Soft-shell headgear, concussion and injury prevention in youth team collision sports: a systematic review.

ABSTRACT: Objectives
To assess the association between soft-shell headgear (HG) use and sports-related concussion (SRC). Secondary objectives were to assess the association between HG and superficial head injury and investigate potential increase in injury risk among HG users.Design
A systematic search in Ovid MEDLINE, Cochrane Library, Scopus, PsycINFO and SPORTDiscus was conducted in April 2020. Inclusion criteria were youth <18, English language, in vivo studies published after 1980 that evaluated SRC and other injury incidence in HG users compared with non-users.Outcome measures
Incidence rates of SRC, superficial head injury or other injuries.Results
Eight studies were eligible. The majority (n=5) reported no difference in the rate of SRC among HG users versus non-users. One rugby study identified significantly lower risk of SRC for non-HG users (risk ratio (RR) 0.63; 95% CI 0.41 to 0.98) compared with HG users, whereas a cross-sectional survey of soccer players indicated higher risk of SRC for non-HG users (RR 2.65; 95% CI 1.23 to 3.12) compared with HG users. Three of the four studies investigating superficial head injury found no significant differences with HG use, though the soccer survey reported reduced risk among HG users (RR 1.86; 95% CI 0.09 to 0.11). Increased incidence of injuries to all body regions for rugby HG users was reported in two studies with adjusted RRs of 1.16 (95% CI 1.04 to 1.29) and 1.23 (95% CI 1.00 to 1.50).Conclusions
HG use was not associated with reduced rates of SRC or superficial head injury in youth soccer and rugby. The possibility of increased injury risk to all body regions for rugby HG users was raised. The need for research specific to youth and female athletes was highlighted.Prospero registration number

Project description:The aim of this systematic review conducted in the topic of youth team-sports players was three-fold: (i) Analyze the variations of decision-making processes between low- and high-level youth players; (ii) analyze the variations of decision-making processes between different age groups; and (iii) analyze the effects of decision-making training-based programs on youth players. Following the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines, this systematic review searched for studies on PubMed, ScienceDirect, Academic Search Complete, SPORTDiscus, and Taylor & Francis Online. The search returned 6215 papers. After screening the records against set criteria, 26 articles were fully reviewed. From the included studies, 9 were focused on comparing the decision-making process between low- and high-level players, 6 compared the decisions made by players from different age categories, and 11 analyzed the effects of decision-making-based training programs on youth players. Comparisons between high- and low-level players suggested that high-level and most talented players present a greater accuracy in the cognitive and executive answers to the game as well as being more adjustable to more complex situations. Considering the comparisons between age groups, a tendency of older players to execute more accurate decisions in the game and to have better tactical knowledge and behavior was observed. Finally, the effects of decision-making training programs suggest a beneficial effect employing practical scenarios (mainly based on small-sided and conditioned games), primarily improving passing decisions and execution. However, the benefits of interventions using videos are not clear.

Project description:INTRODUCTION:Given the growing research on potential adverse outcomes related to concussion and other serious brain injuries and the increased susceptibility for concussion among youth athletes, primary prevention is vital to protect the health and safety of this population. The purpose of this study is to summarize the current research on risk factors and primary concussion prevention strategies focused on specific youth sports, and to identify research gaps. METHODS:A literature search was conducted using six electronic databases. A scoping review method was used to identify studies that addressed risk factors or primary concussion prevention strategies focused on youth athletes (ages 5-18?years) in six sports (football, ice hockey, soccer, lacrosse, basketball, and wrestling). RESULTS:Of the 18 publications identified, the publications focused on risk factors (N?=?11), policy (N?=?1), rule changes (N?=?3), education (N?=?2), equipment (N?=?2), and playing technique (N?=?0). Some articles had information related to multiple topics. CONCLUSIONS:Current research on concussion prevention has primarily been focused on risk factors. There are a dearth of studies that examine primary concussion prevention in sports. When studies do exist, most focus on football and ice hockey. Only a small number of studies focus solely on risk factors or primary prevention in soccer, lacrosse, basketball, and wrestling-all sports in which concussions are common. Practical applications: This scoping review summarizes current research on concussion risk factors and primary prevention strategies in specific sports focused on youth athletes and identifies research gaps to help inform future efforts.

Project description:Some of the more protective and favorable factors for the development and health in children and teenagers are family and sport, so family involvement in the children's sports activities is vital in their sports process. The purpose of this study was to analyze the verbal behavior (positive, negative, and neutral comments) of family spectators of school-age athletes regarding sociodemographic and sporting variables. The sample consisted of 190 family spectators of 215 male and female (Mage = 11.66; SD = 1.60) football, basketball, and volleyball players. The Parents' Observation Instrument at Sport Events (POISE) was used for the observation and LINCE was used to codify the verbal comments made. After registering 38,829 comments, the results showed statistically significant differences in relation to the comments made and the gender of athletes, geographical area, kind of sport, and the sporting category. The findings highlight that in a competitive environment, the comments made by spectators related to athletes do not seem to be initiators of potentially violent situations but rather are dependent on the atmosphere in question. Further research is required in this area to foster positive conduct relating to grassroots sports.

Project description:Objective: Neuromuscular injury prevention programs (IPP) can reduce injury rate by about 40% in youth sport. Multimodal IPP include, for instance, balance, strength, power, and agility exercises. Our systematic review and meta-analysis aimed to evaluate the effects of multimodal IPP on neuromuscular performance in youth sports. Methods: We conducted a systematic literature search including selected search terms related to youth sports, injury prevention, and neuromuscular performance. Inclusion criteria were: (i) the study was a (cluster-)randomized controlled trial (RCT), and (ii) investigated healthy participants, up to 20 years of age and involved in organized sport, (iii) an intervention arm performing a multimodal IPP was compared to a control arm following a common training regime, and (iv) neuromuscular performance parameters (e.g., balance, power, strength, sprint) were assessed. Furthermore, we evaluated IPP effects on sport-specific skills. Results: Fourteen RCTs (comprising 704 participants) were analyzed. Eight studies included only males, and five only females. Seventy-one percent of all studies investigated soccer players with basketball, field hockey, futsal, Gaelic football, and hurling being the remaining sports. The average age of the participants ranged from 10 years up to 19 years and the level of play from recreational to professional. Intervention durations ranged from 4 weeks to 4.5 months with a total of 12 to 57 training sessions. We observed a small overall effect in favor of IPP for balance/stability (Hedges' g = 0.37; 95%CI 0.17, 0.58), leg power (g = 0.22; 95%CI 0.07, 0.38), and isokinetic hamstring and quadriceps strength as well as hamstrings-to-quadriceps ratio (g = 0.38; 95%CI 0.21, 0.55). We found a large overall effect for sprint abilities (g = 0.80; 95%CI 0.50, 1.09) and sport-specific skills (g = 0.83; 95%CI 0.34, 1.32). Subgroup analyses revealed larger effects in high-level (g = 0.34-1.18) compared to low-level athletes (g = 0.22-0.75), in boys (g = 0.27-1.02) compared to girls (g = 0.09-0.38), in older (g = 0.32-1.16) compared to younger athletes (g = 0.18-0.51), and in studies with high (g = 0.35-1.16) compared to low (g = 0.12-0.38) overall number of training sessions. Conclusion: Multimodal IPP beneficially affect neuromuscular performance. These improvements may substantiate the preventative efficacy of IPP and may support the wide-spread implementation and dissemination of IPP. Project description:BackgroundA large volume of systematic reviews and meta-analyses has been published on the effectiveness of sports injury prevention programs.PurposeTo provide a qualitative summary of published systematic reviews and meta-analyses that have examined the effectiveness of sports injury prevention programs on reducing musculoskeletal injuries.Study designSystematic review; Level of evidence, 4.MethodsWe searched the PubMed, CINAHL, EMBASE, and the Cochrane databases for systematic reviews and meta-analyses that evaluated the effectiveness of sports injury prevention programs. We excluded published abstracts, narrative reviews, articles not published in English, commentaries, studies that described sports injury prevention strategies but did not assess their effectiveness, studies that did not assess musculoskeletal injuries, and studies that did not assess sports-related injuries. The most relevant results were extracted and summarized. Levels of evidence were determined per the Oxford Centre for Evidence-Based Medicine, and methodological quality was assessed using the AMSTAR-2 (A MeaSurement Tool to Assess systematic Reviews, revised version).ResultsA total of 507 articles were retrieved, and 129 were included. Articles pertaining to all injuries were divided into 9 topics: sports and exercise in general (n = 20), soccer (n = 13), ice hockey (n = 1), dance (n = 1), volleyball (n = 1), basketball (n = 1), tackle collision sports (n = 1), climbing (n = 1), and youth athletes (n = 4). Articles on injuries by anatomic site were divided into 11 topics: general knee (n = 8), anterior cruciate ligament (n = 34), ankle (n = 14), hamstring (n = 11), lower extremity (n = 10), foot (n = 6), groin (n = 2), shoulder (n = 1), wrist (n = 2), and elbow (n = 1). Of the 129 studies, 45.7% were ranked as evidence level 1, and 55.0% were evidence level 2. Based on the AMSTAR-2, 58.9% of the reviews reported a priori review methods, 96.1% performed a comprehensive literature search, 47.3% thoroughly described excluded articles, 79.1% assessed risk of bias for individual studies, 48.8% reported a valid method for statistical combination of data (ie, meta-analysis), 45.0% examined the effect of risk of bias on pooled study results, and 19.4% examined the risk for publication bias.ConclusionThis comprehensive review provides sports medicine providers with a single source of the most up-to-date publications in the literature on sports injury prevention.

Project description:BackgroundThe use of dedicated training programs for improving decision-making (DM) in team sports players has grown in the last several years. Approaches such as imagery training, video-based training, or game-based drills are some of the interventions used in youth players in order to improve DM. However, no systematic reviews or meta-analyses have been conducted to summarize the main evidence regarding the effects of these programs on the players and identify the magnitude of the effects compared to control groups.ObjectiveThis systematic review (with meta-analysis) was conducted to assess the effects of training programs on the DM of youth team sports players.Data sourcesThe data sources utilized were PubMed, PsycINFO, Scopus, SPORTDiscus, and Web of Science.Study eligibility criteriaThe criteria included the following: (i) youth (≤ 18 years old) team sports players with no restriction on sex or competitive level; (ii) players subjected to training programs to develop DM; (iii) control groups; (iv) pre-post outcomes related to tactical behavior, technical execution, reaction, and decision time; and (v) controlled trials.ResultsThe database search initially identified 2497 titles. From these, six articles were eligible for the systematic review and meta-analysis. The results showed a significant beneficial effect of DM interventions on tactical behavior (ES = 1.12; p = 0.035; I 2 = 80.0%; Egger's test p = 0.066), whereas no significant effect of DM interventions on technical execution was found (ES = 0.74; p = 0.180; I 2 = 69.1%; Egger's test p = 0.873).ConclusionThe DM interventions were significantly effective in improving tactical behavior in youth team sports players independently from the number of sessions to which players were exposed. In addition, DM interventions were significantly effective in improving technical execution. However, the results should be carefully interpreted due to the heterogeneity of the articles' overall methodological quality. Future DM interventions should consider using combined approaches that allow players to develop both tactical behavior and technical execution.

Project description:BACKGROUND:Every year, millions of children in the United States participate in youth full-contact sports, which carry concussion risks-the long-term sequelae of which are not well understood. We examined the attitudes and knowledge of adults in Chicago about youth sports participation, concussion risk, and whether physicians should counsel against youth participation in full-contact sports. METHODS:An anonymous paper survey featuring 13 attitudinal, 13 demographic, and 9 knowledge questions was distributed to a convenience sample of adults ?18 years in hospital waiting areas and four Chicago parks. Participants were asked to hypothetically consider themselves the parent of a 10-year-old child regarding attitudes towards full-contact sports participation. RESULTS:Between June 13 and July 27, 2016, 1091 partial or complete valid surveys were collected. Almost half (46%) of respondents would not allow a hypothetical 10-year-old son to play tackle football. The majority (74%) of respondents agreed that it was appropriate for physicians to counsel against youth participation in full-contact sports. Respondents obtained information about concussions from, on average, 2-3 sources, although only 34% received information from physicians. Respondents demonstrated a high concussion knowledge level (average: 6.75 of 9 questions). However, only 39% of respondents correctly answered that the following statement was false: "After a mild concussion, there are usually visible changes on medical imaging". CONCLUSIONS:Overall, respondents are well-informed about concussions. They are divided about the participation of youth in full-contact sports and are amenable to physician counseling against youth participation in full-contact sports.

Project description:PurposeWater polo is a sport with a high degree of physicality and aggressive play. Unlike most contact sports, epidemiological data on the incidence or prevalence of head trauma in water polo have not been gathered, reported, or made publicly available. The purpose of this study was to begin a systematic characterization of the risks of head impact and concussion in men and women who play water polo at various levels.DesignWe sent an electronic survey to the 44,000+ members of USA Water Polo, asking questions about concussions, head impacts, and symptoms commonly associated with prior concussion. From over 1500 complete responses, we report summary information on the prevalence of concussions and major head impacts in water polo.ResultsWe found that 36% of respondents report sustaining a concussion while playing water polo, with an average of two concussions reported. The prevalence and number of concussions reported varied across positions, levels, and gender. Most strikingly, we found that goalies are at significantly higher risk for concussion, report a significantly more concussions, and appear to experience a qualitatively different type of head impact compared to other positions. Additionally, we found that competition level, gender, and field position are robust predictors of concussion risk.ConclusionOur findings demonstrate that concussions are not uncommon in water polo players. We conclude that there is need for systematic concussion reporting in water polo and suggest that understanding the risk factors of concussion in water polo will require fully considering differences in the head impact exposure between different field positions, competition levels, sexes, and differences in exposure between competition and practice.

Project description:ObjectiveTo evaluate the current literature regarding the utility of global positioning system (GPS)-derived workload metrics in determining musculoskeletal injury risk in team-based field-sport athletes.Data sourcesPubMed entries from January 2009 through May 2019 were searched using terms related to GPS, player workload, injury risk, and team-based field sports.Study selectionOnly studies that used GPS metrics and had injury as the main outcome variable were included.Data extractionTotal distance, high-speed running, and acute : chronic workload ratios were the most common GPS metrics analyzed, with the most frequent sports being soccer, rugby, and Australian rules football.Data synthesisMany distinct workload metrics were associated with increased injury risk in individual studies performed in particular sport circumstances; however, the body of evidence was inconclusive as to whether any specific metrics could consistently predict injury risk across multiple team-based field sports.ConclusionsOur results were inconclusive in determining if any GPS-derived workload metrics were associated with an increased injury risk. This conclusion is due to a myriad of factors, including differences in injury definitions, workload metrics, and statistical analyses across individual studies.

Project description:The objective of this study is to show the links and differences in the expressions of competitive anxiety in the face of the existence of resilient resources in young athletes, according to sporting (years of experience) and personal (gender) characteristics. To meet these aims, the participants answered the Resilience Scale (RS-14) and the Competitive State Anxiety Inventory-2R (CSAI-2R). The sample consisted of 241 adolescent handball and basketball players between 14 and 17 years old. Different analyses were performed, including a differential and multivariate descriptive, a correlation, and a multiple regression. The results showed that anxiety was negatively related to resilience in its acceptance dimension. It was shown that girls showed higher levels of somatic anxiety, while boys showed higher levels of acceptance. Statistically significant differences were found in the resources for acceptance in favor of boys, while there were significantly different indicators in somatic anxiety and self-confidence in favor of girls. The sports experience was positively related to resilience and negatively to anxiety. Although the existence of indicators of cognitive anxiety (e.g., recurrent thoughts or rhyming), coaches and athletes need to understand that they are also indicators of a necessary activation for psychological functioning. Channeling such a process through psychological training of different skills will enhance the capacities for self-confidence.
