Arthroscopic Suture-to-Adjustable Loop Fixation of Adult Anterior Cruciate Ligament Tibial Avulsion Fracture.

ABSTRACT: Adult tibial avulsion fracture of the anterior cruciate ligament (ACL) occurs not as frequently as ACL tear but still is concerning. There are many methods of arthroscopic fixation of this special fracture. However, a simple and effective method is still being pursued. We would like to introduce an arthroscopic suture-to-adjustable loop fixation technique, which features are a reduction of lateral displacement of the fragment by special suture configuration and tensioning, and a final reduction of residual displacement by tensioning the adjustable loop. We consider the introduction of this technique will provide additional choice in the treatment of adult ACL tibial avulsion fracture.

Arthroscopic Suture-to-Loop Fixation of Posterior Cruciate Ligament Tibial Avulsion Fracture.

Project description:Avulsion fracture of the tibial insertion of the posterior cruciate ligament (PCL) receives constant concern. Arthroscopic procedures have long been attempted because of their minimally invasive nature, and various related techniques have been reported. However, the best arthroscopic method is still being pursued. In this article, we introduce an arthroscopic suture ligation and backup adjustable-loop fixation technique for PCL tibial avulsion fracture. The critical points of this technique are proper ligation of the PCL, proper location of the 2 tibial tunnels to create pulleys for posterior-inferior reduction of the bone fragment, and additional backup suture loop fixation. Our experience indicates that this technique is efficient and relatively simple. We consider that the introduction of this technique will provide a reasonable choice in the treatment of PCL tibial avulsion fracture.

Arthroscopic 4-point suture fixation of anterior cruciate ligament tibial avulsion fractures.

Project description:Tibial eminence avulsion fractures are rare injuries occurring mainly in adolescents and young adults. When necessary, regardless of patient age, anatomic reduction and stable internal fixation are mandatory for fracture healing and accurate restoration of normal knee biomechanics. Various arthroscopically assisted fixation methods with sutures, anchors, wires, or screws have been described but can be technically demanding, thus elongating operative times. The purpose of this article is to present a technical variation of arthroscopic suture fixation of anterior cruciate ligament avulsion fractures. Using thoracic drain needles over 2.4-mm anterior cruciate ligament tibial guidewires, we recommend the safe and easy creation of four 2.9-mm tibial tunnels at different angles and at specific points. This technique uses thoracic drain needles as suture passage cannulas and offers 4-point fixation stability, avoiding potential complications of bony bridge fracture and tunnel connection.

Arthroscopic Transosseous Suture-bridge Fixation for Anterior Cruciate Ligament Tibial Avulsion Fractures.

Project description:Few cases of tibial spine avulsion injuries occur in adolescents. An open or arthroscopic surgical approach is indicated for displaced and nonreducible fractures, but evidence for a gold standard is insufficient. Various arthroscopic techniques are available. Suture fixation is popular and shows good results. The proposed technique is a modified suture-bridge fixation using 2 high-strength sutures tied through 2 transosseous tunnels. This simple and low-cost technique avoids the potential complications of hardware fixation within a joint. It represents an arthroscopic treatment option for anterior cruciate ligament tibial avulsion injuries.

Arthroscopic Suture Fixation in Femoral-Sided Avulsion Fracture of Anterior Cruciate Ligament.

Project description:A femoral-sided avulsion fracture of the anterior cruciate ligament (ACL) is a rare and challenging condition. Most reported cases have occurred in childhood or adolescence. Many techniques of ACL repair have been reported, and in recent years, techniques in arthroscopic surgery have been developed and have become ever more popular with orthopaedic surgeons. We created a technique of arthroscopic ACL repair with suture anchor fixation for a femoral-sided ACL avulsion fracture. This technique saves the natural ACL stump. It is available for cases in which creation of a tibial tunnel is not allowed. Moreover, it does not require a skin incision for fixation on the far femoral cortex and, therefore, does not require a second operation to remove the fixation device. The arthroscopic technique also has a good cosmetic outcome.

Arthroscopic Fixation of Anterior Cruciate Ligament Avulsion Fracture Using FiberWire Suture With Suture Disc.

Project description:Controversy still exists regarding how to reduce and fix a displaced tibial avulsion fracture. Open reduction and internal fixation may lead to morbidity due to soft-tissue injury and arthrotomy. As a result, arthroscopic techniques are increasing in popularity. In the literature, sutures, K wires, and screws are suggested to be used as the fixation devices. Screws cannot be used in small or comminuted fractures, whereas K wires and sutures may not provide strong stability. Recently, with the advent of ultrahigh molecular weight polyethylene sutures like FiberWire, it has become possible to use it as the fixation device in even comminuted avulsion fractures with acceptable reduction stability. In this paper, we describe a simple arthroscopic technique using a FiberWire to manage the displaced tibial eminence avulsion fracture.

Arthroscopic Epiphyseal Plate-Sparing Fixation of Anterior Cruciate Ligament Tibial Avulsion Fracture in Skeletally Immature Patients.

Project description:The tibial avulsion fracture of the anterior cruciate ligament (ACL) in skeletally immature patients poses challenges to orthopaedic surgeons due to the necessity of protecting the epiphysial plate during surgical reduction and fixation of the bone fragment. Several epiphysial plate-sparing techniques have been reported. However, the epiphysial plate is still in danger because in most of these techniques the fixation device is approaching the epiphysial plate or passing through it. We would like to introduce a suture fixation technique in which there is no fixation device passing through the fracture interface as well as the epiphysial plate. The critical points of this technique are ligating the ACL, retrieving the fixation suture distally along the anterior surface of the proximal tibia, and tying the fixation suture at an adjustable loop that is set distal to the proximal tibial epiphysial plate. Our clinical experience indicates that this technique is safe and effective. We consider the introduction of this technique will provide more feasible options when surgical treatment is indicated in case of ACL tibial avulsion fracture in skeletally immature patients.

Arthroscopic Single-Tunnel Pullout Suture Fixation for Tibial Eminence Avulsion Fracture.

Project description:Various arthroscopic techniques have been devised for fixation of tibial eminence avulsions, namely percutaneous K-wires, arthroscopy-guided screw fixation, staples, TightRope (Arthrex)-suture button fixation, and transosseous suture fixation. Such techniques provide well-pronounced advantages including less postoperative pain, a reduced hospital stay, and minimal scar with resultant earlier and more compliant rehabilitation. As for transosseous suture fixation, the standard technique comprises the creation of 2 tibial tunnels exiting on both sides of the footprint of the avulsion fracture using an anterior cruciate ligament tibial guide with the angle set at 45°. Our technique entails the creation of a single tibial tunnel directed from the proximal anteromedial tibia to the center of the tibial eminence. The technique uses Ethibond suture (No. 5) and/or FiberWire suture (Arthrex) to fix the tibial eminence by pulling the anterior cruciate ligament fibers and tightening the pullout suture at the tibial exit of the tunnel with a 4-hole button. This modified single-tunnel pullout suture technique is an appealing option that has proved to be effective and economical with a shorter operative time. Moreover, it provides a less invasive option for skeletally immature patients.

Arthroscopic Reduction and Internal Fixation of Posterior Cruciate Ligament Avulsion Fracture Using an Adjustable-Length Loop Device.

Project description:A displaced avulsion fracture at the tibial attachment of the posterior cruciate ligament is considered an indication for surgical reduction and internal fixation because nonunion and remaining posterior instability of the knee are common consequences of conservative treatment. The problems with standard open surgical techniques are that they are relatively invasive despite the limited operative field and it is impossible to explore intra-articular lesions by the posterior approach. An arthroscopic procedure has the advantage of being minimally invasive and allowing the surgeon to detect and treat associated intra-articular injuries. We present an arthroscopic reduction-internal fixation technique using an adjustable-length loop device. A trans-septal portal is created to visualize the fracture fragment directly, and the fragment is reduced and penetrated with a cannulated drill under fluoroscopic guidance. An adjustable-length loop device is relayed from the posteromedial portal and pulled out through the fragment in an anterograde fashion, placing a button on top of the fragment. By tightening the loop, downward compression can be applied to the fragment. Overall, this technique provides good reduction and bone union, and excellent clinical outcomes, including posterior knee stability, can be achieved.

All-Inside Anterior Cruciate Ligament Tibial Avulsion Repair.

Project description:Anterior cruciate ligament (ACL) tibial avulsion occurs predominantly in children and young adults. It is seen in association with injuries due to hyperextension usually involving movements that are similar to riding a bicycle. Bony ACL avulsion is associated with severe restriction of knee range of motion, swelling, inability to bear weight, and continuous pain. Acute swelling does not allow a conclusive clinical examination. Bony ACL avulsion from the tibial side has been treated by various methods ranging from conservative management to a wide range of operative procedures. The various operative procedures that have been described require challenging operative skills, time, and resources, making these techniques demanding and technically challenging. We describe a technique for the treatment of Meyers-McKeever type II, III, and IV bony tibial ACL avulsions that uses regular anterolateral and anteromedial portals with an additional transpatellar portal. The avulsed fragments along with the ACL are held and buttressed with the help of FiberWires and fixed with the intra-articular portion of the proximal tibia. The technique is performed in an all-inside manner and is easy to master, even for beginners.
