ABSTRACT: We present an extremely rare report on gastric inlet obstruction due to giant abdominal aortic aneurysm. Despite the rare occurrence of this manifestation, it should be considered in the differential diagnosis of gastric inlet obstructions.

Repeat Rupture of a Giant Abdominal Aortic Aneurysm after EVAR.

Project description:IntroductionRuptured abdominal aortic aneurysms (AAAs) are known to be associated with high fatal outcomes. Giant AAAs are often defined as having a maximum diameter over 13 cm. Large AAAs over 8 cm have demonstrated a yearly rupture rate of 30-50%, which explains the rarity of giant AAAs. Endovascular repair of ruptured AAAs (rAAAs) is increasingly advocated because of the shorter hospital stay and fewer post-operative complications. Nonetheless, outcomes regarding mortality and cost-effectiveness show a large variability and long-term outcomes are lacking. Few data have been published on treatment of giant AAAs and rAAAs; however, open surgery is generally the preferred option.ReportAn 83 year old presented to the Emergency Department with a history of ruptured abdominal aortic aneurysm treated with an aorto-uni-iliac endograft and a femorofemoral crossover bypass. During follow up, this was complicated by a symptomatic type III endoleak, which was treated by endovascular repair. During the current admission, he presented with a re-rupture of his former aneurysm, which now was 18 cm diameter because of a type IA endoleak. Open surgical repair was performed and the post-operative course was without complications.DiscussionThe current case underlines the value of vascular surgeons being able to perform both open and endovascular surgery in rAAA.

Complicated mycotic saccular aneurysm of the infra-renal abdominal aorta with infected retroperitoneal hematoma: a clinical case report.

Project description:BackgroundRupture of an abdominal aortic aneurysm (AAA) is a life-threatening emergency, with untreated cases nearing a 100% mortality rate. This case presents a rare complication of AAA rupture with an infected retroperitoneal hematoma, emphasizing the importance of timely diagnosis and a multidisciplinary approach.Case presentationA 59-year-old male presented with lower back pain, fever, and difficulty moving, persisting for three weeks. Imaging revealed a saccular infra-renal AAA rupture with an infected retroperitoneal hematoma. Emergency surgery included extracorporeal subclavian-femoral bypass and aneurysm resection. Despite intraoperative complications, the patient recovered after 16 days in intensive care and was discharged in satisfactory condition. Follow-up CT one month later showed functioning bypasses and clinical improvement.ConclusionThis case illustrates the critical need for early diagnosis and coordinated surgical intervention in complex AAA ruptures complicated by infection. Timely multidisciplinary treatment is crucial to prevent further complications and improve patient outcomes.

Wheezing of unexpected etiology: A case report of pea aspiration mimicking bronchus obstruction caused by a tumor.

Project description:IntroductionGeriatric patients with foreign body aspiration (FBA) lack a detailed medical history. Meanwhile, FBA can mimic other diseases and present with wheezing. Here, we report on the difficulty of making a diagnosis of FBA in an elderly man with wheezing.Case presentationAn 84-year-old man presented with wet cough. He had progressive lung cancer, for which only supportive care was provided. His physical examination revealed wheezing. We presumptively diagnosed acute exacerbation of chronic obstructive pulmonary disease (AE-COPD) and administered inhaled procaterol and parenteral methylprednisolone, which did not mitigate his symptoms. Computed tomography revealed a round nodule, 1 cm in diameter, in his right intermediate bronchus. Central airway obstruction (CAO) caused by the tumor was initially suspected. However, bronchoscopy revealed a pea (Pisum sativum) lodged in his right bronchus, which was removed using forceps.DiscussionThe typical clinical presentations of FBA are sudden-onset cough and dyspnea. However, some geriatric patients do not have cough or lack the cognitive capacity to inform of an episode of FBA. FBA can mimic other entities including AE-COPD and CAO, as seen in our case. Clinicians should not terminate the diagnostic process until all available information explaining the patient's signs and symptoms is collected.ConclusionDespite the lack of apparent aspiration, FBA is an important differential diagnosis of new wheezing in the elderly. Physicians need to carefully evaluate "trivial" information or collect additional information when encountering airway symptoms in elderly patients to avoid missing an FBA diagnosis.

Giant mediastinal bronchial artery aneurysm mimicking mediastinal mass: A case report and brief review of the literature.

Project description:Bronchial artery aneurysm and pseudoaneurysm is a rare but life-threatening diagnosis due to catastrophic complications from rupture. Prompt detection and management is key to prevent complications. CT angiogram and digital subtraction angiography are preferred diagnostic imaging modalities. Being very uncommon, these entities can be misdiagnosed as a nonspecific mediastinal soft tissue mass, which can lead to delay in diagnosis and inappropriate or delayed management. We present a case of 72-year-old woman with incidentally detected large bronchial artery pseudoaneurysm, incorrectly classified as mediastinal malignancy at outside facility, receiving follow-up exams for 2 years, before correct diagnosis and management.
