Explaining local variation in referrals from health services to children's social care in England 2013-16: a study using 'children in need' administrative data.

ABSTRACT:

Background

Referral rates from Health service to Children's Social Care (CSC) services vary across England. In 2019, the National Audit Office (re)iterated the urgent need to understand the drivers of such variation.

Methods

Using administrative data (Children in Need Census, 2013-16), we calculated annual referral rates from Health to CSC services (Health referral rate) by Local Authority (LA) areas. We used multilevel linear regression to investigate the relationship between age-adjusted Health referral rates and local need (demand factors) and local practice/systems (supply factors). We present a tool to compare unadjusted and adjusted LA rates.

Results

There was high LA variation in Health referral rates, particularly for infants (mean = 29.0/1000 children < 1 y; range = 6.5-101.8; sd = 12.4). LA variation persisted after age-adjustment. Child poverty (local need) and overall referral rate (local practice/systems) explained 60% of variation in age-adjusted Health referral rates. Overall referral rate was the strongest predictor. Adjusted referral rates were substantially different from unadjusted rates. After adjustment, 57.7% of LAs had higher/lower Health referral rates than expected.

Conclusions

While higher levels of local need are associated with higher Health referrals, some areas have high Health referrals irrespective of local need. Our tool demonstrates the benefits of using adjusted rates to compare LAs.

Project description:Despite growing concerns that children (8-13 years old) tend to avoid the news, the reasons why have received little research attention. Therefore, the current study aims to develop and test a model conceptualizing the relations between children's news consumption, news avoidance, emotional responses (negative emotions and anxiety-related behaviors), and parent and child mitigation strategies. The model was tested using data collected during the first year of the COVID-19 pandemic. The current, preregistered, survey study was part of a longitudinal project and used data from the second wave. Data were collected in November/December 2020 among 510 children (M age = 10.40; 53.72% girls). Findings showed that children who consumed more news during the pandemic avoided pandemic news less often. Children who experienced more anxiety-related behaviors regarding pandemic news avoided pandemic news more often. The relation between news consumption and emotional responses was stronger for children who experienced restrictive parental mediation more often, indicating that this was not an effective parental mediation strategy for tempering their emotional responses. Children with higher levels of emotional responses used reactive coping strategies more often. However, this did not seem to be an effective strategy against pandemic news avoidance because none of the strategies had a negative relation with pandemic news avoidance. Distancing was even positively related to pandemic news avoidance. Although the current study was not able to fully unravel how news avoidance-related constructs relate to one another, we were able to get some important insights guiding future research. Specifically, it is of crucial importance to unravel the mechanisms that increase the chance of children's news avoidance and those that mitigate it, to build interventions to counteract news avoidance and to protect children from the negative emotional consequences by news consumption.

Project description:Implementing major system change in healthcare is not well understood. This gap may be addressed by analysing change in terms of interrelated components identified in the implementation literature, including decision to change, intervention selection, implementation approaches, implementation outcomes, and intervention outcomes.We conducted a qualitative study of two cases of major system change: the centralisation of acute stroke services in Manchester and London, which were associated with significantly different implementation outcomes (fidelity to referral pathway) and intervention outcomes (provision of evidence-based care, patient mortality). We interviewed stakeholders at national, pan-regional, and service-levels (n?=?125) and analysed 653 documents. Using a framework developed for this study from the implementation science literature, we examined factors influencing implementation approaches; how these approaches interacted with the models selected to influence implementation outcomes; and their relationship to intervention outcomes.London and Manchester's differing implementation outcomes were influenced by the different service models selected and implementation approaches used. Fidelity to the referral pathway was higher in London, where a 'simpler', more inclusive model was used, implemented with a 'big bang' launch and 'hands-on' facilitation by stroke clinical networks. In contrast, a phased approach of a more complex pathway was used in Manchester, and the network acted more as a platform to share learning. Service development occurred more uniformly in London, where service specifications were linked to financial incentives, and achieving standards was a condition of service launch, in contrast to Manchester. 'Hands-on' network facilitation, in the form of dedicated project management support, contributed to achievement of these standards in London; such facilitation processes were less evident in Manchester.Using acute stroke service centralisation in London and Manchester as an example, interaction between model selected and implementation approaches significantly influenced fidelity to the model. The contrasting implementation outcomes may have affected differences in provision of evidence-based care and patient mortality. The framework used in this analysis may support planning and evaluating major system changes, but would benefit from application in different healthcare contexts.
