climatic backdrop for Pueblo cultural development in the southwestern United States.

Project description:While climatic triggers for collapse and population migrations of ancestral Pueblo communities have been proposed, little is known about the overall climatic backdrop for the entire pre-Hispanic Pueblo period (ca. 1300 to 460 B2K). Here, we report data from stalagmite HC-1, from Hidden Cave, Guadalupe Mountains, New Mexico, covering the past 3400 years, showing an interval of increased frequency of droughts from 1260 to 370 yr B2K that is coeval with the entire pre-Hispanic Pueblo period. Our record suggests that this puebloan Late Holocene climatic interval was the most arid and highly variable climatic period of the last 3400 years. Climatic conditions favoring the introduction of cultivation existed prior to the Pueblo period during more pluvial-like conditions from at least 3400 to 1260 yr B2K. Hence, the change from the Desert Archaic/Basketmaker to Pueblo cultures was associated with a quick transition to increasing aridity into and through the Pueblo period associated with greater urbanization and the establishment of pueblo population centers.

Synchronous environmental and cultural change in the prehistory of the northeastern United States.

Project description:Climatic changes during the late Quaternary have resulted in substantial, often abrupt, rearrangements of terrestrial ecosystems, but the relationship between these environmental changes and prehistoric human culture and population size remains unclear. Using a database of archaeological radiocarbon dates alongside a network of paleoecological records (sedimentary pollen and charcoal) and paleoclimatic reconstructions, we show that periods of cultural and demographic change in the northeastern United States occurred at the same times as the major environmental-climatic transitions of that region. At 11.6, 8.2, 5.4, and 3.0 kyr BP (10(3) calendar years before present), changes in forest composition altered the distribution, availability, and predictability of food resources which triggered technological adjustments manifested in the archaeological record. Human population level has varied in response to these external changes in ecosystems, but the adoption of maize agriculture during the late Holocene also resulted in a substantial population increase. This study demonstrates the long-term interconnectedness of prehistoric human cultures and the ecosystems they inhabited, and provides a consolidated environmental-cultural framework from which more interdisciplinary research and discussion can develop. Moreover, it emphasizes the complex nature of human responses to environmental change in a temperate region.

Newborn Screening Practices for Beta-Thalassemia in the United States.

Project description:Beta-thalassemia, a heritable condition of abnormal hemoglobin production, is not a core condition on the United States Recommended Uniform Screening Panel (RUSP) for state and territorial newborn screening (NBS) programs. However, screening for sickle cell disease (which is on the core RUSP) also detects reduced or absent levels of hemoglobin (Hb) A and certain other Hb variants associated with beta-thalassemia and, thus, allows for a timely referral to appropriate healthcare to minimize sequalae of the disease. The Association of Public Health Laboratories' Hemoglobinopathy Workgroup administered a comprehensive survey of all U.S. NBS programs to assess beta-thalassemia testing methodologies, the cutoffs for defining beta-thalassemia major, and the reporting and follow-up practices. Forty-six (87%) of the programs responded. Thirty-nine of the 46 responding programs (85%) report some form of suspected beta-thalassemia; however, the screening methods, the percentage of Hb A used as a cutoff for an indication of beta-thalassemia major, and the screening follow-up vary widely. The standardization of technical and reporting procedures may improve access to specialty care prior to severe complications, increase genetic counseling, and provide data needed to better understand the public health impact and clinical outcomes of beta-thalassemia in the United States.
