Mental Health among Spanish Adults with Diabetes: Findings from a Population-Based Case-Controlled Study.

ABSTRACT: The purpose of this study was to assess and compare the prevalence of self-reported mental disorders, psychological distress, and psychotropic drug consumption among people with diabetes mellitus (DM) and matched non-DM controls. A case-controlled study using data from the Spanish National Health Interview Survey was conducted in 2017. We identified 2116 DM adults (aged ≥35 years). Non-DM controls were matched 1:1 by age, sex, and province of residence. Prevalence of mental disorders (25.0% vs. 19.2%; p < 0.001), psychological distress (29% vs. 19.5%; p < 0.001), and consumption of psychiatric medications (29.7% vs. 23.5%; p < 0.001) among DM cases were higher than those among matched non-DM controls. The DM patient variables associated with experiencing a mental disorder, psychological distress, and consumption of psychiatric medications were: being a woman, worse self-rated health, and a visit to a psychologist within the last year. Older age (≥80 years) was associated with a lower probability of reporting mental disorders and psychological distress among DM cases. Not practicing physical exercise was significantly associated with experiencing psychological distress. Adults with DM included in our investigation have a significantly higher prevalence of mental disorders, psychological distress, and consumption of psychiatric medications than non-DM controls. It is necessary to implement screening strategies and psychological interventions to improve the mental health of DM patients in Spain, focusing especially on women and those aged 35 to 59 years.

Project description:BackgroundObsessive-compulsive (OC) disorder is a debilitating disorder with a high delay in help-seeking that could be associated with two barriers that may differ between OC content dimensions: public stigma and mental health literacy.ObjectivesWe aim to describe and analyze the differences among OC content dimensions in public stigma, social distance desire, mental health literacy, and help-seeking intention in a sample of the mental health-naïve population.MethodsA total of 487 participants from the Spanish community with no previous knowledge of or experience with OCD were randomly allocated one of six vignettes describing a person with OC symptoms of one out of six contents (i.e., aggression/harm; sexual; religious/blasphemous/immoral; contamination/washing; doubts/checking; or superstition/symmetry/order). After reading the vignette, participants completed the following questionnaires associated with their vignette: Attribution Questionnaire, General Help-Seeking Questionnaire, Social Distance Scale, and Mental Health Literacy Questionnaire.ResultsThere were significant differences among OC content dimensions on the variables of interest. Aggression/harm OC content was associated with the highest public stigma and discriminating desire, and together with the religious/blasphemous/immoral OC contents were associated with the highest social distance desire. Although no differences appear between contents on intention to seek for formal treatment, participants would request less informal support for their sexual OC content than for the other categories. Contamination, doubts/checking, and superstition/symmetry/order OC contents were more frequently identified as OCD, although only around 50% or less of the sample identified the sexual, superstition/symmetry/order, doubts/checking, and religious/blasphemous/immoral OC contents as a mental health problem.ConclusionsInterventions to reduce stigma and increase help seeking are necessary and should consider OC heterogeneity.

Project description:AimsOur study aimed to (1) identify trajectories on different mental health components during a two-year follow-up of the COVID-19 pandemic and contextualise them according to pandemic periods; (2) investigate the associations between mental health trajectories and several exposures, and determine whether there were differences among the different mental health outcomes regarding these associations.MethodsWe included 5535 healthy individuals, aged 40-65 years old, from the Barcelona Brain Health Initiative (BBHI). Growth mixture models (GMM) were fitted to classify individuals into different trajectories for three mental health-related outcomes (psychological distress, personal growth and loneliness). Moreover, we fitted a multinomial regression model for each outcome considering class membership as the independent variable to assess the association with the predictors.ResultsFor the outcomes studied we identified three latent trajectories, differentiating two major trends, a large proportion of participants was classified into 'resilient' trajectories, and a smaller proportion into 'chronic-worsening' trajectories. For the former, we observed a lower susceptibility to the changes, whereas, for the latter, we noticed greater heterogeneity and susceptibility to different periods of the pandemic. From the multinomial regression models, we found global and cognitive health, and coping strategies as common protective factors among the studied mental health components. Nevertheless, some differences were found regarding the risk factors. Living alone was only significant for those classified into 'chronic' trajectories of loneliness, but not for the other outcomes. Similarly, secondary or higher education was only a risk factor for the 'worsening' trajectory of personal growth. Finally, smoking and sleeping problems were risk factors which were associated with the 'chronic' trajectory of psychological distress.ConclusionsOur results support heterogeneity in reactions to the pandemic and the need to study different mental health-related components over a longer follow-up period, as each one evolves differently depending on the pandemic period. In addition, the understanding of modifiable protective and risk factors associated with these trajectories would allow the characterisation of these segments of the population to create targeted interventions.
