Project description:BackgroundThis study aimed to report the long-term survival of fixed-bearing medial unicompartmental knee arthroplasty (UKA) with a mean of 14-year follow-up, and to determine possible risk factors of failure.MethodsWe retrospectively evaluated 337 fixed-bearing medial UKAs implanted between 2003 and 2014. Demographic and radiographic parameters were measured, including pre-operative and post-operative anatomical femorotibial angle (aFTA), posterior tibial slope (PTS), and anatomical medial proximal tibial angle (aMPTA). Multivariate logistic regression analysis was applied to figure out risk factors.ResultsThe mean follow-up time was 14.0 years. There were 32 failures categorized into implant loosening (n = 11), osteoarthritis progression (n = 7), insert wear (n = 7), infection (n = 4), and periprosthetic fracture (n = 3). Cumulative survival was 91.6% at 10 years and 90.0% at 15 years. No statistically significant parameters were found between the overall survival and failure groups. Age and hypertension were significant factors of implant loosening with odds ratio (OR) 0.909 (p = 0.02) and 0.179 (p = 0.04) respectively. In the insert wear group, post-operative aFTA and correction of PTS showed significance with OR 0.363 (p = 0.02) and 0.415 (p = 0.03) respectively. Post-operative aMPTA was a significant factor of periprosthetic fracture with OR 0.680 (p < 0.05).ConclusionsThe fixed-bearing medial UKA provides successful long-term survivorship. Tibial component loosening is the major cause of failure. Older age and hypertension were factors with decreased risk of implant loosening.

Project description:BackgroundWe sought to determine the 10-year survivorship and reasons for revision for a fixed-bearing unicompartmental knee arthroplasty (UKA) design. In addition, we report on patient-reported outcomes and satisfaction and compare results of medial vs lateral compartment UKA and cemented vs cementless UKA with the same design.MethodsWe performed a retrospective cohort study on a single-surgeon case series using a single fixed-bearing UKA design in 158 consecutive patients who underwent 177 UKA procedures between July 2000 and December 2010. Cases missing follow-up within the last year, clinically or via telephone, were excluded (n = 17, 10%). A cumulative incidence competing risk model was used to evaluate the cumulative incidence of failure.ResultsCumulative incidence of revision at a mean follow-up of 10 years was 13%. The majority of revisions (43%, 10/23) were for aseptic tibial component loosening, followed by progression of osteoarthritis (5/23, 22%). All Patient Reported Outcomes Measurement Information System measures demonstrated mean T-scores within one standard deviation from the US population norm. The mean Knee Injury and Osteoarthritis Outcome Score for Joint Replacement was 96.9 (range, 40-100). The mean pain score was 3.8 (range, 0-8). Eighty-six percent of patients were satisfied with the UKA.ConclusionsAt 10-year follow-up, the most common causes for revision were aseptic tibial loosening and adjacent compartment knee arthroplasty, and similar results were found for medial vs lateral compartment and for cemented vs cementless UKA. Surgeons should consider these findings for future UKA designs, and this represents the first study reporting on survivorship and outcomes of this specific UKA design.

Project description:Background and purpose - Differences in stress distribution in a mobile-bearing and fixed-bearing unicompartmental knee arthroplasty (UKA) design might lead to a difference in fixation of the tibial component. We compared tibial component migration of a mobile-bearing (MB) UKA and a fixed-bearing (FB) UKA using radiostereometric analysis.Patients and methods - In a randomized, patient-blinded clinical trial 62 patients received either the MB Oxford UKA or the FB Sigma UKA. The patients were followed for 24 months with radiostereometric analysis. Clinical outcome was assessed with Oxford Knee Score (OKS), RAND-36 and leg extension power.Results - Migration of the tibial components was similar between groups throughout follow-up. At 12 months, MTPM of the tibial component was 0.44?mm (95% CI 0.34-0.55) for the MB group and 0.40?mm (CI 0.31-0.50) for the FB group. Between 12 and 24 months, the tibial components migrated with a median MTPM increase of 0.03?mm (CI -0.02 to 0.08) in the MB group and 0.03?mm (CI -0.02 to 0.07) in the FB group. Continuous migration of the tibial component was found for 1?MB UKA and 2 FB UKAs. Both groups showed similar and clinically relevant improvement in clinical outcome.Interpretation - MB and FB tibial components had similar good fixation and clinical improvement until 2 years. Based on this study, a low 5- to 10-year revision rate can be expected for both implants.

Project description:BackgroundIsolated osteoarthritis of the lateral compartment of the knee is less common than that of the medial compartment, resulting in significantly fewer lateral unicompartmental knee arthroplasties (UKAs) being performed. This study aimed to evaluate results of a fixed-bearing UKA for the treatment of lateral compartment osteoarthritis of the knee.MethodsA prospectively collected cohort of 255 patients undergoing fixed-bearing UKA of the lateral compartment using the Triathlon PKR (Stryker, Warsaw, IND) implant with a minimum 2-year follow-up was reviewed. The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, radiographic alignment, complications, reoperations, and revisions were recorded. Patient factors and pre- and post-surgical alignment were assessed for their association with a minimum important change (MIC) of the total WOMAC score.ResultsA total of 246 implants with a mean follow-up of 6.6 years (2-10.8 years) were included (4% lost to follow-up). The total WOMAC score increased from 61.3 ± 3.5 to 85.3 ± 7.5, exceeding the MIC in 215 patients (88%). Exceeding the MIC was not associated with age, body mass index, or alignment. The 5-year implant revision rate was 1.6% (3/187).ConclusionsThe fixed-bearing Stryker Triathlon PKR implant for lateral UKA resulted in good clinical outcomes with a low revision rate at midterm follow-up. Body mass index, age, and pre- and post-surgical alignment did not correlate with the clinical outcome. Long-term follow-up is necessary to determine if the clinical improvement and low revision rate can be maintained.

Project description:IntroductionThe surgical technique used in unicompartmental knee arthroplasty (UKA) is crucial for achieving good short and long term clinical outcomes. The medial mobile bearing UKA has shown excellent clinical outcomes and survivorship. But release of the medial collateral ligament during entering joint is cause of mobile bearing dislocation in short term outcomes and lateral compartment osteoarthritis may occur in the mid to long term outcomes. Removing all osteophytes at the time of UKA is sometime impossible due to their large size and extend to the inferior part of medial tibial plateau and removing them completely my result in release of the MCL. But no data exist on clinical outcomes in such patients.MethodsWe conducted a prospective study from 2010 to 2015 of patients undergoing mobile bearing UKA and classified them in to two groups: those with (Gp1) and without (Gp2) residual osteophytes. Osteophyte size was measured using Hernborg's technique. The primary outcomes were pain score, functional score, and knee scores and the presence of reported medial knee pain.Results176 patients who underwent 199 mobile bearing UKAs were recruited: Gp1 = 42 patients (46 knees) and Gp2 = 134 patients (153 knees). Residual osteophyte sizes ranged from 2.13-9.42 mm (mean 4.12). The mean Gp1 Gp2 pain score (49.04, 48.92, p = 0.84), functional score (83.75, 84.04, p = 0.83) and knee score (89.86, 98.7, p = 0.0.78) scores were almost identical and no one complained of medial joint pain. Followed up ranged from 2 - 7 years (mean 4.23). No patients were lost to follow up.ConclusionThe patients with residual osteophytes of length less than 9 mm had good and similar clinical outcomes as patients without residual osteophytes following mobile bearing UKA.Level of evidenceLevel II-2, evidence obtained from well-designed cohort studies or case-control studies, preferably from more than one center or research group.

Project description:PurposeThe argument that patellofemoral osteoarthritis (PFOA) is a contraindication to unicompartmental knee arthroplasty (UKA) remains to be resolved. The purpose of this study was to perform a meta-analysis to determine whether PFOA affects functional outcomes and survivorship after medial UKA.MethodsA literature search was performed in PubMed, Medline, Cochrane Library and Web of science to identify studies published in English (the last search was updated on June 1, 2020). The primary outcome measure was the Oxford Knee Score (OKS), whereas the secondary outcomes included range of motion (ROM) and the revision rate. Patients with patellofemoral joint narrowing or cartilage lesions as assessed intraoperatively or by radiography were assigned to the PFOA group.ResultsA total of 8 studies involving 3504 patients and 3975 knees were included in this meta-analysis. No patients had a severe lateral patellar groove. The results showed that there was no significant difference in the OKS, revision rate and ROM between PFOA patients and patients without PFOA.ConclusionsNo significant difference in functional outcomes and survivorship was found between patients with and without PFOA. Patients with PFOA assessed by radiographs or intraoperatively but without a lateral patellar groove should be considered candidates for medial UKA.

Project description:IntroductionUnicompartmental knee arthroplasty (UKA) is one of the most effective surgical procedures for treating isolated medial compartment knee osteoarthritis. However, previous studies have regarded patellofemoral osteoarthritis as a contraindication for UKA. In contrast, most current research shows that damage to the articular cartilage of the patellofemoral joint, even to the extent of full-thickness cartilage loss, has no influence on the outcome of UKA.Methods and analysisStudy settings: This study is a prospective cohort study that will compare the Forgotten Joint Score and Lonner patellofemoral joint score of patients who have undergone UKA; the patients will be divided into two groups (with and without patellofemoral joint osteoarthritis (PFJOA)).Primary objectiveLong-term follow-up will be used to evaluate the effect of the operation on the above-mentioned scores in both the groups.Secondary objectiveWe will divide the patients from the with PFJOA group into three subgroups according to the localisation of patellofemoral cartilage lesions (medial zone, lateral zone and central zone). We aim to compare knee joint scores among these groups and clarify the impact of different wear sites on clinical efficacy. We will use CT to explore the potential mechanism through which UKA affects patellofemoral joint-related parameters (lateral patellar tilt, lateral patellar shift and tibia tuberosity-trochlear groove distance). We will also record mid-term/long-term post-surgery complications.Ethics and disseminationThis study's protocol is in accordance with the Declaration of Helsinki. Project description:BackgroundA new morphometric fixed-bearing unicompartmental knee arthroplasty (UKA) system has been introduced to address the anatomical patient-specific challenges. It was our hypothesis that accurate restoration of the patient-specific anatomy would restore normal knee kinematics after UKA. Therefore, we aimed in this cadaveric study to analyze the impact of a medial morphometric UKA on (1) the varus-valgus and anterior-posterior stability of the knee, (2) the knee kinematics during standardized activities of the daily living, and (3) the patellar tracking, measured using a dedicated robotic testing protocol.MethodsEight human knee specimens underwent full-leg computed tomography CT scanning and comprehensive robotic assessments of tibiofemoral and patellofemoral kinematics. Specimens were tested in the intact state and after implantation of a fixed-bearing medial UKA. Assessments included passive flexion, laxity testing and simulations of level walking, lunge, and stair descent.ResultsMedial and lateral joint laxity after UKA closely resembled intact laxity across the full arc of flexion. Anterior-posterior envelope of motion showed a close match between the intact and UKA groups. Net rollback and average laxity were both not statistically different. Simulation of activities of daily living showed a close match in the anterior-posterior motion profile between the medial condyle and lateral condyle. Patellar tilt and medial-lateral shift during knee flexion matched closely between groups.ConclusionFunctional assessment of this UKA system shows nearly identical behavior to the intact knee. Fixed-bearing UKA with morphometric, compartment-specific geometry and precise mechanical instrumentation replicates complex knee balance and kinematics.

Project description:AIM:To compare clinical outcomes of patients with and without preoperative genu recurvatum (GR) following mobile bearing unicompartmental knee arthroplasty (UKA). METHODS:We prospectively followed 176 patients for at least 24 mo who had been treated by unilateral, minimally invasive, Oxford UKA. Patients with medial osteoarthritis (OA) knee and preoperative GR (Group I) accounted for 18% (n = 32) and patients without preoperative GR (Group II) accounted for the remaining 82% (n = 144). Knee score, pain scores, and functional scores were assessed for each patient and compared between the two groups. The incidence of postoperative GR and the postoperative hyperextension angles also were recorded and analyzed. RESULTS:The pain score, knee score and functional score were not significantly different between the two groups. Similarly, the incidence of postoperative GR and the measured hyperextension angles were not significantly different between the two groups. The incidence of postoperative GR was 1/32 (3.12%) in Group I and 1/144 (0.69%) in Group II (P = 0.34). The mean postoperative hyperextension angles were 2.40° ± 2.19° (range: 1°-7°) for Group I and 1.57° ± 3.51° (range: 1°-6°) for Group II (P = 0.65). CONCLUSION:Medial OA of the knee and concomitant GR is not a contraindication for the mobile bearing UKA.

Project description:BackgroundThe primary purpose of the present prospective study was to consecutively analyse the outcomes of the return to sports activity of patients with positive patch tests undergoing a medial mobile-bearing titanium niobium nitride (TiNbN) unicompartmental knee arthroplasty (UKA). The secondary purpose was to ascertain if a higher grade of physical activity leads to a reduction in the body mass index (BMI) of the patients.Material and methodsForty-one patients with positive skin patch tests were included in this prospective study. The clinical evaluation consisted of the University of California, Los Angeles (UCLA) activity scale and the High-Activity Arthroplasty Score (HAAS). Each patient was evaluated the day before surgery (T0), after 12.37 ± 0.70 months (T1), and on the day of the final follow-up, after 67.03 ± 18.2 months (T2). Furthermore, the BMI of each patient was analysed before surgery and during the final follow-up.ResultsThe UCLA and HAAS mean preoperative values ranged from 3.68 ± 1.1.7 and 6.15 ± 0.76 to 6.1 ± 0.76 and 10.34 ± 1.3, respectively, at T1 (p < 0.0001) and to the final values of 6.34 ± 0.62 and 11.0 ± 8.9, respectively, at T2 (UCLA: T2 versus T1: p = 0.132; T2 versus T0: p < 0.0001; HAAS: T2 versus T1: p = 0.0027; T2 versus T0: p < 0.001). BMI ranged from a preoperative value of 27.97 ± 3.63 to a final value of 26.84 ± 3.11 (p < 0.0001). The only differences within the subgroups concerned patients with BMI ≥ 28, showing a superior HAAS at each follow-up (p < 0.05). A positive correlation was found between BMI and HAAS at T0 and T2 (p < 0.05).ConclusionsThis is the first study to evaluate the rate of the return to sports activities and change in BMI following hypoallergenic UKA. The majority of patients reduced their weight following UKA and improved their physical activity, showing outcomes that were comparable to the standard cobalt-chrome (CoCr) prostheses, regardless of gender, age, BMI and implant size.Level of evidenceIV - Prospective Cohort Study.
