Genetic Variation in Toll-Like Receptor 5 and Colonization with Flagellated Bacterial Vaginosis-Associated Bacteria.

ABSTRACT: Bacterial vaginosis (BV) is a vaginal dysbiotic condition linked to negative gynecological and reproductive sequelae. Flagellated bacteria have been identified in women with BV, including Mobiluncus spp. and BV-associated bacterium-1 (BVAB1), an uncultivated, putatively flagellated species. The host response to flagellin mediated through Toll-like receptor 5 (TLR5) has not been explored in BV. Using independent discovery and validation cohorts, we examined the hypothesis that TLR5 deficiency-defined by a dominant negative stop codon polymorphism, rs5744168-is associated with an increased risk for BV and increased colonization with flagellated bacteria associated with BV (BVAB1, Mobiluncus curtisii, and Mobiluncus mulieris). TLR5 deficiency was not associated with BV status, and TLR5-deficient women had decreased colonization with BVAB1 in both cohorts. We stimulated HEK-hTLR5-overexpressing NF-κB reporter cells with whole, heat-killed M. mulieris or M. curtisii and with partially purified flagellin from these species; as BVAB1 is uncultivated, we used cervicovaginal lavage (CVL) fluid supernatant from women colonized with BVAB1 for stimulation. While heat-killed M. mulieris and CVL fluid from women colonized with BVAB1 stimulate a TLR5-mediated response, heat-killed M. curtisii did not. In contrast, partially purified flagellin from both Mobiluncus species stimulated a TLR5-mediated response in vitro We observed no correlation between vaginal interleukin 8 (IL-8) and flagellated BVAB concentrations among TLR5-sufficient women. Interspecies variation in accessibility of flagellin recognition domains may be responsible for these observations, as reflected in the potentially novel flagellin products encoded by Mobiluncus species versus those encoded by BVAB1.

Project description:Several fastidious bacteria have been associated with bacterial vaginosis (BV), but their role in lactobacilli recolonization failure is unknown. We studied the effect of 7 BV-associated bacterial species and 2 Lactobacillus species on vaginal colonization with Lactobacillus crispatus CTV-05 (LACTIN-V).Twenty-four women with BV were given a 5-day course of metronidazole vaginal gel and then randomized 3:1 to receive either LACTIN-V or placebo applied vaginally once daily for 5 initial consecutive days, followed by a weekly application over 2 weeks. Vaginal swabs for L. crispatus CTV-05 culture and 9 bacterium-specific 16S rRNA gene quantitative polymerase chain reaction assays were analyzed on several study visits for the 18 women receiving LACTIN-V.Vaginal colonization with CTV-05 was achieved in 61% of the participants receiving LACTIN-V at either day 10 or day 28 visit and 44% at day 28. Participants not colonized with CTV-05 had generally higher median concentrations of BV-associated bacteria compared to those who colonized. Between enrollment and day 28, the median concentration of Gardnerella vaginalis minimally reduced from 10 to 10 16S rRNA gene copies per swab in women who colonized with CTV-05 but increased from 10 to 10 in those who failed to colonize (P = 0.19). Similarly, the median concentration of Atopobium spp. reduced from 10 16S rRNA gene copies per swab to below limit of detection in women who colonized with CTV-05, but increased from 10 to 10 in those who failed to colonize (P = 0.04). The presence of endogenous L. crispatus at enrollment was found to be significantly associated with a reduced odds of colonization with CTV-05 on day 28 (P = 0.003), and vaginal intercourse during the study significantly impaired successful CTV-05 colonization (P = 0.018).Vaginal concentration of certain BV-associated bacteria, vaginal intercourse during treatment, and the presence of endogenous L. crispatus at enrollment predict colonization with probiotic lactobacilli.

Project description:Bacterial vaginosis (BV), the most common vaginal infection among women worldwide, is characterized by an imbalance in the vaginal microbiota with a reduction of protective Lactobacillus species and overgrowth of facultative and strictly anaerobic bacteria. Although the development of a polymicrobial biofilm on the vaginal epithelium is a hallmark of BV, interactions between key BV-associated bacteria (BVAB) [i.e. Gardnerella vaginalis, Fannyhessea vaginae, and Prevotella bivia] present in the biofilm remain largely unexplored. In this study, we aimed to analyze the transcriptome of triple-species biofilms growing in a medium simulating vaginal tract secretions (mGTS) compared to a rich medium, New York City III (NYCIII). As such, triple-species biofilms formed by G. vaginalis, F. vaginae, and P. bivia were grown in either NYCIII or mGTS in anaerobic conditions. After that, RNA was extracted and cDNA libraries were constructed and sequenced. We first characterized the biofilm composition in both media by qPCR and observed that G. vaginalis was the most prevalent species in both conditions. RNA-sequencing data analysis revealed a total of 964, 596, and 656 differentially expressed genes in G. vaginalis, F. vaginae, and P. bivia, respectively, when comparing the transcriptome of biofilms in mGTS versus NYCIII. We identified 927 upregulated and 37 downregulated genes for G. vaginalis, 492 upregulated and 104 downregulated for F. vaginae, and 166 upregulated and 490 downregulated for P. bivia. Gene ontology enrichment showed that metabolic and biosynthetic processes were common within upregulated genes for G. vaginalis, while downregulated genes were associated with transmembrane transporters. For F. vaginae, transmembrane transporters were identified among upregulated genes, while downregulated genes were related to metabolism and cellular process, in which several KEGG pathways linked to metabolism were recognized. For P. bivia, upregulated genes were associated with transmembrane processes while downregulated genes were associated with metabolic processes; KEGG pathways were also identified. This work highlighted the adaptation of 3 key BVAB growing on a medium that simulates in vivo conditions and will contribute to a better understanding of bacterial interactions that are important in the pathogenesis of incident BV.
